The Kentucky Office of Rural Health is pleased to announce the limited availability of “Registration Fee

Waivers” for the upcoming 2014 Patient Centered Care Conference hosted by Kentucky Primary Care

Association.

Interested applicants may complete and submit a Registration Fee Waiver application electronically at

https://ruralhealth.med.uky.edu/registration or by fax at 606-439-0795. All applications must be submitted

prior to May 21st, 2014.

The KORH plans to support up to 10 participants through this competitive process. Applicants will be scored

with those serving rural populations, given priority. Please limit applications to no more than two per

organization. Please note: this waiver does not include transportation, lodging or any other expenses. All

applicants will be notified by May 28th, 2014. This waiver was made possible by the Kentucky Office of

Rural Health through HRSA ORHP SORH funding.

This year’s Patient Centered Care Conference will provide attendees with essential tools and best practices

to:

1) Enhance revenue and improve quality using a patient-centered approach,

2) Support practice transformation and recognition as a Patient Centered Medical Home (PCMH), and

3) Develop the infrastructure needed to sustain the model. This conference has been designed to foster peer

learning with sessions tailored to practices along the spectrum of PCMH recognition and transformation.

The conference will take place June 11th-12th at Hilton Suites in Lexington, KY. For more information

The UK Center of Excellence in Rural Health was established in 1990 to address health disparities in rural

Kentucky, including a chronic shortage of health professionals and residents’ poor health status. The

Center accomplishes this through health professionals education, health policy research, health care

service and community engagement. The Center serves as the federally designated Kentucky Office of

Rural Health (KORH). This program provides a framework for linking small rural communities with local,

state and federal resources while working toward long-term solutions to rural health issues. Kentucky

Home place was created by the Center to address health issues related to lifestyle choices, environmental

factors, inadequate health insurance and general lack of understanding of the healthcare system. Their

mission is to provide access to medical, social and environmental services for the citizens of the

Commonwealth. The center houses the UK North Fork Valley Community Health Center, the host clinic for

the East Kentucky Family Medicine Residency Program.

The Kentucky Office of Rural Health (KORH), established in 1991, is a Federal/State partnership

authorized by Federal Legislation. The Center of Excellence in Rural Health serves as the federally

designated Kentucky Office of Rural Health. The mission of the KORH is to support the health and

well-being of Kentuckians by promoting access to rural health services. The program provides a

framework for linking small rural communities with local state and federal resources while working toward

long-term solutions to rural health issues. The KORH assists clinicians, administrators and consumers find

ways to improve communications, finances and access to quality health care while insuring that funding

agencies and policy makers are made aware of the needs of rural communities.

Elsie Crawford, RN, BSN, MHA, VP of Operations at Wilken’s Group, Jellico, TN

Crawford attended Cumberland College and Carson Newman College for Business Administration and Roane State Community

College for Nursing. She was previously the Office Manager for Lee G. Durham, Jr., M.D. and has been the VP of Operations and

Director of Nursing for Wilken's Medical Group in Jellico, TN since 1974. Elsie has served as Board member of the Tennessee

Technology School (Nursing Division) since 2004, NARHC since 2006, Jellico SDA Church School Board Member (Treasurer) since

2005 and Jellico City Council Member since 2006.

Bill Finerfrock, Executive Director NARHC, Washington, DC

Bill Finerfrock is the co-founder and Executive Director of the National Association of Rural Health Clinics and is a recognized

expert on rural health issues. He also serves as an outside policy consultant for the National Organization of State Offices of Rural

Health. In addition to his work with health care providers and practitioners, he has worked with some of the country's largest

HMO’s as well as assisted pharmaceutical companies with their legislative and regulatory programs. He has worked in and with

the U.S. Congress and Federal agencies for more than 30 yrs. He began his public policy career in Washington, DC in 1976 as a

Lyndon Baines Johnson Congressional Intern for Congressman Bob Edgar (D-PA). Between 1977-1984 he worked for Senator

Edward W. Brooke (R-MA) and Senator Roger Jepsen (R-IA). In 1999 he wrote a book for the Federal Office of Rural Health

Policy on managed care entitled: Understanding and Working with Managed Care, A Guide for Rural Providers. More recently Mr.

Finerfrock co-wrote and edited a publication entitled: “Starting a Rural Health Clinic, A How to Manual”. Mr. Finerfrock is a 1977

graduate of Penn State University. From 1985-1993, he was Director of Federal Affairs for the American Academy of Physician

Assistants.

Robin Rowe, BA , Cabinet for Health and Family Services Office of Inspector General Division of Health Care, Frankfort, KY

Robin Rowe has 14 years of experience with the Cabinet for Health and Family Services Office of Inspector General Division of

Health Care. She holds a Bachelor of Arts Degree from Eastern Kentucky University.

Chris Workman, MASW, Kentucky Department of Public Health/Health Care Access Branch/PCO , Frankfort, KY

For the last 7 years Mr. Workman has been the Branch Manager of the Health Care Access Branch and Director of the Primary

Care Office (PCO) located in the Kentucky Department for Public Health. Mr. Workman has his Master's degree from the

University of Chicago and he has over 25 years working in both the public and private sector.

Steve Salt, MBA , Kentucky Department of Public Health/Health Care Access Branch/PCO , Frankfort, KY

Mr. Salt has been the Shortage Designation program staff for over 5 years. Prior to this administering the shortage designation

program, Mr. Salt spent 10+ years in the Department for Public Health working in Immunizations and local health department

operations. Prior to joining state government, Mr. Salt worked in hospital planning and medical practice management, including

managing a multi-physician hematology/oncology and neurology practices. Mr. Salt has a Master's degree from the University of

Maryland.

Megan Housley, MBA , Business Development Manager Marketing Director, Kentucky Regional Extension Center

Megan Housley is the Business Development Manager and Marketing Director at the Kentucky Regional Extension Center

overseeing and administering all outreach, communication and education projects. Megan works to develop and optimize the

business strategies of the REC to ensure sustainability after completion of the federal grant period. Her job is to ensure that the

Kentucky REC can continue to support healthcare professionals and provide affordable solutions for all Healthcare Transformation

endeavors. Megan serves on the advisory board of the Office of the National Coordinator for Health IT’s Medical Home CoP and is

working towards her NCQA Patient-Centered Medical Home Content Expert Certification. Megan holds a Bachelors of Business

Administration with a concentration in Marketing and Management from the University of Kentucky and a Masters of Business

Administration from Midway College. Megan also serves as an adjunct professor for the business department of Midway College.

Jennifer Wilder, MHA, MU Advisor, Director Coordinator CHFS/Governor’s Office of Electronic Health Information

Jennifer Wilder serves as the Meaningful Use Advisor and Direct Secure Messaging Coordinator for the Kentucky Health

Information Exchange (KHIE) in the Governor's Office for Electronic Health Information (GOEHI) in the Cabinet for Health and

Family Services. Prior to serving in her current role, Jennifer was employed by the Kentucky Regional Extension Center (KY

REC) housed at the University of Kentucky, as an Administrative Fellow specializing in Meaningful Use and Electronic Health

Record (EHR) implementation and training. Jennifer earned her Master's in Healthcare Administration from the University of

Kentucky, College of Public Health, in Lexington, KY. Jennifer is committed to the integration of health information technology

(HIT) for the improvement of public health outcomes, the reduction of health disparities, the reduction of healthcare costs and the

expansion of access to affordable, comprehensive care.

Mary Gaetz, Staff Assistant, Patient Engagement Coordinator CHFS/Governor’s Office of Electronic Health Information

Mary Gaetz currently serves as patient engagement coordinator for the Governor’s Office of Electronic Health Information within

the Cabinet for Health and Family Services were she has been employed for 27 years. She serves on the National e-Health

Collaborative Consumer Engagement Advisory Committee. She is chapter President of the Bluegrass Chapter Society of

Government Meeting Professionals. She is a Health Information Management Services Society (HIMSS) member. She served on

the board of the Lexington Family Counseling Services. Mary is a 1985 graduate of the University of Kentucky.

Sharon Shover, CPC, Manager, Blue & Company, Louisville, KY

Ms. Shover is a Manager with Blue & Co., LLC. She has proven experience in all aspects of revenue cycle management including

knowledge and implementation of best practices in revenue cycle management and associated information systems. Her experience

includes revenue generation, billing/coding and third party reimbursement for hospitals, FQHCs, RHCs and physician office settings.

Ms. Shover frequently performs interim management assignments for hospitals and physician practices. Ms. Shover also supports

the company by providing regulatory risk assessments, performs various instructional and audit/review functions regarding the

medical records for physician practices, including education of physicians and other healthcare professionals, with regard to CPT-4

and ICD -9 coding guidelines and an approved trainer for ICD-10. Ms. Shover's areas of expertise include practice management,

clinical documentation improvement, chart auditing, coding, the accounts receivable cycle from patient access to collections, analyze

commercial insurance contracts, and corporate compliance implementation processes, and policy and procedures. Ms. Shover is a

member of the American Academy of Professional Coders, a member of Healthcare Financial Management Association and

Kentucky Medical Group Management Association, American Health Information Management Association.

Fran Feltner, DNP, MSN, RN, Director University of Kentucky Center of Excellence in Rural Health, Hazard, KY

Fran Feltner received her Doctorate in Nursing Practice from the University of Kentucky, College of Nursing and a Masters in Ad-

vanced Practice Rural Public Health Nursing – Administration from Eastern Kentucky University. She serves as College of Medicine,

Assistant Professor, Family and Community Medicine and Adjunct Faculty, UK College of Nursing and UK College of Health

Science. Currently, Dr. Feltner is the Director of the University of Kentucky, College of Medicine, Center of Excellence in Rural

Health, located in Hazard, Kentucky. Fran has worked for more than 30 years in rural health care, from serving as a nurse in clinical

and hospital settings to her current role as director. One of her most important roles has been developing the community health

worker's scope of practice and training curriculum for Kentucky Homeplace. Since the program's inception in 1994, CHWs have

provided over 4 million services and accessed over $256 million in services and medications for the clients in their communities.

Kentucky Homeplace has been recognized as a model that works and has been duplicated throughout the state and nationally.

Andrew Bledsoe, MBA, PCMH CCE , Executive Director for the Northeast Kentucky Regional Health Information Organization

Andrew Bledsoe currently services as Executive Director for the Northeast Kentucky Regional Health Information Organization

(NeKY RHIO). Mr. Bledsoe has worked in the NeKY RHIO for over three years. He began working as a Practice and Hospital EHR

Consultant working to assist providers (both primary care and specialty) and critical access hospitals in attaining meaningful use.

Mr. Bledsoe then became the Network Director for the Rural Health Information Technology Network Development program and

has since been named Executive Director.

Andrew holds both a bachelor's and master's degree in Business Administration from Morehead State University. Mr. Bledsoe holds

a certificate in Patient Centered Medical Home Content Expert and has completed specialized training in; privacy and security,

project management, ONC Meaningful Use, and federal grant administration. Furthermore, he has completed the Workflow

Redesign Specialist training from the ONC. He serves on the Board of Directors for the Kentucky Rural Health Association (KRHA)

as well as the Chair of the Public Information Committee for KRHA.

Angie Ross, RN, Care Coordinator at Marcum & Wallace Memorial Hospital

Angie received an Associate Degree in Nursing from Hazard Community and Technical College and is currently em-ployed with Catholic Health Partners and dividing her time between three Mercy Medical Physician Practices; including Mercy Primary Care – Irvine, Kentucky, Mercy Medical Clinic of Powell County – Clay City, Kentucky and Mercy Medical Clinic of Lee County, Beattyville, Kentucky. Her professional interests focus on patient education, improving patient out-comes for those patients with multiple chronic disease processes and serving as a resource within family practices to support and promote the Patient Centered Medical Home model and standards. She was recently honored with the Spirit Award through Marcum and Wallace Memorial Hospital for her commitment to the hospital mission and core values.
