1. INTRODUCTION

The International University (IU) is the first public International University of

Vietnam and a member of Vietnam National University – Ho Chi Minh City (VNU).

The IU is currently striving to become a prestigious research institution and training

high quality human resources for the country. It is fully empowered to award all

degrees from undergraduate to post graduate levels. Its internationality is reflected in

international academic environment of IU as a whole, including all degree programs,

teaching staff, languages of instruction, academic and research infrastructure. Its

publicity is reflected in the long-term support from the government and other

2.1 Vision

Take leading in education, research and transfer in the field of Advanced

Technologies and Management.

2.2 Mission

Develop high-quality undergraduate, graduate and PhD education in the field of

Advanced Technologies and Management to meet regional and international

standards.

2.3 Program Objectives

1. Be practicing engineers in the field of production and services, who are able

to:

(i) Design or redesign Industrial, Logistics & Supply Chain systems

(ii) Operate and manage Industrial, Logistics & Supply Chain systems

(iii) Improve the existing Industrial, Logistics & Supply Chain systems

(iv) Support for wise decision making

2. Engage in lifelong learning to maintain and enhance professional skills

3. Work effectively with people and demonstrate leadership, professional skills

and ethical behavior in the workplace

4. Fulfill the needs of community and industrial sector of Vietnam in solving

technical and management problems using industrial and systems engineering

2.5 Career Opportunities

IEM engineers can take on and perform excellently tasks in various fields, namely

Logistics & Supply Chain Management, Procurement Management, Project

Management, Inventory Management, Quality Management, Production

Management, Optimization in Production and Service, developing an integrated

solution to reduce the operation cost, etc. Qualified graduated engineers can achieve

the top-level positions in organizations such as Production Director and Chief

Executive Officer.

2.6 Programs

School of Industrial Engineering and Management has two different programs:

Industrial and Systems Engineering (ISE) and Logistics and Supply Chain

Management (LSCM).

• ISE trains students to be engineers who have ability to design, operate,

coordinate, manage production systems and services to create products and

services.

• LSCM trains students to be engineers who have ability to design, operate,

coordinate, manage activities in Logistics and supply chain. These systems

support the supply and storage of raw materials for production, services,

distribution and delivery the finished goods to customers.

2.7 Course Design

The course of the B.S Degree in ISE & LSCM in the IU – VNU HCM provides the

students the flexibility to join either the IU program (4 years at IU) or the twining

program (2 years at IU and 2 years at Rutgers University, or at University

Binghamton, the State University of New York). Students will receive bachelor

degree awarded by the IU if taking the IU program and bachelor degree awarded by

the overseas partner universities if taking the twining program.

The IEM programs are designed with consideration of students’ English level which

is estimated on TOEFL or TOEFL IBT scores. First year students are classified into

three levels:

• English level 1: TOEFL ≥ 500 (TOEFL IBT ≥ 60 or IELTS > 6.5): Students

are admitted to take full program.

• English level 2: 430 ≤ TOEFL < 500 (40 ≤ TOEFL IBT < 60): Students are

admitted to take partial program together with English preparation classes

(IE2).

• English level 3: TOEFL < 430 (TOEFL IBT < 40): Students are required to

take only intensive English course (IE1) for the first semester of the first year

at school.

2.8 Scholarship

Students with entrance examination scores equal or above 25 are eligible to receive

full scholarship which is worth 168.000.000 VND (~ $6,000 USD) for the whole

course (4 years for domestic programs and 2 years for twinning programs). Partial

scholarship, which is worth 84.000.000 VND (~ $ 3,000 USD for the whole course),

is offered to students with entrance examination scores from 24 to 25. To maintain

the scholarship throughout the whole course, students need to keep their GPA equal

or above 70 and scores of all subjects equal or above 50.

To encourage students with good merits, each semester the International University

spends around 24,000 USD awarding those who achieve excellent merits in the

second semester of the first year. Every semester, 40 scholarships, each worth

12.620.000 VND (~ 600 USD), are granted to the best students who receive neither

full nor partial scholarships.

2.9 Course Assessment

• Mid-term exam: 10% - 20%

• Final exam: 40% - 60%

• Others (e.g. In-class quizzes, group presentation, etc): 20% - 40%`

EN007IU Writing Academic English 1 2 credits

This course provides students with instruction and practice in essay writing,

including transforming ideas into different functions of writing such as definitions,

classifications, cause – effects, arguments. Through reading a few representative

university-level texts, students will develop the ability to read critically and write

accurately, coherently, and in appropriate academic style in response to those texts.

They will also practice necessary skills to write a research report.

To provide students with the study skills needed to listen to academic lectures, take

effective notes and prepare for examinations.

EN011IU Writing Academic English 2 2 credits

This course provides an overview of the organizational format for a research paper

and assists students in completing research projects in any content area course by

providing assistance in writing effective research papers using a step-by-step process

approach. Course content includes the components of a research paper, and

techniques of selecting and narrowing topics; writing argumentative thesis

statements; outlining; locating and documenting sources; taking notes. Students also

have to read extensively about a chosen topic to explore different ideas of multiple

authors about that topic. Students work with projects relating to their content area

courses.

Capstone design is a semester-long course, this work will be done by group project

including 2 – 3 students taken at the senior year. This project reflects practical

industrial system engineering. Each project includes the use of open-ended

problems, development and use of design methodology, formulation of design

problem statements and specification; consideration of alternative solutions,

feasibility consideration and detailed system descriptions. It also includes realistic

constraints (such as economic factors, social impact, and environmental problem).

The students choose the particular design project with approval of appropriate

faculty, advisors. The capstone design topics should be related to the real case from

industry, e.g. projects in the internship company of students in the previous

semester. Also, this project emphasizing the use of knowledge in previous courses

to design, evaluate a system such as facility designs, mathematical modeling and

This course provides students with an understanding of the principles, processes and

the supply chain. The course examines both the independent demand and dependent

Product Design and Development course introduces to the students the role of

the students will apply their learned principles and methods of product development

This is an introductory course to Logistics and supply chain management (SCM). It

provides an overview of fundamental concepts, business processes and models/tools.

The objective of this course is to identify problems, issues and strategies in today’s

supply chain operations via real-world cases. Analytical models and technical tools

are introduced as needed. This course combines SCM business knowledge with

analytical thinking and pinpoints the role of SCM relative to other business

disciplines. It serves as a roadmap to more in-depth courses on related topics.

The simplest definition of economic forecasting is that it is a process that has as its

objective the prediction of future events or conditions to reduce that uncertainty so

that our decisions will be better ones.

Specific objectives are to instruct you in:

1. The formulation and specification of forecasting models;

2. Data collection, interpretation, organization, and analysis for building forecasting

models;

3. Fundamental statistical and probability concepts used in forecasting;

4. The existence of a hierarchy of forecasting models;

5. The use of econometric software in a lab setting.

IS059IU Materials Handling Systems 3 credits

Proper methods for material handling and storage including safety practices, proper

equipment usage, engineering controls, and personal protective equipment. Included

are procedures for storage of non-hazardous and hazardous materials, material

handling equipment preventative maintenance, and motor fleet safety.

IS057IU Warehouse Engineering Management 3 credits

This course provides the students with an understanding of the principles, processes

and techniques for the effective planning, management and operation of warehouses.

Through this exposure, students will gain insights into how warehousing adds value

to the organization’s supply chain and how warehousing decisions impact the

performance of the organization.

There is global experience and examples that show how comprehensive

organizational environmental sustainability and archaeological criteria integrated

into the supply chain management/procurement process and decision-making of

public and private agencies, organizations and corporate entities can improve

financial and environmental performance, while addressing ethics, social

regeneration, resource/waste impacts and economic development concerns. This

course will allow students to participate in applied research projects that include

designing supply chain management and procurement systems and products, which

address environmental, social and ethical considerations in organizational and

corporate policy, program and reporting.

IS064IU Entrepreneurship in Supply Chain 3 credits

The nature and importance of entrepreneurship; forms of entrepreneurship; the

entrepreneurial process; the entrepreneurial mind; creativity, ideas and innovation;

screening entrepreneurial opportunities; identifying resources to support

entrepreneurial activities; intellectual property issues; accessing finance and other

resources; the entrepreneurial team; assessing risk; business structure and ethics;

entrepreneurial strategy; finding and reaching customers and marketing innovation;

feasibility planning.

IS062IU E-Logistics in Supply Chain Management 3 credits

Comprehensive inquiry into the role of e-commerce in collaborative distribution and

logistics relationships. Special attention is afforded to resource and technology

interdependencies, exchange governance mechanisms and relationship management

bench-marking. Emphasis is given to the tools for creating value in the supply

chain.

Supply security and risk management have become major business concerns in view

of the need to protect the supply chain and maintain business continuity in the wake

of high-consequence disruptive events. This course provides a broad overview of

key supply chain security areas and issues in the context of homeland security.

IS066IU Data Mining in Supply Chain 3 credits

Data mining refers to a family of techniques used to detect interesting nuggets of

relationships/knowledge in data. With the availability of large databases to store,

manage and assimilate data, the new thrust of data mining lies at the intersection of

database systems, artificial intelligence and algorithms that efficiently analyze data.

The distributed nature of several databases, their size and the high complexity of

many techniques present interesting computational challenges.

An overview of business intelligence in the field of supply chain management and

marketing. Addresses how to leverage business intelligence systems to define KPIs,

sharpen the accuracy of forecasting and planning, track business activities, and

deliver dashboards, scorecards, strategic reporting, and operational/real-time

reporting to enhance decision making for supply chain and marketing. SAP business

intelligence solution is introduced to illustrate the concepts.

IS067IU International Transportation & Logistics 3 credits

Students learn the significance of international traffic and transport logistics.

Student will learn basic methods and applications of operations research to

implement, operate and optimize overall company material flow technical networks.

This applies in particular to the subject of the optimal arrangement of sources and

outflows and their dimension as well as their optimal interconnection from a

transport technology point of view.

and sea transport; competition in international transport; competition in

international transport; cost accounting for freight forwarding; price setting in road

freight, rail, air and sea transport; information management in freight forwarding.

This unit covers the following: the role of Purchasing and Procurement in Supply

Chain Management, purchasing procedures, supplier sourcing and management,

negotiations, supplier relationships, specifying product quality, matching supply

with demand and support tools for purchasing and procurement. Comprehensive

theories and models developed by practitioners are examined.

IS072IU Port Planning and Operations 3 credits

This course provides the students with an understanding of port system,

geographical location of ports, related planning and operational issues. Methods and

processes for port planning and design. Besides that, the students are provided the

knowledge about Inland connectivity, port’s linkage to transport infrastructure,

intermodal connections, and marine operations in ports. Traffic management, cargo

handling, terminal operations, facilities and equipment, port security.

IS073IU Business Law 3 credits

The aim of this course is to:

• Familiarize the student with legal language; basic concepts, principles and

general knowledge of business Law.

• Introduce to students about main business forms in Vietnam and regulations

for each. Also, possibility of reorganization and Insolvency for enterprises,

as main subject matter of this course.

• Increase the student’s understanding of the Vietnamese regulations over

The basic objective of this course is to provide to students with the necessary

knowledge, skills and foundations for acquiring a wide range of rewarding careers

into the rapidly expanding world of Import & Export Management. In addition, this

course aims at imparting knowledge of trade procedures and documentation

formalities with a view to enable the participants to develop a systematic approach in

handling trade transaction and incidental paper work.

IS076IU Introduction to Computing – Matlab

Application

3 credits

Introduction to MATLAB, a powerful programming package for engineers and

scientists. Students will learn the fundamentals of MATLAB, how to write programs

in MATLAB, and how to solve engineering problems using MATLAB. Emphasis on

problem-solving skills and mathematical tools of importance in engineering.

C++/C#, Python

Introduction to programming in C++. Operators and the C++ system; fundamental

data types; flow of control; functions; arrays, pointers, and strings; application of

C++ for solving engineering problems and numerical analyses.

This course is offered for undergraduate students at School of IEM, IU. It aims to

improve students’ academic and scientific writing in English, and helps them

successfully complete course reports, thesis, dissertations, and articles for

publication as well as doing a proper presentation, etc. Upon completion of the

course, we hope our students become more effective, more efficient, and more

confident writers.

This course provides the student with a comprehensive view of retailing and an

application of marketing concepts in a practical retail managerial environment. As a

potential marketing manager, this course will give students insight into the retailing

environment of which students will be a part and allow students to make informed

decisions in your interaction with retailers. The course also provides a good

foundation for those interested in owning or running a small retail business or those

interested in pursuing a retail career as a merchandise buyer or store manager.

IS081IU Deterministic Models in Operations Research 4 credits

Elements of problem solving and algorithmic design. Use of numerical analysis and

linear algebra to solve industrial engineering problems. Topics to be covered

include: problem formulations, simplex method in tableau form, duality theory, an

introduction to the geometry of the simplex method, sensitivity analysis,

transportation and network flow problems, optimality conditions and basic

numerical methods for nonlinear programs.

applications of accounting and financial reporting, essentials in the US standard,

including topics such as the theory of debit and credit, accounts, special journals, the

Để đảm bảo môi trường sinh hoạt học tập tốt tại trường Đại học Quốc tế - Đại học

Quốc gia TP.HCM, trường Đại học Quốc tế đề ra Nội quy sinh viên như sau:

1. Nghiêm cấm mọi hành vi gian lận với bất kỳ hình thức nào trong nhà trường.

2. Lên lớp đúng giờ, trang phục gọn gàng lịch sự kín đáo, ứng xử giao tiếp văn

minh lịch sự phù hợp với môi trường sư phạm. Ngôn ngữ giao tiếp trong

khuôn viên Nhà trường bằng tiếng Anh.

3. Phải tôn trọng lễ phép với thầy cô giáo, cán bộ viên chức, lịch sự và hiếu

khách, hòa đồng và sẵn sàng giúp đỡ bạn bè.

4. Đi xe buýt cần xếp hàng thứ tự trước sau khi lên xe, giữ gìn vệ sinh, văn

minh lịch sự và trật tự trên xe buýt.

5. Giữ gìn vệ sinh chung, sử dụng nhà vệ sinh theo đúng qui định. Không mang

hoặc sử dụng các loại thức ăn, nước uống vào phòng học, giảng đường,

phòng thí nghiệm.

6. Không được nằm ở khu vực hành lang, trên lối đi, được nghỉ trưa tại phòng

học từ 11 giờ 30 đến 12 giờ 30.

7. Phải đảm bảo an ninh trật tự trong khuôn viên nhà trường, không gây tiếng

ồn, không sử dụng các loại phương tiện gây tiếng ồn; không chơi thể thao,

chạy nhảy đùa giỡn trong phòng học, khu vực gần phòng học, khu vực hành

chính gây ảnh hưởng tới hoạt động của nhà trường.

8. Tuân thủ theo tất cả quy định chung của tòa nhà Điều hành ĐHQG-HCM.

Các loại phương tiện vào trường phải gửi đúng nơi quy định.

9. Giữ gìn bảo quản tài sản và các trang thiết bị của nhà trường. Thực hiện tốt

quy định về việc mượn và trả thiết bị phục vụ công tác giảng dạy. Không tự

ý di chuyển, sắp xếp, điều chỉnh sử dụng các thiết bị phục vụ giảng dạy.

Không dán giấy, viết vẽ trên tường, bàn học. Chỉ được phép tổ chức hội họp,

di chuyển bàn ghế, trang trí phòng học, sử dụng các trang thiết bị khi có sự

chấp thuận của Phòng Quản trị Thiết bị.

10. Không được sử dụng trang thiết bị của nhà trường vào các mục đích khác với

học tập và nghiên cứu.

11. Không đùa giỡn trong thang máy, tùy tiện bấm chọn các tầng khi không có

nhu cầu.

12. Cấm hút thuốc, uống rượu bia trong khuôn viên trường học.

13. Cấm thành lập, tham gia các hoạt động mang tính chất chính trị pháp luật, tệ

nạn xã hội; tổ chức tham gia các hoạt động tập thể mang danh nghĩa nhà

trường khi chưa được Hiệu trưởng cho phép.

14. Cấm sản xuất, buôn bán, vận chuyển, tàng trữ, phát tán, sử dụng các loại vũ

khí, chất gây nổ, chất ma túy, hàng cấm …; các tài liệu, ấn phẩm phản động,

đồi trụy, cấm truy cập các nguồn thông tin không lành mạnh trên Internet.

Nghiêm cấm tổ chức, tham gia truyền bá các hoạt động mê tín dị đoan, các

hoạt động của các tổ chức, tôn giáo trong nhà trường và các hoạt động vi

phạm đạo đức khác.

15. Nghiêm cấm các hình thức trộm cắp hoặc phá hoại tài sản công và tài sản cá

nhân người khác.

16. Nội dung này được áp dụng kể từ ngày ký. Mọi vi phạm tùy mức độ cụ thể

sẽ được xử lý từ khiển trách đến buộc thôi học. Trường hợp nghiêm trọng

giao cho cơ quan chức năng xử lý theo qui định của pháp luật.

A1. QUY ĐỊNH CHUYỂN ĐỔI CHƯƠNG TRÌNH

1. Chuyển đổi ngành học do trường cấp bằng:

Nguyên tắc chung:

- Thực hiện các văn bản: công văn số 1479/ĐHQG-ĐH&SĐH ngày

11/9/2008, Quy chế 1368 và công văn 1391/ĐHQT-ĐH&SĐH ngày

09/8/2010.

- Ngành được chuyển sang còn chỉ tiêu đào tạo; số lượng sinh viên chuyển

sang không quá 10% tổng chỉ tiêu được giao của ngành.

- Trường phải quy định các môn học cần chuyển đổi, môn học được công nhận

đảm bảo tích lũy kiến thức của chương trình đào tạo (CTĐT) ngành chuyển

đến. Các môn học tương đương phải được thể hiện trong CTĐT và quy định

học vụ của nhà trường.

Đối với các trường hợp không đạt yêu cầu xét tuyển xét tuyển đầu vào đối với khóa

tuyển sinh tương ứng của ngành xin chuyển đến thì phải thỏa các điều kiện sau:

- Có lý do chính đáng về việc chuyển ngành học;

- Kết quả học tập ít nhất 3 học kỳ đầu tiên (không tính học kỳ học ngoại ngữ)

đạt từ loại khá trở lên và đã tích lũy đủ số tín chỉ tối thiểu cho mỗi học kỳ

theo quy định.

2. Chuyển đổi ngành học từ chương trình liên kết do trường nước ngoài cấp

bằng sang chương trình do trường Đại học Quốc tế cấp bằng:

Nguyên tắc chung:

- Chương trình liên kết có Quyết định của ĐHQG-HCM về việc liên kết đào

tạo.

- Thỏa các quy định về chuyển đổi ngành học trong các chương trình so

trường cấp bằng theo nội dung mục 1.1 của văn bản này.

Đối với các trường hợp không đạt điều kiện trúng tuyển đầu vào của khóa tuyển sinh

tương ứng của ngành xin chuyển đến thì phải thỏa các điều kiện sau:

- Chương trình liên kết có cùng phương thức tuyển sinh, cùng nhóm ngành với

chương trình do trường ĐH Quốc tế cấp bằng.

- Sinh viên có hoàn cảnh thực sự khó khăn không thể theo học tiếp giai đoạn 2

của chương trình liên kết.

- Đã hoàn thành giai đoạn 1 của chương trình liên kết (không tính học kỳ học

ngoại ngữ) và có điểm trung bình tích lũy tại thời điểm xin chuyển ngành

thuộc top 10% hoặc >= 7,0 điểm (thang điểm 10).

- Số lượng sinh viên diện 2.2 chuyển sang không quá 5% tổng chỉ tiêu được

giao của ngành.

A2. QUI CHẾ ĐÀO TẠO THEO HỌC CHẾ TÍN CHỈ - BẬC ĐẠI HỌC

1. Chương trình đào tạo (CTĐT)

CTĐT là tập hợp các môn học được bố trí giảng dạy học tập kế tiếp nhau theo một

trình tự khoa học nhằm đào tạo người học có đủ kiến thức, khả năng và tiềm năng

cho một lĩnh vực khoa học kỹ thuật. Người học hoàn thành một CTĐT thì được cấp

một văn bằng tương ứng.

Nội dung đào tạo trong toàn khóa học của từng ngành ở mỗi trình độ được thể hiện

thành CTĐT. CTĐT của mỗi ngành đào tạo do Trường xây dựng trên cơ sở khung

chương trình do Bộ GD&ĐT qui định và tham khảo chương trình đào tạo của các

trường đại học danh tiếng nước ngoài. Mỗi CTĐT có thời lượng từ 135 đến 145 tín

chỉ và thời gian đào tạo 4 năm (không tính chứng chỉ Giáo dục quốc phòng, Giáo

dục thể chất) bao gồm các loại môn học như sau:

1.1 Nhóm môn học bắt buộc gồm những môn học chứa đựng những nội dung chính

yếu của ngành đào tạo và bắt buộc sinh viên phải tích lũy.

1.2 Nhóm môn học tự chọn là những môn học mà sinh viên được tự chọn để tích lũy

đủ số tín chỉ qui định. Trong đó có thể chia ra:

• Môn học tự chọn định hướng: là những môn học tự chọn được xác định theo

định hướng chuyên ngành của một CTĐT, chứa đựng những nội dung cần

thiết mà sinh viên phải chọn trong số các môn học tự chọn do trường qui

định theo nhóm ngành và ngành.

• Môn học tự chọn tự do: là các môn học mà sinh viên có thể chọn tùy ý theo

học.

• Việc hoàn tất đạt yêu cầu của CTĐT là điều kiện để người học được cấp

bằng cho mỗi cấp học.

2. Tín chỉ

Tín chỉ là một đơn vị dùng để lượng hóa khối lượng lên lớp và tự học bắt buộc đối

với một sinh viên để đạt được các yêu cầu học tập. Một tín chỉ được qui định bằng

15 tiết lý thuyết, hoặc 30 đến 45 tiết thực hành và thời gian tự học cần thiết theo ước

lượng chung của Nhà trường trong học kỳ: học 1 tiết lý thuyết sinh viên phải học 2

tiết tự học.

3. Khóa học, năm học và học kỳ

3.1 Khóa học

Khóa học là thời gian qui định để sinh viên hoàn tất một CTĐT của một ngành nhất

định. Khóa học theo một CTĐT được quy thành số học kỳ chính hay thành số tín chỉ

tối thiểu phải tích lũy. Tùy ngành nghề đào tạo, khóa học được thiết kế từ 8 đến 10

học kỳ chính cho bậc đại học. Tùy theo khả năng, sinh viên có thể rút ngắn hoặc kéo

dài thời gian học tập, nhưng không được kéo dài quá thời gian qui định cho toàn

khóa học như sau: rút ngắn tối đa 2 học kỳ, kéo dài tối đa 4 học kỳ.

3.2 Năm học và học kỳ

Năm học gồm 2 học kỳ chính và 1 học kỳ hè. Học kỳ là một khoảng thời gian nhất

định gồm một số tuần dành cho các hoạt động giảng dạy, học tập, và đánh giá kết

quả học tập.

• Học kỳ chính gồm 16 đến 20 tuần, trong đó có ít 'nhất 14 tuần dành cho các

hoạt động giảng dạy, học tập và các tuần còn lại dành cho việc kiểm tra, thi,

bảo vệ luận văn, đánh giá và đăng ký học tập.

• Học kỳ hè là học kỳ không bắt buộc phải học. Khi sinh viên có nhu cầu và

Nhà trường có điều kiện sẽ tổ chức học kỳ hè, gồm 8 đến 10 tuần trong đó có

ít nhất 7 tuần dành cho việc giảng dạy, học tập, kiểm tra và một tuần dành

cho việc thi cử, đánh giá.

4. Lớp học

Lớp sinh viên:

Được tổ chức cho các sinh viên học cùng ngành chuyên môn trong cùng một khóa

đào tạo. Lớp sinh viên được tổ chức tương đối ổn định từ đầu đến cuối khóa học

nhằm duy trì các sinh hoạt đoàn thể, các phong trào thi đua, các hoạt động chính trị-

xã hội, văn hóa thể thao và để quản lý sinh viên trong quá trình học tập theo qui định

của Nhà trường. Nhà trường căn cứ vào số lượng sinh viên của mỗi khóa, mỗi ngành

đào tạo, để qui định số lớp sinh viên.

Lớp môn học:

Bao gồm các sinh viên theo học cùng môn học, có cùng thời khóa biểu của môn học.

Mỗi lớp môn học đều được ký hiệu bằng một mã số riêng. Số lượng sinh viên của

lớp môn học được Trường quy định như sau:

• Với môn cơ bản như Toán, Lý, Hóa v.v.v. . .: tối đa là 120 sinh viên.

• Với môn học cơ sở ngành và môn chuyên ngành: tối đa 60 sinh viên.

• Với môn ngoại ngữ: tối đa 30 sinh viên.

• Sĩ số tối thiểu của một lớp môn học sẽ do Ban Giám hiệu quyết định từng

trường hợp cụ thể.

5. Môn học- Môn học tương đương - Điểm môn học

5.1 Môn học

5.1.1 Môn học là tập hợp tất cả các dạng hoạt động học tập của một phần khối lượng

kiến thức. Tùy theo khối lượng kiến thức của môn học yêu cầu, mỗi môn học được

lượng hóa bằng một số tín chỉ và định danh bằng mã số môn học.

5.1.2 Các dạng hoạt động học tập của một môn học bao gồm số tín chỉ lý thuyết, số

tín chỉ rèn luyện kỹ năng: bài tập, thí nghiệm, thực tập, seminar, tiểu luận, v.v... và

số tín chỉ tự học.

5.1.3 Tùy theo yêu cầu về mặt kiến thức, môn học có thể có một hay nhiều môn học

tiên quyết, một hay nhiều môn học trước. Đối với mỗi sinh viên theo học một CTĐT

nào đó, các môn học được xếp thành: các môn học bắt buộc, các môn học tự chọn

định hướng, các môn học tự chọn tự do.

• Môn học tiên quyết: các môn học là tiên quyết đối với môn học A là các môn

học mà sinh viên phải theo học trước và phải thi đạt mới được theo học môn

học A.

• Môn học trước: môn học A được gọi là môn học trước của môn học B nếu

như sinh viên đã đăng ký học và có điểm thi của môn học A, kể cả điểm I,

W, X, Z.

• Môn học tương đương: một hay nhiều môn học được gọi là tương đương với

môn học A của CTĐT khi chúng có nội dung và thời lượng đáp ứng được

yêu cầu của môn học A đó (có nội dung giống nhau từ 80% trở lên và có số

tín chỉ tương đương hoặc lớn hơn). Danh mục các môn học tương đương

được Nhà trường quy định theo đề nghị của Trưởng Khoa.

• Môn học thay thế: môn học thay thế là môn học được sử dụng để thay thế

một môn học khác có trong chương trình đào tạo nhưng không còn tổ chứa

giảng dạy nữa.

5.2 Điểm môn học

5.2.1 Điểm cuối cùng dùng để đánh giá một môn học được gọi là điểm môn học.

Điểm môn học được đánh giá dựa trên kết quả học tập toàn diện của sinh viên trong

suốt học kỳ đối với môn học đó.

5.2.2 Điểm môn học có thể bao gồm các loại điểm thành phần sau:

• Thực hành: bài tập, thực hành, seminar, tiểu luận...

• Kiểm tra giữa học kỳ.

• Thi cuối học kỳ.

5.2.3 Điểm thành phần:

Các điểm thành phần được cho dưới dạng số nguyên từ 0 đến 100 và điểm tổng kết

môn học là con số đã làm tròn thành số nguyên.

Tỷ lệ các điểm thành phần trong kết quả cuối cùng của một môn học được qui định

như sau :

• Điểm thực hành, bài tập, tiểu luận : từ 20% - 40%

• Điểm kiểm tra giữa kỳ : từ 20% - 40%

• Điểm thi cuối học kỳ : từ 40% - 60%

Riêng đối với các môn thực hành, được quy định như sau:

• Điểm các bài thực hành trong học kỳ : từ 70 – 80%

• Điểm thi cuối học kỳ : từ 20 – 30%

Các trường hợp không thi giữa kỳ mà thay thế bằng hình thức khác phải có đề xuất

của lãnh đạo Khoa và có sự phê duyệt của Ban Giám Hiệu trong vòng 2 tuần (1 tuần

đối với học kỳ hè) kể từ khi môn học bắt đầu. Những qui định này thay đổi tùy theo

môn học và phải được ghi trong đề cương chi tiết các môn học và đề cương được

công bố trước sinh viên ở buổi lên lớp đầu tiên. Đối với các môn học đã đăng ký và

đã được chấp nhận, sinh viên có trách nhiệm thực hiện đầy đủ và nghiêm túc mọi

yêu cầu của cán bộ giảng dạy về việc lên lớp, làm bài tập, thực hành thí nghiệm,

thực tập, tham gia thảo luận, dự kiểm tra giữa môn học và thi kết thúc môn học. Nếu

không thực hiện phần việc nào, sinh viên sẽ nhận điểm không (0) cho điểm thành

phần tương ứng.

Mức điểm đối với một môn học

Điểm môn học được làm tròn đến một chữ số thập phân của điểm trung bình tính

theo phân lượng của các điểm thành phần. Trường sẽ tiếp tục qui đổi điểm theo

thang điểm 4 hay 100 ra thang điểm chữ khi cần thiết.

5.2.4 Những ký hiệu sau đây được sử dụng trong các trường hợp đặc biệt: Cấm thi

hoặc Vắng thi không phép (F); Miễn học hoặc Bảo lưu (WH); Vắng thi có phép

hoặc điểm chưa hoàn tất (I); Chưa có điểm thi (NA);

5.2.5 Điểm chưa hoàn tất (điểm I): là điểm tạm thời do cán bộ giảng dạy phụ trách

môn học xem xét và cho đối với những sinh viên vì những lý do chính đáng không

thể hoàn tất các yêu cầu của môn học vào cuối mỗi học kỳ hay khóa học.

• Điều kiện để sinh viên có thể xin nhận điểm “I” của môn học: Sinh viên đã

dự học, dự các lần kiểm tra giữa học kỳ, các hoạt động liên quan đến môn

học như thí nghiệm, thực hành.

• Đã nộp học phí của môn học.

• Vì các lý do bất khả kháng nên vắng mặt trong buổi kiểm tra, thi cuối học kỳ

(ốm đau, tai nạn…).

• Những thủ tục sau đây phải được thực hiện:

• Trường hợp đơn xin điểm I xảy ra trong thời điểm trước ngày thi: sinh viên

làm đơn kèm theo

• chứng cứ hợp lệ, trình bày với CBGD. Dựa vào ý kiến của CBGD và ý kiến

đồng ý của Lãnh đạo

• Khoa, phòng Đào tạo xem xét về việc ra quyết định cho phép sinh viên nhận

điểm I. Trường hợp sinh viên đau ốm bất thường hoặc lý do chính đáng bất

khả kháng không tham gia thi: sinh viên làm đơn kèm chứng lý hợp lệ tại

phòng Đào Tạo trong vòng 3 ngày kể từ ngày thi

• Những trường hợp đặc biệt sinh viên làm đơn gởi Ban chủ nhiệm Khoa và

Khoa đề xuất để BGH phê duyệt. Sinh viên không cần đăng ký học lại các

môn học được nhận điểm I trong học kỳ tiếp theo, mà chỉ nộp đơn xin dự thi

xóa điểm I. Trong thời hạn là 1 năm tiếp theo, nếu sinh viên không có đủ

điểm để đánh giá môn học, điểm I tự động được chuyển sang điểm F (điểm

không).

6. Điểm trung bình học kỳ, điểm trung bình tích lũy

6.1 Điểm trung bình học kỳ (ĐTBHK)

• ĐTBHK là điểm trung bình của tất cả các môn học mà sinh viên đã đăng ký

học và được trường xếp lớp trong học kỳ đó.

• Các môn học chưa hoàn tất (điểm I), Miễn học, bảo lưu (WH), chưa có điểm

thi (NA) không tính trong điểm trung bình học kỳ.

• ĐTBHK được làm tròn đến 1 chữ số thập phân.

• ĐTBHK được dùng để xét học bổng, khen thưởng, học vượt, học cùng một

lúc ở nhiều ngành, nhiều trường và xử lý học vụ

6.2 Điểm trung bình tích lũy (ĐTBTL)

ĐTBTL là điểm trung bình có trọng số của tất cả các môn học mà sinh viên đã đăng

ký học và được trường xếp lớp từ lần đăng ký đầu tiên đến thời điểm tính. ĐTBTL

được tính theo thang điểm 100 và được làm tròn đến 1 chữ số thập phân. ĐTBTL

được dùng để xét phân loại kết quả học tập của khóa học, xử lý học vụ, xét tốt

nghiệp.

7. Số tín chỉ tích lũy

Số tín chỉ tích lũy là tổng số tín chỉ của các môn học (kể cả môn học bảo lưu) đã

hoàn tất và đạt điểm môn học ≥ 50 từ lúc bắt đầu khóa học đến thời điểm được tính.

A3. CÁC DIỆN SINH VIÊN

1. Sinh viên hệ chính quy

Sinh viên hệ chính quy là sinh viên đã hoàn tất các thủ tục nhập học và học chương

trình chính qui theo phương thức tập trung toàn thời gian. Một số trường hợp đặc

biệt cũng sẽ được xem xét để thu nhận vào hệ chính qui tập trung như: sinh viên

đang học ở nước ngoài có nguyện vọng muốn về nước học tiếp, sinh viên các trường

đại học khác ở trong nước (cùng khối thi, cùng nhóm ngành đào tạo) có nguyện

vọng muốn chuyển trường.

2. Sinh viên tạm dừng

2.1 Nếu có lý do chính đáng sinh viên có thể làm đơn xin tạm dừng học tập và phải

nộp đơn tại Phòng Đào tạo. Sinh viên chỉ được phép tạm dừng khi có quyết định cho

phép của Ban Giám hiệu.

2.2 Thời gian tạm dừng do thi hành nghĩa vụ quân sự không tính vào thời gian học

của sinh viên. Thời gian tạm dừng vì các lý do khác phải chịu sự ràng buộc nói trong

Mục 3.1-Điều 2.

2.3 Sinh viên thuộc diện ưu tiên 1, theo qui định của quy chế tuyển sinh đại học, có

tổng thời gian cho phép tạm dừng tối đa là 4 học kỳ chính cho toàn khóa học có thời

gian đào tạo từ 5-6 năm. Tổng thời gian tạm dừng được tính vào quỹ thời gian cho

phép học của khóa học và không được tạm dừng quá 2 học kỳ liên tiếp.

3. Sinh viên

3.1 Tìm hiểu, nghiên cứu để nắm được kế hoạch học tập, chương trình đào tạo, và

những qui định, chế độ của nhà trường. Khi cần thiết, sinh viên nên đến các phòng

chức năng, CVHT, và cán bộ giảng dạy môn học để được hướng dẫn và giúp đỡ.

3.2 Phải liên hệ với CVHT để được hướng dẫn và thông qua danh sách các môn học

dự kiến chọn trong mỗi học kỳ để thực hiện việc đăng ký học tập theo đúng qui

định.

3.3 Tất cả sinh viên (trừ trường hợp được giải quyết cho tạm dừng hoặc chưa hoàn

thành đầy đủ nghĩa vụ qui định đối với nhà trường) đều phải thực hiện việc đăng ký

học tập theo đúng qui định.

3.4 Khối lượng học tập đăng ký chỉ được coi là chính thức khi đã được nhà trường

chấp thuận và xếp được lớp môn học. Sinh viên có trách nhiệm theo dõi thông báo

về kết quả học tập của học kỳ trước và kết quả đăng ký môn học tại phòng Đào tạo

hoặc Văn phòng Khoa quản ngành.

A4. ĐĂNG KÝ MÔN HỌC

Việc đăng ký môn học nhằm giúp cho sinh viên chủ động trong việc xây dựng kế

hoạch học tập của mình, lựa chọn các môn học cho từng học kỳ trong toàn khóa học

sao cho phù hợp với năng lực, hoàn cảnh cá nhân nhằm đạt hiệu quả học tập cao

nhất.

• Phải đăng ký tối thiểu 12 tín chỉ cho mỗi học kỳ, trừ học kỳ cuối khóa học

• Chỉ được đăng ký tối đa 24 tín chỉ cho mỗi học kỳ, trừ học kỳ cuối khóa học,

đối với sinh viên có ĐTBTL >= 65.

• Bảng đăng ký môn học phải có sự chấp thuận và phê duyệt của GVCN.

• Các trường hợp đặc biệt, sinh viên phải có đơn và được sự phê duyệt của

lãnh đạo Khoa.

1. Quy trình đăng ký môn học:

Sinh viên năm thứ nhất học theo thời khóa biểu quy định, Phòng Đào tạo phối hợp

với các khoa tổ chức và hướng dẫn cho sinh viên từ năm thứ 2 trở lên đăng ký môn

học theo quy trình đăng ký môn học như sau:

• Trước khi kết thúc học kỳ 1 tháng, Phòng Đào tạo chuyển về cho Khoa: sổ

tay sinh viên, phiếu đăng ký môn học, danh sách sinh viên.

• Khoa thông qua hệ thống giáo viên chủ nhiệm tổ chức phát sổ tay, phiếu

đăng ký môn học (phải có ký nhận) và tư vấn hướng dẫn sinh viên đăng ký

môn học. Giáo viên chủ nhiệm phải tập trung sinh viên lớp mình chủ nhiệm

ít nhất 1 lần để thực hiện công tác này.

• Sau khi hết thời hạn đăng ký môn học (ghi trong sổ tay sinh viên) sinh viên

nộp phiếu đăng ký môn học về cho giáo viên chủ nhiệm, thông qua Khoa

nộp về Phòng Đào tạo theo lịch quy định: phiếu đăng ký môn học của sinh

viên đã đăng ký có ký xác nhận của giáo viên chủ nhiệm, sổ tay và phiếu

đăng ký môn học dư và danh sách sinh viên ký nhận để lưu giữ đối chiếu khi

cần.

• Chậm nhất là 2 tuần sau khi kết thúc thời hạn đăng ký môn học, Phòng Đào

tạo chuyển đến các Khoa: thời khóa biểu tạm thời của Khoa, giảng viên, của

từng sinh viên. Khoa chuyển thời khóa biểu giảng viên đến cho các giảng

viên và thông qua giáo viên chủ nhiệm chuyển cho sinh viên thời khóa biểu

cũng chính là kết quả đăng ký môn học chính thức của sinh viên (có ký

nhận)

• Sau khi nhận thời khóa biểu mọi sai sót phải được gửi về Phòng Đào tạo

thông qua Khoa trong vòng 1 tuần kể từ ngày Khoa nhận được thời khóa

biểu.

• Sau 1 tuần thực học, sinh viên được đăng ký thêm môn học hoặc hủy môn

học do những lý do khách quan, năng lực học tập (để hạn chế thay đổi này

giáo viên chủ nhiệm cần tư vấn kỹ cho sinh viên để sinh viên đăng ký số

môn học phù hợp khả năng). Sinh viên nộp đơn có ý kiến của giáo viên chủ

nhiệm, phiếu đăng ký môn học có chữ ký xác nhận của giáo viên chủ nhiệm

(trường hợp đăng ký thêm môn học) gửi về khoa.

• Phòng Đào tạo sẽ xử lý các thay đổi về thời khóa biểu trong tuần thực học

thứ 2. Sau đó sẽ trình Ban Giám hiệu ký duyệt và công bố thời khóa biểu

chính thức. Thời khóa biểu chính thức là văn bản chính thức của nhà trường,

là căn cứ để tính khối lượng giảng dạy của giảng viên và khối lượng học tập

của sinh viên, và là cơ sở để thực hiện các hoạt động khác.

• Đối với các trường hợp nộp phiếu đăng ký môn học bị trễ: 3 ngày sau thời

hạn đăng ký môn học quy định, sinh viên nộp phiếu đăng ký môn học và đơn

cho Trưởng Khoa xem xét, có ý kiến chuyển cho Phòng Đào tạo xử lý. Sau

hơn 3 ngày trễ nhất là 1 tuần sau thời hạn đăng ký môn học quy định sinh

viên nộp trễ có lý do chính đáng nộp đơn kèm theo bằng chứng và phiếu

đăng ký môn học có xác nhận của giáo viên chủ nhiệm nộp trực tiếp cho

Phòng Đào tạo xử lý. Sinh viên phải theo dõi và nhận kết quả xử lý tại nơi đã

nộp đơn.

2. Đăng ký môn học:

- Sinh viên căn cứ vào các thông tin sau đây để đăng ký môn học: quy chế , quy

định về đào tạo, thời khóa biểu dự kiến để biết các thông tin về môn học, thời

khóa biểu từng môn học, lịch đăng ký môn học; kết quả học tập để chọn đăng ký

môn học phù hợp với khả năng.

- Việc đăng ký môn học của sinh viên được xếp theo thứ tự ưu tiên như sau:

• Đăng ký các môn học đúng chương trình của khóa – ngành học.

• Ưu tiên cho trả nợ môn học để kịp tiến độ chuyển tiếp (đối với chương trình

liên kết).

• Đăng ký các môn học học vượt đối với sinh viên khá giỏi.

- Sinh viên cần phải nhận sự tư vấn của giáo viên chủ nhiệm để được tư vấn về kế

hoạch học tập, đăng ký môn học cho phù hợp. Sinh viên phải chịu trách nhiệm

với việc đăng ký môn học của mình không chấp thuận mọi trường hợp chỉnh sửa

do nhờ người khác đăng ký hộ.

- Sau khi cân nhắc và ghi chính xác đầy đủ nội dung yêu cầu của phiếu đăng ký

môn học, sinh viên ký và ghi rõ học tên vào phiếu đăng ký môn học và nộp cho

giáo viên chủ nhiệm, GVCN phải ký duyệt và chịu trách nhiệm liên đới về tính

chính xác, hợp lý của phiếu ĐKMH.

- Quy trình chỉnh sửa thời khóa biểu sinh viên hoặc đăng ký bổ sung

2.1 Chỉnh sửa thời khóa biểu:

Khi nhận được TKB của mình sinh viên phải kiểm tra các thông tin trên thời khóa

biểu: số môn học đăng ký, học phí v.v.v . . nếu có bất kỳ sai sót nào cần chỉnh sửa,

hoặc khiếu nại về thời khóa biểu sinh viên phải nộp đơn về Khoa trong vòng 3 ngày

kể từ ngày công bố thời khóa biểu. Khoa có nhiệm vụ kiểm tra (thông qua giáo viên

chủ nhiệm) và cho ý kiến xử lý vào đơn của sinh viên, sau đó tổng hợp gửi về Phòng

Đào tạo để xử lý. Phòng Đào tạo sẽ chuyển kết quả xử lý về Khoa để trả lời cho sinh

viên.

2.2 Quy trình đăng ký bổ sung hoặc hủy môn học:

Trong 1 tuần thực học đầu tiên, căn cứ thời khóa biểu, khả năng và điều kiện học tập

sinh viên có thể xin bổ sung hoặc hủy môn học, theo quy định và quy trình như sau:

2.2.1 Quy đinh về việc bổ sung hoặc hủy các môn học:

- Không được hủy các môn học thực hành thí nghiệm, đồ án môn học.

- Không được hủy môn học tiên quyết.

- Được đăng ký thêm môn học chỉ được chấp nhận nếu còn chỗ, hoặc bố trí được

giảng viên hướng dẫn: thực hành, thí nghiệm.

2.2.2 Học cùng lúc hai chương trình:

- Sinh viên học cùng lúc hai chương trình là sinh viên có nhu cầu đăng ký học

thêm một chương trình thứ hai để khi tốt nghiệp được cấp hai văn bằng.

- Điều kiện để học cùng lúc hai chương trình:

• Ngành đào tạo chính ở chương trình thứ hai phải khác ngành đào tạo chính ở

chương trình thứ nhất;

• Sau khi đã kết thúc học kỳ thứ nhất năm học đầu tiên của chương trình thứ

nhất;

• Sinh viên không thuộc diện xếp hạng học lực yếu ở chương trình thứ nhất

- Sinh viên đang học thêm chương trình thứ hai, nếu rơi vào diện bị xếp hạng học

lực yếu của chương trình thứ hai, phải dừng học thêm chương trình thứ hai ở học

kỳ tiếp theo.

- Thời gian tối đa được phép học đối với sinh viên học cùng lúc hai chương trình

là thời gian tối đa quy định cho chương trình thứ nhất, quy định tại Khoản 3

Điều 2 của Quy chế này. Khi học chương trình thứ hai, sinh viên được bảo lưu

điểm của những môn học có nội dung và khối lượng kiến thức tương đương có

trong hai chương trình và đã được học ở một chương trình.

- Sinh viên chỉ được xét tốt nghiệp chương trình thứ hai, nếu có đủ điều kiện tốt

nghiệp ở chương trình thứ nhất.

2.2.3 Điều kiện chuyển ngành, chuyển chương trình:

- Điều kiện đề chuẩn ngành, chuyển chương trình: Sinh viên được phép xin

chuyển ngành, chương trình học trong khoảng thời gian 4 học kỳ đầu.

- Điều kiện để sinh viên xin chuyển từ chương trình đào tạo do trường Đại Học

Quốc Tế cấp bằng sang chương trình liên kết:

• Nếu sinh viên xin chuyển tại thời điểm học kỳ I năm thứ nhất, nhà trường sẽ

xem xét dựa trên các cơ sở sau đây:

▪ Chỉ tiêu của ngành muốn chuyển đổi phải còn chỉ tiêu.

▪ Điểm trúng tuyển của sinh viên phải ≥ điểm chuẩn của ngành thuộc

chương trình muốn chuyển đổi.

• Nếu sinh viên xin chuyển tại thời điểm sau khi học ít nhất một học kỳ, nhà

trường sẽ dựa trên các cơ sở sau đây:

▪ Chỉ tiêu của ngành muốn chuyển đổi còn phải còn chỉ tiêu.

▪ Sinh viên không bị các hình thức kỷ luật, xử lý học vụ.

- Điều kiện để sinh viên xin chuyển từ chương trình liên kết do trường Đại học

nước ngoài cấp bằng sang chương trình do trường Đại học Quốc tế cấp bằng:

sinh viên chỉ được xem xét chuyển đổi sau khi đã hoàn tất ít nhất 1 học kỳ, nhà

trường sẽ căn cứ vào các cơ sở sau đây để xem xét trình Đại học Quốc gia quyết

định:

• Chỉ tiêu của ngành muốn chuyển đổi phải còn chỉ tiêu.

• Điểm trúng tuyển của sinh viên phải ≥ điểm chuẩn của ngành thuộc chương

trình muốn chuyển đổi.

2.2.4 Điều kiện để chuyển trường

- Sinh viên được xét chuyển trường nếu có các điều kiện sau đây:

• Trong thời gian học tập, nếu gia đình chuyển nơi cư trú hoặc sinh viên có

hoàn cảnh khó khăn, cần thiết phải chuyển đến trường gần nơi cư trú của gia

đình để thuận lợi trong học tập;

• diện nằm ngoài vùng tuyển quy định của trường xin chuyển đến; Sinh viên

năm thứ nhất và năm cuối khóa;

• Sinh viên đang trong thời gian bị kỷ luật từ cảnh cáo trở lên.

- Thủ tục chuyển trường:

• Sinh viên chuyển trường phải làm hồ sơ xin chuyển trường theo quy định

của nhà trường;

• Hiệu trưởng trường có sinh viên xin chuyển đến quyết định tiếp nhận hoặc

không tiếp nhận; quyết định việc học tập tiếp tục của sinh viên, công nhận

các học môn học mà sinh viên chuyển đến được chuyển đổi kết quả và số

môn học phải học bổ sung, trên cơ sở so sánh chương trình ở trường sinh

viên xin chuyển đi và trường xin chuyển đến.

A5. ĐÁNH GIÁ VÀ XỬ LÝ KẾT QUẢ HỌC TẬP

Trong buổi lên lớp đầu tiên của môn học, cán bộ giảng dạy phụ trách môn học sẽ

thông báo cho sinh viên biết nội dung tóm tắt của môn học, vị trí môn học trong

CTĐT, cách thức học, và các hình thức được áp dụng để đánh giá kết quả học tập

môn học liên quan (các tiêu chuẩn, phân lượng của các điểm thành phần trong kết

quả đánh giá cuối cùng, số lần kiểm tra trong học kỳ, các điều kiện để được dự thi

cuối học kỳ và những vấn đề liên quan đến kỳ thi cuối học kỳ), lịch thi, kiểm tra và

thí nghiệm.

Kiểm tra thường kỳ và thi kết thúc môn học

1. Điều kiện để được tham dự kiểm tra và thi kết thúc môn học

1.1 Mỗi môn học có thể có nhiều kỳ kiểm tra nhưng chỉ tổ chức một kỳ kiểm tra

chính vào giữa học kỳ và một kỳ thi kết thúc môn học vào cuối học kỳ. Không có kỳ

kiểm tra lại hoặc kỳ thi lại cho những sinh viên đã dự thi không đạt yêu cầu hoặc

vắng thi.

1.2 Điểm để đánh giá kết quả học tập của môn học bao gồm điểm kiểm tra giữa kỳ,

điểm thực hành và bài tập (nếu có) và điểm thi kết thúc môn học.

1.3 Vắng mặt khi lên lớp:

• Sinh viên (SV) vắng mặt quá 20% số giờ trên lớp sẽ bị cấm dự thi cuối học

kỳ, và phải nhận điểm không (0) cho môn học đó.

• Nếu sinh viên vắng mặt (có và không có lý do) quá 20% số giờ thực hành

(bài tập, thí nghiệm, seminar, ...) và 50% số kỳ kiểm tra trong học kỳ sẽ phải

nhận điểm không (0) cho môn học thí nghiệm và cho phần kiểm tra của môn

học đó.

1.4 Sinh viên phải hoàn thành tất cả các bài thí nghiệm và thực tập, tham dự các kỳ

kiểm tra và thi cuối học kỳ.

• Sinh viên vắng mặt trong các giờ thí nghiệm hay thực tập, đều phải có đơn

kèm theo chứng lý trình bày với cán bộ giảng dạy phụ trách môn học. Thời

hạn phải trình bày muộn nhất là 01 ngày sau ngày vắng mặt (không kể ngày

chủ nhật và lễ). Nếu lý do được công nhận thì sinh viên được xếp cho làm bù

theo sự sắp xếp của giảng viên hướng dẫn thực hành. Nếu sinh viên không

trình bày hoặc lý do vắng mặt không được công nhận thì phải nhận điểm

không (0) cho các buổi đã vắng.

• Sinh viên vắng mặt trong các giờ kiểm tra và thi phải có đơn kèm theo chứng

lý trình bày với Trưởng Khoa quản lý sinh viên, trong thời hạn muộn nhất 01

ngày sau ngày kiểm tra hay thi. Nếu sinh viên không trình bày hoặc lý do

vắng mặt không được công nhận thì phải nhận điểm không (0) cho kỳ kiểm

tra hay thi đã vắng. Chỉ sau khi Trưởng Khoa quản lý sinh viên duyệt đơn và

phòng Đào tạo ra quyết định công nhận thì sinh viên được xem là vắng có lý

do chính đáng, và trong bảng điểm ghi điểm chưa hoàn tất (điểm I).

1.5 Đến trễ giờ kiểm tra, thi: Sinh viên đến trễ giờ thi từ 15 phút trở lên sẽ không

được vào phòng thi và sẽ nhận điểm không (0) đối với môn kiểm tra hoặc môn thi

đó.

1.6 Lịch thi, kiểm tra và thí nghiệm:

• Lịch kiểm tra môn học hoặc lịch thi sẽ được công bố ở Phòng Đào tạo và

Khoa ít nhất 2 tuần trước thời gian thi hoặc kiểm tra.

• Lịch thí nghiệm chung sẽ được công bố cụ thể ở các Phòng thí nghiệm, Văn

phòng Khoa quản lý sinh viên. Sinh viên có nhiệm vụ theo dõi để biết lịch

học vụ của mỗi học kỳ.

2. Cách tính điểm môn học, điểm trung bình học tập và xếp loại kết quả học tập

Cách tính điểm trung bình học tập: điểm trung bình học tập của mỗi học kỳ, mỗi

năm học, hoặc mỗi khóa học được tính theo công thức sau:

ni là số tín chỉ của môn học thứ i

N là tổng số môn học

2.1 Điểm trung bình học kỳ (xem Mục 6.1 -Điều 2) sau từng học kỳ, từng năm học

được dùng để xét học bổng, khen thưởng, được lấy theo kết quả thi kết thúc môn

học ở lần thi thứ nhất.

2.2 Điểm trung bình chung tích lũy (xem Mục 6.2 -Điều 2) được dùng để xét học

cùng một lúc ở nhiều ngành, phân loại kết quả học tập của khóa học, xét tốt nghiệp,

ngừng học hay thôi học, được tính theo điểm thi cao nhất trong các lần thi.

2.3 ĐTBHK, ĐTBTL được tính theo thang điểm 4 hoặc theo thang điểm 100 và

được làm tròn đến 1 chữ số thập phân.

2.4 Xếp loại sinh viên: ĐTBTL là cơ sở để xếp loại học lực sinh viên theo bảng như

sau:

Giỏi 80 <= GPA < 90 A-

2.5 Không tính kết quả thi các môn học Giáo dục quốc phòng vào điểm trung bình

học kỳ, năm học hay khóa học. Việc đánh giá kết quả và điều kiện cấp chứng chỉ đối

với môn học này theo qui định của Bộ Giáo dục và Đào tạo.

3 Thông báo kết quả

3.1 Tất cả các kết quả của các hoạt động học tập đều được thông báo cho sinh viên

tại Văn phòng Khoa quản lý sinh viên đối với các môn học thuộc nhóm ngành và tại

Phòng Đào tạo đối với môn học chung cho toàn trường. Điểm kiểm tra được CBGD

thông báo tại lớp. Các điểm kiểm tra và kết quả học kỳ đối với mỗi môn học đều

phải được CBGD gởi đến Phòng Đào tạo hoặc Khoa, chậm nhất là 15 ngày sau ngày

thi của môn học đó. Thời gian dán thông báo là 5 ngày (không kể ngày lễ và chủ

nhật)

3.2 Mỗi học kỳ, phòng Đào tạo thông báo cho sinh viên kết quả điểm của các môn

học trong học kỳ, ĐTBHK và ĐTBTL tính đến hết học kỳ đó.

3.3 Trong quá trình học, sinh viên có thể đề nghị trường cấp hoặc gởi bảng điểm đến

nơi cần thiết theo yêu cầu của sinh viên. Để thực hiện công việc này, sinh viên phải

làm đơn và nộp lệ phí. Mức lệ phí do trường qui định.

3.4 Đối với sinh viên bị cảnh cáo học vụ và bị buộc thôi học, Trường gửi thư báo

cho gia đình sinh viên. Riêng sinh viên bị buộc thôi học Trường còn gửi thư báo về

địa phương nơi cư trú chậm nhất là 01 tháng sau khi có quyết định buộc thôi học.

4 Chấm phúc tra

Sinh viên có quyền đề nghị chấm phúc tra kết quả thi. Đơn xin phúc tra kết quả thi,

kiểm tra phải được gởi đến phòng Đào tạo hoặc Khoa trong vòng 1 tuần kể từ ngày

công bố kết quả. Tất cả các đề nghị phúc tra kết quả đều phải đóng lệ phí, mức lệ phí

do trường qui định.

5 Xử lý kỷ luật đối với sinh viên

5.1 Khiển trách áp dụng đối với những sinh viên phạm lỗi một lần: nhìn bài của bạn,

trao đổi với bạn (hình thức này do cán bộ coi thi quyết định tại biên bản được lập).

Sinh viên bị khiển trách trong khi thi môn nào sẽ bị trừ 25% số điểm thi của môn đó.

5.2 Cảnh cáo đối với các sinh viên vi phạm một trong các lỗi sau đây:

• Đã bị khiển trách một lần nhưng trong giờ thi môn đó vẫn tiếp tục vi phạm

Quy chế.

• Mang vào phòng thi vũ khí, chất gây nổ, gây cháy, bia, rượu hoặc các vật

dụng gây nguy hại khác.

• Trao đổi bài làm hoặc giấy nháp cho bạn.

• Chép bài của người khác. Những bài thi đã có kết luận là giống nhau thì xử

lý như nhau. Nếu người bị xử lý có đủ bằng chứng chứng minh mình bị quay

cóp thì trường có thể xem xét giảm từ mức kỷ luật cảnh cáo xuống mức

khiển trách. Người bị kỷ luật cảnh cáo trong khi thi môn nào sẽ bị trừ 50% số

điểm thi của môn đó. Hình thức kỷ luật cảnh cáo do cán bộ coi thi lập biên

bản, thu tang vật và ghi rõ hình thức kỷ luật đã đề nghị trong biên bản.

5.3 Đình chỉ thi đối với các thí sinh vi phạm một trong các lỗi sau đây:

• Đã bị cảnh cáo một lần nhưng trong giờ thi môn đó vẫn tiếp tục vi phạm Quy

chế.

• Khi vào phòng thi còn mang tài liệu, phương tiện kỹ thuật thu, phát, truyền

tin, ghi âm, ghi hình, thiết bị chứa đựng thông tin có thể lợi dụng để làm bài

thi.

• Đưa đề thi ra ngoài hoặc nhận bài giải từ ngoài vào phòng thi.

• Viết vẽ những nội dung không liên quan đến bài thi.

• Có hành động gây gổ, đe dọa cán bộ có trách nhiệm trong kỳ thi hay đe dọa

sinh viên khác. Hình thức đình chỉ thi do cán bộ coi thi lập biên bản, Điều

hành thi quyết định.

• Sinh viên bị kỷ luật đình chỉ thi trong khi thi môn nào sẽ bị điểm không (0)

môn đó; phải ra khỏi phòng thi ngay sau khi có quyết định của Điều hành thi.

5.4 Đối với sinh viên đi thi hộ hoặc nhờ người thi hộ đều bị kỷ luật ở mức buộc thôi

học.

Xử lý kết quả học tập:

1. Học lại một môn học

1.1 Nếu sinh viên không đạt yêu cầu đối với một môn học bắt buộc, thì phải đăng ký

học lại môn học đó khi môn học được tổ chức giảng dạy.

1.2 Đối với môn học tự chọn không đạt, sinh viên phải

• Đăng ký học lại môn học đó, hoặc

• Đăng ký học lại môn học khác trong số các môn học qui định của môn học

tự chọn.

2. Cải thiện điểm

Đối với các môn học có kết quả đạt, sinh viên được phép đăng ký học lại để cải

thiện ĐTBCTL nếu có nguyện vọng. Điểm cuối cùng của môn học cải thiện là điểm

cao nhất trong các lần thi. Việc đăng ký học cải thiện điểm phải theo qui định của

trường.

3. Cảnh cáo học vụ

3.1 Hằng năm, Hội đồng học vụ trường sẽ xét xử lý học vụ sau học kỳ 1 và sau học

kỳ hè. Kết quả học tập của học kỳ hè sẽ được nhập vào học kỳ 2 của năm học tương

ứng khi xét xử lý học vụ.

3.2 Sinh viên vi phạm các qui định sau sẽ bị cảnh cáo học vụ:

• Không đạt số tín chỉ tối thiểu quy định trong một học kỳ.

• Có ĐTBTL dưới 35 theo thang điểm 100.

• Có ĐTBTL của 2 học kỳ liên tiếp dưới 50 theo thang điểm 100.

Thời hạn cảnh cáo học vụ kéo dài trong 1 học kỳ chính tiếp theo. Sinh viên sẽ được

xóa tên trong danh sách cảnh cáo học vụ trước thời hạn, nếu có kết quả học tập ở

cuối học kỳ kế không vi phạm Mục 3.2 của Điều này. Đến cuối thời hạn cảnh cáo

học vụ nếu kết quả học tập của sinh viên vẫn vi phạm Mục 3.2 của Điều này thì sẽ bị

cảnh cáo học vụ lần thứ hai. Khi đến cuối thời hạn cảnh cáo học vụ lần thứ hai nếu

kết quả vẫn rơi vào mục 3.2 thì sẽ bị xử lý theo Mục 4.2 của Điều này.

4. Đình chỉ học tập và xóa tên khỏi danh sách sinh viên

4.1 Trường sẽ xóa tên khỏi danh sách sinh viên nếu sinh viên gửi đơn xin thôi học

và được Trường ra quyết định cho phép nghỉ học.

4.2 Trường sẽ ra quyết định đình chỉ học tập và xóa tên khỏi danh sách sinh viên nếu

sinh viên rơi vào một trong các trường hợp sau:

• Đã hết thời gian tối đa được phép học.

• Đã tự ý bỏ học từ một học kỳ chính trở lên.

• Sau thời hạn bị cảnh cáo học vụ hai lần vẫn vi phạm các qui định tại Mục 3.2

của Điều này.

• Vi phạm quy chế học vụ và các qui định khác của trường theo đến mức buộc

thôi học.

• Không làm thủ tục đăng ký môn học.

• Không đóng học phí theo đúng qui định của trường.

• Không thỏa các điều kiện để được cấp bằng trong khoảng thời gian qui định

của khóa học.

• Đã đăng ký và được trường xếp lớp học, nhưng sinh viên không đến nhận

phiếu kết quả đăng ký môn học (chậm nhất là trong tuần lễ đầu tiên của học

kỳ).

5. Điều kiện để được học tiếp

Sinh viên không thuộc diện qui định ở Mục 4 của Điều này thì được đăng ký học

tiếp.

6. Chế độ ưu tiên cho việc xét học tiếp, thôi học

Sinh viên thuộc diện ưu tiên 1 và 2 theo Quy chế tuyển sinh đại học được giảm tiêu

chuẩn xét học tiếp và thôi học như sau:

Khu vực 1 Khu vực 2

Ưu tiên 1 Giảm 0.6 điểm Giảm 0.4 điểm

Ưu tiên 2 Giảm 0.4 điểm Giảm 0.2 điểm

Sinh viên thuộc diện ưu tiên 1 được ngừng học để củng cố kiến thức tối đa không

quá hai năm cho toàn khóa học đối với các ngành có thời gian đào tạo từ 3-4 năm,

không quá ba năm cho toàn khóa học đối với các ngành có thời gian đào tạo từ 5-6

năm. Các năm ngừng học này không được kế tiếp nhau.

Điều kiện để nhận đồ án, luận án tốt nghiệp, thi cuối khóa:

Sinh viên không bị cảnh cáo học vụ và các sinh viên đã hoàn số môn học và tín chỉ

đã quy định, không bị nợ môn học.

Điều kiện xét và công nhận tốt nghiệp:

Sinh viên sẽ được công nhận tốt nghiệp nếu hội đủ các điều kiện sau: (tính cho toàn

khoá học)

- Cho đến thời điểm xét tốt nghiệp không bị kỷ luật đình chỉ học tập hoặc

không bị truy cứu hình sự.

- Đạt tất cả các môn học qui định trong CTĐT cho ngành đào tạo.

- Có các chứng chỉ Giáo dục Quốc phòng.

Sau mỗi học kỳ, Hội đồng xét tốt nghiệp của Khoa và Trường sẽ xem xét để lập ra

danh sách những người đủ điều kiện tốt nghiệp. Dựa trên đề nghị của Hội đồng xét

tốt nghiệp, Hiệu trưởng ra quyết định công nhận tốt nghiệp cho những sinh viên đạt

điều kiện theo qui định.

Mỗi sinh viên tốt nghiệp được Trường cấp cho 01 bằng tốt nghiệp và 01 bảng điểm

tốt nghiệp, trong bảng điểm có ghi lĩnh vực chuyên sâu hoặc chuyên môn phụ nếu

có.

Những sinh viên không đủ điều kiện cấp bằng tốt nghiệp, nhưng hết thời gian tối đa

cho phép học ở trường thì được cấp bảng điểm, chứng nhận kết quả các môn học đã

học trong thời gian theo học tại trường. Ở mỗi phần kiến thức, sinh viên đăng ký và

thi theo qui định của trường.

Kết quả thi viết được công bố chậm nhất là 10 ngày sau khi thi.

Xếp hạng tốt nghiệp:

1. Loại tốt nghiệp được xác định căn cứ vào ĐTBTL của các môn học qui định cho

ngành đào tạo. Cách phân loại được thực hiện theo qui định tại Mục 2.4-Điều 10.

2. Đối với loại giỏi và loại xuất sắc, nếu rơi vào một trong các trường hợp sau thì

loại tốt nghiệp bị giảm một cấp:

• Thời gian học tại trường của sinh viên vượt quá một học kỳ so với tiến độ

quy định đối với khóa học.

• Có số tín chỉ phải thi lại vượt quá 5% so với tổng số tín chỉ qui định cho toàn

khóa học.

• Đã bị kỷ luật trong thời gian học từ mức cảnh cáo ở cấp trường trở lên.

Thu hồi văn bằng hay chứng chỉ đã cấp:

Các văn bằng chứng chỉ sẽ bị thu hồi trong các trường hợp sau:

• Bị phát hiện có chứng cớ là có sự gian lận trong kỳ thi tuyển sinh và quá

trình học đại học của người được cấp văn bằng hay chứng chỉ.

• Bị phát hiện có chứng cớ là có sự vi phạm “Qui chế văn bằng bậc đại học”

do ĐHQG-HCM ban hành.

Hiệu trưởng sẽ ra quyết định hủy bỏ việc công nhận tốt nghiệp, thu hồi văn bằng

hoặc chứng chỉ trong các trường hợp trên và đương sự sẽ bị xử lý theo luật định.

Chỉnh sửa, bổ sung:

Qua quá trình hoạt động, các điều khoản sẽ được điều chỉnh, bổ sung cho phù hợp

với thực tế. Việc điều chỉnh, bổ sung các điều khoản của quy chế do Hiệu trưởng

quyết định.

I/ Học kỳ 1 và học kỳ 2

Học bổng của học kỳ đang học được xét trên điểm của học kỳ kế trước (gọi là học

kỳ được dùng xét học bổng).

- Điểm học kỳ 3 được tách riêng với điểm học kỳ 2.

1/ Điều kiện để được xét học bổng

- Sinh viên phải hoàn tất môn AE1 tính đến học kỳ dùng xét học bổng.

- Trong học kỳ được dùng xét học bổng:

+ Số tín chỉ đăng ký > 12 tín chỉ.

+ Điểm tổng kết từng môn học >50 (kể cả môn Giáo dục thể chất, Giáo dục

quốc phòng và các môn không tính điểm trung bình).

+ Điểm trung bình học kỳ >70.

- Không cấp học bổng cho các sinh viên đã có quyết định nghỉ học hoặc đang tạm

dừng ở học kỳ nhận học bổng. Đối với sinh viên xin tạm dừng, nếu sinh viên nhập

học lại đúng quy định thì sẽ được nhận học bổng.

- Trong cùng một học kỳ, sinh viên đã nhận học bổng toàn phần hoặc bán phần sẽ

không được xét học bổng khuyến khích học tập.

- Trong cùng một học kỳ, nếu sinh viên đạt cùng lúc học bổng tuyển sinh và học

bổng khuyến khích học tập thì sinh viên sẽ được nhận loại học bổng có giá trị cao

nhất.

2/ Công thức tính điểm trung bình học bổng

Điểm xét học bổng = điểm trung bình học kỳ X (0.7 + 0.3 X (Tín chỉ tích lũy

học kỳ/ tín chỉ tối đa))

Trong đó:

+ Tín chỉ tích lũy học kỳ không tính tín chỉ của các môn nhận điểm I, NA, miễn

học, bảo lưu, môn giáo dục thể chất, môn giáo dục Quốc phòng và các môn

không tính điểm trung bình.

+ Tín chỉ tối đa là 24 tín chỉ. Đối với những sinh viên đăng ký nhiều hơn 24 tín

chỉ thì được tính 24 tín chỉ.

3/ Số lần xét nhận học bổng

Đối với sinh viên chương trình không liên kết: mỗi sinh viên chỉ được xét học bổng

tối đa 8 lần theo đúng tiến độ của khóa trúng tuyển, trong đó:

• Một lần xét điểm tuyển sinh.

• Tối đa bảy lần xét điểm học tập ở học kỳ kế trước.

Đối với sinh viên chương trình liên kết: mỗi sinh viên chỉ được xét học bống tối đa

4 lân theo đúng tiến độ của khóa trúng tuyển, trong đó:

• Một lần xét điểm tuyển sinh.

• Tối đa ba lần xét điểm học kỳ kế trước.

(Riêng sinh viên chương trình Auckland chỉ xét toi đa hai lần theo đủng tiến độ

của khóa trúng tuyển).

4/ Chỉ tiêu và giá trị học bổng

- Số suất học bổng: 40 suất phân bổ theo phần trăm tỷ lệ tổng số sinh viên của

từng khoa, ngành, khóa học (không tính cho những sinh viên đã nhận học bổng

toàn phần hoặc bán phần).

- Phần trăm được làm tròn theo quy tắc:

• Nếu số sinh viên của nhóm xét học bổng>100, quy tắc làm tròn được sử

dụng, giá trị > 0.5 làm tròn 1.

* Nếu số sinh viên của nhóm xét học bổng < 100. quy tắc làm tròn trên được

sứ dụng, giá trị > 0.1 làm tròn 1.

- Giá trị học bổng: 600 USD/suất, học bổng quy đổi sang VNĐ (tỷ giá quy đổi

được nhà trường quy định theo từng học kỳ).

5/ Cách gán loại học bổng cho sinh viên

- Cách chọn sinh viên thuộc học bổng loại nào là lấy theo điểm trung bình xét học

bổng từ cao xuống thấp trong từng nhóm xét học bổng, xét từ nhóm Giỏi xuống

Khá cho đến khi hết chỉ tiêu.

- Vì tính công bằng cho sinh viên:

+ Nếu sinh viên có cùng điểm trung bình xét học bổng với của sinh viên đạt

học bổng loại Giỏi (có điểm trung bình xét học bổng nhỏ nhất) thì cũng sẽ đạt học

bổng loại Giỏi (có thể số % sẽ dôi lên).

+ Nếu sinh viên có cùng điểm trung bình xét học bổng với của sinh viên đạt

học bổng loại Khá (có điểm trung bình xét học bổng nhỏ nhất) thì cũng sẽ đạt học

bổng loại Khá (có thể số % sẽ dôi lên).

II/ Học kỳ 3

1/ Điều kiên để được xét học bổng

- Học bồng xét cho những sinh viên có đăng ký môn học trong học kỳ 3.

- Sinh viên phải hoàn tất môn AE1 tính đến học kỳ dùng xét học bổng.

- Trong học kỳ 3:

+ Số tín chỉ đăng ký > 6 tín chỉ.

+ Điếm tong kết từng môn học > 50 (kể cả môn Giáo dục thể chất, Giáo dục

quốc phòng và các môn không tính điểm trung bình).

+ Sinh viên phải có GPA > 70.

- Không cấp học bổng cho các sinh viên đã có quyết định nghỉ học hoặc đang tạm

dừng ở học kỳ nhận học bổng. Đối với sinh viên xin tạm dừng, nếu sinh viên nhập

học lại đúng quy định thì sẽ được nhận học bổng.

2/ Công thức tính điểm trung bình học bổng

- Tương tự như học kỳ 1 và học kỳ 2.

- Tín chỉ đăng ký tối đa là 12 tín chỉ, sinh viên đăng ký nhiều hơn 12 tín chỉ được

tính như 12 tín chỉ.

3/ Số lần xét nhận học bổng

Theo đúng tiến độ của khóa trúng tuyển:

- Đối với sinh viên chương trình không liên kết: mỗi sinh viên chỉ được xét học

bổng học kỳ 3 tối đa 3 lần.

- Đối với sinh viên chương trình liên kết: mỗi sinh viên chỉ được xét học bổng học

kỳ 3 tối đa 1 lần.

4/ Chỉ tiêu và giá trị học bổng

- Số suất học bổng: 20 suất phân bổ theo phần trăm tỷ lệ tổng số sinh viên đăng ký

môn học trong học kỳ 3, của từng khoa, ngành, khóa học.

Phần trăm được làm tròn theo quy tắc:

• Nếu số sinh viên của nhóm xét học bổng > 100, quy tắc làm tròn được sử

dụng, giá trị > 0.5 làm tròn 1.

• Nếu số sinh viên của nhóm xét học bổng < 100, quy tắc làm tròn trên được sử

dụng, giá trị > 0 . 1 làm tròn 1.

- Giá trị học bổng: 300 USD/suất, học bổng quy đổi sang VNĐ (tỷ giá quy đổi

được nhà trường quy định theo từng học kỳ).

5/ Cách gán loại học bổng cho sinh viên

Tương tự như học kỳ 1 và học kỳ 2.
