Older People’s Mental Health Services
This leaflet introduces the North
Lincolnshire integrated support,
therapy and reablement (STaR) team
for older adults. We provide services
for people usually over 65 who
are experiencing difficulties in their
mental health and require a period of
support, therapy and reablement for
a short period of time.
Our service will provide a
comprehensive assessment of your
needs, and where appropriate, timely
support, therapy and reablement.
This includes support with a personal
budget where this is appropriate from
this team. The team also provide
information and signposting to other
agencies and if you choose you can
contact them for support.
Our aim is to work with you to
promote your individual choices
and independence in your local
community in the short term, so that
you will be able to continue with
support if required in the longer term
when the STaR team complete your
support.
What is an integrated support, therapy and reablement team?
The team is a multi-disciplinary team
of mental health professionals who
work with the wider mental health
service for older adults to provide
support for your mental and physical
wellbeing. We work towards a
seamless transition to independence
and engagement to other services
where appropriate.
Team members include:
• Team manager
• Occupational therapist
• Occupational therapy associate
practitioner
• Occupational therapy assistant
• Physiotherapist
• Nursing assistants
• Reablement support officers
• Self-directed support workers
• Administrative support workers.

How does the team get to know you?
Most of the referrals to our service
come from other mental health
professionals who you will have
already seen. These include the
community nurse, occupational
therapist, physiotherapist and
consultant psychiatrist involved in
your care. If you and the professionals
feel that you would benefit from the
team’s support and services for your
mental health, they will request a
referral the STaR team.
What happens next?
Once a referral has been accepted, a
member of the team will contact you
or your family / friend to offer to visit
you at home. If you prefer to be seen
elsewhere, we will try to arrange this.
The person who visits may need to
ask you some questions about your
life, interests and what you would
like support to do. We will try to not
to repeat questions you may have
been asked on previous assessments.
However, to help the team to identify
what support, therapy or reablement
would be beneficial to you, we
would like to find out all about
you and how your mental health
problem is affecting your day-to-
day life, so we can support you to
become as independent as possible.
The visit is usually for about an hour
and may include assessments from
occupational therapy / physiotherapy
if this is required from our team.
The assessment and information
gathered at this visit may require the
team to share relevant information
with other agencies who work with
the team. We will ask your permission
to do this as part of the assessment.
Care planning
Following the visit, the team will
discuss the best course of action to
support you. They will then arrange
to discuss the options with you and
start to formulate a care plan. If you
wish to have a family member or
friend to help you with this, please let
us know.
The care plans are person-centred,
which means they are individually
written and agreed with you to
ensure the team is delivering a service
that is beneficial to your mental
wellbeing.
Progress and evaluation
The team who are supporting you,
will ensure that the agreed care plan
is still meeting your agreed outcomes.
If there are any change, for example,
you have already achieved some or all
of the outcomes, then this is the time
to discuss and make any changes to
meet any new outcomes.

Discharge
Your discharge from the STaR team
will be planned with you and the
mental health professionals involved
in your care. At this point, if you need
signposting to services, the team will
support this process to ensure it is a
seamless transition from the team.
