ROOSEVELT PROJECT GRADUATION 2014
Dear Roosevelt Seniors, Parents & Guardians:
The Friends of Roosevelt Project Grad is a group of volunteer students, parents,
guardians and concerned adults in the community who plan and host an annual
Project Graduation celebration for Roosevelt High School seniors.
Project Graduation is a nationally recognized drug and alcohol free, adult supervised
all-night party that has been a great success in many states. The purpose of this
special graduation celebration is to help to prevent teenage deaths caused by
drinking and driving on graduation night. Due to the efforts of a group of volunteers,
Roosevelt High School graduates were the first from a public high school in Hawaii to
enjoy a Project Graduation celebration in 1990. It has become a tradition that all
Roosevelt seniors look forward to each year.
The Class of 2014 will be graduating on Saturday, May 24, 2014, and so will many other
high school students. We need your help to make their special night a safe and joyous
occasion.
This once-in-a-lifetime event begins graduation night:
When: May 24, 2014 9:00 p.m. (Roosevelt Cafeteria – CHECK-IN)

May 25, 2014 7:30 a.m. (Roosevelt Cafeteria – PICK UP)

Fee: $100 for each Graduate (good to March 7, 2014 )
The registration fee of $100 is non-refundable. Note: the actual cost for each student
is more than $250. Thanks to year-round fundraising and donations from the
community, we are able to defray costs. The last day to register for $100 will be Friday,
March 7th. Seniors who register after March 7th will be assessed a late registration
fee of $150 through April 30, 2014. The late registration fee increases to $200 up
through and including the day of graduation. A limited number of sponsorships are
available for students who would otherwise be unable to attend (an application form
is included with this packet). Registration fees must be paid up front and will be
refunded based on any sponsorship (full or partial) approved/granted.

Friends of Roosevelt Project Grad volunteers will be at the Roosevelt High School Senior Patio on the following dates to register seniors for Project Graduation 2014. Please bring all completed forms with your payment or sponsorship application. Seniors: Be sure to register early with your friends to reserve seats together
on the same bus…maximum of 30 students per bus!
~~There will be NO bus-switching once sign-ups are completed!!!~~
~~Late registrations will not be guaranteed shirts or reservations!!!~~ **For the seniors who register AFTER March 7th, the registration fee will be:

PERMISSION/WAIVER FORM

9:00 p.m. to 7:30 a.m. Sunday, May 25, 2014.
I fully understand the potential problems involving alcohol and/or an illegal substance that could arise and
will cooperate fully with all volunteers of the Friends of Roosevelt Project Grad. In the event that my
son/daughter is found to be in possession of or under the influence of alcohol and/or an illegal substance, I
understand that he/she will be removed from the rest of the students and I will be notified and must pick
him/her up immediately.
I understand that students participating in this event cannot leave the premises of the Project Graduation
celebration at (Secret Destination). Students will not be released until the designated ADULT parent or
guardian signs them out on the morning of May 25, 2014. I understand that I am responsible for my child once
he/she is picked up from the RHS cafeteria. This is to protect my son/daughter from any traffic accidents or
fatalities due to fatigue in my child. Telephone use for personal reasons will be prohibited.
I know this celebration is a gift offered to all graduates and that it is being planned by a group of volunteer
students, parents and supportive adults. I understand that participation in this celebration party is fully
voluntary. In consideration of the benefits to be derived, and having full confidence that every precaution
will be taken to ensure the safety of my child, my signature below acknowledges my understanding of this
event and agreement to waive all claims, not to place suit or to hold liable any volunteer person, Friends of
Roosevelt Project Grad, Roosevelt High School Alumni Association and Roosevelt Alumni Foundation,
Roosevelt High School, Hawaii State Department of Education, or any of the vendors, including those
providing transportation, for any incident or injury that may occur to my child while attending the Project

Xtreme Fun Rentals, Inc. rides we suggest that you do not enter the amusement

PATRON RESPONSIBILITY There are inherent risks in the participation in or on any amusement ride, device, or attraction.
Patrons on an amusement ride, device, or attraction, by participation, accept the risk inherent in
such participation of which the ordinary prudent person is or should be aware. Patrons have a
duty to exercise good judgment and act in a responsible manner while using the amusement ride,
device, or attraction and to obey all oral or written warnings, or both, prior to or during
participation, or both.
Patrons have a duty to not participate in or on any amusement ride, device, or attraction when

safety equipment provided.
WAIVER OF LIABILITY I voluntarily release, indemnify, hold harmless and discharge Xtreme Fun Rentals, Inc. (hereinafter
collectively referred to as “X.F.R.”), from any and all liability, claims, demands actions or rights of
actions, whether personal to me or to a third party which are related to, arise out of or are in any
way connected with my use or participation on the X.F.R. attractions including those allegedly
attributable to negligent acts or omissions. I agree to reimburse any reasonable attorney’s fees
and costs which may be incurred by X.F.R. in the defense of any such liability claim, demand,
action or right of action. In the event that I file a cause of action against X.F.R, I agree to do so
solely in the state of Hawaii, and I further agree that the substantive law of Hawaii shall apply in
that action without regard to any conflict of law or rules of Hawaii.

IMPORTANT REMINDERS – PLEASE KEEP & POST AT HOME

SA TU RDAY, Ma y 24 t h :
Check-in at the RHS auditorium prior to your final rehearsal by name and confirm your bus
number.
After the graduation ceremonies, check-in at the cafeteria at 9:00 p.m. Check-in will be by
bus number. Please be prompt when reporting to the cafeteria after graduation.
REQUIRED ATTIRE FOR SENIORS ATTENDING:
LONG PANTS (ankle length; no shorts, capris or pedal pushers)
PROJECT GRAD T-SHIRT (no alterations allowed)
COVERED SHOES.
NO Jewelry, Leis, Purses/Bags, Hats, Sunglasses, Pagers, Cell Phones or other electronic
devices are allowed. If these items and any other unauthorized/contraband items are found
on you, they will be confiscated and held for you until your return on Sunday, May 25th.
If you check-in without the proper attire, your parents will be called to bring you the
necessary items. You will NOT be allowed to participate in any activities until you have the
necessary items.
Graduates may bring with them only a jacket or sweater and required prescription
medication in a re-sealable plastic bag with their full name printed on it.
Graduates will be escorted from RHS to the Secret Destination, leaving at approximately 9:30
p.m. They will make one pass in front of the school on Nehoa Street. Family and friends are
encouraged to line Nehoa Street to cheer their graduate.
REMINDER: Any attendee, who is disorderly or disruptive or is found to have
unauthorized/contraband items, including drugs or alcohol, may be sent home at the
discretion of the chaperones.
S UNDAY, M ay 2 5 t h :
Parents/Guardians may PICK UP their graduates Sunday morning, May 25th, at the RHS
cafeteria at 7:30 am. PLEASE BE PROMPT, both the graduates and their chaperones will be
tired!
Parents/Guardians or other authorized person(s) are REQUIRED TO BRING A PICTURE ID to sign
out their graduate at the end of the Project Graduation celebration.
Graduates will only be released to the authorized Parent/Guardian orADULT designated on
the authorization form. NO EXCEPTIONS! Under NO circumstances will the graduate be
allowed to drive or walk home themselves.
Graduates will not be allowed to borrow cell phones to call someone to pick them up.
Parents/Guardians are expected to know when the graduates are to be picked up.

Tell-a-Grad is a special message to graduates from parents, family members and friends to be read the morning after attending the Project Graduation celebration. The messages will be in personalized goodie bags each attendee receives. Imagine how wonderful your grad will feel to receive a special note from a loved one!
