Department SpotlightCLEARWATER FAMILY PRACTICETri-State Clearwater Family Practice consists of a team of nine providers and twenty-seven staff members. The providers in family practice include Celso Chavez, MD; Robin Hight, NP-C; Doris Ziegeldorf, FNP-BC; Theresa Smith, NP-C; Sherry Sweikert-Smith, DNP; Christine Norwood, NP-C; Cody Harris, DNP; Cheryl Loetscher, NP-C; Jessica Six, FNP-BC; and Theresa Smith, NP-C. Tri-State Clearwater Family Practice provides primary care for patients of all ages; newborn to geriatric. Services include preventative healthcare (including screenings, physicals, immunizations, and vaccines), managing chronic medical concerns, sports physicals, Employment & DOT physicals, drug screenings, and post-accident screening. Other services provided include contraceptive services, Medical Assisted Treatment for Opioid Addiction (MAT), and bioidentical hormone therapy.

Tri-State Memorial Hospital & Medical Campus honored Katie Hasenoehrl, LPN, of Clearwater Family Practice as its first DAISY Award® recipient, on January 13, 2020. Katie’s nomination for this award is on behalf of the outstanding lasting impression she leaves with patients. To better support the safety of patients, visitors, and staff, Tri-State Memorial Hospital & Medical Campus will adopt standardized Emergency Code Calls starting on February 17, 2020. The new Emergency Code Calls will help quickly communicate and mobilize resources in the event of an emergency on Tri-State Memorial Hospital’s campus!

EVERY Tri-State Memorial Hospital employee (clinical and clerical) has an Emergency Code Calls HealthStream task available. This HealthStream is due by Monday, February 17, 2020. This education will ensure that all staff are familiar with and know how to call an Emergency Code Call.

New signage and code call stickers for your name badge will be distributed the week of February 10, 2020. Below is an example of the new Emergency Code Call signage.
