acEvely  involved  in  extending  the  case  management  pilot  within  a  mulE-­‐disciplinary  team.    Totalling  up  to  1.67  wte  

(i.e.  62  hours/week)  worked  on  a  full-­‐Eme,  part-­‐Eme  or  job  share  basis.  

The  pracEce  would  also  be  supporEve  of  the  post  holders  developing  other  PracEce  Nursing  skills  which  would  

benefit  the  PracEce  Nursing  Team  and  the  post  holders  in  the  future.

Dependant  upon  experience,  the  role  is  currently  graded  as  Cligon  Grade  5  (£21-­‐25k/wte)  or  Grade  6  (25-­‐30k/wte)
