PETITION COMES NOW the petitioner, by the undersigned counsel, and unto this Honorable Court, respectfully alleges: 1. That the petitioner is a resident of ________________________________, is the (relationship to deceased) of ____________________, deceased, who died intestate on ___________________ (complete date of death) ; 2. That the said deceased ________________________ left real estate located in ____________________________________. The gross value of which does not exceed TEN THOUSAND PESOS (P10,000.00) as evidenced by a certificate of assessed value of said property, issued by the Assessors Office of ________________________, hereto attached as Annex "A" and made an integral part of this petition; 3. That the following persons, all of legal age, (if some are minors, request for guardian ad litem for them from the Court), are the sole, lawful and exclusive heirs of the said deceased____________________________________; 4.
