I certify that in applying to register as a third party qualified elevator inspector for the State that I will comply with the following:

1. I will conduct inspections as a Maryland third party qualified elevator inspector to ensure the highest degree of safety possible.

2. I understand that a registered third party qualified elevator inspector is a position of public trust and that I will uphold the integrity, independence, objectivity and fairness necessary to comply with the ASME QEI-1 Standard.

3. I understand that I am prohibited from conducting inspections of a unit that is manufactured, owned, repaired or maintained by my current employer or an entity for whom I have an ownership interest or receives compensation.

4. I understand that I am required to report to the Safety Inspection Unit any information which could be used to question my objectivity and integrity including but not limited to terminations or suspensions from employment or criminal violations.

5. I understand that my failure to comply with the requirements listed in this statement could result in a bar from third party inspections in Maryland for 3 years.

Third Party Periodic Inspection Report Article-Public Safety Title 12, subtitle 8 requires that all elevator units be inspected periodically in conformance with the applicable sections of the Safety Code (ASME A17.1). Documentation of the periodic inspection performed shall be submitted to the Commissioner of Labor and Industry on this form only.

When a periodic annual inspection revels that the elevator unit is in violation of the Safety Code, the owner of the elevator unit must be given a copy of the Violation Notice report form.

By indicating that the elevator unit is in compliance the third party inspector is recommending to the Commissioner that a Certificate of Inspection be issued. Elevator units that are not identified as in compliance will not be issued a certificate.

TPQEI must submit inspection report and associated documentation to the Commissioner within 30 days of the inspection.

TPQEI report indicates no alleged violations; a certificate will be issued by the Commissioner.

Clearing Alleged Violations

1. Owner required to schedule a follow-up inspection by TPQEI within the 30 days of inspection due date.

2. If follow-up inspection by TPQEI is not reported within 30 days, owner will receive notice letter. Follow-up inspection by State inspector after 40 days.

Scheduling 5-Year Inspections With Commissioner

5-year comprehensive inspections are required on elevator units that are suspended by wire ropes, belts, chains or a similar suspension means, or elevator units with rack and pinion drive.

State inspectors will witness the 5-year periodic (full load) test and perform the comprehensive 5-year inspection.

The owner must coordinate with the Commissioner the date and time of the 5-year comprehensive inspection 60 days in advance of inspection date.
