TRUST PROFILE

Buckinghamshire Hospitals NHS Trust was formed on the 1 April 2003 from the merger of South Buckinghamshire NHS Trust and Stoke Mandeville Hospital NHS Trust. It operates from three hospital sites in Amersham, Stoke Mandeville and Wycombe. The hospitals offer a wide range of high quality acute services at both Stoke Mandeville and Wycombe and some specialist services including:

The National Spinal Injury Centre (Stoke Mandeville) Burn Care and Plastics sub regional centre (Stoke Mandeville) Dermatology inpatient centre (Amersham)

The Trust serves residents in Buckinghamshire, Thame (Oxfordshire), Tring (Hertfordshire) and Leighton Buzzard (Bedfordshire) - a combined population of 550,000. It serves a much larger population of 1.5m for Burns and Plastic Services and 14m for Spinal Injuries.

Medical Education and Training

The Trust has a reputation for teaching and training excellence in medical education. There are two district clinical tutors and one director of medical education. A Modernising Medical Careers committee oversees the implementation of foundation training programmes and changes to basic specialty training.

The Trust has a strong partnership with Oxford clinical school and a number of other medical schools, and provides teaching attachments for clinical students in a range of specialties. Consultants who contribute substantially to teaching students are eligible for an honorary senior clinical lecturership. Further expansion in student numbers, with development of a faculty model for delivering undergraduate education, is planned.

There are modern, well-equipped education centres and libraries on all three sites with an active programme of continuing education for senior medical staff, training grade doctors and general practitioners. The education centres are fully equipped with lecture theatres, seminar rooms and state of the art audiovisual equipment.

The Libraries are housed within the education centres and are accessible 24 hours a day providing access to many healthcare databases and access to over 1000 journals in electronic full text that can be accessed at work or at home from any computer with internet connection.

Each directorate undertakes monthly academic, audit and governance meetings, which are multidisciplinary and involve staff from other sites. Routine clinical commitments are usually cancelled to enable all staff to fully participate.

Amersham represents a fascinating blend of ancient and modern. Amersham is made up of two parts - top Amersham, a thriving commercial centre, and Old Amersham, a popular tourist spot with its wide sweeping High Street, half-timbered buildings and picturesque period cottages. The Old Town is famed for its shopping with a wide selection of antique and craft shops, designer boutiques, and an impressive range of restaurants, snack bars and coaching inns.

1. DUTIES OF THE POST

a. Principal responsibilities. This post is based in Buckinghamshire Hospital Trust which comprises of 3 hospitals; Stoke Mandeville Hospital, Aylesbury, Wycombe General Hospital, High Wycombe and Amersham General Hospital, Amersham. The majority of the clinical aspect of this post will be based in the Department Of Orthodontics in Stoke Mandeville, although occasional visits to the department in Amersham may also be required in the final year. The academic portion of training is completed through the Eastman Dental Hospital and University College of London (UCL)

Candidates will be expected to have three years postgraduate experience in either Hospital or General Dental Practice, having obtained a Fellowship from one of the Royal College of Surgeons of Great Britain or Ireland. The appointed candidate will be expected to participate in New Patient Clinics for diagnosis, advice and treatment planning, treat a wide range of malocclusions, including multi-disciplinary cases, ie orthognathic and restorative patients. They will attend and fully participate in the multi-disciplinary clinics, participate in tutorials, lectures, journal clubs and postgraduate courses as directed. They will be required to undertake an active role in personal research and participate in audit projects both interdepartmentally and regionally.

An NTN is allocated to the post holder, and with successful ARCP/RITA outcomes will lead to the award of a CCST and entry onto the Specialist list. Candidates will be entered for the MSc in Orthodontics through the University of London and Membership in Orthodontics through a Royal College to complete their training in orthodontics and be entered onto the specialist list.

2. TRAINING AND RESEARCH

a. Training SchemeThe post is based in Buckinghamshire Hospital Trust and the Eastman Hospital, London. It is part of the UCL training programme in Orthodontics.


b. ResearchThe post holder will be encouraged to undertake an active role in personal research leading to the achievement of an MSc. The department also undertakes regular Audit and clinical governance projects which the post holder will be expected to participate in.

c. Facilities for study and trainingGood library facilities are available at Stoke Mandeville Hospital, and the Eastman Dental Hospital. On line access is also available through the Eastman Dental hospital Library.

d. Postgraduate Medical Education Courses AvailableThere are excellent Postgraduate facilities at all Hospitals.

3. SPECIALTY

1. BUCKINGHAMSHIRE HOSPITAL TRUST Stoke Mandeville Hospital

The Orthodontic Department shares the facilities with the Oral & Maxillofacial Surgery Department. There are five well equipped surgeries and the Laboratory is on site.

A full range of orthodontic techniques is undertaken. Most cases are treated with 2-arch fixed appliances, mainly Straight-wire and some Tip-edge systems being used. The department also has some self ligating appliances, which the candidate would be encouraged to try. A large number of cases are treated with functional appliances. Multidisciplinary planning extends to both orthognathic, cleft and restorative treatments and joint clinics are held with the appropriate clinicians on a regular basis.

The radiology department is situated in the Outpatient department and has full radiological facilities including OPT, Lateral Ceph machines and Intraoral views. The radiology department has recently converted to digital images and are available through networked computers in surgery’s and offices.

Clinical photography is undertaken by the Medical Illustration Department although the post holder will also be encouraged to take digital images of their patients in the chair. A digital camera is available in the department. OPAL image is available for analysis of Lateral Cephalograms.

The laboratory employs a Senior Chief Technician, a Chief Technician and a Senior Technician. There is a dedicated orthodontic secretary and reception staff dedicated to the combined Departments of Oral Surgery and Orthodontics.

Amersham General Hospital


The Orthodontic Department is shared with Maxillofacial Surgery and has three well equipped surgeries. Two full time secretaries are based in the department and reception is shared with general outpatients.

Full range of orthodontic treatments are undertaken in the department, as in Stoke Mandeville. Radiology is fully digital with networked access in both surgeries and offices. OPAL image is also available for analysis of cephalograms. The hospital has an excellent Medical Illustration Department for all photographic requirements.

Wycombe General Hospital There is no outpatient department for either Orthodontics or Maxillofacial surgery.

NB. All In-patient work is undertaken at Oxford Radcliffe Hospitals Trust under the care of the appropriate Consultant Maxillofacial Surgeon.

Eastman Dental Hospital and Institute For Oral Health Care Sciences

The Eastman Dental Institute and Hospital is a unique postgraduate centre for oral health care sciences offering first class research, teaching and clinical training. The Institute is part of University College London and the Hospital is part of University College London Hospitals NHS Foundation Trust. Together they are able to enhance the experience of those working and training at the forefront of clinical research and patient care.

FacilitiesClinicalThe unit is located in the basement of the main Eastman building and has extensive clinical facilities consisting of 17 dental chairs in an open plan clinic and 3 separate surgeries. The larger enclosed single surgeries are used for multidisciplinary consultation clinics and teaching clinics. Most new patient clinics are held in the side surgeries and patient treatment is in the open clinic.There is a recently opened dedicated 2 chair craniofacial suite.

Orthodontic LaboratoryThere are good technical facilities with a recently refurbished postgraduate trainees’ laboratory and a production laboratory with a separate area for the Institute Instructor Technicians. Although much of the technical work is done within the laboratory some of the less technically demanding work is sent to an outside laboratory.

Computer Cluster RoomThe unit has its own computer cluster room for image processing and computerized surgical planning using the Dolphin™ package. All computers are equipped for standard word processing, clinical data collection and spreadsheets for statistical analysis.

Research LaboratoriesThe basic science research is undertaken in collaboration with staff from the research divisions. There are dedicated research facilities in the Sussex wing. The research teams help with the supervision of MSc projects for those based at the Eastman undergoing clinical training.

Records storesOrthodontic records for those undergoing active treatment are held within the unit in the reception area. There are separate areas for storage of models and an archive of treated case records is held for research purposes.

Offices and Other facilitiesThe secretarial offices are housed adjacent to the main clinical area. Other staff offices include facilities for all senior staff and communal office space for trainees. Computer, printing and photocopying equipment is available together with relevant equipment for clinical photography, radiographic digitization and cephalometric analysis.

The work of the Unit

ClinicalPatients are referred to the unit from various sources, a large proportion of those referred being from specialist practitioners and an increasing number require a


multidisciplinary approach. The unit has set criteria for acceptance of patients including the most difficult cases and those deemed to be of value for teaching purposes.

A large proportion of the treatment carried out in the unit is done by Specialist Registrars and postgraduates in training under the supervision of the teaching staff.

A variety of multidisciplinary clinics are held with the senior staff having individual areas of interest and many of the combined clinics are formally organised as teaching clinics in order to give trainees access to some of the more complex clinical problems. Some clinics are held jointly with consultant staff for other centres, for example, Great Ormond Street Hospital.

Teaching and TrainingSpecialist Registrars and other postgraduates have a well structured training programme incorporating chairside clinical teaching, lectures, seminars, tutorials and technical demonstrations. There are also a number of trainees from regional hospitals who interlink with the academic component of the training. Those linked in this way attend the didactic teaching but carry out their clinical chairside training and their project work at their regional unit.

All staff members are actively encouraged to participate in audit and the unit operates an excellent process of appraisal and systematic training review to ensure that all trainees’ needs are catered for.

Orthodontic Unit Research and Development

The unit has a large number of staff with wide ranging research interests. Biomaterials science incorporates materials testing and microbiological testing in conjunction, studies on friction, force delivery, bond strengths, the study of biofilms and fluoride release of adhesives.

The basic science research is focused on the molecular aspects of processes involved in normal and abnormal facial development as well as response to treatment. In particular the unit has an interest in muscle ultrastructure, the control and regulation of extracellular matrix and matrix interactions. This has included studies on myosin protein phenotype regulation, extracellular matrix regulation, cell/matrix interactions and the molecular response to muscle stretch.

The clinically orientated research is directed towards the efficacy and cost effectiveness of different treatment modalities and treatment effects on the quality of life. Three dimensional modelling systems for detailed facial analysis and treatment planning are under development in conjunction with the department of Biomedical Informatics.

Training towards PhD qualification and postdoctoral research is available for both scientists and clinicians. The clinical trainees undertake project work as part of their


training and facilities are made available to senior clinical staff interested in carrying out research personally.
