A Registered and Licensed Dietitian, or designee, working for the SDPI Diabetes Prevention Program will let a physician know the participant is here for a fasting glucose, Hemoglobin A1C, fasting lipid panel, and a A/C ratio. d. No vitals are required for this visit. e. For all follow-up appointments, a Registered and Licensed Dietitian, or designee, working for the SDPI Diabetes Prevention Program will submit orders signed by a physician for a fasting glucose, Hemoglobin A1C, fasting lipid panel, and a A/C ratio f. A Registered and Licensed Dietitian, or designee, working for the SDPI Diabetes Prevention Program will grab a urine cup from the medical storage room and place the patient’s sticker on it. The urine cup will be given to the patient who will be instructed to place full urine cup into the small door that goes to lab in the patient bathroom. 1. Purpose: To screen for the Special Diabetes Program for Indians (SDPI) Diabetes Prevention Program to determine eligibility using a fasting glucose, a 2 hr OGTT, and a fasting lipid panel.

2. Equipment Needed:a. Supplies for a fasting glucose, a 2 hr OGTT, a fasting lipid panel, a

Hemoglobin A1C & a fasting lipid panel.

3. Procedure:a. Potential participants will check in at the front desk where a charge ticket

and stickers will be generated.b. A Registered and Licensed Dietitian, or designee, working for the SDPI

Diabetes Prevention Program will be paged after the potential participant checks in.

c. A Registered and Licensed Dietitian, or designee, working for the SDPI Diabetes Prevention Program will let a physician know the potential participant is here for fasting glucose, a 2 hr OGTT, a fasting lipid panel, a Hemoglobin A1C & a fasting lipid panel to determine eligibility for the SDPI Diabetes Prevention Program.
