    Missing documentsIf you do not have your original documents,try to obtain the required documents fromthe relevant educational institution, authorityor employer organisation. Most educationalinstitutions, authorities and employerorganisations keep relevant records and willreissue these documents if requested. In somecircumstances, VETASSESS will accept astatutory declaration or other sworn affidavitin place of the required documents butassessment cannot be issued on the basis ofstatutory declarations only. If you cannotobtain the required documents, contactVETASSESS for advice.

    Your assessment will not begin until all yourdocumentation is received. If missingdocuments are not received within fourmonths of receipt of a letter from VETASSESSrequesting documentation, your applicationwill expire. You must then submit a newapplication and pay another application fee.

    CertificationAll documents must be certified as a truecopy of the original by a person who isauthorised to certify documents in yourcountry of residence. In many countriespeople such as lawyers, Justices of the Peaceand Public Notaries (among others) areauthorised to certify documents.

    1. Your nominated occupation

    This section deals with your nominatedoccupation and reason for assessment.

    Choose your occupation carefully. Makesure that you read the description ofthis occupation thoroughly and thatyou meet all requirements.

    1.1 This is the title of the occupation thatyou nominate from the SkilledOccupation Lists on the Department ofImmigration and Border Protection(DIBP) website at www.immi.gov.au

    We will only assess occupations ifVETASSESS is listed as the relevantassessing authority on DIBP's SkilledOccupations Lists. Nominate oneoccupation only on this form. If youwish to nominate more than oneoccupation, you must complete aseparate application form and pay anadditional fee for each.

    1.3 Cross one of the visa types - GeneralSkilled Migration (GSM), EmployerNomination Scheme (ENS), RSMS,Temporary Work (Skilled) visa (subclass457) or Temporary Graduate visa(subclass 485).

    1.4 Cross whether you require an AdvisoryLetter to give to DIBP for Points Testpurposes. GSM applicants, includingthose applying for state or territorysponsorship, may apply to DIBP forpoints towards the Points Test. DIBPhas authorised VETASSESS to providean opinion on qualifications and/oremployment for DIBP's considerationof a claim for points for these items inthe GSM Points Test. VETASSESS willassess their highest qualification up topostgraduate doctoral degree levelunless the applicant requests anotherqualification to be assessed for pointstest purposes. In some cases VETASSESSwill need to consider two or morequalifications to determine whethertogether they are comparable to theeducational level of a qualification onthe Australian Qualification Framework.

    For Points Test purposes, VETASSESS willconsider any Australian or overseasemployment undertaken during the

    last 10 years to provide an opinion onwhether it is closely related to thenominated occupation. See Section 7for a definition of closely relatedemployment for Points Test purposes.Please visit the DIBP website for furtherinformation about the Points Test.

    VETASSESS does not provide advice onwhich migration scheme or visa categoryis suitable for you. Please refer to theDepartment of Immigration and BorderProtection (DIBP) for information aboutmigration issues (www.immi.gov.au).

    2. Your personal details

    This section asks for all your personaland contact details.

    Complete this section carefully andmake sure you provide all requiredinformation. If you are nominating anagent, their details are to be entered atSection 3.

    3. Authorising an agent

    Complete this section if you wish toappoint an agent to communicate withVETASSESS on your behalf. Please notethat all correspondence will be sent toyour agent. Go to Section 4 if you do notwish to appoint an agent.

    Fill this area in if you know the nameof the person who will be your agentfor your application for a VETASSESSSkills Assessment. If you do not knowthe agents name, the company namewill be sufficient. To operate legally inAustralia, a migration agent must beregistered with the Migration AgentsRegistration Authority (MARA) andmust have a MARA number.     4. Your school education

    This section is about your school yearsup until you began tertiary or highereducation.

    4.1 Write the start and finish dates (monthand year only), and the total numberof years that you attended primaryschool . Pr imary school ing (orelementary schooling) usually refers tothe first six to eight years of compulsoryschooling. It may also include acompulsory kindergarten year. Thenumber of years of primary educationvaries with different school systems.

    4.2 List the country or countries in whichyou completed these years of schooling.

    4.3 Write the year when you completedprimary schooling.

    4.4 Write the start and finish dates (monthand year only), and the total numberof years that you attended secondaryschool. In some countries secondaryschooling may vary between fourto eight years following primaryschooling. Include any upper secondaryor senior secondary schooling you havecompleted.

    4.5 List the country or countries in whichyou completed these years of schooling.

    4.6 Write the name of the highest (eithersecondary school or upper secondaryschool) certificate you obtained inyour language and translated intoEnglish.

    4.7 Write the year when you completedthat certificate.

    5. Your post-secondary education

    This section covers your education aftersecondary schooling. Fill in eachqualification separately. If you have morethan two qualifications, provide detailsof any additional qualifications in thesame format on a separate piece ofpaper. Sign and date this piece of paperand attach it to your application form.

    VETASSESS will assess the most relevantqualifications, which are at the requirededucational level for your nominatedoccupation. More than one qualificationmay appear on your final assessmentletter. If you hold both Australian andoverseas qualifications, provide detailsof both and VETASSESS will assess asrequired.

    For Points Test purposes, VETASSESS willassess the qualification with the highesteducational level in Australian terms,unless you request otherwise. Please alsolist any underpinning qualifications(including the entry qualification to theprogram leading to the qualification youwant assessed).

    5.1 Fill this section in if you know whatyour student registration number was.

    5.2 Write the title of your qualification inEnglish.

    5.3 What is the major field of study in yourqualification?

    Your qualification or academictranscript/mark sheet may state yourmajor field of study. The major field ofstudy is the concentration of subjectswhich was the main focus of theeducational program leading to yourqualification. The major field of studyusually includes a progression ofsubjects of increasing depth and/orspecialisation. If your qualification hastwo majors, please provide details forboth.

    5.4 Name the awarding body. This is theorganisation that awarded yourqualification and it will be written onyour qualification. This may be differentto the campus of study.

    5.5 Write the full address of the awardingbody.

    5.6 Write the name of the campus that youattended.

    5.7 Write the name of the institution youattended, if this institution was not theawarding body.

    5.8 List the normal entry requirements foryour course or the name of theexamination that you had to completeto gain entry. Give the name of anyeducational qualification normallyrequired for admission to this course.

    6. Verification of Chinese qualifications

    This section is for applicants forassessment of qualifications awardedin the Peoples Republic of China. Go toSection 7 if you do not requireassessment of qualifications fromChina. If you have more than twoqualifications, provide details of anyadditional qualifications in the sameformat on a separate piece of paper.Sign and date this piece of paper andattach it to your application form.

    6.1 Cross Yes if you have qualification(s)awarded by an institute in the Republicof China.

Beforesubmitting transcripts to VETASSESS,please check them to ensure that theyare officially issued and all subjectnames, scores and credits of the Englishversion match the original Chinese one.Inconsistent information in thedocuments may cause considerabledelay or a negative verification result.

    7. Your employment

    This section is about your employmentcompleted anytime in the five years upto the date of lodgement of yourapplication to VETASSESS for a SkillsAssessment. If you also require youremployment considered for Points Testpurposes, you may include employmentcompleted anytime in the past ten years.Go to Section 9 if you are applying for aQualifications Assessment for 485 visapurposes.

    For the Skills Assessment, only paidemployment within the last five yearswhich is highly relevant to thenominated occupation and at anappropriate skill level for your nominatedoccupation will be considered. You maydescribe other employment in your CV/rsum.

    VETASSESS defines highly relevantemployment to the nominatedoccupation as paid employment of atleast 40 hours per fortnight in which themajor everyday tasks closely match themajor tasks undertaken in thenominated occupation in Australia and

    are performed at an appropriate skilllevel for the nominated occupation inAustralia. The employment must alsomatch the lead statement in theAustralian and New Zealand StandardClassification of Occupations (ANZSCO)for the nominated occupation.

    For the optional Points Test advice toDIBP about employment within thelast ten years, VETASSESS applies DIBPsd e f i n i t i o n o f c l o s e l y re l at e demployment to the nominatedoccupation. DIBP defines closely relatedemployment as paid employment of atleast 40 hours per fortnight in anoccupation which has the same skill leveland draws on a similar skill set andsubject matter as the nominatedoccupation. Usually closely relatedemployment is in an occupation whichis in the same Unit Group of ANZSCO asthe nominated occupation.

    If you have worked in more than threepositions, please provide details of anyadditional employment in the sameformat on a separate piece of paper. Signand date the paper and attach it to yourapplication form.

    In calculating years of paid employment,VETASSESS will count employment of atleast 40 hours per fortnight. Do notinclude weeks of less than 40 hours perfortnight even if the average hours ofyour employment was 40 hours perfortnight or more.

    Please list all positions which are relevantto your nominated occupation. List yourmost recent employment first.

    7.1 Write the business name of theemployer for whom you work(ed).

    7.2 Write the former or other names thatthe business is known by.

    7.3 Write the complete street address ofyour employer, including the streetname, number, suburb or town, stateor territory, postcode and the country.

    7.4 Write the name of the employer/supervisor or manager for the person(usually the Manager or HumanResources representative of thecompany that employed you) whoVETASSESS may contact in order toverify any of your employment history.

    7.6 Write your position/job title with thisemployer as statement on the officialevidence. If you held more than oneposition/job with the same employer,you can describe your other roles inyour CV/resume.

    7.7 Write the date when you startedemployment in this position.
