The goods and services shown below are those we can provide to our customers. You may choose only the items

you desire. However, any funeral arrangements you select will include a charge for our basic services and

overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will

explain the reason in wri1ng on the statement we provide describing the funeral goods and services you selected.

BASIC SERVICES OF FUNERAL DIRECTOR & STAFF……… $2195.00 This fee for our basic services and overhead will be added to the total cost of the

funeral arrangements you select. (This fee is already included in our charges for

direct crema�ons, immediate burials, and forwarding or receiving remains.) This

fee includes, but is not limited to, staff and equipment to respond to ini1al

request for service 24 hours every day; ini1al sheltering of remains;

arrangement conference with family or responsible party; prepara1on and filing

of necessary authoriza1ons and permits; filing of death cer1ficate; coordina1on

with cemetery, crematory or other par1es involved with the final disposi1on of

the deceased; prepara1on and placement of obituary no1ces; accoun1ng and

billing; postage, prin1ng and supplies. Also included in this fee is overhead

expenses such as administra1ve, regulatory, maintenance, employee wages and

benefits, insurance, u1li1es, inventory, and equipment costs.

EMBALMING……………………………………………………………….. $850.00 Except in certain special cases, embalming is not required by law. Embalming

may be necessary, however, if you select certain funeral arrangements, such as

a funeral with viewing. If you do not want embalming, you usually have the

right to choose an arrangement, such as direct crema1on or immediate burial,

that does not require you to pay for it.

PRIVATE VIEWING OF UN-EMBALMED

BODY………………………………………………………….. $250.00 to $850.00 Level of fee is dependent on condi1on of the body. Private viewing is limited to

immediate family (up to 10 people), during normal business hours, and is limited

to 30 minutes. Outside of these circumstances, our normal fee for visita1on will

be added to the above fee charged for prepara1on of the body.

USE OF FACILITIES & STAFF FOR VIEWING OR MEMORIAL

VISITATION…………………………………………………………….……. $750.00 Our services include coordina1on of service with third par1es, set-up of

visita1on area, placement of encased remains, display of floral arrangements,

supervision of and a:endance during the visita1on. (Fee is based on up to a 3-

hour visita1on on a day separate of funeral/memorial service. Each addi1onal

hour held the same day shall be billed at $200 per hour.)

USE OF FACILITIES & STAFF FOR FUNERAL CEREMONY

of and a:endance during the ceremony. (Fee includes 1-hour visita1on directly

DIRECT CREMATION Our charge for a direct crema1on includes our basic service fee, local

transporta1on from the place of death, temporary shelter of remains and

crematory fee. If you want to arrange a direct crema1on, you can use an

alterna1ve container. Alterna1ve containers encase the body and can be made

of materials like fiberboard or composi1on materials (with or without an outside

covering). The containers we provide are made of cardboard.

charge for the first 7 days for crema1ons, and 3 days for burials, which in most

cases is adequate. A?er that, if crema1on or embalming is unable to be

performed due to a delay on behalf of the family or authorized agent, the

charge will be implemented. This charge will also apply from the first day for all

bodies being held for transfer to another loca1on.

CASH ADVANCE ITEMS The previous list does not include fees for certain items that you may ask us to

arrange and pay for on your behalf. This would include such items as cer�fied

copies of the death cer�ficate, hairdressing fees, clergy honorariums, musician

fees, cemetery fees, medical examiner or coroner’s fees, newspaper obituary

fees, airline fees, flowers or any other item provided by a third party. The fees

for these items will be shown on your Statement of Goods and Services Selected

provided at the arrangement conference. Some of these items may be es�mated

at the �me of the arrangement conference but will be itemized as soon as the

fees are presented to us.

PAYMENT POLICY All service fees and merchandise purchased from our funeral home and cash

advance items (including es�mated cash advances) are due and payable upon

receipt of the cremated remains or on the day of any memorial services. Verified

insurance assignments are gladly accepted. We will work with you and the

insurance company to file any claims and assignments. We do not accept estate

claims.
