System requirements • This is a web based system. • Recommended web browser: - Internet Explorer 9 and above. - or Google Chrome. • Require network access: - via DAMA network (proxy enabled). - or using Internet.

Step by step 1) Enter username and password. Masukkan nama pengguna dan kata laluan.

2) Click Login. Klik pada Login.

3) You will be redirect to home page. Anda akan dibawa ke halaman utama.

2) Re-enter the new password. Masukkan semula kata laluan baru.

3) Click Change Password. Klik Change Password.

How to answer the questions 1) Each selection of answer will be auto saved

and can be update before the end of due date.

Setiap pilihan jawapan akan disimpan secara automatik dan boleh dikemaskini sebelum tarikh tutup. 2) If you select “Good” as the answer. Jika anda memilih “Good” sebagai jawapan. “Good” means YES. i) You know/aware. ii) You have done it/complied. iii) System is already in place. “Good” bermaksud YA. i) Anda tahu/sedar mengenainya. ii) Anda sudah melakukannya/mematuhinya. iii) Sudah adanya sistem.

How to answer the questions 1) If you select “Poor” as the answer,

you are require to specify grade of poor level before you can proceed to next questions.

Jika anda memilih “Poor” sebagai jawapan, anda hendaklah memilih “grade of poor level” sebelum anda boleh menjawab soalan seterusnya.

“Poor” means NO/NOT DONE. “Poor” bermaksud TIDAK/TIDAK DILAKSANAKAN. 2) “This issue is not recognized” means (i) you are not aware of the requirements. (ii) you do not know it is not allowed. (iii) you do not know the law/regulations/ policies/procedure/system. “This issue is not recognized” bermaksud (i) anda tidak mengetahui akan keperluannya. (ii) anda tidak tahu ianya tidak dibenarkan. (iii) anda tidak tahu mengenai undang- undang/peraturan/polisi/prosedur/sistem tersebut.

How to answer the questions 3) “Policies/procedure are not established” means (i) There are no company policies/procedures. (ii) There are no appointed personnel. “Policies/procedure are not established”

bermaksud (i) Tiadanya polisi/prosedur syarikat. (ii) Tiadanya orang yang ditugaskan.

4) “ No actions are taken” means (i) The policies/procedures are not implemented. (ii) The policies/procedures are not followed. “No actions are taken” bermaksud (i) Polisi/prosedur tidak dilaksanakan. (ii) Polisi/prosedur tidak diikuti. 5) Select “Others” under grade of poor level if none of the above answer choices is applicable. Please fill in the comment box. Example of comment: I don’t understand the questions. Pilih “Others” di bawah “grade of poor level” jika tiada pilihan jawapan yang sesuai. Sila isi ruang “comment box”. Contoh komen: Saya tidak memahami soalan tersebut.
