Enter Admin Console User ID and Password

Click on the Assessments tab to Create a new ASSESSMENT

Click CreateNew Assessment

Select Assessment Type as SURVEY

Select Next to move to next screen

No. of days the assessmentshould show

Select Question Type: based on type of responses to be captured -Rating: option to rate by selecting from 5 starsLikert Scale: option to select from a scale of 1 to 5/10/20/30/100Progress Bar: Shows progress on a scale of 1-100Radio: Yes / No or True /False answersMulti Select: option to select more than one answer from multiple choicesSingle Select: Single answer can be selected from multiple choicesComments: Text box can be entered in a text boxDate Picker: drops down a calendar to select date fromSelect Save Question at the bottom of the page to move to next screen

Select Next to move to next screen

Select Add Question to add more question

Select PUBLISH to see the entered assessment in the iPad .
