Samples centrifuged then aliquoted into 0.6 mL microcentrifuge tubes.

Please DO NOT Parafilm or tape sample tubes.

0.6 mL tubes labeled with a letter and consecutive numbers (ie.S1, S2, S3)

0.6 mL tubes placed in a secondary package (ie. rack, box).

Dry ice added (4-10 kg) AND a dry ice label filled out (UN 1845). Please Get this Label

from your shipping department or the courier. If it is not available , cut out and use

the label on page 6.

Print Eve Tech order confirmation email and place between Styrofoam and

Cardboard INSIDE box (not in shipping pouch).

Record your sample's species, sample type, and infectious nature on waybill and on

other documents requiring a commodity description. (ie. Human Serum, non-infectious)

Please fill out, print, and tape page 5's address label to the outside of your box

provided on page 5. (this is insurance in case waybill is lost during shipping)

International shipping: Additional items to include in document pouch:

3 copies of commercial invoice. (See page 3 and fill out yellow fields)
