    The wakalah charge for this investment-linked certificate might be different from which illustrated in the brochure, if loading(s) applies.

    You should satisfy yourself that the plan serves your needs and that you can afford the contribution. You should ensure that the allocation of

    your takaful contribution towards protection and investment meets your financial circumstances. If you need clarification, please contact yourfriendly PruBSN representative.

    PruBSN believes it is important that you fully understand all the benefits under the certificate, and that you also understand how the cost ofTakaful protection and distribution affect these benefits.

    Participating in a regular contribution family Takaful certificate is a long-term commitment. It is not advisable to hold this certificate for a shortperiod of time in view of the high initial costs.

    The information set out below explains the individual items in the marketing illustration table.

    NOTE:

    Should you stop making regular contributions, the plan may continue for as long as there are sufficient units in the account. The Tabarru'deduction and service charges for the respective accounts will be deducted through cancellation of units, subject to consent given by you.

    This is the minimum amount to be received on death or when you become disabled. The benefit is only applicable as long as the certificate isin force.

    1. Death Sum Covered

    You have the ability to increase your sum covered without increasing your regular contribution with minimum RM 10,000 per increment as longas you have enough slack in your PUA. This option is subject to underwriting.

    This is the amount that will be payable upon diagnosis of a critical illness. The benefit is only applicable as long as the certificate is in force.

    2. Critical Illness Sum Covered

    Contribution Paid is the amount that you (the certificate owner) pay for this certificate for the particular certificate year. Take note that not all ofthe amount paid will be invested into the investment fund(s) you selected. See explanation on unallocated and allocated contribution below.

    3. Contribution Paid and Cumulative Contribution

    Cumulative Contribution is the total amount that you (the certificate owner) pay for this certificate up to the end of respective certificate year.

    4. Allocated Contribution and Unallocated Contribution (Up Front Wakalah Charge)

    7. Asset Management Charge

    This is the projected value of units at any particular point in time that you may receive if you surrender the certificate and is net of tax and allapplicable charges.

    8. Cash Value

    If the certificate is terminated early, you may get less than the amount of contributions paid.

    Other Charges include service charge.

    The illustrated Tabarru' and other charges are based on current levels. These are not guaranteed and may be varied from time to time bygiving 90 calendar days notice to certificate owners.

    Tabarru' are deducted monthly from the value of your units to pay for your Takaful coverage. The Tabarru ' for Takaful coverage varies by agenext birthday, gender, occupation, medical rating and smoking status. The Tabarru' will increase as you grow older. Please refer to Illustrationof Tabarru' and Other Charges Table for the Tabarru breakdown.

    This is the commissions payable to agent comprises of basic commissions, production and persistency bonus.10. Direct Distribution Cost

    It is inclusive of Takaful Saver commissions ( if any). If you decide to stop the Takaful Saver contribution, the Takaful Saver commissiondeduction will stop unless you continue to pay for the Takaful Saver contributon.

    If the death is due to suicide within the first certificate year, no Takaful coverage will be payable, but the remaining value in PUA and IUA willbe payable.

    9. Total Death Benefit

    This is the amount that will be payable on your death.

    Service Charge

    Any charges that the Participant pays to PruBSN for the services rendered. These charges consist of commission and distribution relatedcharge, asset management charge and service charge.

    Wakalah Charge

    An arrangement where a portion of your contribution is donated into a fund to assist fellow participants in need through the benefits entitled tothem.

    Tabarru'

    The surplus arising from the Tabarru' fund, inclusive of investment profit after claims payable. The surplus, if any, is calculated and distributedbetween the Participant and PruBSN in the stated proportion.

    Investment Unit Account (IUA)

    Definitions

    The account where the amount from the Takaful Saver contribution will be used to purchase units for investment purpose and non-forfeitureoption.

    The account where units are allocated, depending on the amount of contribution paid for basic and/or additional benefit and surplus arising.

    Protection Unit Account (PUA)

    The Takaful Concept

    Takaful is a mutual assistance concept based on the principles of shared responsibility, cooperation and common interest. Each participantcontributes on the basis of Tabarru' (donation) into a fund that will be used to support each other in times of need.

    Free Look PeriodYou are required to study the plan illustration and pay particular attention to the benefits which are guaranteed and benefits which are notguaranteed. If there are discrepancies between the plan illustration and the certificate documents, you may cancel the certificate within 15calendar days from the delivery date of the certificate documents.

    Tabarru' Fund

    It is the pool of funds for the purpose of solidarity and cooperation among the participants that is used to help all participants in time ofmisfortune.

    In the event of death, the value of units in the PUA and the value of units in the IUA will be payable. In addition, the basic sum coveredfrom the Tabarru fund will also be payable.

    Benefits

    - In the event of total and permanent disability before age 70, the basic sum covered is payable. This is paid in lieu of basic sum covered.The units remain in the PUA.

    If basic sum covered is less than or equal to RM 4 million, basic sum covered ceases. If basic sum covered is more than RM 4 million,the excess of basic sum covered can be continued with payment of contributions. Riders (without disability plus) can be continued withpayment of contributions.

    - In the event of surrender, the surrender value of the certificate is payable. The value of the units will be based on the unit price at thenext valuation date following the notification of surrender. The participant will be able to withdraw the notification before the closingvaluation time.

    Loyalty Bonus

    Loyalty Bonus is a benefit to eligible participants subject to terms and conditions specified in the certificate document and this benefit isnot guaranteed.

    This bonus pays 5% of your yearly PUA contributions upon completion of the 10th certificate year and every 3 years after that.

    You are only entitled to the loyalty bonus if you pay all the contributions due for each prevailing certificate year before the next certificateanniversary.

    Fund switching:

    If you find that the fund you have chosen is no longer appropriate, you have the flexibility to switch funds. You are allowed 4 freeswitches per year without any fee. Any subsequent switches within the year, you will be charged a processing fee of 1% subject tomaximum of RM50 per switching application. Units can be switched between Takafulink funds at any time.

    Top-ups can be made at any time. The minimum amount required for top-ups is RM500. Apart from 5% wakalah charge on every singlecontribution top-up, there will be an aditional one-off top-up fee of RM25 for top-up less than RM5,000. For single contribution top-uppayment of RM5,000 and above, there will be no top-up charge imposed.

    Crisis Shield pays the sum covered on diagnosis of a critical illness. This is paid in lieu of the basic plan sum covered. Units remain inthe plan. If the sum covered of Crisis Shield paid is less than that of basic plan sum covered, the excess sum covered of the basic plancan be continued with the payment of contributions.

    1. What is this product about?This investment-linked Takaful certificate (ILT) offers a combination of Takaful protection and investment.     Lapse of certificate - The ILT certificate will lapse when the value of investment units is insufficient to pay for the Tabarru' and other charges.

    Cash value - the cash value of the ILT depends on the performance of the investment-linked funds selected. The higher the level of Takafulcoverage selected, the more units will be absorbed to pay for the Tabarru' and fewer units will remain to accumulate cash values under yourcertificate.

    General

    6. What are some of the key terms and conditions that I should be aware of?

    Importance of disclosure - you must disclose all material facts such as medical condition, and state your age correctly.

    Free-look period - you may cancel your ILT by returning the certificate within 15 calendar days from the delivery date of the certificate. PruBSNwill refund to you the unallocated contributions, the value of units that have been allocated (if any) at unit price at the next valuation date andany Tabarru' and certificate charge that have been deducted less any medical fee incurred.

    In the event of total and permanent disability before age 70, one lump sum payable under all certificates with PruBSN on a person is up toRM1 million per person. Any excess is paid on the first anniversary of the Covered Person's total and permanent disability subject to proof ofcontinued disability. Upon earlier death, the balance of any unpaid benefit is paid immediately. The maximum payable under all certificates isRM4 million per person.

    For Medical & Health Takaful5

    Qualifying/waiting period - The eligibility for:

    Critical illness benefits under the certificate will only start 60 days after the effective date of the certificate for heart attack, coronaryartery disease that needs surgery, cancer or other serious coronary artery heart disease and 30 days for other critical illnesses.

    - Medical Cover and/or Hospital Benefit and/or Medic Essential under the certificate will start immediately for hospitalisation due to

    accidents and 30 days for any other causes.

    Takaful Health2 and/or TH Annual Limit Waiver under the certificate will only start 120 days after the effective date of the certificate forspecified illnesses and 30 days for all other illnesses. Cover for hospitalisation due to accidents commences immediately.

    If the death is due to suicide within the first certificate year, no takaful coverage will be payable, but the remaining value in PUA and IUA willbe paid.

    7. What are the major exclusions under this certificate?

    Critical Illness

    Illness is directly or indirectly caused by acquired AIDS or any HIV infection, except when AIDS is full blown or is caused by a bloodtransfusion.

    Plastic/Cosmetic surgery, circumcision, eye examination, glasses, lens and refracrion or sugical correction of nearsightedness and/ orfarsightedness and the use or acquisition of external prosthetic appliances or devices such as artifical limbs, heatring aids, cochlear apparatus,implanted pacemaker and prescriptions thereof.

    Pre-existing condition if such condition was not disclosed in the proposal form or any other form relating to your health status after thecertificate is in-force.

    Medical and Health

    Dental conditions including dental treatment or oral surgery as necessitated by accidental injuries to sound natural teeth occuring whollyduring the period of takaful. Expenses arising from placement of denture, root canal treatment and prosthetic services such as bridges,implants an crown or their replacement will not be payable.

    Private nursing (except for Home Nursing Care Benefit), rest cures or sanitaria care, illegal drugs, intoxication (including and not limited toalcohol and drugs ), sterilization, venereal disease and its sequele, AIDS and AIDS related complex and HIV related diseases, and anycommunicable diseases requiring quarantine by law.

    Any treatment or surgical operation for congenital abnormalities or deformities including hereditary conditions.

    Pregnancy, current and previous pregnancies (and related complications), child birth (including surgical delivery and any surgical or nonsurgical procedure of the female reproductive system during surgical delivery), miscarriage, abortion and prenatal or postnatal care andsurgical, mechanical or chemical contraceptive methods of birth control, treatment pertaining to infertility as well as erectile dysfunction, and

    Investigation and treatment of sleep and snoring disorders, hyperhidrosis treatment, hormone replacement therapy, stem cells therapy (otherthan for haemopoeitic blood disorder), hyperbaric oxygen therapy and alternative therapy such as treatment, medical service or supplies,including but not limited to chiropractic services, acupuncture, acupressure, reflexology, bonesetting, herbalist treatment, massage or aromatherapy or other alternative treatment.

    Care or treatment for which payment is not required or to the extent which is payable by any other takaful or indemnity covering the coveredperson and disabilities arising out of duties of employment or profession that is covered under a Workman's Compensation Takaful Contract.

    Psychotic, mental or nervous disorders and those due to senile disorders (including any neuroses and their physiological or psychosomaticmanifestations).

    Costs/expenses of services of a non-medical nature, such as television, telephones, telex services, broadband services, electricity bills, radiosor similar facilities, admission kit/pack and other ineligible non-medical items.

    Sickness or Injury arising from racing of any kind (except foot racing), hazardous sports such as but not limited to parachuting, skydiving,water skiing, underwater activities requiring breathing apparatus, winter sports, professional sports and illegal activities.

    Private flying other than as a fare-paying passenger in any commercial scheduled airlines licensed to carry passengers over establishedroutes.

    Accidental

    War, invasion, act of foreign enemy hostilities whether it is declared or not, civil war, rebellion, revolution, insurrection, military or usurpedpower, direct participation in riot, strike or civil commotion.

    Engaging in professional sports, scuba diving, racing of any kind, aerial flights other than as crew member of or as a fare-paying passenger ona licensed passenger-carrying commercial aircraft operating on a regular scheduled route or any dangerous activities or sports (includingbungee jumping, hang-gliding, ballooning, parachuting and sky-diving), unless agreed to by a special endorsement.

    Taking drugs or narcotic unless taken as prescribed by a qualified doctor or physician.

    Attempted suicide or self-inflicted injuries while sane or insane.

    Any violation or attempted violation of the law or resistance to arrest.

    Pregnancy, current and previous pregnancies (and related complications), child birth (including surgical delivery and any surgical or nonsurgical procedure of the female reproductive system during surgical delivery), miscarriage, abortion and prenatal or postnatal care andsurgical, mechanical or chemical contraceptive methods of birth control, treatment pertaining to infertility as well as erectile dysfunction, andtests or treatment related to impotence or sterilization.

    Pre-existing physical or mental defect or infirmity.

    Alcohol and drugs intoxication.

    Any insect bite including mosquito bites and worm infestation.

    8. Can I cancel my certificate?

    Participating in a regular contribution ILT is a long-term financial commitment. It is not advisable to hold this certificate for a short period oftime in view of the high initial costs. If you find that the fund you have chosen is no longer appropriate, you have the flexibility to switch thefund. You are allowed four switches per certificate year without any charge. You may cancel your certificate by giving a written notice toPruBSN.

    9. What do I need to do if there are changes to my contact details?

    It is important that you inform us about any change in your contact details to make sure that all correspondence reaches you on time.
