If you have received this questionnaire via email, please complete it online, save a completed copy to your computer, and then provide a completed version to Angelyn Dale at adale@hallestill.com This process will enable her to have your results ready as soon as possible. If you have received a hard copy of the questionnaire, please complete it and drop it off at the Information Desk at the church. The receptionist will contact Angelyn to come get it and score it for you so you can get the results. The results will be completely confidential.

The following is a list of 80 statements. Before considering them, please make sure you have set aside an uninterrupted quiet time so that you may focus on your responses. Begin by praying for insight and candor with yourself. Answer based on how true these statements really are regarding your life experience, both past and present, not as you wish you would be.
