1.0 Executive Summary

Kerr Environmental Services, Inc. performed the subsurface soil, groundwater, and vapor

intrusion investigation at the CVS Pharmacy Property (hereinafter referred to as the "project

site") located at 3425 West Sixteenth Street in Indianapolis, Indiana, at the request of Mr. Robert

McCormack, 3425 Partners, LLC, with reliance on this report extended to Americana Bank of

McCordsville, Indiana.

The project site consists of a commercial building of approximately 10,852 square feet situated

on approximately 1.09 acres. The remainder of the project site is parking lots, drives, and

landscaped areas. The project site is bounded by West Sixteenth Street to the north with an

access drive to Tibbs Avenue to the east. Surrounding properties include commercial properties

to the north, a car wash and residential properties to the west, a church to the south, and a Burger

King to the east with a gasoline station and residential properties across Tibbs Avenue.

Recognized environmental concerns were discovered during the performance of a Phase I

Environmental Site Assessment (ESA) of the project site. The Phase I ESA was placed on hold

pending the outcome of this investigation. The recognized environmental concerns identified

included the presence of former underground petroleum storage tanks located on properties

immediately east and west of the project site. The adjacent Burger King was formerly a gasoline

service station. The adjacent car wash formerly utilized a fuel tank located within a few feet of

the project site. In addition, there are other existing or former gasoline service stations located

within close proximity to the project site.

To investigate these environmental concerns at the project site, Kerr Environmental Services,

Inc. conducted a subsurface soil and groundwater investigation. A total of six subsurface soil

samples and five groundwater samples were collected. The analytical results revealed

subsurface soil contamination at a depth of 27 to 28 feet below ground surface in sample GP-5D

collected immediately adjacent to the car wash on the west side of the project site. This sample

contained 1-methylnaphthalene, 2-methylnaphthalene, naphthalene, 1,2,4-trimethylbenzene,

xylenes above residential migration to groundwater screening levels and 1,2,4-trimethylbenzene

was also above residential direct contact screening levels. These compounds are associated with

coal, crude oil, naphthalene solvents, and other petroleum products.

The analytical results of groundwater samples showed the presence of trichloroethene (TCE), a

solvent, above residential vapor intrusion screening levels and residential tap water (drinking

water standards) screening levels. Three samples were also above the commercial/industrial

vapor screening levels.

Due to the TCE concentrations in the groundwater, Kerr Environmental Services, Inc. conducted

a vapor intrusion investigation at the project site, consisting of the collection of indoor air and

sub slab air samples. Analytical results revealed the presence of benzene in the indoor air.

However, the benzene was present well below residential and commercial/industrial action

levels. Two sub-slab samples also contained benzene as well, but also below screening levels.

One of these two samples, collected near the northeastern corner of the building, also contained

TCE and tetrachloroethene (also known as perchloroethene or PCE). PCE was detected below

screening levels. TCE was detected above both residential and commercial/industrial screening

levels. The indoor air samples were collected immediately adjacent to each of the sub-slab

samples and did not contain a detectable concentration of TCE or PCE.

In conclusion, the subsurface soil has been impacted by an apparent release at the adjacent car

wash. Groundwater has been impacted with TCE. Sub-slab air samples revealed the presence of

TCE. However, the air samples in the CVS Pharmacy building were below action levels and are

not believed to be a concern. The vapor intrusion investigation was taken during a worst-case

scenario, during cold weather, which would indicate that levels would be lower during other

times of the year.

2.0 Objective and Scope

The objective of this subsurface soil, groundwater, and vapor intrusion investigation was to

determine the presence or absence and relative concentrations of contaminants at the project site.

This investigation is not a guarantee or certification that the project site is free of contamination

or hazardous materials, but rather it is an opinion of the potential for contamination to exist

based on the scope of this investigation.

The scope of this investigation included the collection of subsurface soil and groundwater

samples, collection of sub slab and indoor air samples, laboratory analyses of compounds or

chemicals of concern, and the preparation of this Subsurface Soil, Groundwater, and Vapor

Intrusion Investigation Report. Certain information presented herein has been provided by

regulatory agencies and interviews. That information is assumed to be correct and Kerr

Environmental Services, Inc. makes no representation or warranties regarding such information.

Kerr Environmental Services' opinions stated herein are based on generally accepted

environmental site assessment methodologies and procedures with respect to due care customary

3.0 Site Description

The project site is the CVS Pharmacy property located at 3425 West Sixteenth Street in

Indianapolis, Indiana. The project site is located in the Northeast Quarter of the Southeast

Quarter of Section 32, Township 16 North, Range 3 East, in Wayne Township, Marion County.

The project site is located less than a mile west of the White River and less than a mile east of

the Indianapolis Motor Speedway. The project site is bounded by West Sixteenth Street to the

north with an access drive to Tibbs Avenue to the east. See Figure 1, Project Location Map, and

Figure 2, Site Map.

The project site consists of a commercial building of approximately 10,852 square feet situated

on approximately 1.09 acres. The remainder of the project site is parking lots, drives, and

landscaped areas. Surrounding properties include commercial properties to the north, a car wash

and residential properties to the west, a church to the south, and a Burger King to the east with a

gasoline station and residential properties across Tibbs Avenue.

The project site lies within the Tipton Till Plain physiographic province of the Central Till Plain

Region. The Tipton Till Plain is a till plain of low relief with extensive areas of ice

disintegration features. Bedrock in the area of the project site is dolomite and limestone of the

Muscatatuck Group of Devonian Age. The bedrock is overlain with undifferentiated outwash.

The depth of the unconsolidated deposits is over 100 feet. The depth to groundwater is likely

seasonal to some extent and was encountered at approximately 27 feet below the ground surface.

Soils on the project site consist of Urban Land Fox Complex (UfA). These soils are dominantly

Fox loam soils that have been altered by site development. The slope in the area is 0-2 percent.

These soils are well drained and found on terraces of the till plains.

4.0 Subsurface Soil and Groundwater Investigation

On October 27, 2014, Kerr Environmental Services, Inc. collected six subsurface soil samples

and five groundwater samples from the project site. These samples were collected utilizing a

Geoprobe, a track-mounted hydraulic sampling device. The sample locations were strategically

placed adjacent to properties that have, or have had, underground storage tanks (USTs). Three

sample locations were immediately adjacent to the Burger King property to the east of the

project site and two sample locations were immediately adjacent to the car wash property to the

west of the project site. The subsurface soil samples were taken directly from soil core sleeves.

The groundwater samples were collected by utilizing new tubing with a decontaminated check

valve and peristaltic pump through the hollow stem rods of the Geoprobe. All samples were

placed in containers supplied by the laboratory, labeled with appropriate identifying information,

and placed in an iced cooler for delivery to the laboratory under strict chain of custody protocol.

The sample locations are shown on Figure 2.

5.0 Vapor Intrusion Investigation

On November 13 and 14, 2014, Kerr Environmental Services, Inc. collected ten air samples at

the project site to evaluate the potential for vapor intrusion. Five samples of the air beneath the

slab foundation were collected, as well as five samples of the indoor air. The air and sub slab

samples were collected at the same location. The indoor air samples were collected with Suma

canisters with a vacuum and regulated valve. The sub slab samples were collected in the same

way except that the regulated valve was connected to a tube that was affixed through the

concrete floor. The indoor air samples were collected during normal operations of the facility.

All samples were placed in packaging supplied by the laboratory, labeled with appropriate

identifying information, and delivered to the laboratory under strict chain of custody protocol.

The sample locations are shown on Figure 2.

6.0 Analytical Results

6.1 Comparative Action Levels

Comparative action levels were selected from the Indiana Department of Environmental

Management’s (IDEM’s) Remediation Closure Guide, updated in 2014. The residential and

industrial screening levels were both utilized.

6.2 Laboratory Results

The subsurface soil and groundwater samples were delivered to ENVision Laboratories, Inc. of

Indianapolis, Indiana for laboratory analyses. The air samples were delivered to EnvisionAir of

Indianapolis, Indiana for laboratory analyses.

The analytical results revealed subsurface soil contamination at a depth of 27 to 28 feet below

ground surface in sample GP-5D collected immediately adjacent to the car wash on the west side

of the project site. This sample contained 1-methylnaphthalene, 2-methylnaphthalene,

naphthalene, 1,2,4-trimethylbenzene, xylenes above residential migration to groundwater

screening levels and 1,2,4-trimethylbenzene was also above residential direct contact screening

levels. These compounds are associated with coal, crude oil, naphthalene solvents, and other

petroleum products. The subsurface soil analytical results are summarized on Table 1.

residential vapor intrusion screening levels and residential tap water (drinking water standards)

screening levels. Three samples were also above the commercial/industrial vapor screening

levels. The groundwater analytical results are summarized on Table 2.

Analytical results revealed the presence of benzene in the indoor air. However, the benzene was
