This Safety Manual is for the management, staff and workers of Japuz-JansolEnterprises, this will serve as a guideline for accident prevention in the
construction site.

II. SAFETY AND HEALTH COMMITTEE
1. OBJECTIVE
The pro"ect safety committee shall !e formed to comply with re4uirementsfrom the 2epartment of +a!or and Employment as detailed in the ccupational
Safety and )ealth Standards, and to assist in the accomplishment of safety
o!"ectives.
2. FUNCTION AND COMPOSITION OF THE COMMITTEE
1.0 / pro"ect safety committee as re4uired !y the company safety program
will !e organized and a regular monthly meeting will !e held at the pro"ect site.

1. PROJECT IN CHARGE
Ensure the creation of a $ro"ect Safety $lan that complies with the
re4uirements of the company safety policy. Ensure that work is e6ecuted according to the Safety $lan.
Ensure that the implementation of the $ro"ect Safety $lan is monitored and
that the plan is reviewed as and when necessary.

V. ONSITE SAFETY!HEALTH PROMOTION AND EDUCATION
1. OBJECTIVE
Safety knowledge, attitude, skills and ha!its at all levels of the pro"ect
workforce shall !e promoted, maintained and enhanced !y regular and continuing
safety training and education.
2. RE"UIREMENTS
1.0 Safety rientation
#ew workers will !e re4uired to attend a Safety rientation course !efore
deployment for work. *pon completion of the orientation, a new worker will !e
issued with a site 2 card or gate pass and a safety sticker for his hard hat. )e will
then !e re4uired to sign a safety orientation form.
nduction seminar of workers will also include the company safety and healthpolicy, pro"ect clinic procedures, and the company code of employee discipline.
The seminar will !e participated !y the pro"ect nurse or first-aider and the pro"ect
administrative officer of his representative.
1.1 Tool o6 Talks
Tool!o6 meetings provide a forum to discuss items of safety relevant to the
pro"ect and permit an opportunity for open communication !etween employees
and supervisors on the pro"ect.
Safety talks will !e scheduled on a weekly !asis. Such talks will !e carried out
!y the supervisors >leadmen, capatazes, foremen? assisted !y the safety officer.
The talks will !e of short duration on a relevant safety topic and records of
attendance will !e maintained. Each talk will likely last for at least 07 minutes
every Monday morning or as determined !y the pro"ect.
1.5 Safety seminars for supervisors and engineers
The $& or the pro"ect safety officer will arrange safety course for supervisors
and engineers. The seminar shall reinforce the responsi!ility of supervisors and
engineers for their workers@ safety and welfare.

VI.i SAFETY PROMOTIONS
1. OBJECTIVE
$ertinent information shall !e communicated to all employees at the work site
!y practical means, to make employees well informed a!out matters important to
their safety and well !eing.
2. RE"UIREMENTS
1.0 Safety shall !e included in all "o! instructions.
1.1 Safety ulletin oards
The pro"ect shall have an ade4uate num!er of !ulletin !oards

while standing in a casual manner.
They shall !e well lighted for good viewing during all shifts.

The safety officer shall provide posters and other safety information
materials for the !ulletin !oards.
VI.ACCIDENT!INCIDENT INVESTIGATION AND REPORTING
1. OBJECTIVE
n compliance to ule 0(7' of the S)S of 2+E, records of accidents and
illness shall !e kept and maintained open at all times for inspection !y authorized
personnel.
/ll accidents and downgrading incidents shall !e thoroughly investigated to
find the root causes. Measures to prevent recurrence of the accidents;incidents tend
to !e more effective if targeted to the root causesA otherwise, treating thesymptoms would "ust increase the waste of resources.

/n immediate investigation will !e completed !y crew supervision,
foremen;leadmen. The investigation process shall !e supervised !y the pro"ect
incharge and pro"ect safety officer.
2. RE"UIREMENTS
1.0 The ncident;/ccident nvestigation eport 8orm should !e used
we can use the forms from the S)S manual.
1.1 /ll accidents;incidents shall !e reported on the standard form !y
the supervisor.
1.5 )igh potential accidents and near-accidents shall !e reported to the
attention of upper levels of management to assure full
investigation and action !y appropriate authorities.
1.' The safety officer shall ensure that all corrective actions listed for
ma"or high-potential loss accident are implemented.
3. MAJOR LOSS ANNOUNCEMENTS
Summaries of vital information on ma"or personnel accidents must !e
written and promptly su!mitted to the $&.
$rompt summaries of ma"or property loss or damage and fires must !e
written and circulated to the $&.
These reports must contain key information on causes and prevention.
4. CONTENTS
The pro"ect incharge will report in writing to the owner. /narrative of accident
details and what was done or is !eing done to prevent recurrence. This written
report may !e preceded !y a telephone call for possi!le representation of the
owner. The narrative report will include the following informationA
n"ured employee>s? name>s?, occupation, length of employment, age,
se6, date, and time of accident.
2escription of accidentA detail of what the employee was doing and
what tools, e4uipment, structures, or fi6tures were involved.
#ature of in"uries or illness
/ccident causes that contri!uted to the incident.
Steps to !e taken to see that this is done
$rovide a detailed sketch, if needed to e6plain the accident scene.
$hotograph shall accompany the written report for serious accident which
results to lost time in"ury.
/fter the conduct of investigation, the employer thru the pro"ect safety officer
shall su!mit a report to 2+E egional office on or !efore 1( thof the month

#. RECORDKEEPING PROCEDURES
The 8irst /id personnel with the assistance of the pro"ect safety officer must
maintain a variety of records for protection of the employees, the company and
client.
0. 8irst /id +og
This is a chronological listing of all visits to the clinic or first aid
station. Every in"ury illness reported, no matter how slight must !e
recorded. / copy of the log!ook and necessary forms shall !e provided
to the clinic or first aid station.
1. ndividual Medical ecord
This is a confidential record of the employees illness and in"uries
reported to the clinic or first aid station. The 8irst /ider is responsi!le
for keeping the medical record of every employee in the site. /t the
end of the month, he;she shall su!mit to the $ro"ect ncharge and
pro"ect safety officer a summary report of all cases treated in the clinic.
VII. PROTECTION OF THE GENERAL PUBLIC
Safety measures shall !e implemented to protect the pu!lic from hazards
connected with construction operations and activities.
1. RE"UIREMENTS
Standard warning signs, traffic signs and !arricades shall !e installed as
re4uired !y government regulations and !y the S)S for !uilding
pro"ects.
8lagmen directing traffic shall follow standard signals and procedures.
peration of heavy e4uipment and hoists shall use standard signals. 3hen
the distance !etween the operator and the signalman e6ceeds 1(( feet, a
porta!le radio communication system shall !e used.
nly trained and 4ualified personnel shall !e assigned as vehicle drivers,
e4uipment operators, signalmen and flagmen.
Safety nets, inverted um!rella, and covered walkway shall !e installed as
re4uired to protect the pu!lic from falling de!ris.

Signs, !arricades and other safeguards installed shall !e protected from
misuse, a!use, damage or interference.
2. RESPONSIBILITIES
The $ro"ect-in-charge shall anticipate the pro"ect needs and order the
preparation the re4uired devices.
The pro"ect supervisors shall install re4uired traffic signs and !arricades,
and safety nets as re4uired !y the pro"ect. They shall also protect these
devices and pu!lic safeguards from misuse, a!use, damage or interference.
Supervisors shall instruct and monitor traffic flagmen.
Supervisors and the E4uipment-in-&harge shall instruct and monitor their
operators for heavy and critical e4uipment.
The $ro"ect-in-charge shall strictly enforce the assignment of 4ualified
personnel to do certain "o!s as stated a!ove.
The pro"ect safety officer shall conduct daily inspection of the installed
safeguards.
I$. CONSTRUCTION SAFETY PRACTICES
1. E$CAVATION
E6cavation is defined as making a depression or cavity in the earth@s surface
including the removal of previously installed roadways, sla!s or foundations at
and !elow the surrounding ground level. The principal hazards associated with
e6cavation are%
Suffocation, crushing or other in"ury from falling material.
2amage;failure of installed underground services and conse4uent hazards.
Tripping, slipping or falling.
$ossi!ility of e6plosive, flamma!le, to6ic or o6ygen-deficient atmosphere
in e6cavation.
$rior to starting e6cavation, proceed as follows%
2etermine the presence and location of any underground oil, chemical or
gas.

/reas to !e e6cavated shall !e identified and segregated !y means of
!arricades, ropes and;or signs to prevent access of unauthorized personnel
and e4uipment. Suita!le means shall !e provided to make !arriers visi!le
at all times.
$rovide means of diverting surface water from e6cavation.
Sloping, shoring or !racing that may !e re4uired to sta!ilize the soil shall
!e designed !y a competent engineer and installed as designed. Special
attention shall !e paid to !racing designs when the e6cavation is near
operating e4uipment.
2. CONCRETE, FORMWORK AND SHORING
Some of the hazards associated with concrete installations are%
8ailure of inade4uately designed;installed forms and shoring.
/wkward confined workspace while forming and placing.
e!ar, wire mesh and tie wires pose puncture and cutting hazards.
8alling or flying concrete and gunite while placing.
$re-stressed concrete includes use of e6posed steel !ars or strands with much
latent energy.
Safety for the installation of a finished concrete structure starts with its design.
/ competent engineer should design the formwork for all structures re4uiring the
use of shoring and for large pours to !e made at and !elow ground level.
$rovide ade4uate !earing capacity, >a function of soil conditions?, for
formwork vertical support mem!ers.
$rovide !racing of formwork lateral for loads to !e imposed during
form erection and the pour.
$rovide support for and anchor reinforcing steel as it is installed.
Se4uence of form erection should minimize e6posure of workmen to
installed reinforcing steel. $rovide covers as re4uired to prevent
possi!le impalement or cutting !y workers !y e6posed steel.
/de4uate work platforms with guardrails should !e provided for
workmen during form erection. *se safety !elts when platforms cannot
!e provided.

emoval of forms and shores should follow approved scheme of the Structural
2esigner. /ll nails, e6posed tie wires, etc. are to !e removed or !ent over as forms
are removed. 8orm material shall !e neatly stacked for reuse or disposal.
3. SCAFFOLDING AND LADDERS
i. The ma"or hazards associated with the use of
scaffolding and ladders are falling men, material or tools.
ii. Scaffolds are generally classified !y the loading of
work platforms as light, medium or heavy duty with design
platform loadings of 17 >011?, 7( >11'?, and C7 >5BB? psf
>kg;m1?, respectively.
iii. The following re4uirements are applica!le to all
scaffold work platforms%
/ guardrail shall enclose all working platforms more than 0( ft. >5m?
a!ove ground or floor level. Duardrail shall !e located '1 in. >0(7(
mm? a!ove platform.
3ork platform shall !e completely decked with no openings.
/de4uate access to each work platform shall !e provided. f ladders
are used, they shall !e securely tied to scaffolding.
$lanks shall !e fastened;secured and kept clean.

iv. /s applica!le, continuous scaffolding is to !e tied and
secured to structure !eing scaffolded at 17 ft. intervals
horizontally and 17 ft. vertically.
v. /ll materials used for scaffolding shall !e periodically
inspected to confirm condition satisfactory for continued use.
vi. Suita!le footings shall !e provided to prevent uneven
settlement and loading of vertical support for scaffolding
vii. f ad"usting screws are used, they shall !e installed only
at !aseplates. They shall never !e used on scaffolds with
casters.
viii. 3hen erecting or using rolling scaffolds, casters shall
!e locked in position. $ersonnel are not permitted on rolling
scaffold while it is !eing moved. /ny tool or material onscaffold to !e moved shall !e secured.
i6. Straight or e6tension ladders shall !e used at an angle
of a!out C7 degrees with the horizontal >' to 0 rise? with the
top e6tending at least 5 ft. >9(( mm? a!ove the point of top
support and securely tied at that point.
6. $ersonnel shall have !oth hands unencum!ered when
clim!ing ladders. /ll clim!ing and descending shall !e done
facing the ladder.
6i. Metal ladders shall not !e used in the vicinity of
electrical e4uipment or circuits.
6ii. 3hen using stepladder considerations should !e given
to tying off or holding to sta!ilize.
6iii. Scaffolds must !e plum! and level at all times.
4. WELDING, BURNING AND CUTTING

hazards.
/ll gas cylinders shall have their contents clearly la!eled.
Das cylinders shall always !e stored, transported, used and secured in an
upright position. alve keys must !e on cylinder when in use.
Das hoses shall !e regularly checked for a!rasion, cuts, leaky "oints or other
damage. 3elding ca!le shall !e similarly checked for e6ternal damage and for
properly connected and insulated "oints. n running hoses and ca!les to work,
prefera!ly they should !e located overhead. f not possi!le, and particularly in
heavy traffic areas, protective cover should !e provided when run on the
ground.
a? #ever use grease or oil on e4uipment >cylinders, hoses and torches?
handling o6ygen.
!? 6ygen shall not !e used for ventilation or cooling, !lowing dust or
cleaning work.
c? 8rames of electric welding machines are to !e grounded.
n preparing for welding and !urning, it shall !e ensured that:
a? Sparks or molten metal will not fall on people or com!usti!lematerials particularly from work at elevated location.
!? 3ork is screened or isolated to preclude e6posure of ad"acent
workmen to welding flash.
c? 3ork is properly grounded for electric welding. $ipe containing oil,
gas or other com!usti!le material and conduits and structures for
electricity transmissions shall not !e used for grounds.
d? 8ire e6tinguisher is availa!le within 7( ft. >07 m? of work. 8or work at
elevated location or in confined space, e6tinguisher shall !e at work
location.
e? 3ork location is ade4uately ventilated. 8or welding or !urning
materials containing zinc, lead, cadmium, chromium or !eryllium,
respirators or fresh air masks are re4uired for poorly ventilated or
confined workspaces.
f? 3ork pieces shall !e properly supported for the operation to !e done.
3hile doing the work, welders and !urners must use the personal protective
e4uipment re4uired for the "o!. #ormally this will include tinted eye protection,which greatly restricts the user@s visi!ility. 8or this reason, thorough preparation

for the "o! is important !efore starting work. )and or power tools are fre4uently
used in con"unction with welding and !urning operations re4uiring additional
protective e4uipment.
#. HAND AND POWER TOOLS
The hazards associated with the use of hand and power tools are%
8ailure or disintegration of tool.
$ro6imity to moving or cutting parts of tool.
8lying particles from tool and work.
)eat and sparks.
Electrical shock.
&rushing or pinching.
Tools that are right for the "o!, in safe condition and used properly will provide
productivity as well as safe work conditions.
3ood handles on tools shall !e free of splinters, cracks, and solidly attached to
their working heads.
Electric powered tools shall !e dou!le insulated or properly grounded. Electric
cords shall !e maintained free of cuts, a!rasion and kinks. &ords shall not !e
used for hoisting tools.
Tools with e6posed moving parts shall !e provided with guards to minimize
e6posure of tool operator.
/ll powered tools shall !e stopped and, as applica!le, air or electric supply
disconnected, to make any ad"ustments to tool. 8uel powered tools shall !e
shutdown for refueling.
mpact-type >percussion? tools shall have suita!le retainers to hold tool !its in
place during operation.
E6tension or leverage increasers shall not !e used on tool handles.

&. INDUSTRIAL MOBILE E"UIPMENT
nherent with the use of productive mo!ile e4uipment are the many potential
hazards that can result from%

$rior to work assignment on any piece of e4uipment, all operators shall !e given
the necessary instructions, practice time, and tested !y a competent o!server for
capa!ility to operate the e4uipment safely.
$rior to starting work each day, operator shall check proper functioning and
condition of horn, lights, tires, !rakes >service and emergency?, windshields,
mirrors, and make a visual and operational check of engine and functional devices
of the e4uipment. /ny defects shall !e rectified or logged for correction as "udgednecessary !y the operator or his supervisor.
/ll e4uipment shall !e periodically inspected and serviced !y a competent
mechanic. ecords of this maintenance shall !e kept for each piece of e4uipment.
The load capacity and any operational restriction for each piece of e4uipment
shall !e posted at the operator@s station. /s applica!le, the capacity and
re4uirements for any e4uipment to transport personnel shall !e posted also.
Safe means of access to operator@s station shall !e provided on each piece of
e4uipment.

$ermanently mounted fire e6tinguishers shall !e provided on all industrial
mo!ile e4uipment.
The engine shall !e shut down and parking !rake set each time%
The e4uipment is !eing refueled.
/ny maintenance work is !eing performed.
The operator leaves his station for any reason.
/ny load e6tending !eyond the confines of the e4uipment shall !e marked or
tagged for easy visi!ility.
+oading of container-type e4uipment shall !e limited to a capacity that will avoid
spillage along the route of travel.
3hen operator@s visi!ility is restricted in direction of travel, or when working in
a congested or confined space, a signalman on the ground with clear visi!ility
shall direct the movement of the e4uipment.
8or !est control, mo!ile e4uipment shall always !e operated in gear, i.e., never#ote% / fatality could occur if a worker walks out from !ehind a truck and !e
run over.?
&ondition and capa!ility of roads or routes of travel for mo!ile e4uipment shall
!e ascertained prior to starting work. 3eather will affect these conditions, and
they shall !e rechecked as appropriate.
'. ELECTRICITY
The ma"or hazards of electricity are electrical shock and the accompanying
possi!ility of death. 8atal electrical shock can !e sustained with voltages of 01(
or less with current as low as 7( F1(( milliamperes. These num!ers indicate thatcare must !e e6ercised with any work utilizing electricity since the hazard is

present over the range of currents and voltages in service at construction site.
