PrivacyPolicyKeeneBeautyAcademyagreeswith,andmaintainsaspolicy,theprinciplesandpracticesofPublicLaw93-579:ThePrivacyActof1974oftheUnitedStates.Wedonotreleaseinformationaboutstudentswithoutthewrittenpermissionofthestudentorguardianofdependentminorstudent.KeeneBeautyAcademydoesprovideaccesstostudentandotherschoolrecordstoitsaccreditingagencytoensurecomplianceofallaccreditationstandards.TheStudenthastherighttogainaccesstohis/herfiles,underthesupervisionoftheDirector.Adequatenoticeisappreciatedforformerstudentfiles.Allstudentrecordsaremaintainedforatleastsix(6)years.Noinformationwillbereleasedtoathirdpartywithoutthestudent’swrittenpermission.

CounselingKeeneBeautyAcademycounsels’studentsonan“asneeded”basis.Iftheadministrationfeelsastudentisinneedofcounselingwhetherforpersonalissues,behaviorproblems,unsatisfactoryprogressinattendanceorgradepointaverage,thestudentismetinaprivatesettingtodiscusstheappropriatemeasurestobetaken.Resourcesarereadilyavailableforoutsideassistanceifneeded

• ExamsmissedduetoabsencemustbemadeupnolaterthanthefirstFridayfollowingthestudentsreturn.Makeupexamsaretobescheduledattheconvenienceoftheinstructor.

• Ifanentiresubjectismissedduetoabsence,thestudentisrequiredtoparticipateinthesubjectsnextavailabletimeframe.

Educational Support Planning:

AftercompletingalladmissionsrequirementsandacceptingenrollmenttoKeeneBeautyAcademystudentswhofeeltheymayneededucationalsupportoraccommodationsbeyondstandardteachingortestingmethodsmustmeetwiththeDirectortodiscussaccommodationsnolaterthanthetenthdayoftheirattendanceinordertoensureaccommodationscanbeputinplaceassoonaspossible.Requireddocumentationmayincludebutisnotlimitedto;individualsecondaryorpost-secondaryeducationalplanreports,medicalorpsychologicalevaluations,orVocationalRehabilitationassessments.Additionalstaffrequiredtomeetaccommodationswillbeattheexpenseofthestudent.

TerminationTermsforterminationinclude,butarenotlimitedtothefollowing:

1. Unsatisfactoryprogress,ingradepointaverageand/orattendanceaverage.2. Seriousdisregardofschool’srulesandregulations.3. Aviolationofschool’sdrugandalcoholpreventionpolicy.Atermination

noticewillbegiveninwritinginaccordancewithNACCAS,KBA’saccreditingagency.

AnAppealmustbereceivedwithin5businessdaysoftheAcademy’swrittennotification.Uponreceiptofwrittenappeal,adecisionoftheschoolboard,consistingofAdministrator,Director,oneinstructorofastudent’schoiceandoneoutsidememberoftheAcademy’schoice,willberenderedwithin15businessdays.Decisionsoftheboardarefinal.

Graduation&LicensingUponsuccessfulcompletionofacourse,thestudentisawardedadiplomacertifyingtraininginallbranchesoftheirchosenfield.Thegraduateistheneligibletotakethestateboardlicensingexam.Thegraduatemayalsoapplyforaninterimortemporarypermittowork(NH)whilewaitingforthestateexam.

GraduationRequirementsStudentswillgraduateuponcompletionoftherequirednumberofhourslistedbelow,subjecttothefollowingconditions:

A. TherequirednumberofpracticalassignmentsarecompletedB. Thestudentmustachieveacumulativeacademicaverageofseventy-

fourpercent(74%)atthedateofgraduation.C. Thestudentwillhavefulfilledallfinancialobligationstotheschool,withthe

studentunderstandingthatpriortobecominglicensedtheymustpassthestateboardexam.

Cosmetology:InordertograduatefromKeeneBeautyAcademyincosmetology,studentsmusthavecompleted1500hoursoftraining,achievedaminimumaverageof74%onacademicwork,practicalworkandattendance,andcompletedtherequirednumberofpracticalassignments.

Esthetics:InordertograduatefromKeeneBeautyAcademyinesthetics,studentsmusthavecompleted600hoursoftraining,achievedaminimumaverageof74%onacademicwork,practicalworkandattendance,andcompletedtherequirednumberofprojects.

arrangements made regarding tuition • State of NH Cosmetology Board application completed and licensure fee ($25.00)

paid • Four (4) passport photos submitted • Proof of age, i.e. driver's license, birth certificate, government issued

identification. • Keene Beauty Academy can admit an applicant who exhibits professional

potential and can provide proof they have graduated high school, or passed a G.E.D. exam as well as

o A certificate demonstrating that the student has passed a state authorized examination that the state recognizes as the equivalent of a high school diploma;

o An academic transcript of a student who has successfully completed at least a two-year program that is acceptable for full credit toward a bachelor’s degree.

• Keene Beauty Academy does not discriminate on the basis of age, sex, race, color, ethnic origin, or religion.

• Applicant who received online secondary education; validity of source of certification or diploma will be thoroughly researched and verified though Advanc-ed.org and a full secondary education transcript will need to be submitted by the applicant. In order for a student to receive Federal Student Aid funding self-certification will not be sufficient

• Homeschooled students though not considered to have a high school diploma or equivalent, they are eligible to receive FSA funds if their secondary school education was in a homeschool that state law treats as a home or private school. Some states issue a secondary school completion credential to homeschoolers. If this is the case in the state where the student was homeschooled, they must obtain this credential to be eligible for FSA funds.

• Applicants who are of compulsory age (NH 16y) will be considered when they can provide verifiable proof of completed secondary education. It should be noted that NH State Board of Barbering Cosmetology and Esthetics does not issue professional license applicants until the age of 18.

• Keene Beauty Academy does not admit students under the Ability to Benefit program at this time.

• Keene Beauty Academy does not admit foreign students at this time. • Keene Beauty Academy will consider re-entry of terminated or students who have

failed to complete on a case by case basis. All standard application procedures will be followed to apply for re-entry

• InorderforKeeneBeautyAcademytoconsiderrequeststoaccepttraininghoursfromsecondarytrainingprograms,thefollowingpolicymustbefollowed.SubmissionoftranscriptofCosmetologyorEstheticsgradesandclassesattendedforbothjuniorandsenioryears(ifapplicable)aswellasacompletesecondaryschooltranscript.Eachyearisevaluatedindividuallyforfinalgrade.ApplicantmusthavefinalCosmetologyorEstheticsgradeof74%orbettereachyearinbothwrittenandpracticalworktobeconsideredfortransferofhoursaswellasanoverallGPAof74%orbettereachyearinfullsecondarytranscript.$500.00institutionalscholarshipisissuedtoapplicantsfromsecondaryCosmetologyprogramswhoexhibitGPAof74%orbetterwhetherornottheyrequesttransferofhours.Aforementionedinstitutionalscholarshipwillexpireoneyearafterthegraduationdateoftheapplicant.

• AllTransferapplicantsmustcompleteallstandardKeeneBeautyAcademyentrancerequirements.Transferhoursfromsecondaryprogramswillbeappliedtostudent'scompletedhoursafter900hoursofsuccessfullycompletedtrainingfallingwithintheguidelinesofKeeneBeautyAcademySatisfactoryAcademicProgresspolicy.EnrollmentagreementwillreflectacceptanceoftransferhoursandwillbeamendedifSatisfactoryProgressisnotachievedat900completedhours.Studentmayelecttoremoverequestfortransferofhoursatanypointintheirprogram.

• KeeneBeautyAcademyreservestherighttorefusethetransferofhoursfromanyprevioustraining.

• AllstandardKeeneBeautyAcademyadmissioncriteriawillbefollowed.

SatisfactoryAcademicProgressPolicy

TheSatisfactoryAcademicProgressPolicyisconsistentlyappliedtoallstudentsenrolledatKeeneBeautyAcademytodeterminewhetheranotherwiseeligiblestudentismakingSAPintheireducationalprogramandmayreceiveassistanceundertheTitleIV,HEAprograms34C.F.R668.16(E).Itisprintedinthecatalogtoensurethatallstudentsreceiveacopypriortoenrollment.ThepolicycomplieswiththeguidelinesestablishedbytheNationalAccreditingCommissionofCareerArtsandSciences(NACCAS)andthefederalregulationsestablishedbytheUnitedStatesDepartmentofEducation.NOTE:StudentsreceivingfundsunderanyfederalTitleIVprogrammustmaintainsatisfactoryprogressinordertocontinueeligibilityforsuchfunds.Ifastudent’seligibilityforfinancialaidhasbeennegativelyimpactedbySAP,thestudentwillbenotified.ThisistruewhetherastudentisusingTitleIVstudentaidornot.ThisistrueforanyVAorMilitaryFinancialBenefits.Inordertobeconsideredmakingsatisfactoryacademicprogress(SAP)whileattendingKeeneBeautyAcademy,astudentmustachieveSAPrequirementswhichwillbeevaluatedatthepointatwhichthestudentachievesthefollowingactualattendedhours.TheSAPevaluationperiodsforeachprogramareasfollows;Cosmetology;450,900,1200actualattendedhoursEsthetics;300actualattendedhours

Cosmetologyprogramconsistsof12/3academicyears.900hrs+600hrs.Estheticsprogramconsistsof2/3ofoneacademicyear.600hrsAlltransferstudentswillbeevaluatedatthemidpointoftheircontractedhoursortheestablishedevaluationperiodsabove,whichevercomesfirstineachcontractedperiod.

AstudentmeetingtheminimumrequirementsatanevaluationshallbeconsideredmakingsatisfactoryprogressuntilthenextscheduledSAPevaluation.Thestudentmustmeetboththeattendanceandacademicprogressrequirementstobeconsideredmakingsatisfactoryprogress.Satisfactoryacademicprogressdefined;Qualitativeandquantitativerequirements:

Thequalitativeelementusedtodetermineacademicprogressisareasonablesystemofgradesasdeterminedbyassignedacademiclearning.Studentsareassignedacademiclearningandaminimumnumberofpracticalexperiences.Academiclearningisevaluatedaftereachunitofstudy.Practicalassignmentsareevaluatedascompletedandcountedtowardcoursecompletion.Severalcomprehensivepracticalskillsevaluationswillbeconductedduringthecourseofstudy.Practicalskillsareevaluatedaccordingtotextproceduresandsetforthinpracticalskillsevaluationcriteriaadoptedbytheschool.Studentsmustmaintainacumulativewrittenandpracticalgradeaverageof74%andpasstwofinalwrittenexamspriortograduation.Studentsmustmakeupmissedtestsandincompleteassignments.Numericalgradesareconsideredaccordingtothefollowingscale:

DETERMINATIONOFPROGRESSSTATUS

Studentsmeetingtheminimumrequirementsforacademicsandattendanceattheevaluationpointareconsideredtobemakingsatisfactoryacademicprogressuntilthenextscheduledevaluation.StudentswillreceiveahardcopyoftheirSatisfactoryAcademicProgressDeterminationatthetimeofeachoftheevaluations.StudentsdeemednotmaintainingSatisfactoryAcademicProgressmayhavetheirTitleIVFundinginterrupted,unlessthestudentisonwarningorhasprevaileduponappealresultinginastatusofprobation.

WARNING

Studentswhofailtomeetminimumrequirementsforattendanceoracademicprogressareplacedonwarningandconsideredtobemakingsatisfactoryacademicprogresswhileduringthewarningperiod.Thestudentwillbeadvisedinwritingontheactionsrequiredtoattainsatisfactoryacademicprogressbythenextevaluation.Ifattheendofthewarningperiod,thestudenthasstillnotmetboththeattendanceandacademicrequirements,he/shemaybeplacedonprobationand,ifapplicable,studentsmaybedeemedineligibletoreceiveTitleIVfunds.

PROBATION

Studentswhofailtomeetminimumrequirementsforattendanceoracademicprogressafterthewarningperiodwillbeplacedonprobationandconsideredtobemakingsatisfactoryacademicprogresswhileduringtheprobationaryperiod,ifthestudentappealsthedecision,andprevailsuponappeal.Additionally,onlystudentswhohavetheabilitytomeettheSatisfactoryAcademicProgressPolicystandardsbytheendoftheevaluationperiodmaybeplacedonprobation.Studentsplacedonanacademicplanmustbeabletomeetrequirementssetforthintheacademicplanbytheendofthenextevaluationperiod.StudentswhoareprogressingaccordingtotheirspecificacademicplanwillbeconsideredmakingSatisfactoryAcademicProgress.Thestudentwillbeadvisedinwritingoftheactionsrequiredtoattainsatisfactoryacademicprogressbythenextevaluation.Ifattheendoftheprobationaryperiod,thestudenthasstillnotmetboththeattendanceandacademicrequirementsrequiredforsatisfactoryacademicprogressorbytheacademicplan,he/shewillbedeterminedasNOTmakingsatisfactoryacademicprogressand,ifapplicable,studentswillnotbedeemedeligibletoreceiveTitleIVfunds.

RE-ESTABLISHMENTOFSATISFACTORYACADEMICPROGRESS

Studentsmayre-establishsatisfactoryacademicprogressandTitleIVaid,asapplicable,bymeetingminimumattendanceandacademicrequirementsbytheendofthewarningorprobationaryperiodandwithintheMaximumTimeFrame.

INTERRUPTIONS,COURSEINCOMPLETES,WITHDRAWALS

IfenrollmentistemporarilyinterruptedforaLeaveofAbsence,thestudentwillreturntoschoolinthesameprogressstatusaspriortotheleaveofabsence.Hourselapsedduringaleaveofabsencewillextendthestudent’scontractperiodandmaximumtimeframebythesamenumberofdaystakenintheleaveofabsenceandwillnotbeincludedinthestudent'scumulativeattendancepercentagecalculation.Studentswhowithdrawpriortocompletionofthecourseandwishtore-enrollwillreturninthesamesatisfactoryacademicprogressstatusasatthetimeofwithdrawalifthestudentreturnswithin180days.

APPEALPROCEDURE

Ifastudentisdeterminedtonotbemakingsatisfactoryacademicprogress,thestudentmayappealthedeterminationwithintencalendardays.Reasonsforwhichstudentsmayappealanegativeprogressdeterminationincludedeathofarelative,aninjuryorillnessofthestudent,oranyotherallowablespecialormitigatingcircumstance.Thestudentmustsubmitawrittenappealtotheschoolonthedesignatedformdescribingwhytheyfailedtomeetsatisfactoryacademicprogressstandards,alongwithsupportingdocumentationofthereasonswhythedeterminationshouldbereversed.Thisinformationshouldincludewhathaschangedaboutthestudent’ssituationthatwillallowthemtoachieveSatisfactoryAcademicProgressbythenextevaluationpoint.Appealdocumentswillbereviewed,andadecisionwillbemadeandreportedtothestudentwithin30calendardays.Theappealanddecisiondocumentswillberetainedinthestudentfile.Ifthestudentprevailsuponappeal,thesatisfactoryacademicprogressdeterminationwillbereversedandfederalfinancialaidwillbereinstated,ifapplicable.

NONCREDIT,REMEDIALCOURSES,REPETITIONS

Noncredit,remedialcourses,andrepetitionsdonotapplytothisinstitution.Therefore,theseitemshavenoeffectupontheschool'ssatisfactoryacademicprogressstandards.

TRANSFERHOURS

WithregardtoSatisfactoryAcademicProgress,astudent’stransferhourswillbecountedasbothattemptedandearnedhoursforthepurposeofdeterminingwhentheallowablemaximumtimeframehasbeenexhausted.

ThispolicyisincludedinthisFirstDayHandbook,theKeeneBeautyAcademyCatalogaswellasthepre-enrollmentinterviewdocumentationyoureceive.

RefundPolicy5. RefundPolicy:

Thispolicyappliestoallapplicantrejections,terminationsforanyreason,byeitherparty,includingstudentdecision,courseprogramcancellation,orschoolclosure.

Cancellationorwithdrawalshalloccurontheearlierofthedatesthat:a. Anapplicantisnotacceptedbytheschool.Theapplicantshallbeentitledtoa

refundofallmoniespaid,excluding$100.00applicationfee.b. Astudent(orlegalguardian)cancelstheirenrollmentinwritingwithin

threebusinessdaysofsigningtheenrollmentagreement.Inthiscaseallmoniescollectedbytheschoolshallberefundedexceptingtheabove-mentionedapplicationfee,regardlessofwhetherornotthestudenthasactuallystartedclasses.

c. Astudentcancelstheirenrollmentafterthreebusinessdaysofsigningthecontractbutpriortostartingclasses.Inthesecases,theyshallbeentitledtoarefundofallmoniespaidtotheschoolexcepttheapplicationfeeintheamountof$100.00

d. Astudentnotifiestheschooloftheirwithdrawalinwritinge. Astudentonanapprovedleaveofabsencenotifiestheschoolthattheywillnot

bereturning.Thedateofwithdrawalshallbetheearlierofthedateofexpirationoftheleaveofabsenceorthedatethestudentnotifiestheschoolthatthestudentwillnotbereturning.

f. Astudentisterminatedbytheschool.(Unofficialwithdrawalswillbedeterminedbytheinstitutionbymonitoringattendancedaily.)Studentswhoareabsentfor14consecutivedayswillbeterminatedandrefundcalculationwillbebasedonlastdateofattendance.

g. Intype5b,c,d,orfofficialcancellationsorwithdrawals,thecancellationdatewillbedeterminedbythepostmarkonthewrittennotification,orthedatesaidnotificationisdeliveredtotheschooladministratororownerin

h. person.ThereturnofTitleIVfundsasprescribedinSection484BoftheHEAAmendmentsdeterminestheamountofTitleIVaidastudenthasearnedatthetimeastudentwithdraws.TheamountofTitleIVaidastudenthasnotearnedisthenreturnedtotheTitleIVprograms.Itisaproportionalcalculationbasedonthedateofwithdrawalthroughsixtypercent(60%)ofthepaymentperiod.Allrefundsarebasedonscheduledhours.

i. Forstudentswhoenrollandbeginclassesbutwithdrawpriortocoursecompletion(after3businessdaysofsigningthecontract),orstudentswhohavebeenexpelled,thefollowingscheduleoftuitionearnedbytheschoolapplies.Allrefundsarebasedonscheduledhoursonstudentslastdayofattendance.Theformulainbriefisasfollows:

c. Theinstitutionwillcomparetheamountearnedtotheamountdisbursed.Ifthe

amountofaiddisbursedexceedstheamountearned,thisamountofTitleIVaidmustbereturned.

d. Theinstitutionallocatestheresponsibilityforreturnedunearnedaidbetweenthe

schoolandthestudentaccordingtotheportionofdisbursedaidthatcouldhavebeenusedtocoverinstitutionalchargesandtheportionthatcouldhavebeendisburseddirectlytothestudentonceinstitutionalchargeswerecovered.

e. Theinstitutionand/orthestudentmustthendistributetheunearnedTitleIVaidback

totheTitleIVprograms.

Acancellationfeeof$150willbechargedforstudentswhowithdraworterminateatanytimeduringtheirenrollment.

Allcostofextraitemstothestudent,suchasbooksandtheinstructionalsuppliesorequipmentservicecharges,studentactivityfees,deposits,rentalandotherchargesneednotbefiguredinrefundcomputations,providedsuchchargesareitemizedseparatelyintheenrollmentagreement,catalogs,orinotherdatafurnishedthestudentbeforeenrollment.Unmetobligationsbythestudenttotheschoolwillbecollectedbylegalandethicalbusinesspractices.

PleasenotesuchitemsarerefundableunderthecircumstancesdescribedinItems5a,b,and,cabove.Ifacourseiscanceledsubsequenttoastudent’senrollment,andbeforeinstructioninthecoursehasbegun,theschoolshallprovideafullrefundofallmoniespaidincludingapplicationfeeandanyunusedregistrationfeetotheStateofNH.Ifaschoolclosespermanentlyandceasestoofferinstructionafterstudentshaveenrolled,andinstructionhasbegun,theschoolwillprovideapro-ratarefundofallmoniespaid.Ifaschoolcancelsacourse

and/orprogramandceasestoofferinstructionafterstudentshaveenrolledandinstructionhasbegun;theschoolshallatitsoption:provideapro-ratarefundforallstudentstransferringtoanotherschoolbasedonthehoursacceptedbythereceivingschool.

Theschoolmayretainarefundof$25orlessthatwouldbeallocatedtoaTitleIVprogram.

Thestudent’srightsunderthisagreementmaynotbeassignedtoanyotherperson.

Itemsofimportance:

AstudentshouldprovidewrittennotificationtoKeeneBeautyAcademyofhisorherwithdrawal.Inthecaseofdisablingillnessorinjury,deathinthestudents’immediatefamily,oranyotherdocumentedmitigatingcircumstances,areasonableandfairrefundsettlementwillbemade.Ifacourseand/orprogramiscanceledsubsequenttoastudent’senrollment,andbeforeinstructioninthecourseand/orprogramhasbegun,theschoolshallprovideafullrefundofallmoniespaid.Ifpermanentlyclosedornolongerofferinginstructionafterthestudenthasenrolled,theschoolwillprovideaproratarefundoftuitiontothestudentORprovidecoursecompletionthroughapre-arrangedteachoutagreementwithanotherinstitution.

StudentswillbenotifiedofamountofTitleIVbenefitstheycanexpecttoreceive,andwhen,viaa“PreliminaryAwardLetter”TherefundtothestudentmayexceedtheMinimumTuitionAdjustmentSchedule.Ifastudenthas14consecutivedaysofabsence,notdocumentedasaleaveofabsence,KeeneBeautyAcademywillwithdrawthatstudentfromenrollment.ThelastdateofacademicattendancewillbedeterminedbyKeeneBeautyAcademyfromourattendancerecords.ThestudentmayprovidewrittennotificationtoKeeneBeautyAcademyoftheirwithdrawalpriortothedatethatKeeneBeautyAcademywouldnormallydeterminethatthestudentwithdrew.

Thedateofdeterminationisthedateofthenotification.Ifastudentwithdrawsfromenrollmentwithoutnotifyingtheadministration,orofficiallywithdraws/terminatesenrollment,KeeneBeautyAcademywillmakeanynecessaryrefundswithin45calendardaysofthestudent’slastdayofphysicalattendance.
