First in Fort Bend County Methodist Sugar Land Hospital offers LARIAT stroke-preventing procedure

Patients who have atrial fibrillation (irregular heartbeat) are at partic-ular risk for developing clots in

the left atrial appendage (LAA) at the top of the left chamber of the heart. The clots can break off, travel within the body and cause a stroke. Most pa-tients with atrial fibrillation take blood-thinning medications to prevent these embolic strokes. Some patients, because of bleeding risks and other side effects, cannot take these medications.

Methodist Sugar Land is the first hospital in Fort Bend County to offer the LARIAT procedure to treat patients who have atrial fibrillation and are at increased risk of stroke. The LARIAT procedure is nonsurgical and noninvasive—performed in a catheterization lab instead of an operating room.

“The LARIAT Suture Delivery Device provides an

innovative technology that may help atrial fibrillation patients who cannot take blood thinners,” says Jie Cheng, M.D. , a board cer t i f ied electrophysiologist at Methodist Sugar Land Hospital who performed the first such procedure in Texas.

“By using state-of-the-art 3 dimensional echocardiography (a sonogram of the heart), we are able to

guide the LARIAT procedure without the need for open heart surgery,” says Kesavan Shan, M.D., board certified cardiologist and director of cardiac imaging at Methodist Sugar Land Hospital. We are very pleased to provide this potentially lifesaving therapy to residents of Fort Bend and surrounding areas.”

Transmittable to humans, leptospirosis is a po-tentially fatal, worldwide disease of domestic

animals and wildlife, caused by several strains of the Leptospira bacteria. Occurring in both sub-urban and rural environments, "Lepto" out-breaks usually occur during the seasons of heavy rainfall, and infections are most commonly ac-quired through cuts in the skin, or via the mu-cous membranes, by coming in contact with infected urine or standing water. Ingestion of infected animals, soil or infected bite wounds can also cause infection.

Once the bacteria have entered the body, they travel throughout the bloodstream to several parts of the body affecting multiple organs, most commonly the kidneys, liver and lungs, causing disease and organ failure. Once in the kidneys, the bacteria replicate and are shed in the urine, contaminating the environment, and putting our pets and us at risk of infection.

Infected dogs may show lethargy, pale mucous membranes, increased drinking and urination, little to no appetite, and vomiting. They usually have a concurrent fever, shivering, and muscle tenderness. It is important to take your pet to your veterinarian immediately if you see any of these signs and have a recent history of coming into contact with wildlife urine or standing water by visiting the dog park, swimming in nearby lakes or going on a recent hike.

Survival rates vary, and largely depend on how long symptoms have been present, how progressive the disease has become, how your pet responds to treatment, and if permanent damage has occurred.

Due to the zoonotic risk (transmission to humans) of leptospirosis, it is important to ask your veterinarian if you and your pet are in an endemic area for this type of bacteria, and to discuss preventative measures and clinical signs to look for in your pet. Vaccination of dogs against certain strains of Leptospira can greatly lessen the chance of transmission and severity of the disease and may save your pets life.

I have been told that I am a pre-diabetic. What does that mean?

Pre-diabetes is a condition that indicates an early impairment of carbohydrates or sugar tolerance. In other words, the patient cannot properly process dietary carbohydrates or sugars. This condition is usually a precursor for diabetes. In most cases, the patient is asymptomatic and does not suffer from any particular concerns. Pre-diabetes is usually diagnosed through a routine fasting blood work. Most patients with this condition tend to be overweight or obese. The initial treatment plan for patients with pre-diabetes usually consist of a course of conservative management by encouraging low sugar, low fat diet, aerobic exercises and weight loss. In the following few months, the patient’s blood sugars are checked and monitored closely to make sure that the patient does not develop diabetes. Annual eye exams, preventive care and regular follow up visits are recommended for these patients. .
