Congratulations!

The presentation of the AFPM Safety Award plaques is part of a comprehensive safety awards program which the Association’s Safety & Health Committee has developed to promote accident prevention in the petroleum refining, petrochemical manufacturing, and contracting industries and to publicly recognize the excellent record of safety in operations which the industries and contractors have achieved.

AFPM Safety Awards are based on records kept for employees in accordance with OSHA record keeping requirements as defined by law and entered on the OSHA 300A summary form.

The Safety Awards Program honors AFPM Regular member companies operating U.S. refineries and petrochemical manufacturing plants as well as Associate member contractors working in those facilities. The program consists of the following awards.

Distinguished Safety Award

AFPM’s most prestigious award, the Distinguished Safety Award or DSA, as it is known, recognizes those member company refineries and petrochemical plants that have attained a superior level of safety performance in the domestic refining and petrochemical manufacturing industries. Recipients are chosen by a selection committee represented by members of the AFPM Safety & Health Committee. It is the selection committee’s responsibility to carefully examine the safety performance records of individual plant locations using the specific screening and selection criteria detailed below.

Screening CriteriaIn order to qualify for the DSA, a facility must first pass the screening criteria as described below.1. A location must achieve the following

requirements for the calendar year:a. an employee total recordable incidence

rate of 0.6 or less, b. no employee lost workday cases

involving days away from work, restriction, or transfer (DART)

2. A location must achieve the following requirements for the calendar year:

a. A contractor total recordable incidence rate of 0.6 or less,

b. no contractor lost workday cases involving days away from work, restriction, or transfer (DART)

3. A location must have had no employee workplace related fatalities and no non-employee (contractor, visitor, etc.) workplace related fatalities for five consecutive years.

4. A facility will be excluded from applying if it has any of the following during the calendar year:

a. a Process Safety Event (PSE) that resulted in a days away from work case or a restricted/transferred case of an employee, contractor, or subcontractor.

b. a PSE that resulted in a fatality of a third-party individual,

c. a PSE that resulted in an officially declared community evacuation

d. a PSE that resulted in an officially declared community shelter-in-place

e. any workplace fatality of an employee or non-employee (contractor, visitor, etc.).

Selection CriteriaAfter passing the screening criteria, the candidate facility’s overall safety performance record will be evaluated by the selection committee based on the applicant’s written application.

Meritorious Safety Performance Award

Recipients must achieve an employee total recordable incident rate of 0.8 or less at their site for calendar year 2011.

A facility will be excluded from receiving this award if it has any of the following:a. a Process Safety Event (PSE) that

resulted in a days away from work case or a restricted/transferred case of an employee, contractor, or subcontractor.

b. a PSE that resulted in a fatality of a third-party individual,

c. a PSE that resulted in an officially declared community evacuation

d. a PSE that resulted in an officially declared community shelter-in-place

e. any workplace fatality of an employee or non-employee (contractor, visitor, etc.)

Award for Safety Achievement: 1,000,000+ Employee HoursRecipients must have worked 1,000,000 or more employee hours without a lost employee workday involving days away from work.

A facility will be excluded from receiving this award if it has any of the following during the evaluation period:a. a Process Safety Event (PSE) that

resulted in a days away from work case or a restricted/transferred case of an employee, contractor, or subcontractor.

b. a PSE that resulted in a fatality of a third-party individual,

c. a PSE that resulted in an officially declared community evacuation

d. a PSE that resulted in an officially declared community shelter-in-place

e. any workplace fatality of an employee or non-employee (contractor, visitor, etc.)

The evaluation period for this award does not have to encompass an exact calendar year or an even million hour increment. For example, an award may be presented for 1,235,792 hours from February 10,

2011 to December 5, 2011. Please note that the intention of this award is to recognize achievements in 1,000,000 hour increments and therefore, awards are only presented to those facilities that have reached the next million hour threshold. In other words, a facility that received an award at last year’s banquet for 1,200,000 hours worked and then worked an additional 500,000 hours without a lost day case involving days away from work, would not be eligible for an award this year since a cumulative total of only 1,700,000 hours was reached. A cumulative total of at least 2,000,000 would be required to receive the award again.

Award for Safety Achievement: 1+ YearsRecipients must have worked one or more years (365 consecutive days) without a lost workday case involving days away from work.

A facility will be excluded from receiving this award if it has any of the following during the evaluation period:a. a Process Safety Event (PSE) that resulted

in a days away from work case or a restricted/transferred case of an employee, contractor, or subcontractor.

b. a PSE that resulted in a fatality of a third-party individual,

c. a PSE that resulted in an officially declared community evacuation

d. a PSE that resulted in an officially declared community shelter-in-place

e. any workplace fatality of an employee or non-employee (contractor, visitor, etc.)

The evaluation period for this award does not have to encompass an exact calendar year. For example, an award may be presented for 1 year without a lost workday case from October 10, 2010 to October 9, 2011, etc. Please note that awards are given for 365 day intervals only. In other words, an award for one year and 5 months would not be granted.

Contractor Award For Meritorious Safety Performance

This award recognizes maintenance contractors working a minimum of 20,000 hours per calendar year at a regular member facility which has achieved a total recordable incidence rate of 0.8 or less with no workplace related fatality during 2011.

AFPM HONORS INDUSTRY ACHIEVEMENTS

AT SAFETY AWARDS EVENT

WASHINGTON – The American Fuel & Petrochemical Manufacturers yesterday recognized facility operators in the petroleum refining and petrochemical manufacturing community for outstanding occupational and process safety achievements in safe operations in 2011. AFPM Chairman James Mahoney, executive vice president of operations excellence and compliance for Koch Industries, presented the awards during the Safety Awards Event, a program associated with AFPM’s annual National Occupational and Process Safety Conference, held May 16-17. The awards are part of a comprehensive program developed by AFPM’s Safety and Health Committee to promote accident prevention in the petroleum refining and petrochemical manufacturing communities. The awards publicly recognize the excellent operational safety that the businesses have achieved. The recipients of this year’s Distinguished Safety Award, AFPM’s highest honor, are ConocoPhillips’ Santa Maria Facility, Marathon Petroleum’s Michigan Refining Division, and Valero Energy’s Houston Refinery. “We are extremely proud to honor the facilities for their commitment to safety and for continually striving to set higher standards. AFPM members have no greater priority than ensuring the safety and security of their employees, facilities and the communities surrounding their operations. Those being honored with this award exemplify the industry’s overall commitment to safety and their employees, and we’re thrilled to recognize them,” said AFPM President Charles T. Drevna. This year 42 regular member companies represented by 64 refineries and 66 petrochemical plants were honored with a total of 242 awards. Forty Associate member companies were recognized for their excellent safety performance with 415 awards for their efforts in 143 facilities.
