Success Planning provides a framework for gathering and summarizing important information about each student in the class.

Tasks and activities can be arranged in various ways depending upon the needs of the individual student.

Student Success planning should foster, support, monitor and document:

Communication between school and parent(s) and among school personnel Goal setting and related activities Social/emotional/physical skills with connection to school/community Active, responsible student participation in the plan development and continual evolution.
