Sample Bloodborne Pathogens Exposure Control Plan
OSHA requires all employers that can “reasonably anticipate exposure” of employees to infectious materials to prepare and implement a written Exposure Control Plan. The following sample Bloodborne Pathogens Exposure Control Plan was adapted from Appendix D of OSHA’s Bloodborne Pathogens Directive, which was published in 2001. The purpose of the model plan is to provide small employers with an easy-to-use format for developing a written exposure control plan. Each employer must modify the plan to meet the unique needs and circumstances of its workplace.
This sample plan includes provisions for: • job classifications and exposure determinations • program administration • compliance methods • engineering and work practice controls • personal protective equipment • cleaning of contaminated areas • employee hygiene and housekeeping • hepatitis B vaccination and post-exposure follow-up • a sample form for an employee to decline a hepatitis B vaccine • communication of hazards to employees • training • recordkeeping • sharps injury log This sample plan does not include provisions for HIV/HBV laboratories and research facilities. Those facilities must comply with special provisions found at 29 CFR 1910.1030(e).
Employers Covered All employers whose employees can “reasonably anticipate” contact with blood and other infectious materials while performing their job duties are covered. OSHA has not issued an exhaustive list of specific occupations in which exposures are likely to occur. However, in OSHA’s recent directive, it did identify several types of workers who are likely to have the potential for occupational exposure, including the following: • doctors, nurses, physicians’ assistants, and other employees in clinics, physicians’ offices,
hospitals, and other health care settings

• dentists, dental technicians, and dental hygienists • employees of clinics, tissue banks, blood banks, and diagnostic labs • housekeepers in health care and other facilities • personnel in hospital laundries or commercial laundries that service laundry from health
care or public safety personnel • employees designated to provide emergency first aid • home health care, nursing home, and hospice employees • staff of institutions for the developmentally disabled • funeral home and mortuary employees • employees at HBV and HIV research facilities • employees of substance abuse clinics • custodial workers who clean up contaminated sharps or spills of blood or other bodily
fluids • employees who handle regulated waste • paramedics and other medical emergency personnel • police officers, firefighters, and personnel of correctional institutions • maintenance workers in health care facilities • medical equipment service and repair personnel The above types of employees are not automatically covered by the Bloodborne Patho-gens Standard unless they have the potential for occupational exposure. In addition, employees other than those listed above are covered by the standard if they have the potential for occupational exposure.
Legal Source This sample plan for bloodborne pathogens exposure control follows the OSHA require-ment located at 29 CFR 1910.1030. The plan was adapted from Appendix A to OSHA’s Bloodborne Pathogens Directive, which was published in 2001, with additions based on various OSHA letters of interpretation and guidance documents that have been issues since that time. In 2001, OSHA revised its own bloodborne pathogens regulations to implement the Needlestick Safety and Prevention Act. These revised regulations require affected employers to do all of the following: • consider safer medical devices such as sharps with engineered protections and needle-
less systems for use as “engineering controls” • solicit and document input regarding engineering and work practice controls from
nonmanagerial employees who: - are responsible for direct patient care, and - may be injured from contaminated sharps.

• review the annual plan to update and document technological changes that eliminate or reduce bloodborne pathogens exposure
• keep a sharps injury log for percutaneous injuries from exposure incidents with con-taminated sharps, including:
- the type and brand of device, - the department and area involved, and - an explanation of how the incident occurred. However, employers that are exempt from OSHA’s general recordkeeping requirements also are excluded from keeping a sharps injury log. This includes small employers (those with 10 or fewer employees), as well as employers in designated SIC codes (such as drug stores and doctor’s offices and clinics).

This ECP includes: • determination of employee exposure • implementation of various methods of exposure control, including:

EMPLOYEE EXPOSURE DETERMINATION
All employees who have occupational exposure to bloodborne pathogens are covered by this ECP, including part-time, temporary, contract, and per diem employees. For some job titles, all employees who perform that job are at risk of occupational exposure. For others, only some employees who perform that job are at risk. All job titles and, where necessary, job tasks and procedures that involve a risk of exposure are identified in this section. METHODS OF IMPLEMENTATION AND CONTROL
Universal Precautions All employees will utilize universal precautions. “Universal precautions” require that blood and OPIM be treated as if it is known for sure that the blood, etc., is infected with HIV, HBV, or other bloodborne pathogens. Since it is not possible to know in advance if someone’s blood is contaminated with a pathogen, employees will be instructed to treat the blood and OPIM of all individuals as potentially infectious. Employees that treat the blood or OPIM of a particular group (e.g., children or the elderly) as not potentially infectious will be considered to be in violation of this facility’s policies and ECP. Under a 2001 directive, OSHA allows employers to use alternative measures that are even stricter than universal precautions. These include “body substance isolation” and “standard precautions,” in which all body fluids and substances are regarded as infectious. Where these practices are used within this facility, they are deemed to be in compliance with this facility’s policies and ECP.

The specific engineering controls and work practice controls used are listed below: [For example: non-glass capillary tubes, sharps with engineered sharps injury protection (SESIPs), needleless systems] One vital control measure is the careful monitoring and timely replacement of sharps disposal containers to ensure that they are not overfilled. blood or OPIM pose a hazard to the eye, nose, or mouth.

OPIM, in such a way as to avoid contact with the outer surface.

Contaminated sharps are discarded immediately or as soon as possible in containers that are closable, puncture-resistant, leak-proof on sides and bottoms, and labeled or color-coded appropriately. Sharps disposal containers are available at . location

HEPATITIS B VACCINATION will provide training name of responsible person or department
to employees on hepatitis B vaccinations, addressing safety, benefits, efficacy, methods of administration, and availability. The hepatitis B vaccination series is available (at no cost to the employee) after training and within 10 days of initial assignment to employees identified in the “Employee Exposure Determination” section of this plan. Vaccination is encouraged unless: • documentation exists that the employee has previously received the series; • antibody testing reveals that the employee is immune; or • medical evaluation shows that vaccination is contraindicated. If an employee chooses to decline vaccination, the employee must sign a declination form. Employees who decline may request and obtain the vaccination at a later date at no cost. A sample declination form is included in this plan. Documentation of refusal of the vaccination is kept at

Following initial first aid (cleaning the wound, flushing eyes or other mucous membrane, etc.), the responsible person listed above must ensure that the following activities are completed: • Document the routes of exposure and how the exposure occurred. • Identify and document the source individual (unless the employer can establish that
identification is infeasible or prohibited by state or local law). • Obtain consent and make arrangements to have the source individual tested as soon as
possible to determine HIV, HCV, and HBV infectivity; document that the source individual’s test results were conveyed to the employee’s health care provider.
• If the source individual is already known to be HIV-, HCV-, or HBV-positive, new
testing need not be performed. • Assure that the exposed employee is provided with the source individual’s test results
and with information about applicable disclosure laws and regulations concerning the identity and infectious status of the source individual (e.g., laws protecting confiden-tiality).
• After obtaining consent, collect the exposed employee’s blood as soon as feasible after
the exposure incident and test his or her blood for HBV and HIV serological status. • If the employee does not give consent for HIV serological testing during collection of
blood for baseline testing, preserve the baseline blood sample for at least 90 days; if the exposed employee elects to have the baseline sample tested during this waiting period, perform testing as soon as feasible.

• information on the appropriate actions to take and persons to contact in an emergency
involving blood or OPIM • an explanation of the procedure to follow if an exposure incident occurs, including the
method of reporting the incident and the medical follow-up that will be made available • information on the post-exposure evaluation and follow-up that the employer is
required to provide for the employee following an exposure incident • an explanation of the signs and labels and/or color-coding required by the Standard and

Sharps Injury Log All percutaneous injuries from contaminated sharps will be recorded in the OSHA 300 and 301 Logs, as well as the Sharps Injury Log. The record will include at least: • the date of the injury; • the type and brand of the device involved; • the department or work area where the incident occurred; and • an explanation of how the incident occurred. Needlesticks and sharps injuries involving blood or OPIM will be treated as “privacy cases” for purposes of the 300 and 301 Logs. Therefore, no personal identifiers will be used on the 300 and 301 Logs. The employee will instead be identified on a separate, confidential list maintained for the recording of privacy cases. The confidentiality of the people whose names are on the Sharps Injury Log will also be protected. If the Sharps Injury Log is made available to other parties (e.g., supervisors, safety committees), any information that directly identifies the employee or that could reasonably be used to identify the employee will be withheld. The Sharps Injury Log will be reviewed at least annually as part of the annual evaluation of the bloodborne pathogens exposure prevention program and will be maintained for at least five years following the end of the calendar year covered.
