• Staff Incentive programs utilize the same targets as
management
• Staff are eligible for a quarterly Patient Satisfaction
Bonus (PSB) based on their entity results.
– System PSB performance has exceeded threshold for the last
18 quarters straight
– Quarterly bonuses can range from $150 - $300
• Staff are eligible for an annual Quality Bonus.
– Quality Bonus is based around Hand Hygiene results
– Annual bonuses can range from $50 - $150
Staff Incentives
• Houston Methodist has also periodically paid special
bonuses such as annual Thank You bonuses.
• A System staff employee who worked full-time in 2013
received $1,172 in incentive compensation.

• It is important that you read and understand all dependent eligibility
requirements – see page 6 of your Benefits Guide.
• Verifications are conducted to ensure only eligible dependents are
covered on our benefit plans. These verifications are administered by
the ConSova Corporation, a benefits verification firm.
• You will be contacted by ConSova to provide proof of dependent eligibility
for any dependent on the Houston Methodist Medical Plan.
• If you cover your spouse under the Medical Plan and indicate that
he/she is not eligible for his/her employer’s medical plan, you will have
to provide additional documentation that will validate this statement.
• If you do not respond, provide incomplete information, or a dependent is
found to be ineligible, the affected dependent will automatically be
removed from the Medical Plan and all other benefit plans for which
he/she is determined to be ineligible. You may also be requested to
repay Houston Methodist for benefits paid out on the ineligible
dependent.
SPOUSAL SURCHARGE
In addition to the Medical Plan premium when the following
conditions are met Spousal Surcharge applies:
1. Employee’s spouse is eligible to participate in an employer’s medical
plan (regardless of type of plan) AND
2. Employee’s spouse chooses to participate in the Houston Methodist
Employee Medical Plan.
The amount of the surcharge is $100 per pay period.
Does not include spouses who are covered under Medicare, who
are not eligible for their employers’ plan or who are self-employed
and do not have access to a group plan.

• Two Plans:
1. Choice
2. Choice Plus
• Both provide comprehensive coverage
• Plans are identical “In Network”, Choice Plus allows for “Out of
Network” at % of charges covered after deductible.
• The “In Network” is the entire Choice/Choice Plus UHC network
which is a nationwide plan with approximately – 19,683
specialists in Houston and 74 hospitals.
• Choice Plus costs significantly more than Choice Plan, with the
only additional benefit being able to go out of the UHC network.
• Over 95% of employees enroll in the Choice Plan.

You and your covered spouse must both: Complete the WebMD Health Assessment
Complete a HealthyDirections Biometric Screening
Be tobacco/nicotine-free by:
• Screening negative for tobacco/nicotine* use during your Healthy
Directions Biometric Screening
• The Health Assessment and Biometric Screening must
be completed within 60 days of hire or entering a
benefits eligible status to be eligible for a reduced
deductible.
*If it is unreasonably difficult for your spouse to be tobacco/nicotine-free, an alternative lower
Medical Plan deductible qualification may be available. For more information, contact HR Benefits

CARRYOVER FROM
CHRISTUS – DED/OOPM
• Employees will receive a credit of 60% of out of pocket costs* and
up to $250 towards the deductible* if they complete the following
criteria** by March 2, 2014:
• Health Assessment through WebMD on MMB
• Onsite Biometric Screening
*For out of pocket and deductible costs incurred from July 1, 2013
through January 31, 2014 and paid through February 28, 2014.
**This criteria will also count towards the 2014 Medical Plan
Rewards criteria to receive the lower Medical Plan deductible in
2014. However, please note, additional criteria apply for the 2014
Medical Plan Rewards.
WHAT IS A BIOMETRIC SCREENING AND
WHERE TO GET ONE DONE
• A Biometric Screening is a key component in helping
you learn more about your health and wellness.
• The screening includes height, weight, waist, lipid
profile, tobacco utilization status and glucose.
• Visit the HealthyDirections Hot Spot on
mymethodistbenefits.com to find the current Biometric
Screening schedule.
• Employees and covered spouses need to make an
appointment at a Houston Methodist location that is
convenient for them, regardless of the employee’s work
location.
• It is not required that you be fasting prior to the
screening, but it is preferred. 16
IMPORTANT INFORMATION ON
QUALIFYING FOR LOWER DEDUCTIBLE
• All employees and spouses are required to attend a
Biometric Screening on campus.
• Exceptions to attending a screening on campus will
only be made for one of the following two reasons:
1. Spouse resides more than 50 miles from ANY Houston
Methodist facility
2. Spouse will be out of the area 30 days during Enrollment
events (documentation required)
• Spouse will have to wait to complete Health
Assessment
Only ees on LOA will be allowed to use the form, that is the only exception
other than spouses. Forms will only be granted after eligibility is
confirmed.

• Specialty drugs must be filled at a specialty pharmacy after obtaining a one-month supply (mail order pricing not available)
