TheUnitedStatesHepatitisAcademicMentorshipProgram(US-HAMP)isamultistatecollaborativelearningnetwork.ItisdesignedtoempowerandtrainkeystateleadersastheybuildstrongteamstoEDUCATE,ADVOCATEandCOLLABORATEatastateleveltoprovidemomentumfortheeliminationofhepatitisCintheUS.US-HAMPwasdesignedanddevelopedusingthesuccessfulKentuckyHepatitisAcademicMentorshipProgram(KHAMP)atele-mentoringtrainingmodel.TheWestVirginiaHepatitisAcademicMentoringPartnership(WVHAMP)replicatedandextendedtheHAMPmodeltoincludeaUS-HAMPHIPAAcompliantDataSystem.US-HAMP’sgoalistoimprovethehealthofindividualsinunderservedcommunitiesthroughouttheUnitedStatesbytrainingKeyStateLeadersinteam-basedmanagementstrategies.

ThekeystateleaderswillbeeducatedontheHAMPmodeltoempowerprimarycareproviders.

US-HAMPwilleducatestateleadersonharmreductionandhowtheycanidentifypatientpopulationsatthegreatestriskforHepatitisC.Bestpracticeswillbediscussedonhowuniversalscreeningandtestingcanbridgethegapbetweenpreventionandmanagementofhepatitisinfectionsthroughoutthestateaspatientsaretreatedandcured.Thefinalgoal:US-HAMPwillbeeasilyaccessible,regardlessoflocationtoassistkeystateleadersastheybuildstrongteamstodeveloptheirHAMPmodel.

Who'sBehindUS-HAMP

US-HAMPwasinspiredanddevelopedduetothesuccessandbestpracticesoftwoprograms,KHAMPandWVHAMP.

ThefoundationalandfirstHAMPprogram,KHAMPwasdevelopedbyastrongmultidisciplinaryleadershipteam,including,theKentuckyRuralHealthAssociation,theKentuckyDepartmentforPublicHealthAdultViralHepatitisProgram,UniversityofLouisville,andhepatitisCclinicalexpertsfromacrosstheCommonwealth.

TheWVHAMPreplicatedtheKHAMPmodelfortheirpatientpopulationandextendedthemodelwiththedevelopmentofaUS-HAMPHIPAA-CompliantDataSystem.Theregistrycanbeusedindividuallyforeachstateandcollectivelyformultiplestatesinbothdatamanagementandreporting.TheWVHAMPmodelwasalsodevelopedbyastrongmultidisciplinaryleadershipteam,including,theWestVirginiaRuralHealthAssociation,theWestVirginiaDepartmentforPublicHealthAdultViralHepatitisProgram,WestVirginiaUniversityandWestVirginiaMedicaid.

TheultimategoalofUS-HAMPistotrainkeystateleadersandsupporttheireffortwitheducation,advocacyandcollaborationastheydevelopahepatitisCeliminationmodelfortheirstate.TheHAMPmodelisaHepatitisCenterofExcellenceModelthatcanbeeasilymodifiedandusedtoimprovethehealthofunderservedcommunitiesthroughouttheUnitedStatesandultimatelyprovidemomentumforeliminatinghepatitisCintheUS.
