• In order to submit a renewal request that requires a CBC, existing fingerprints must be on record.
• If existing prints are not available, the following three scenarios are possibly occurring:

No Fingerprints on NMLS (last result was a Namecheck)
CBC request must be submitted through the Individual (MU4) Form and fingerprints captured by Fieldprint.
Pending Fingerprint Status
Individual must schedule a fingerprint appointment through Fieldprint website.

• Workflow used if company plans to pay for license renewal
• Applicable only for sponsored licenses
Attest and Pay
• Workflow used if individual will pay for license renewal
• Available for any license eligible for renewal (regardless of sponsorship status)
• Available for licenses that may have already gone through the Attest for Company workflow
Do Not Renew
• Workflow used if individual does not plan to renew a license for 2015
• Available only to individuals, companies do not have the ability to submit a Do Not Renew request on behalf of an individual.
• When license is attested, notification is sent to sponsoring company
• Individuals still have the option to pay renewal fees
