It is the mission of Southwestern Pennsylvania Human Services, Inc. is to identify and provide for health and social services needs throughout the southwestern Pennsylvania region.

Monessen Family Center

Monessen Family Center The Monessen Family Center provides services to support families in Westmoreland County to ensure the healthy growth and development of their children. Our services include:

• Parents as Teachers: A home visiting program for children 0-5 that focuses on child development. • Developmental Screenings: Trained staff will use Ages and Stages Developmental screening tool

to screen the child and make appropriate referrals and recommendations. • Group Connections: Facilitated playgroups for parents and children ages 1 and 2 that focus on the

four domains of development. • Pregnant and/ or Parenting Program: Teen moms can take classes online at the Center to earn

their high school diploma from Monessen School District and staff will be the liaison between the Intermediate Unit and the student. They will also participate in the PAT home visiting program.

• Playgroups: Staff will provide social and educational opportunities for 3 and 4 year olds. • Parenting Classes: Staff will provide either group or one on one parenting instruction using

researched base curriculum. • Time Limited Family Reunification Services: A Masters level staff will provide reunification or

preservation services to families that have been referred by Westmoreland County Children’s Bureau. This is a fee for service program.

• Fatherhood Support Services: A support program for fathers. The program can consist of emergency assistance, adult education, parenting instruction, home visits, parent/child bonding opportunities and resource referral.

• Additional resources and referrals are provided to families, including Private Industry Council, WIC, Primary Care, Drug and Alcohol Services, Domestic Relations, Welfare Office, Westmoreland Intermediate Unit, Head Start, Pre-K Counts, Food Bank, CCIS, Valley Women's Health, Salvation Army, library, Monessen School District, Westmoreland County Children's Bureau, Tickets for Kids, Westmoreland Case Management, among others.

• Transportation is also provided as needed.

The CARE Center, Inc.

STTARS The STTARS program offers counseling, legal and medical advocacy and a 24 hour hotline service to survivors of sexual violence. Prevention/education services are also available to schools and the community.

Drug and Alcohol Provides outpatient (OPT), intensive outpatient (IOP) and Partial (PHP) drug and alcohol treatment to adults and adolescents. Specialized treatment services includes a Women and Family program which provides preferential treatment to pregnant women.

PACE (Parenting & Communicating Effectively) PACE is an educational intervention program for families encountering custody issues within the Washington County court system.

CTT The Community Treatment Team (CTT) program is a community based treatment program based in Greene County that provides a comprehensive approach to reintegrate individuals into the community. CTT is a self-contained, person centered, strengths based and recovery focused program that provides intensive services to individuals.

ACT The Assertive Community Treatment Team (ACT) serves individuals 18 years of age and older with serious mental illnesses that have effects on daily functioning. Individuals receiving ACT services have individual treatment teams providing case management, therapy, nursing and psychiatric care.

Crisis SPHS Crisis offers mental health crisis intervention services for Washington and Greene Counties. The levels of crisis intervention services include: telephone, walk-in and mobile crisis services. Telephone and mobile crisis intervention services are offered 24 hours per day, 7 days per week.

Diversion SPHS offers a crisis stabilization/diversion unit for residents of Washington County. The unit offers an alternative to behavioral health hospitalizations or is an option to step down from the hospital prior to reintegrating into the community. At the crisis unit, staff develop treatment plans and coordinate care for individuals upon discharge from the unit.

Psych Rehab Psychiatric Rehabilitation helps individuals who have persistent and serious mental illness develop the emotional, social and intellectual skills needed to live, learn and work in the community with the least amount of professional support.

Certified Recovery Specialist Certified Recovery Specialist staff are specially trained substance use recovering peers who help empower individuals who are struggling with substance abuse to build community recovery supports for long term recovery. An individual does not need to be in treatment services to receive recovery support services. PCIT PCIT is an evidence-based treatment for young children with behavioral and emotional disorders that places emphasis on improving the quality of the parent-child relationship and changing parent-child interaction patterns.

Greene County Navigator The Greene County Navigator assists individuals with Opioid Use Disorder (OUD) and/or any individual who is seeking help for someone with OUD in navigating the system to receive treatment and support. The Navigator will also provide education to the community about the opioid epidemic.

Long Term Structured Residence (LTSR) The long term structured residence provides 24 hour services in structured setting for individuals, ages 18 and older, who have demonstrated severe and persistent psychiatric disorders. Individuals receive therapeutic interventions, specialized and individualized treatment services in a controlled environment.

Community Rehabilitation Residence (CRR) Butler CRR provides a safe, secure and recovery focused placement for mental health consumers who are seeking discharge from state and community based inpatient units. The program teaches independent living skills to enable consumers to successfully seek and maintain independent living.

Butler Permanent Supportive Housing Butler Permanent Supportive Housing provides rental subsidy for up to 24 individuals in the Butler Community. Support services are provided by other community services and assistance can be short term or on-going.

Galleton Commons Galleton Commons is an enhanced supportive housing program designed to create an atmosphere of security, comfort, and recovery, all within the least restrictive environment. Staff are onsite 24/ 7 and are mobile throughout the grounds 24 hours of the day and available to the clients at all times to provide oversight, support and assistance with transportation to appointments, meal preparation, general housekeeping, and basic daily needs.

Fayette Street Commons

The goal of this program is to provide the support to Washington County residents, aged 18-25, with mental illness who do not function well in larger CRR or similar program environments, to continue to increase independence and interaction in the community. Staff provide professional support and referrals for these individuals, as well as assist them in learning the necessary skills to live independently in the community. Staff provide transportation assistance to physical and behavioral health appointments and linkage to other community agencies. Staff support the individuals in goal planning and learning to take their medications independently as well as laundry, shopping and meal preparation. Finally, staff will work with the young adults to create a homelike environment where they feel safe and comfortable while embarking upon their own recovery journeys.

Connect, Inc.

Washington Family Center The Washington Family Center provides a STEAM focused after-school and summer program providing academic enrichment, social development, parental and community involvement. The Center works with the following schools in Washington County:

Washington Family Shelter The Washington Family Shelter provides emergency housing to homeless families up to 60 days. The program offers case management, assistance in securing basic needs, housing and support services referrals. After the 60 day period expires, the families will move in to permanent housing or the most appropriate housing option.

Welcome Home Welcome Home offers emergency housing to families and single women up to 60 days. After the 60 day period expires, the residents will move in to permanent housing or the most appropriate housing option. Welcome Home offers additional cot spaces during the months of November – March when the weather drops below 25 degrees. The program offers case management, assistance in obtaining basic needs, housing and support service referrals.

It is the mission of Connect, Inc. is to provide a full continuum of care to homeless, near-homeless, and low income individuals, children and families in Washington, Greene and Westmoreland Counties.

Crossing Point Crossing point offers 20 income based permanent Supportive Housing Single Room Occupancy units throughout Washington County. The program provides case management, life skills and linkage and referral to community and support services.

Washington Permanent Supportive Housing Washington Permanent Supportive Housing offers 46 income based single family units throughout the different Washington County communities. The program provides case management, life skills and linkage and referrals to community and support services.

Whitehill Place – Transitional Housing Offers transitional housing (assistance for up to 18 months) for single individuals facing homelessness. Residents are afforded their own apartment and pay 30% of their income towards rent and utilities. The program offers case management and support services in obtaining income and eligible benefits and services while assisting them in finding permanent housing.

Greene County Permanent Supportive Housing Greene County Permanent Supportive Housing operates as an income based supportive housing program in scattered throughout Waynesburg, Pa. A combination of one, two and three-bedroom apartments are leased from private landlords to provide long-term supportive housing to house 12 disabled individuals and families who may suffer physical disabilities, mental health, drug and alcohol, or co-occurring disorders. The program offers case management, life skills and linkage and referral to community support services.

Greene County Rapid Rehousing Greene County Rapid Rehousing provides temporary rental assistance up to seven single individuals and one family at a time. The program also offers case management, life skills and linkage and referral to support services.

Determination, Wellness and Empowerment for Life (D.W.E.L.) D.W.E.L. operates a 21 unit income based, site based and scattered site permanent supportive housing program in Westmoreland County. The program also offers case management, life skills and referrals to support services.

Developmental Disabilities Services In order to receive our developmental disability services, an individual needs to contact their Intellectual Disabilities Supports Coordinator. If they do not have a Supports Coordinator, they should call their resident county’s behavioral health department and ask for intake.

Our developmental disability services include:

• Community Living Arrangement: CLA programs provide independent, but supervised, community-based group home living for eligible persons with developmental or intellectual disabilities.

• Therapeutic Activity Center: “Riverside Horizons” is a therapeutic activity center for adults with developmental or intellectual disabilities where they learn activities of daily living.

• Behavioral Supports: Provides functional assessment, development of strategies and

interventions and training to individuals, staff, parents and caregivers of individuals with developmental or intellectual disabilities who are experiencing behavioral challenges.

• In-Home and Community Supports: A service provided in home and community settings to assist individuals with intellectual disabilities in acquiring, maintaining and improving the skills necessary to live in the community.

• Community Participation Supports: A service through Riverside Horizons with the focus on developing basic skills necessary for individuals with intellectual disabilities to pursue competitive integrated employment, develop connections in the community and promote a spirit of personal contributions and community connection.

Mental Health Association in Butler County

Compeer and Compeer Corps

Compeer and Compeer Corps are a friendship programs that links volunteers with individuals in mental health recovery. The programs offer supportive friendships and mentoring relationships to both civilians and veterans to help bring happiness and offset feelings of loneliness that can sometimes occur.

Educations Advocacy

Our Educations Advocacy Program offers support to families to help children and young adults acquire the best education that they can attain. The advocate can provide support at Individual Educations Plan meetings, Inter-Agency Service Treatment Plan meetings, and Evaluation Report meetings.

Adult Advocacy/Ombudsman Our Adult Advocacy/Ombudsman Program can provide individuals information about managed care services and can assist with filing complaints and grievances if they feel dissatisfied with provided services and/or decisions made by the managed care organization.

Representative Payee Program Representative Payees provide assistance in bill paying and financial accountability to individuals with a mental health diagnosis who are unable to manage their own personal finances. The program is available to individuals with no close family or friends that are able to assist them with managing their funds.

The missions of the MHA are: to promote public awareness of mental health and acceptance of mental illness; to promote public recognition of mental health disorders and reduce stigma; to assist persons with mental illness and their families to enrich their lives in the hospital and in the community; and to

serve as advocates for those with mental illness, their friends and families.

Recovery is the central focus of all services that seek to help those with mental illnesses. The goal of recovery should be available to all who struggle with mental illness.

Southwest Behavioral Care, Inc.

Henry House Henry House operates a duplex in Belle Vernon as an enhanced supportive housing program for three men and three women. The program is a co-housing community designed to provide a home like environment that provides a level of security to the residents, while they receive a complement of services designed to meet their individual needs.

Day Reporting Center (DRC) The Fayette DRC is designed to serve individual’s 18 years of age or older that are involved with the justice system. The DRC is a concept which provides an opportunity to assist the offender in managing and addressing all his/her mental health and substance abuse clinical needs. The DRC is a joint venture with Adult Probation where offenders report to the Day Reporting Center 1-6 times per week to meet with their probation officer or attend services as identified by their assigned therapist.

Emergency Services Program Emergency services are provided to individuals, family members who request assistance with admission to a behavioral health unit under the Mental Health Procedures Act. Mobile Medication The mobile medication program provides assistance to individuals who experience difficulties with medication management.
