HOSA Advisors,

This packet will provide you with important information regarding the registration process, deadlines, and the competitive events schedule for the Utah HOSA State Leadership Conference on March 19-20, 2020. You will find all of the necessary registration materials at the Utah HOSA website www.uthosa.org .

Registration Procedures, Pre-Conference Submissions & Due Dates Provided are step-by-step instructions for registering your chapter for competition. Please don’t hesitate to ask if you have any questions. We would like to remind you to PLEASE ADHERE TO DEADLINES so we have enough time to ensure your students compete in their desired events. Students MUST be registered as HOSA members before you register them as delegates for competition. Don’t forget that you also MUST be registered for SLC as an Advisor. The registration fee is $40 per participant. There are no refunds after March 5, 2020.

We will be hosting our 14th Annual Utah HOSA 5K Scholarship Fun Run on Friday, March 20 at 7:00 AM. The Fun Run is optional and costs an additional $10 for each runner and an additional $10 for a Fun Run t-shirt, please fill out request on the registration form with shirt size if purchased. (T-shirts may be purchased even if you are not participating in the run.) All proceeds from the Fun Run will be donated to the Utah HOSA Scholarship Fund.

At the Utah HOSA SLC, delegates may be registered to compete in multiple events that do not overlap or conflict. Help your delegates learn the rules and guidelines of their event(s) and what supplies they are required to bring with them. Delegates MUST attend the event orientation to be eligible to compete. There are several events that have two rounds. The test score from Round ONE will be used to qualify the competitor for Round TWO. Round TWO qualifiers will be posted near HOSA headquarters and Utah HOSA social media sites (Twitter, Instagram, and Facebook). Remind your delegates to bring a Photo ID with them to the event orientation. School issued ID is acceptable for Utah HOSA State Leadership Conference (SLC) only. HOSA International Leadership Conference (ILC) will require a government issued ID. Please Note: Delegates attending the HOSA ILC in June will be eligible to participate in only ONE EVENT, excluding Recognition Category Events.

Necessary Forms Please have all conference participants review, fill out and sign each of the following forms: Code of Conduct, HOSA Dress Code, Medical Liability Release, and Media Release forms. This includes competitors, advisors, chaperones, and guests. Advisors are required to bring a copy of each participant’s forms to the conference. Utah HOSA will request a copy for all ILC qualifiers and/or in the event of conference violations. Please strictly enforce the Dress code by being proactive in educating your delegates on proper professional dress for the conference. As a reminder, no delegate will be permitted on stage during the award ceremonies if not in proper business attire. Competitors will need to set up an account on a new platform Tallo by the Submission Deadline of March 13, 2020. This platform will be used at ILC as well. Please plan to attend the Mandatory Advisor Meeting on Thursday, March 19, immediately following the opening ceremonies.

Utah HOSA 2020 State Leadership Conference Advisor Preparation Checklist

October ___ Attend the FLC Advisor Meeting ___ Join our SLC Team by signing up with a Category Chair to help with a Competitive Event, or become an Event

Manager, or a Judge Manager, etc. November - December ___ Choose a Hotel and reserve a number of hotel rooms for you and your students who plan to attend the SLC. ___ Help students identify, review, prepare, and study for their competitive events. Valuable resources may be

found at http://www.hosa.org under the Competition section. This will help your students decide which event(s) is(are) best for them. We encourage all participants to become familiar with the Judge Rating Sheets, found in the event guidelines, and to practice scoring each other with them.

___ If you are assisting in an event, review the competitive event guidelines for that event. ___ Visit the Utah HOSA Google drive, Quizlet, and other on-line resources for study material. Events that require

time such as Community and Awareness and Healthy Lifestyle should be in process. ___ Submit travel request forms for SLC and ILC to your District. January ___ SLC Registration opens – Wednesday, January 1, 2020 through Sunday, March 1, 2020. ___ Judge Managers – contact judges for your event (see the Judge Manager Checklist). ___ Applications for the Executive Council (State Officers) are available. ___ Practice with your students, have students use the Judge Rating Sheets and practice verbalizing skills.

Consider arranging a competition with and against other schools in your district/region. February ___ Judge Managers – Complete and send your event scenarios and supply list to your Category Chair. ___ Identify those events that require a Tallo submission and help your students practice uploading info into Tallo ___ Prepare ALL special events, awards, officer and scholarship applications. ___ Verify your delegates have the appropriate supplies including notebooks and portfolios as required by different

competitions. ___ Practice with your students, have students use the Judge Rating Sheets and practice verbalizing skills.

Consider arranging a competition with and against other schools in your district/region. ___ Cancel any unneeded hotel rooms - Before canceling any room PLEASE send an email to Denise so she

Utah HOSA 2020 State Leadership Conference Registration Procedures

1. Registration begins on Wednesday, January 1, 2020 through Sunday, March 1, 2020. There is a

late registration period between March 2-5th ($100 late fee per chapter) where you may add or make substitutions for some competitive events. Be aware that not all events are available for late submissions. There will be NO refunds after March 5th.

2. Instructions for Online Registration: a. Go to the HOSA website at www.hosa.org.

b. Select login from the menu on the top of the screen.

c. Find the Local Chapter Advisors section and click on the Access Chapter Advisor Services.

d. Enter your Charter Number and Password and login.

e. Select Conference Registration from the menu.

f. You will be prompted to enter your Charter Number and Password again.

g. Select “Utah State Conference” and then click “Register Members” at bottom of the screen.

h. A complete list of your students that have been registered for HOSA will appear. Beside each student’s name, there is a link to register that student for the conference. Click register for the person/student you want to enter and a screen with all of the options will come up. Fill in all necessary information.

i. Don’t forget that ALL advisors and chaperones must be registered for the conference as HOSA members or guests. These names are also viewable.

j. You will need to do the following for each conference attendee: i. Specify Male or Female. ii. Select Registration Type (i.e. Advisor, Chapter Officer, HOSA State Officer, Middle School,

Secondary, Post-secondary, Chaperone, Guest). iii. Email addresses are also required. iv. Select the Activities and the Competitive Event(s) the person will participate in. v. All team member must be added to the event. vi. Once you have made all of the above selections, you will click save at the bottom of the

page. vii. You may make any necessary changes until March 1, 2020.

Utah HOSA Code of Conduct

A good reputation enables HOSA members to take pride in our organization. Your conduct at any HOSA function should

make a positive contribution to the reputation that has been established.

1. Your behavior at State Leadership Conference (SLC) should at all times be such that it reflects credit to you, your school/college, your state and HOSA.

2. You will be responsible and held accountable for your own conduct. Your chapter advisor will need to be informed of your activities and whereabouts at all times. (HOSA Conference name badges shall be worn at all times at SLC).

3. You may not use personal vehicles to travel to and from the State Leaderships Conference center or any offsite event facilities. State and District transportation will be provided.

4. You are expected to attend all SLC general sessions and other scheduled conference activities. Please be prompt and show respect to those in the audience and on stage.

5. You are to report any accidents, injuries or illnesses to your local chapter advisor immediately.

6. Personal prescriptions and over-the-counter drugs must be labeled properly and only administered in the presence of the local chapter advisor. Sharing of such medications is strictly prohibited.

7. The Utah HOSA SLC is a non-smoking conference (this includes vaping, e-cigarettes and Juuling, etc.).

8. All students/participants attending the Utah HOSA SLC may not purchase, consume or be under the influence of alcohol or drugs at any time. Violators will be subject to stringent disciplinary action.

9. No individuals from the opposite sex, other local chapters (unless permitted by the Advisor), or outside groups are to be allowed into your hotel room.

10. You are expected to observe the designated curfew 11pm unless otherwise posted. (Curfew means that you are in your own room by the designated hour.)

11. If you are found responsible for stealing or vandalism, you and your parents will be obligated to pay any and all damages.

12. Students who disregard the rules will be subject to disciplinary action and will be sent home at their own expense. Parents will be notified as well as School and District Administrators.

13. Do not charge anything to your hotel room. Do not charge anything to the Utah HOSA account. You will be asset a fine.

14. You are to abide by the HOSA Dress Code at all competitive events, general sessions and other conference activities.

GENERAL SESSION PROTOCOL: The general sessions should be fun and enthusiastic while maintaining a respectful attitude towards others. Please use phones in a professional manner. It is important to remain seated until the end of the session.

Utah HOSA Dress Code

Dress for success and maintain the professional image of HOSA Future Health Professionals while you are at the State Leadership Conference (SLC). Pay attention to the requested Dress Code for the different conference situations. Students not adhering to this Dress Code will not be allowed on stage to receive awards. Professional attire: Professional attire is generally conservative and should look appropriate in any office setting. It should not look like party or picnic clothing. Don’t confuse club attire with business attire. If you would wear it to a club, you probably shouldn’t wear it in a business environment. Women may wear a skirt or pant suit with comfortable heels. While men may wear a blazer/suit jacket and pants with a button down shirt, a tie and dress shoes. Avoid tight or baggy clothing. Your clothing should be clean and well pressed. Hair and make-up should be conservative. Perfume or cologne should be used sparingly. Avoid smelling of body odor or smoke. Men’s Business Casual attire:

• Dressy slacks or pressed khakis; no jeans or shorts. • Button down shirts, polo’s with collar and pullovers or sweaters worn over a collared shirt are appropriate. • Ties and belts are optional and always appropriate. • Dress shoes with socks should be worn; no sandals, athletic shoes or hiking boots.

Women’s Business Casual attire:

• Slacks, pressed khakis or skirts are appropriate. Avoid skirts that are above the knee or have revealing slits above the knee. Colors should generally be conservative; navy, black, gray, brown or tan.

• Blouses, button down shirts, polo’s with collars and sweater sets are appropriate attire. Avoid velvets and shimmery fabrics which are more suitable for parties are not appropriate at this conference. Avoid low cut or tight fitting shirts. Cleavage should be covered and is not appropriate.

• Keep your makeup, hair and jewelry choices simple and conservative. • Shoes should be flats, loafer or closed-toe pumps that you can walk comfortably in. Avoid thin strapped shoes, high-heels,

platform shoes, flip flops and sneakers. • Dress socks or hose is recommended. • If you carry a purse, keep it small and simple.

HOSA Casual attire: HOSA casual attire is for all social functions in conjunction with the SLC. Clothing may be casual and used for everyday wear. EXCEPT the following: tank tops, halter-tops, extremely short shorts or skirts and cut-offs. The length of shorts and skirts must go to your fingertips while standing. Shirt straps must be two inches wide. Competitive Event attire:

• Competitors and event personnel should reference the event guidelines for desired attire. General Session attire:

• Attendees should be in either professional attire or they may wear their clinical attire that was worn for skill event testing. International Leadership Conference General Session attire:

• Black or Navy-Blue Suit with white blouse or button down shirt. • Males wear a tie, dress socks and shoes. • Females may choose to wear knee-length skirt or slacks, dress socks or hose with closed-toe blue or black shoes.

Official HOSA Uniform:

• Tailored navy blazer with emblem affixed over the heart, with matching slacks or knee length skirts. • White button down collared shirt or blouse. • Female officers choose either maroon HOSA scarf or tie as an accent, and dress socks or hose with closed-toe navy or black

shoes. • Male officers choose a navy or maroon tie and wear dress shoes that are navy or black with matching socks. • Official name tag is to be worn.

Competitive Events Management Team HOSA Competitive Events Management Team Everyone who works with HOSA is committed to the highest standards when implementing the Competitive Events program. Integrity and honesty are core values of HOSA. The Competitive Events Management Team is responsible to manage the events as outlined by HOSA HQ. The CE Team and their roles are listed below. The State Advisor – Denise Abbott The State Advisor oversees all aspects of the state conference and competition. They ensure all competitors are correctly registered for their events, all event results are tabulated accurately and reported to HOSA HQ. They work with the competitive events chair to resolve any issues to assure that all events are managed equitably. The Competitive Events Chair – Robbie Rauzi The Competitive Events Chair is the conference and competitive event lead. They also work directly with the onsite and offsite facility personnel. They serve as the liaison between the Category Chairs and the State Advisor. They are stationed at HOSA headquarters and visits the events during the competition. They serve as the liaison with the hotel, conference center and/or the competitive events sites. The Category Chair (CC) The Category Chairs provide training and support to the Event Manager & Judge Manager to ensure they have the tools, knowledge, and skills to successfully manage a HOSA competitive event. They may help in recruiting judges and writing/reviewing scenarios and topics. During SLC they are on-site able to help their team as needed. The Event Manager (EM) Event Managers coordinate and assist the Judge Manager with pre-event planning and the scheduling of judges and event personnel. During the conference, the EM ensures the integrity of the event by conducting and managing the details, personnel, timekeepers, patients and proctors, etc. This includes making sure the event personnel are where they are supposed to be, that everyone follows the guidelines, the event runs on time and oversees the cleanup of the site after the event. Judge Manager (JM) Judge Manager is in charge of the judging process of their event. This includes finding/scheduling judges, writing/reviewing scenarios, and making sure that the guidelines are followed. Other members of the Event Management Team may assist in the process. At the competition, they meet with and orient the judges. They will triple check all rating sheets and timekeeper logs for accuracy.

Category Chair Checklist Category Chair The Category Chairs provide training and support to the Event Manager & Judge Manager to ensure they have the tools, knowledge, and skills to successfully manage a HOSA competitive event. They may help in recruiting judges and writing/reviewing scenarios and topics. During SLC they are on-site able to help their team as needed. 2-3 Months Before Competition ________ Familiarize yourself with the events within your category.

________ Read and understand all event guidelines within your category.

________ Finalize your list of Event Managers and Judge Managers.

________ With the help of your Judge Managers, finalize your list of Judges for each event. Event Managers will need to create a final list of Event Personnel. Send this list to Robbie and Denise by March 1st.

________ Skilled event(s) - Judge Managers will need to select event procedures and written scenarios which include skills along with a list of necessary supplies/equipment. If you need ideas, please use the Event Managers to assist. Send this list to Robbie and Denise by March 1st.

1-2 Weeks Before Competition ________ Skilled event(s) - Make sure that your Judge Managers have and bring all

necessary equipment for their skills. The offsite locations will not be providing the equipment.

________ Contact your Event Managers and Judge Managers to remind them of their event assignments. Ask them to do the same with their Event Personnel and Judges.

3-Days Before Competition ________ Send reminder e-mail to your EM’s and JM’s reminding them when to arrive and where to

check-in. Ask them to do the same with for their Event Personnel and Judges.

At Competition ________ Welcome your EM’s and JM’s and confirm that everything is in order for their event. ________ Help the JM welcome and check-in judges, they will receive a judge badge, map, meal ticket,

and a gift. ________ 20-minutes prior to each event assure that rooms are set up and ready.

________ Ensure that all events run equitably and on time.

________ Assist with any issues: registration, dress code violations, Student Identification (ID), etc. Report these on the competitor list. Please remember that violations do not disqualify a competitor. Please communicate ALL major concerns and violations to HOSA HQ (Competitive Event Chair & State Advisor).

After Competition ________ Confirm that event boxes, top ten lists, and competitor sheets for ALL events are submitted

Properly through Google Sheets.

________ Gather Judge information sheets from your JM’s and turn in to HOSA HQ so that we can contact judges in the future.

1-Day Before Competition ________ Assist the JM in contacting Judges and reminding them of the conference.

At Competition ________ Report to your Category Chair. ________ Sign out your event box at HOSA Headquarters and prep your rooms 20 minutes prior to event.

________ Run an equitable and timely event.

________ At event check-in: - Check competitors for valid Photo ID (school issued ID is acceptable for SLC only).

HOSA International Leadership Conference (ILC) will require a government issued ID. - Check for appropriate dress, report on competitor sheet if any violations occur. - Have competitors remove nametags along with any items that may distinguish their school

or chapter affiliations. - Please remember that violations do not disqualify a competitor.

________ On written tests make sure the answer sheets are identified as Middle School, Secondary or Post-secondary. We will provide fluorescent dots to use.

________ For round 2 events, please complete your qualifier list quickly. If round 2 is off-site then remind qualifiers of the bus schedule and to plan accordingly.

After Competition ________ Complete your top ten list quickly and submit this as well as the competitor list to HOSA HQ.

________ Sign in your box to HOSA HQ when you have completed your event.

Utah HOSA 2020 State Leadership Conference Conflicting Event Guide

At the Utah HOSA SLC, delegates may be registered to compete in multiple events that do not overlap or conflict. Delegates MUST attend the event orientation to be eligible to compete. There are several events that have two rounds. The test score from Round ONE will be used to qualify the competitor for Round TWO. Round TWO qualifiers will be posted near HOSA headquarters and Utah HOSA social media sites (Twitter, Instagram, and Facebook). Remind your delegates to bring a Photo ID with them to the event orientation. School issued ID is acceptable for Utah HOSA State Leadership Conference (SLC) only. HOSA International Leadership Conference (ILC) will require a government issued ID. Help your delegates learn the rules and guidelines of their event(s) and what supplies they are required to bring with them. Please Note: Delegates attending the HOSA ILC in June will be eligible to participate in only ONE EVENT, excluding Recognition Category Events.
