IntroductionPsychosis is a severe mental illness which affects up to 3% of the population and is associated with significant impairment in social functioning and shorter life expectancy. Although psychosis can emerge at any age, the peak of onset is in late adolescence and early adulthood. Psychosis is ranked as one of the top causes of disability and most expensive illnesses worldwide through costs related to hospital admissions, physical health co-morbidities and unemployment.

Early Intervention in PsychosisEarly intervention in psychosis (EIP) are multi-disciplinary teams which set up to seek, identify and reduce treatment delays at the onset of psychosis and promote recovery by reducing the probability of relapse following a first episode of psychosis. EIP is critical for ensuring that the impact of psychosis is minimised in people’s lives at a time when most are just starting out in higher education and employment. Timely access to EIP is shown to have a significant positive long-term impact on the lives and livelihoods of individuals with psychosis and their families.

ContextIn 2016, the introduction of the psychosis standards marked a historical moment as for the first time, a severe mental illness i.e. psychosis, was subject to access and quality standards. The standards were introduced to ensure the majority of people with first episode of psychosis can:

1. Access specialist psychosis treatment within 2 weeks of referral 2. Receive a National Institute of Clinical Excellence (NICE) concordant package of care

As well as assessing patients within 2 weeks of referral and providing NICE concordant care, EIP teams were also tasked with extending their age range to include those aged over 35 and to treat people with At Risk Mental States (ARMS).

The South EIP ProgrammeThe South of England comprises of 13 Sustainability & Transformation Partnerships, 50 CCGs and 16 mental health providers with 32 Early Intervention in Psychosis (EIP) teams. The South EIP Programme set up platforms for exchanging ideas and expertise, defining optimal clinical pathways and service models. The programme has developed partnerships with national and local NHS England, NHS Improvement (NHSi) and Health Education England (HEE) teams in order to build staff capacity and competencies required to deliver on the standards. The programme maintains a whole system overview in order to identify emerging problems as they arise and provide expertise to support quality improvement.

PurposeThis document provides NHS England and its stakeholders an update on the equity of early intervention in psychosis in the South of England including investment per patient, resulting capacity, quality and outcomes. In doing so, we aim to honour our duty to patients and their families by driving for equitable access to evidenced based early intervention for psychosis, regardless of where people live in the South of England.

Executive SummaryEarly Intervention in Psychosis (EIP) has an overwhelming collection of robust research evidence showing its clinical and economic effectiveness. It was therefore welcome to see that NHS England listed EIP as a ‘must do’ in the Five Year Forward View for Mental Health1 (FYFV-MH).

teams are restricting access in order to manage demand.

MethodThe EIP Matrix is a web-based data analytics software which allows registered users to enter data and receive feedback in the form of graphed charts. The software uses questions on NICE concordance which were developed by a national expert reference group as part of the Royal College of Psychiatry’s Clinical College for Quality Improvement (CCQI). In addition to caseloads and NICE concordance, the EIP Matrix also has questions about investment, workforce and outcomes. Managers of EIP services were invited to update their data between 1st August – 30th September 2018. After data was submitted, each EIP team manager, their director and commissioners received a draft summary report to review and correct as needed. Webinars and face to face forums were additionally used to share initial findings with providers and commissioners.

Findings

InvestmentTo calculate investment per patient, we asked EIP teams to provide their total annual budget. The question used to elicit this information is: ‘What is the total annual budget of your EIP team at the time of entering your data? i.e. actual budget under the control of EIP team manager including vacant posts. Please exclude overheads e.g. Trust on-costs such as facilities and any additional resources from other teams such as CMHTs or Crisis Teams.’ Investment per patient is calculated by dividing the EIP team annual budget with the total caseload.

Based on NICE reference costs, approximately £8,250 investment per patient is required to deliver a NICE concordant package of care2. None of the EIP services in the South of England are funded at this level:

EIP Caseloads - Sept 2017 versus Sept 2018

With the exception of Oxford, 2Gether, Solent and Southern Health the caseloads of EIP teams in the South of England have stayed largely the same or reduced this year. Our interpretation is that teams have struggled to meet demand without additional resources and have resorted to limiting access to the service by raising referral thresholds, limiting the age range for the service, or by discharging people earlier.

PrevalenceThe Psymaptic psychosis incidence tool (www.psymaptic.org) uses epidemiological data to predict the number of new cases of psychosis that can be expected by each EIP service per year. Prevalence is calculated by multiplying annual incidence by 3, for the three years that new cases are expected to remain on the EIP caseload. It is worth noting that the current Psymaptic incidence is based on population census data from 2011. In some instances predictions underestimate actual new cases taken on by an EIP service. An updated version of Psymaptic is due to be published in 2019. The below table illustrates each EIP service’s caseload as compared to what is expected according the current Psymaptic tool:

CaseloadsEngagement with treatment is a key function for early intervention in psychosis services, and this requires care coordinators to use assertive means to engage the person and family through more flexible and frequent contacts. This is why there is a maximum recommended Service User-Care Coordinator ratio of 15 Service Users per Care Coordinator. Higher caseloads are associated with higher rates of disengagement and relapse3.

Cognitive Behavioural Therapy for Psychosis (CBTp)There has been an increase in the number of people receiving CBTp from 21% in 2017 to 29% in 2018. There has been significant investment in this area from Health Education England (HEE) in terms of funding for training, and from providers in terms of releasing staff and arranging backfill. A second cohort of graduates has qualified this year bringing the number of CBTp therapists up from 27.2 to 44.2 Whole Time Equivalents (WTE). We therefore expect to continue to see improvements in this area so long as teams are able to retain their newly qualified CBTp therapists.

Family Interventions (FI)The proportion of families who received Family Interventions has remained relatively low at 20%. Family Interventions are typically delivered by care coordinators in pairs at times when the family is most likely to be available, which can be during evenings. The low level of delivery of family interventions is likely due to the lack of capacity within the teams to deliver this intervention.

Physical Health EIP teams in the South of England have made some improvements in the screening of physical health, from 41% in 2015 to 51% in 2018. Although this is an improvement, it is still some way off from the 90% standard set by the 2017-19 Commissioning for Quality and Innovation (CQUIN) framework. Some teams, like the Pan-Dorset and Gloucestershire Recovery in Psychosis (GRIP) EIP teams have made significant improvements and provided a comprehensive physical health check for 93% of people on their caseload. Some of the learning from the Pan-Dorset and Gloucestershire Recovery in Psychosis (GRIP) EIP team is explored further as part of a good practice case studies in Appendix 1.

Individual Placement and Support (IPS) Individual Placement and Support (IPS) has been shown to be an effective intervention for supporting people with severe mental illness back into paid employment5. NHS England has set an ambition to double the number of people accessing IPS by 2020. Of the people with psychosis who are not in employment or education on the EIP caseload, 33% have had 2 or more appointments with an IPS worker. Alongside NHS England’s ambition to double those accessing IPS, there has also been transformation funding. The following Sustainability and Transformation Partnerships in the South of England were successful in securing IPS transformation funding:

1. Buckinghamshire, Oxfordshire and Berkshire West 2. Devon 3. Frimley 4. Somerset5. Sussex and East Surrey

A second tranche of transformation funding is expected later this year and should provide more resources for EIP to make further improvements in the delivery of IPS.

DiscussionEarly Intervention in Psychosis Services are the most clearly specified and evidence-based area of mental health care delivery making them transparent to commissioners, people with psychosis and their families. EIP teams in the South of England have significantly improved waiting times and increased access to CBT for psychosis, Individual Placement and Support (IPS) and physical health screening. These improvements have been made despite the majority of teams not receiving any increase in funding. With the exception of the Devon CCG, the 6 CCGs in Surrey, the 8 CCGs in Kent and Medway and the 4 CCGs in Hampshire, none of the other 31 CGGs in the South of England have made funding increases for EIP this year or where funding has been given to providers, it has not reached the EIP frontline. Without this investment the vision of the Five Year Forward View for Mental Health will not be realised.

Appendix 1: Good Practice Examples

Physical Health

EIP Team Pan-Dorset EIP Team

AchievementThe team has excelled at physical health screening by providing a comprehensive physical health check to more than 90% of people on their caseload.

Enablers

The Pan-Dorset EIP team has a named clinical lead for physical health. His main duties are care coordination and a smaller caseload allows him to lead the team in ensuring physical health checks are completed and recorded.

Where the physical health checks results indicate an issue, the team will offer and deliver an intervention which is also recorded. The Information and Technology team have built a business intelligence solution with a dashboard which allows managers and clinicians to have a near real time view of performance.

The dashboard refreshes every 24 hours and is able to forecast the number of patients that will require a new physical health check in the coming months, so that clinicians can plan ahead and set time aside to complete them. This allows the clinicians and team to be proactive rather than having to constantly respond to breaches.

EIP Team Gloucester Recovery in Psychosis (GRiP) EIP Team

Achievement2gether NHS Foundation Trust is one of only 3 Mental Health Trusts to consistently achieve targets set for compliance for this CQUIN.

Enablers

The GRIP EIP credits some of their success to consistent support from their board and IT department. The Trust’s physical health workstream is treated as a transformation project, with clear milestones and deliverables set against those who are accountable. The EIP team has a physical health lead but each clinician is responsible for ensuring physical health checks are completed, and any interventions required are offered.

Before embarking on the project, a comprehensive clinical engagement exercise was conducted to find out the barriers to completing and recording physical health checks. One of the barriers was the forms for recording physical health checks were in multiple areas and hard to find on the electronic health record. The Trust IT team worked with clinicians and people who use services to simplify the process of recording to involve one screen which is easy to find.

The GRIP EIP team’s initial plan was to hold clinics where patients can get physical health tests completed, but the DnA rate to these clinics was high. The team adapted their method to go to people’s homes with a mobile physical health bag with all necessary recording devices. The results are now much improved with 93% of the team’s caseload having had a comprehensive physical health check in the last 12 months.

To sustain this improvement, the IT department has also deployed a physical health tracking dashboard so that individual clinicians, their managers and the board can have near real-time updates on progress.

EIP Team Sussex EIP Team and Southdowns Housing

AchievementSouthdowns Housing is a centre for excellence for Individual Placement and Support (IPS). Working in partnership with Sussex EIP teams, the service has delivered IPS to 83% of those who were not in employment in the last year.

Enablers

The relationship between Southdowns and Sussex Partnership NHS Foundation Trust is a longstanding partnership with a solid governance structure which ensures that IPS is embedded within every mental health team that treats people with severe mental illness. The group were successful in securing NHS England funding in the Wave 1 IPS bid for expanding the existing IPS service within mental health teams. The funding enabled the recruitment of additional employment specialists, who are co-located with EIP teams, enabling them to work side by side with EIP clinicians to deliver IPS interventions earlier and to more service users. As well as co-location, the employment specialists also have access to the Trust’s electronic health record system, so that they can be informed about the activity of other team members with service users, and record IPS activity. Future work will focus on strengthening ties between the EIP clinicians and the teams’ IPS workers through developing an EIP IPS Partnership Forum and to improve reporting of interventions and outcomes to stakeholders.
