Sub:- APPCB – ZO – VSP – M/s. Community Health Centre, Kotturu (V&M), Srikakulam District – Application for Consent for Operation(CFO) of the Board – BMWA, HWA, Air and Water Consent applications – Payment of balance consent fee for the current year to issue CFO & BMWA - Reg.

2. CFO, BMW & HWA applications received at Regional Office, Srikakulam on 13.06.2019 through OCMMS.

3. RO’s inspection report received at ZO, Visakhapatnam on 25.06.2019.

*** The Board vide reference 1st cited directed all the Health Care Facilities(HCF) to file

a BMW authorization application and CFO under Air & Water Act along with the following instructions:

1. The prescribed fee for the current year i.e., April 2019 to March 2020 at the concerned Regional Office of APPCB immediately.

2. The BMWA fee shall be paid based on bed strength is Rs. 100/- per bed per year.

3. The CFE & CFO fee are based on the total investment in the hospital(entire investment in the hospital including the cost of land and building without depreciation i.e., gross block of fixed assets).

M/s. Community Health Centre, Kotturu (V&M), Srikakulam District submitted the CFO, BMW & HWA applications vide reference 1st cited.

The HCF has paid Rs. 2500/- towards BMW fee, CFE fee & CFO fee but not submitted the gross block of fixed assets to assess the project cost on which the actual fee has to be paid by the HCF for the current year i.e., April 2019 to March 2020. Further, the HCF has not submitted the details of i) water consumption, ii) waste water generation, iii) mode of disposal and iv) BMW authorization application and Bio Medical Waste generation details.

Hence, the BMW & CFO application cannot be processed without proof of project cost and the BMW, CFE & CFO fee required for the current year.

In view of the above, you are hereby directed to submit the balance sheet/CA certified letter as proof of project cost and the required fee for the current year that will be informed after submission of balance sheet for processing the application.

To The Occupier, M/s. Community Health Centre, Kotturu (V&M), Srikakulam District Copy to the Environmental Engineer, RO, Srikakulam for information and directed to pursue the payment of balance fee by the HCF. Further, the RO, Srikakulam is directed to submit consolidated statement to Head Office about the fee to be paid by all the Government HCFs from 2010 onwards for taking further course of action.
