Casey and Hannah are our Occupational Therapists,

who are trained to assist people to overcome

limitations caused by injury or illness, psychological or

emotional difficulties, developmental delay of the

effects of ageing.

The goal is to maximise your client’s skills for living.

Occupational Therapists can help your client to maintain your

independence by assisting you to:

Achieve a safer, more accessible home environment

How To Make A Referral For Your Clients

Social Work services operate from a multidisciplinary perspective to ensure that clients and families needs are met in a person centred manner. Social workers can provide short term counselling, practical and emotional support and information to help clients through a difficult time. They can also refer them to other support services.

Social Work services aim to improve well-being in the community by offering supportive counselling,

practical support, referrals to other services and advocacy for individuals or groups.

Services are provided on a one-to-one basis and can assist with a wide range of issues including, however

not limited to:

Capacity to adjust to illness and associated lifestyle changes

Interfamily relationships and dynamics and conflict between client and family members regarding

decisions

Grief and Loss

Formal and informal support networks

Accommodation

REDHS social workers are trained in Advance Care Planning and can support your client to document

their future healthcare wishes.

There is no cost associated with this service however appointments are necessary.

A nurse can help your client manage a health condition at home. Examples might be changing

dressings on wounds, or perhaps monitoring your blood pressure.

Nursing care is in place to assist your client with minor health conditions and works in

conjunction with other assistance available to your clients through the health system.

How To Make A Referral For Your Client

REDHS District Nursing Team provides the community

based nursing care and health advice to the

communities of Rochester, Elmore and Goornong for

people who may be unwell, aged and frail, people

with disabilities and their carers

Wound care and dressings, medication monitoring and assistance, Blood pressure and general health

monitoring. Injection of daily insulin (depending on location of residence), and blood glucose

monitoring. Clexaine - anticoagulant injections pre and post operatively. Administration of eye drops

twice daily. (We are unable to offer an evening service for medication administration) Palliative care.

Referrals to other allied health clinicians /services following assessments Eg; Podiatry, Physio, OT.

Education to clients and carers where required. Indwelling and suprapubic catheter changes. Pathology

collection. Continence advice.

If wound care is provided there is a fee for the wound care products supplied.

RURAL WITHDRAWAL NURSE

Our Rural Drug Withdrawal Nurse can help people to overcome the difficulties of drug dependence by referral to

residential detox and rural (home based detox). Liaison with GPs and other health care professionals, as well as

supportive others, is crucial for the recovery of an individual.

In consultation with the client, Tony can develop a specific case plan for the person to develop strategies and

realistic goals for the person to aid recovery and maintain a drug free lifestyle, or strategies aimed at harm

minimisation/reduction as appropriate.

How To Make A Referral For Your Clients

Tony is a Rural Drug Withdrawal Nurse who is

committed to helping people with substance

dependencies to overcome the difficulties in

remaining drug free, with help in the areas of

holistic health, which encompasses emotional,

physical and spiritual health.

Conduct comprehensive client assessments including alcohol, tobacco and other drug specific information to determine appropriate intervention.

Provide withdrawal advice to facilitate the most appropriate option for withdrawal. To liaise with medical practitioners and other health care providers where medical and other specialised interventions are required for withdrawal management.

Provide advice and support as appropriate for hospital staff where clients are undertaking hospital based withdrawal.

Facilitate referral to residential withdrawal facilities as appropriate.

Provide an on call, out of hour’s service for clients and support people undertaking withdrawal.

Utilise appropriate interventions such as motivational interviewing, cognitive behavioural approaches and relapse prevention strategies to facilitate client engagement and achievement of goals.

Develop an Individual Treatment Plan with each client, identifying significant treatment goals and strategies for achieving goals.

How To Make A Referral For Your Clients

Transition care is for older people who have been in

hospital, but need more help to recover and time to make a

decision about the best place for them to live in the longer

term. Transition care may be provided either in your clients

own home or in a ‘live-in’ setting. The REDHS live in setting

is in the hospital acute ward. Your clients can only access

transition care directly from hospital.

Care Support Program is a new program to REDHS

providing outings and giving Carers the much needed

time off and short break.

Transport is provided to and from the activities.

1 Day a month

Provide respite and support for carers

Promote and maintain good health and social support and interaction with others

Provide a close and supportive environment to share the day with others

Provide activities to promote independence and enjoyment.

RESIDENTIAL RESPITE

REDHS Aged Care is an approved service provider for short term Respite Care. To be

eligible for Respite Care an ACAS approval is required and once approved entitles the

applicant to 63 Respite days per financial year. There is a cost per day for Respite Care.

A room with an ensuite is available in our Low Care facility for this specific purpose. It

can be booked in advance or if available, can be used for emergency Respite (pending

bookings).

Receive 24 hour care under the supervision of a Registered Nurse and receive help with

all personal care. Meals provided in a lovely homely environment.

NRCP offers support to carers providing a service

of Day Program to In Home respite.

This service allows quality time for Carers to have

a break .

Service is available for Day Respite on a Friday

when we join with the Planned Activity Program

at REDHS or In home respite.

Promote and maintain Carer Support

Provides Carers with valuable time out from

the everyday caring role.

Provide emotional support to the Carer.

Provides social interaction and

engagement with others.

Provides emotional, health and

wellbeing support to the care recipient.

Eligibility

The NRCP is designed to support and assist Primary Carers of people who are unable to care for

themselves because of frailty or a disability. NRCP services are targeted to assist carers of:

Frail Aged Australians (aged 65 years and over, or aged 50 years and over if Indigenous)

People with Dementia

People with Dementia and Challenging Behaviours;

Younger People with disabilities (people under the age of 65 years, or under the aged of 50 years if

Indigenous)

People with a terminal illness requiring Palliative Care

People with High Care Needs

Employed Carers.

Ashley is our Exercise Physiologist who

specialises in the delivery of exercise for

the prevention and management of chronic

diseases and injuries. AEPs provide support

for clients with conditions such as

cardiovascular disease, diabetes,

osteoporosis, mental health problems,

cancer, arthritis, pulmonary disease and

more.

Our Strength & Balance Program Level 1 has been designed for participants that aren’t

able to stand for long periods of time. The majority of the program is performed in a

seated position working on increasing muscle tone, strength and range of movement.

Supervised Gym Group

Get You Clients To Come Join The Fun

Supervised by Exercise Physiologist Ashley Watson, the aim of this group is to provide

exercise programs in an individualised setting. Following an initial assessment your clients

are provided with a program where specific exercises are designed to meet their own

personal goals. During the sessions there is individualised supervision of exercise

technique and progression of exercises.

When: Thursday 2pm Cost: $5.00

Rochester & Elmore District Health Service, offers a range of

low to high impact exercise classes.

Your clients can join and participate in a fun and safe

environment to improve their health and wellbeing.

Our strength training program designed for men & women over 50 years of age. Your

clients will enjoy social interaction in a group setting while exercising to their ability in a

fun & safe environment. For your clients to be eligible for the group program, they must

have the ability to walk independently with good standing balance.

When: Thursday 9:00-10:00 or Group 10:30-11:30 Cost: $5.00

Cardiac Rehabilitation is a program of exercise, and support to help maximise your clients

physical, emotional and social recovery after a cardiac event or condition. This is achieved

by individually prescribed exercises to suit your condition and general fitness. The clients

must be 3 months post cardiac event.

When: Wednesday 1pm-230pm Cost: $5.00

What Will Your Clients Need To Do Before They Come To The

Sessions

Participants are required to complete an assessment which can be conducted by a

physiotherapist or exercise physiologist, who will identify your needs and suitability for

appropriate class prior to commencing group programs.

The T2DM group program is designed to provide group education and exercise to people

with Type 2 Diabetes. This program is eight weeks in duration and is Medicare funded so

there are no out of pocket expenses to the individual. The education component consists

of sessions facilitated by a Diabetes Educator, Dietitian and Exercise Physiologist. As well

as education, the group performs group exercises weekly in a fun environment.
