PREAMBLESafety at work can not be seen as different from work itself It is directly linked with the quality of the work procedure employed and the person involved in the work It naturally falls that systems to improve safety and the methods to improve work will be by and large the same

A Safety Work Permit is a format to be filled in by the concerned so as to ensure that the work which is being attended to is commenced and maintained at safe condition during the course of activity The safety permit is a mandatory document and in no circumstances can be override the authority of the safety permit for the purpose of putting into operation the equipment of the plant placed out of action under the authority of safety work permit

The procedure enumerated for issue of safety work permit shall cover the entire plant area

SAFETY WORK PERMIT- CLASSIFICATION ndashSafety work permit is issued for the activities as under ndash

bull Working on electrical operated equipment- Electrical Work Permit- (xxx)bull Working at height ndash Permit to work at height- (xxx)bull Working at the confined places- Permit to work in confined places- (xxx)

ISSUE OF SAFETY PERMIT

Safety Work Permit shall be issued for the activities which are carried on in the plant as detailed hereunder ndash

bull Any work which is to be done involving soldering welding by arc or flame cutting etc in high hazardous area or equipment carrying inflammable liquids or gasesbull Any work carried on involving electrical shockbull Any work carried on at the height of 10rsquo or above involving falling hazards viz All structuring erections and fabrication work civil masonry and shuttering work pipe line equipment and working on ground girder column roof trusses etc cleaning of hopper and chimney etc

Any work in the confined area viz trenches drains culvert or sumps below ground level cyclone and hopper etc

PROCEDURE OF WORK UNDER SAFETY WORK PERMIT ndash

When the work is being carried on under safety work permit it should be ensured that certain precautions are observed so as to make the work and place safe while carrying out the activities

1048633 Persons engaged to carry out work are experienced and have been imparted necessary instructions for execution of the job safety

1048633 While working on electrical operated equipment it should be ensured beyond doubt that the plant can not be energized in error The switches must be locked on the outposition and the fuses locked up Ensure that the plant can not be energized through any other circuits

1048633 When the work is being carried on at height the use of safely harness should be ensured similarly the work platformscaffold of sound construction is provided Area below work place is cordoned and warning board displayed apart from complying with other safety measures

1048633 Whenever the work is being carried on in a confined area it should be ensured that atmosphere is tested before the personnel assigned the job entered the confined area Maintain an observer above with means of communication to personnel in a confined area and rescue apparatus in use or in readiness as required

RESPONSIBILITIES ndash

bull Primary responsibility for issue of Safety Work Permit shall be of the concerned department taking up repairs and maintenance of the equipment

bull It shall be the responsibility of the officer or workmen to take the required safety work permit before commencement of the work

bull It will be responsibility of the issuer to issue the safety permit with all out precaution as indicated on the permit form itself

bull Date time name and signature of the issuer and receiver should be clearly mentioned on the permit

bull It shall be the responsibility of the section head to ensure that proper personnel protective equipment are provided for the work as required and mentioned on the work permit

bull Termination or extension of the permit should be issued duly signed by receiver or issuer of the permit No permit shall be cancelled over phone Copy of permit issued is signed and sent back for cancellation physically

bull Sectional Head should ensure compliance of the safety checksmeasures before the issue of safety work permit

RECORDS

The Safety Work Permit issued shall be recorded and a copy of the same is retained for a period of one year to strengthen the system

REVISION AND MODIFICATION ndash

The Safety work permit system shall be reviewedrevised and modified as and when required at the discretion of the Management
