That said property, person(s) or thing(s) are described as follows:

1. Any traces of blood, hair, skin or other biological material, latent fingerprints to include palms or other contact mark which may be used for identification of the occupants.2. Any material fibers found in or on the vehicle which may be needed to include or exclude materials found on and with the occupants.3. That the following facts establish probable cause for believing that grounds for issuance of a search warrant for the aforementioned property, persons or things exist:

On October 16, 2021, at approximately 0509 hours, a single-vehicle collision occurred west bound State Route 202 (SR202), mp-7, at the end of the off ramp for Scottsdale Road. A red Nissan Altima drove on the off ramp, in the middle (#2) lane, preparing to go south on Scottsdale Road. The vehicle was traveling too fast, and the driver locked the brakes. The vehicle continued straight, jumped a curb, and struck a steel sign support for a traffic signal. The passenger side of the vehicle sustained significant damage, causing severe injuries to the passenger.The driver was positively identified by independent witnesses, and later identified by Law Enforcement Personnel as George E. Johnson, Date of Birth November 6, 1983. Johnson had red, watery, eyes, and an odor of intoxicating beverage. Johnson was observed within the driver seat of the vehicle by independent witnesses. Johnson was transported to Scottsdale Osborn Hospital for non life threatening injuries. Preliminary results from the hospital blood draw showed Johnson’s Blood Alcohol Content (BAC) was 0.331. The passenger in the vehicle, identified as Tamirah Tianna Washington, Date of Birth August 21, 1983, was transported to Scottsdale Osborn Hospital with serious injuries. She was pronounced deceased at 0559 hours.

Wherefore Affiant requests that a search warrant be issued commanding that a search be made in the daytime (between 06:30 AM and 10:00 PM local time) of the person(s) premise(s), building(s), vehicle(s) and/or other item(s) described herein for the property and thing(s) described herein, and that the same be retained in the custody of the Affiant or in the custody of the Arizona Department of Public Safety and disposed of according to the law, pursuant to A.R.S. §13-3920.
