Statement of Qualificationsfor

At-the-Ready (ATR) Construction Inspection Services Vermont Agency of Transportation Municipal Assistance Bureau

SOQ Introduction

In response to the Vermont Agency of Transportation Municipal Assistance Bureau (VTrans-MAB) Request for Qualifications (RFQ), dated November 28, 2016, Staff Sterling Management, LLC (SSM) is pleased to present this technical proposal for performing Construction Inspection Services on an “At-the-Ready” basis for municipalities developing a variety of local transportation improvement projects.

As stated in the RFQ, this is the first stage of a two-stage process and no contract between a consultant and VTrans will result from this RFQ. Instead, VTrans will use responses to the RFQ to make a determination on which consultants are the most qualified to assist municipalities. Those consultants judged to be most qualified will then be listed on a publicly-available Qualified Roster for local government transportation projects. Municipalities will subsequently, under the second phase, send a Request for Proposals (RFP) to a consultant listed on the Qualified Roster when services are required for a local transportation project. The municipality’s RFP will detail the specific Scope of Work needed at that time. The consultant’s response to the RFP may result in a contract between the municipality and the consultant.

All work performed by consultants for municipalities will be accomplished in accordance with the following: A Scope of Work (SOW) to be separately identified by the Municipality for each project, in an RFP

developed by the Municipality in consultation with VTrans. Such RFP will then be sent to a consultant on the Qualified Roster.

Introduction to the Firm

Staff Sterling Management, LLC (SSM), was founded by Michael Stafford in January, 2000. The company is owned and operated by Mr. Stafford and based in Morrisville, VT. Mr. Stafford is a Viet Nam-Era Veteran and the company is registered with the Vermont Secretary of State to operate as a limited liability company in Vermont. SSM specializes in providing construction inspection, owner representation, construction project management andother construction-related services to clients with a focus on projects in the transportation, public infrastructure, environmental and commercial building arenas. Since the inception of the business, SSM has worked on projects in Vermont, several other states and in a few foreign countries.

Collectively, SSM’s professional team has multiple decades of experience as construction resident engineers, construction managers and construction field inspectors. Mr. Stafford, Company Principal, and our designated Project Manager for this assignment are both members of the Project Management Institute (PMI) and all services performed by SSM follow the high standards established by PMI.

Regarding projects coordinated through the VTrans Municipal Assistance Bureau Local Projects Section(MAB-LP), SSM is currently or has provided Construction Inspection Services for twelve Vermont municipalities and a state agency on eighteen such undertakings since 2008. Descriptions for these projects, along with references for each, may be found in the “Qualifications and Experience of Firm”section of this technical proposal.

SSM is also currently providing Municipal Project Manager (MPM) Services to four Vermont municipalities developing transportation-related projects through the VTrans MAB-LP.

In addition to the aforementioned projects coordinated through VTrans MAB-LP, SSM has provided Project Management and Construction Inspection services for several other municipal and State of Vermont building and infrastructure construction projects that were developed outside of the MAB-LP process. This includes managing fifteen construction projects for the Vermont State Parks.

SSM is fully insured in accordance with the Consultant Contract Provisions as described in the VTrans MAB-LPGuidebook for Locally Managed Projects. A properly completed certificate of insurance will be provided to any municipal clients upon request.

It is SSM’s policy to make good faith efforts to utilize Disadvantaged Business Enterprises (DBEs) to provide services for us whenever possible. We do have a copy of the latest VTrans DBE Registry on hand.

SSM does have a current AF38 Form on file with VTrans.

Company Principal Biography

Staff Sterling Management is owned by Mr. Michael Stafford, EIT. He has over 40 years of construction and project management experience and is a member of the Project Management Institute. Michael also holds a licensure as an Engineer-in-Training. His range of project history includes; land surveying for subdivisions, infrastructure construction, institutions construction, and building of power plants, petrochemical plants and public service plants. He directed operations for a division of a multi-national corporation, where he was heavily involved with environmental remediation work, including: composting, pumping, treating and disposal.

Michael has worked throughout the United States and abroad and has foreign experience coordinating multi-national participants on projects. He is currently performing consulting work for the federal government on chemical demilitarization projects and has served in management positions on eight nuclear power plant projects. These assignments have provided him with a wealth of knowledge regarding federal procurement guidelines and federal contracting regulations. He is a Certified Excavation Supervisor and for eight years held a certification as a Hazardous Waste Supervisor. Michael has also received training in Hazardous Materials Management and OSHA regulations.

Regarding expertise in the transportation arena: Michael is a former member of the Lamoille County Transportation Advisory Committee, which interfaces closely with VTrans on transportation planning; he has conducted trainings on the 2004 Manual on Uniform Traffic Control Devices; and he is fully familiar with the standards set forth in the 2002 VTrans Pedestrian and Bicycle Facility Planning and Design Manual. Michael is also a member of the board of directors of the Vermont Environmental Consortium and formerly served as chair of the Vermont Trails and Greenways Council.

General Approach to Municipal Construction Inspection Contracts

On any given Construction Inspection contract with a municipality, it would be our plan to assign a team that includes a Project Manager and a Construction Specialist (On-Site Construction Inspector) to work with thatmunicipality:

Mr. Pat Travers will serve as SSM’s Project Manager. In addition to coordinating, overseeing and supporting the activities of our Construction Specialist, he will participate in all planning and scheduling activities, manage the project budget, moderate project meetings, formally sign contractor payment requisitions, provide progress reports to the Municipal Project Manager (MPM) and other stakeholders as necessary, and coordinate all contract administrative tasks. Pat will be our primary administrative liaison to the Municipality, the Design Engineer and VTrans and he will be directly responsible for ensuring that SSM fulfills all of its contractual obligations as the Construction Inspection Consultant.

Working under Mr. Travers, SSM has a team of four Construction Specialists, i.e., On-Site Construction Inspectors, whom we could assign to any municipal Construction Inspection contract. The assigned Specialist will be at the project site daily; overseeing the work performed by contractors, keeping track of quantities, writing and submitting daily work reports and supervising all required materials testing. He will inspect all work in progress to ensure that it is performed to plans and specifications and will interface with the Design Engineer when any field issues arise. He will communicate daily with the MPM, to keep him/her abreast regarding work progress and make them aware of any challenges that arise in the field. He will also be our liaison to the general public and will handle any concerns raised by municipal officials, nearby residents and abutting property owners.

Our Project Manager has worked together with each of our Construction Specialists on several prior projects coordinated through the VTrans MAB-LP Program. All possess a thorough knowledge of the VTrans Standard Specifications and are intimately familiar with the proper procedures and requirements laid out by the MAB-LP Program. In addition, they have established an efficient methodology for working together, making sound judgment calls and quickly solving any challenges that arise in the field.In addition to the Project Manager and Construction Specialist, on each project we will engage the services of a materials testing sub-consultant to perform on-site and laboratory materials testing services. These services will be engaged through fully-executed sub-consultant contractual agreements that will adhere to the Specifications for Contractor Services as described in the VTrans-MAB Guidebook for Locally Managed Projects.

SSM’s approach as a Construction Inspection Consultant is to establish the Project Owner’s interest as our primary objective. To that end, we have developed the following work protocols:

1. We diligently oversee the work in progress to ensure that it is built to the Contract Documents and is in compliance with the Contract Drawings the VTrans Standard Specifications, Special Provisions, permits and other requirements. This will provide the Municipality with a properly constructed and high quality end product.

2. We keep a close eye on work progress versus the planned project schedule; in-place quantities versus original estimates; and actual project costs versus the budget. Should any troubling variances arise, we quickly take actions to try to identify the causes and formulate solutions for keeping the Municipality’s costs under control and getting the project back on its proper track.

3. If any design issues arise, our Construction Specialist will promptly inform our Project Manager and the MPM and then work on a solution with the Design Engineer, thereby minimizing any project delays.

4. We closely examine all monthly payment requisitions put forth by the General Contractor to ensure that quantities being billed are accurate and in agreement with the quantities recorded by our Construction Specialist. This will prevent any over-billing charged to the Municipality.

5. When a situation arises where a change order to the contractor is merited, we will first develop our own estimate of what we believe the change should realistically cost. We will then scrutinize the Contractor’s proposed cost and compare it to our estimate to verify that the Contractor is being fair. Should there be a discrepancy between our estimate and the Contractor’s proposal, we will work with the Contractor on coming up with a cost that is fair to the Contractor while protecting the Municipality’s budget.

6. It is our policy to be reasonable regarding our Construction Specialist’s hours at any project site. To that end, he will be on this project site only as the project’s needs dictate. Therefore, at times when he is not needed on site for a full day, he will not be there. This will prevent any unnecessary fees being charged to the Municipality on our part, thereby keeping the Municipality’s budget for inspection services in line. Conversely, we will not short-change the project by unnecessarily cutting the hours that our Construction Specialist is present at the project. We will ensure that he is on site when critical activities are taking place to verify that they are carried out properly. SSM will also coordinate and schedule on-site materials testing activity so that it is carried out in the most efficient and cost-effective manner.

For all required laboratory and field materials testing services, we will, depending upon project location and availability, utilize the services of one of these three firms: Alpha Testing and Engineering, LLC, of Milton, VT; Beck Engineering, PC, of Bondville, VT; or Optimum Testing of Derby, VT.

Alpha Testing and Engineering, LLC:

Alpha Testing and Engineering was founded by Bob Lovgren and Patti Reed in 2009. The company offers a full-service soils and concrete testing laboratory; which provides pre-construction materials qualification as well as quality assurance / quality control lab and field testing during construction. Additionally, Alpha has developed an extensive network of consultants with specialized expertise in a variety of disciplines to supplement their in-house capabilities. Alpha’s clients include; contractors, engineers, architects, municipalities, state agencies and private individuals.

Specific services offered by Alpha Testing include: construction material laboratory testing; compressive and flexural testing for concrete, masonry and soil-cement samples, using a 400,000 pound hydraulic compression testing machine; comprehensive field inspection and testing during all phases of construction; in-place soil and bituminous concrete density testing, utilizing nuclear moisture-density gauges; pile driving and load test observation and inspection; structural steel inspection; evaluation of steel erection practices and procedures; paint and coating inspections; fireproofing material inspection; equipment calibration services; and forensic testing of failed or damaged construction materials.

The professionals at Alpha Testing have a combined 60+ years of experience in the field and their technicians are certified. The company has an impressive background in performing test procedures in accordance with ASTM, AASHTO, VTrans, ACI and FAA specifications and regulations.

Alpha Testing and Engineering is certified by VTrans as qualified for performing work on VTrans-funded projectsand has performed testing services for SSM on several VTrans MAB-LP projects over the past few years.

Beck Engineering was founded in 1997 by Matthew H. Beck, P.E., who is licensed as a Professional Engineer inVermont and New Hampshire. Since its start twenty years ago, the company has offered a range of on-site andlaboratory testing services for concrete and soil materials. In addition to Mr. Beck, who has 34 years of experienceas a civil engineer, the company employs one other civil engineer and a testing technician, who is certified as aconcrete technician and as a Troxler technician, with competency in the use of nuclear moisture density gauges. Optimum offers a full-service soils and concrete testing laboratory; which provides pre-construction materials qualification as well as quality assurance / quality control lab and field testing during construction. Optimum has provided a variety of materials testing services on a wide range of projects stretching from the Barre / Montpelier area to the Canadian border, on projects ranging in size up to $60 million

Qualifications and Experience of Firm

Since 2008, Staff Sterling Management (SSM) has provided Construction Inspection services on eighteen locally managed transportation projects that were developed through the VTrans-MAB Local Projects Section. These projects have been located statewide and have included sidewalks, shared-use recreation paths, road reconstruction and intersection reconfiguration. Many of these have also involved significant drainage system improvements, curbing installations and the construction of retaining walls. Projects have ranged in size from $100,000.00 to $1.7 million.

SSM has also performed Construction Inspection services for three City of Burlington transportation projects that were built to the VTrans Standard Specifications, but were financed and developed outside of the VTrans-MAB LP Process. Similar services were performed for the Town of Morristown for two sidewalk construction projects in Downtown Morrisville.

Additionally, SSM provided Construction Inspection services for the VT Dept. of Fish and Wildlife on a $700,000.00 boat launch, parking area and access road project that was constructed to the VTrans Standard Specifications.

Experience with Federally Funded Projects and Familiarity with VTrans Specifications

Local transportation projects covered in the Agency’s RFQ will be partially funded by the Federal Highway Administration (FHWA) through the VTrans Municipal Assistance Bureau Local Projects Section. Hence, all of those projects will have to conform to all of the procedures and regulations required by FHWA and VTrans. For the past eight years, the SSM team has worked with several Vermont municipalities and a state agency on transportation projects funded through the VTrans MAB-LP Program, both as a Construction Inspection Consultant and as a Municipal Project Manager. This experience has bolstered us with a thorough working knowledge of the VTrans MAB Project Development Process, other MAB requirements, the permit process and the state and federal funding guidelines surrounding public infrastructure construction. And in addition to the VTrans MAB-LPassignments, SSM has been providing consulting services on federal government projects for many years. Thisexperience has provided us with a comprehensive knowledge of the Code of Federal Regulations as well as federal government procurement guidelines.

Pat Travers, EIT:

Pat Travers, SSM Northeast Manager, is our designated Project Manager and supervisor of inspection for thelocally managed transportation projects. Pat possesses a degree in civil engineering, holds an Engineer-in-Training license and is a member of the Project Management Institute. He has nearly forty years of professional experience in engineering, construction management and business management. Pat’s career has included working for municipal agencies, earth moving contractors, general contractors in commercial construction, design-build firms, a multi-national engineering corporation, a state agency and Staff Sterling Management. He has also owned businesses in the construction, education and agriculture arenas.

Pat commands a strong expertise in the area of public infrastructure projects and has fulfilled responsibilities as a construction inspector, resident engineer and project manager on many public sector projects throughout Vermont, New York State and Connecticut; including those funded through the VTrans MAB-LP Section.

Project management experience for Pat on municipal Construction Inspection Services contracts coordinated through the VTrans MAB-LP Program includes:

Engaged since 2009 by Vermont State Parks to perform project management and owner representation services on multiple capital improvement and restoration projects for historic buildings and other facilities within several state parks in northern Vermont. To date, Pat has been involved with 15 such projects for the State Park system.

Serving as construction manager, coordinator of design services and supervisor of construction inspection for the Town of Morristown regarding its sidewalk and curbing reconstruction project on Portland and Bridge Streets.

Pat’s personal strengths include: strong verbal and written communication skills, diplomatic public relations capabilities, excellent public speaking and public presentation skills, and being able to effectively coordinate effortsamong all those involved in a project, such as owners, design firms, contractors and the general public. Robert “Bob” Rossi

Bob Rossi is one of our four Construction Specialists (On-Site Construction Inspectors). As a member of the third generation in a general contracting partnership, S.V. Rossi Construction Co., Bob grew up watching, studying and eventually working in heavy construction. Reviewing blueprints from an early age, Bob worked on various highway projects; including the multi-million dollar undertaking that involved the construction of a portion of Interstate 91 in the Northeast Kingdom of Vermont. Over a period of three years, Bob fulfilled multiple responsibilities on this project, including assignments as a field engineer, laborer, equipment operator and record keeper.

In addition to his highway construction background, Bob has over 40 years of experience in building construction, building materials and manufacturing. He has been a general contractor in both the residential and commercial arenas as well as a manufacturer’s representative working with builders, architects and customers. As a contractor, he bid on municipal, institutional, and commercial projects, which involved performing quantity takeoffs from blueprints, determining manpower needs and figuring the types and amounts of materials required. He also interfaced closely with architects, builders and purchasing agents.

Other experience for Bob includes having been a sales & project manager for a lumber yard, where he managed all materials and personnel. He also conducted training seminars for the public and other stakeholders, including energy conservation groups and state employees. Early in his career, Bob worked for a large manufacturing company and held the positions of foreman and plant superintendent. In these roles, he was responsible for production scheduling along with the supervision of ten union drivers and two shifts of plant workers.

Bob prides himself on bringing a positive attitude and good work ethic to any project. He strives for efficiency and safety in order to accomplish the goals at hand.

Thomas “Tom” O’Brien

Another one of our Construction Specialists (On-Site Construction Inspectors) is Tom O’Brien. Tom has fifty years of experience in the construction and facilities management industries. In addition to working as a construction inspector for SSM, Tom has spent time as a journeyman carpenter, crew foreman, project superintendent, clerk-of-the-works and owner of a general contracting business. This experience includes time in Vermont, the Boston area and New York City.

Over the course of his career Tom has been involved in a wide array of assignments on a diverse variety of construction projects, including; residential building and renovation, commercial building construction, municipal buildings, schools, site infrastructure and historic restorations. Restaurants, large grocery stores, saw mills and public libraries are among the types of projects with which he has been involved. Tom has also adept at project design and he has worked closely with architects and engineers on design efforts for retail store fronts, house foundations, public buildings and commercial spaces.

Darrell Martin

Darrell Martinis our fourth Construction Specialist (On-Site Construction Inspector) whom we can assign to locally managed transportation projects.

Darrell has over forty years of experience in the construction industry; the vast majority of those as the owner of acontracting company, D. Martin & Company Construction. Darrell has also worked for general contractors in theStowe and Northeast Kingdom regions and has been responsible for supervising up to 22 employees. During histenure as business owner, he managed construction projects in the residential, commercial building and public arenas. Projects involved: remodeling, new building construction, highway construction and earth moving. Clientsincluded: homeowners, developers and the State of New Hampshire.

Through his extensive experience in the construction industry, Darrell has acquired a thorough knowledge of modern building construction methods, materials, practices and procedures. He can also read and interpret complex construction blueprints and specifications and can provide suggestions for improved constructability. Other applicable experience includes: hiring and managing employees, contract administrating, managing of sub-contractors, estimating, bidding, construction scheduling, materials purchasing, residential design and quality control managing. Darrell is computer-literate and is adept with communication via email. He also possesses keen interpersonal skills and is extremely diligent in dealing with residents and property owners who neighbor a project.

Darrell is a Viet Nam-era veteran and retired from the US Army National Guard after 24 years of service. His duties included direct supervision, logistics and short and long term training and planning for as many as 120 soldiers. He also became operations sergeant of a regional training institute, which trained upwards of 500 students per year.
