Introduction Color My Nails School of Nail Technology

Color My Nails Salon was founded in 1984 by Monica Bruin. Through Monica’s 26+ years in the nail industry she opened four nail salons, and a tanning salon which she owned with her sister Jamie Comstock. Monica served as board member and chairperson for the Utah Division of Occupational and Professional Licensing. She was vital in the regulation of licensing for Nail Technology in the cosmetology field.

Monica and Jamie began teaching nail classes in Color My Nails Salon and continued teaching until the State of Utah approved licensing for nail technicians in 2001. In May of 2002 Monica opened Color My Nails School of Nail Technology, the first accredited nail technology school in the state of Utah.

With courage and conviction Monica battled breast cancer for 19 years. She passed away June 2010, fighting until the very end. Monica was passionate about the nail industry and Color My Nails School and Salon. She left behind an amazing legacy. With Monica's passing the school was purchased by Jamie Comstock and was owned and operated by her until 2016.

Color My Nails is now owned and operated by Julianne Young and her husband Jeff Young. Julianne attended Color My Nails School of Nail Technology in 2015. She graduated and received her Nail Technician license in June of 2015. In August of 2015 Julianne earned her Nail Technician Instructor license and began working as an instructor at the school. This allowed Julianne to combine both her passion for nails and teaching. Throughout the remainder of 2015 Jamie and Julianne worked closely to bring Color My Nails School into the 21st Century. Jamie recognized Julianne's passion for the schools success and when she was ready to "retire" offered Julianne and Jeff the opportunity to purchase the school.

Julianne is a practicing Nail Technician as well as a certified SOGel Nails Instructor. In addition she is a member of the Utah Board of Cosmetology. She is also the Mom of two kids, a boy and a girl, three dogs and resides with her family and does nails in West Jordan.

Julianne is committed to operating Color My Nails School of Nail Technology with the same high standards it was founded on. She loves the school, its students and instructors and comes to work each day ready to help others build their dreams!

Melissa Hendon is Color My Nails School Director. Melissa attended Color My Nails School to become a Nail Technician and has worked as a Nail Tech since 2010. Melissa also received her Nail Technician Instructor license in 2010 and began working at the school that same year. In addition to being the school director, instructing and managing the schools inventory and supplies Melissa teaches the state practical testing classes. She stays up to date on all state testing requirements and makes sure all students that graduate from Color My Nails School of Nail Technology are ready to take and pass their state practical exam.

Melissa is a practicing Nail Technician as well as a certified SOGel Nails Instructor. She is also the Mom of two boys and loves watching her boys play baseball and football. She resides with her husband and boys and does nails in Sandy.

Melissa and Julianne work closely together, as well as with the rest of the staff to make sure that each student attending Color My Nails School of Nail Technology receives the best education possible.

All educators at Color My Nails School of Nail Technology are practicing nail technicians with a combined 20+ years of experience in the nail industry. The years of experience they bring allows our students to receive the most up to date information about the industry and has helped create some of the best nail techs in Utah.

Color My Nails School of Nail Technology teaches from the 7th edition of Milady Standard; Nail Technology. All lessons are taught by experienced nail technicians who are licensed with the state of Utah.

Mission Statement and General Objectives

The primary goal of Color My Nails School of Nail Technology is to offer the highest level of quality education in the nail industry for our students. This will be accomplished through our licensed instructors who will provide training techniques involving the latest nail products, skills and procedures available. Furthermore, our students will be educated in the concept of total customer satisfaction, and will acquire the necessary skills to pass state board exams and be prepared for a Career in Nail Technology.

This will be accomplished the following ways: • To be well organized in our courses of study.• To ensure professional training by providing one instructor for every twenty students.• To maintain a well organized and supervised practical room where students can improve

their knowledge and skills.• To provide a program of supportive services including guidance and counseling to

students, this includes counseling and employment assistance.• To operate a successful school that the community and nail industry can respect.• To prepare students for the Utah State licensing exams and employment using effective

• Be aware that sitting for long periods of time can be strenuous on the back and neck.• Take care of hands and wrists to prevent Carpel Tunnel Syndrome.• There is exposure to various chemicals. (glasses and gloves should be worn to help with overexposure)• Be willing to work long hours to build clientele to earn desired income.• Work with cuticle nippers and nail clippers.• Enjoy working with the public and be able to follow a client’s direction.• Make a strong commitment to the education of the nail industry.

The practice of safety and sanitation is essential for effective and successful performance within the industry. Methods of compensation vary and may include hourly pay, salary, commission or booth rental.

Color My Nails School of Nail Technology feels that students interested in pursuing a career in nail technology or a related field should consider all aspects of such a decision. Persons who want to become professionals in this field must:

• Have finger dexterity and a sense of form and artistry• Enjoy dealing with the public and be able to follow a client's direction• Keep abreast of the latest trends and beauty techniques• Work long hours while building a personal clientele in order to earn the desired income• Make a strong commitment to the educational process and complete the course of study• Learn the skills necessary to operate a personal business

In addition, applicants and students should be aware that:

* The work can be arduous and physically demanding because of long hours standing with handsat shoulder level or sitting over a nail technician's station or esthetician's chair.

* A personal investment may be required for advertising and promotions such as printing ofbusiness cards.

* There will be exposure to various chemicals and fumes which may cause allergic reactions orcould be harmful if used incorrectly.

* The practice of safety and sanitation is essential for effective and successful performance withinthe industry.

* Methods of compensation vary and may include straight salary, salary plus commission, straightcommission, sliding scale commission, retail commission or independent contracting (rentingspace and equipment from an existing salon).

The school will assist graduates in finding suitable employment by posting area employment opportunities and teaching job readiness classes, but placement is not guaranteed.

Scholarship Policy Color My Nails School of Nail Technology does not offer any scholarship programs at this time.

Housing Color My Nails School of Nail Technology does not provide institutional housing for attending students. However, if an applicant needs assistance in finding local housing, the school will refer the applicant to an appropriate agency.

Admissions and Enrollment Color My Nails School of Nail Technology does not discriminate in its employment, admission, instruction, or graduation policies on the basis of age, race, color, creed, religion, sex, financial status, physical handicap, ethnic origin or residence. The school does not recruit students attending or admitted to another school. The school requires that each student enrolling in the Nail Technology program has:

• High School Diploma OR GEDIf the applicant does not hold a high school diploma or equivalent, he/she must provide proof ofbeing above compulsory age (16) and

• Applicants between the age of 16 and 18 who do not hold a high school diploma or equivalent,and are currently enrolled in high school may be admitted if they have obtained permission(letter) from their school principal or guidance counselor.

Each student is allowed to make one contract change (Enrollment agreement), at no charge. There will be a $25.00 fee to make any additional changes once school has begun. All applicants will receive and review the school’s catalog before they sign an Enrollment Agreement. Transfer Students: Color My Nails School of Nail Technology may accept applicable verified hours from a student’s previous schooling, from an accredited school. All transfer hours must be verified and will be credited as both attempted and completed hours. Any costs associated with verification are the responsibility of the student. Hours applied toward Color My Nails School of Nail Technology training are at the discretion of the administration. Upon transfer hour acceptance the student will be required to pay the Enrollment Fee of $200.00 and Tuition of $7.50 per hour to meet the state hour requirement and kit fee of $700.00 Re-Entry Students: Former students of Color My Nails School of Nail Technology who wish to re-enter must request approval from the school administration. The request will be reviewed and a decision made within 30 days of the request. Students who re-enter within 30 days of their last clock-in date will re-enter at the same progress status as when they left, they will also be charged the same contract amount as the original contract. ($7.50 multiplied by the number of clock hours needed to complete the state requirement of 300 clock hours equal’s total tuition due tuition = $2100.00.) Students who re-enter more than 90 days after their last clock-in date must start the course from the beginning. Tuition rates current at the time of re-entry will apply and tuition balance from previous enrollment(s) must be paid in full prior to re-entry. Students are only allowed to re-enter the school a total of two times. Acceptance of re-entry students is based on availability and a $150.00 re-entry fee will apply to all students.

Requirements for State Licensing Upon graduating from Color My Nails School of Nail Technology, the following are requirements for obtaining a state license:

• Be a graduate of a school approved by the department of commerce, having the requirednumber of hours for the selected course. (300 hours for Nail Technician/Manicuring)

• Pass a written & practical examination on the knowledge of the occupation administered by NIC(National-Interstate Council of State Boards of Cosmetology).

Each student is responsible for scheduling their own testing!!!

Registration can be accomplished through e-mail, telephone or by fax with a VISA or MASTERCARD credit card. Nail Technician Theory Exam……………………………………. $87.00 Nail Technician Practical Exam…………………………………. $87.00

The passing score needed for each test is 75%

Before testing, students must have the school owner or instructor complete the test entry form to release their hours for test eligibility. Please refer to graduation requirements in this catalog to determine eligibility of hours being released. After taking both the practical and theory tests with NIC, you will need to send in required paperwork (Application for Licensure) to DOPL. There is a $60.00 fee for a state license and takes approximately 2-4 weeks to receive once all paperwork is complete. This fee should be paid by check payable to DOPL and should accompany your Application for Licensure. We strongly recommend you make a copy of your State Test Results for your records.

*In order to apply for a Nail Technician license in the state of Utah all applicants must fill out Qualifying Questionnaire or criminal history. Based on the answers to the questions students could potentially be required to appear before the Utah State Board of Cosmetology and subsequently issued a probationary license. If you have additional questions about the Qualifying Questionnaire please talk to Julianne or Melissa.

Nail Technology/Manicuring Course Written Outline 300 hours (Minimum required by the State)

English Course Course Description:The primary purpose of the Nail Technology Course is to train the student in the basic manipulative skills, safety judgments, proper work habits, and desirable attitudes necessary to pass the State Board examination and for competency in job-entry level positions as a Nail Technician or related career avenue. Color My Nails School of Nail Technology trains students in a manner to address proper: • Technical Skills

• Communication Skills• Safety Skills• Life long Learning Skills

Educational Objectives: It is the goal of Color My Nails School of Nail Technology to ensure that graduates are able to:

• Perform the skills necessary to become marketable in the Nail Technology industry or related field.• Practice proper grooming and effective communication skills• Pass the state board exam for licensing.• Exhibit professionalism and a good work ethic.

Instructional Methods: At Color My Nails School of Nail Technology, students receive education through a sequential set of learning steps, which address specific tasks necessary for state board preparation, graduation, and job entry-level skills. Clinic equipment, implements, and products are comparable to those in the industry. The course is presented through well developed lesson plans which reflect current educational methods. Specific methods used are:

• Lecture• Demonstration• Audio-visual aids• Printed materials• Text books• Guest Educators

References: A library of references, periodicals, books, texts, and audio/visual tapes are available to support the course of study and supplement the students’ training.

Grading Procedures: Students are assigned theory study and a minimum number of practical/teaching requirements. Theory is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion ONLY when rated as satisfactory or better. Practical skills are evaluated according to text procedures and performance standards established by the state licensing agency. Students must maintain a theory grade average of 80% and pass a FINAL written and practical exam prior to graduation. Students must make up failed or missed tests and incomplete assignments. Numerical grades are considered according to the following scale:

85%-92% Very Good Satisfactory 75%-84%

74% and under Unsatisfactory

Excellent - Flawless performance in practical work. Very Good - Work is above average and acceptable for practical floor performance. Satisfactory - Work is acceptable for practical floor performance. Unsatisfactory - Needs further instruction. Not able to work on practical floor.

Dress Code This is an industry of appearance and image. You determine the clients you want and the money you

make by the way you present yourself.

Students must come to school with their hair and makeup in a professional manner. The student must be clean and concerned with every aspect of personal and oral hygiene. The students clothes must be cleaned and pressed, and be in good condition with no rips, holes, or fraying (stylish fashion jeans with appropriate rips/tears are ok with approval from an instructor). Each student is given a smock and name tag with their kit and are required to wear them every day. This is part of the school dress code. If the student arrives to school without their smock and/or name tag they will be asked to clock out and retrieve them. Additionally, smocks and name tags may be purchased for $30.00 and $5.00, respectively. No one is allowed on the floor without a smock and name tag. Remember we are in a professional industry. The following clothes are not allowed:

• No rubber flip-flops, slippers of any kind• Clean, good repair fashion tennis shoes are ok• No tank tops, halter tops or strapless tops.• Shorts and sweats are not allowed. (Knee length shorts are the only exception)• No hats, caps or bandannas of any kind.

In addition, cell phones, tablets, IPods or MP3 players are not allowed in school while you are clocked in. If emergency circumstances arise, your instructor will make that exception. If a student comes to school looking unprofessional, they may be asked to go home and change clothing on their own time.

Right to Privacy Policy

In compliance with the Family Educational Rights and Privacy Act of 1974 Color My Nails School of Nail Technology follows policies that:

a) guarantee each student (or parent or guardian if the student is a dependent minor, 17 or younger)access to that student’s records;

b) require written consent from the student or guardian for release of records in response to each thirdparty request unless otherwise required by law;

c) Color My Nails does not publish “directory information” about students;d) provide and permit access to student and other school records as required for any accreditation process

initiated by the institution or by NACCAS or in response to a directive of the Commission.Access to records must be arranged previously and a staff member must be present while the records are being reviewed. Copies of file documents may be obtained at a cost of fifteen cents per copy. Cumulative education records are maintained for a minimum of seven years after graduation or termination in accordance with state and federal law. The school must make its student files available to accrediting, regulatory and other governing agency representatives who have the legal right to examine such files for compliance reviews. No portion of a file may be removed or reproduced without the permission of the owner of the school as all material and records contained therein are property of Color My Nails School of Nail Technology.

Attendance and Tardy Policy Color My Nails School of Nail Technology desires students to succeed in the professional world and school attendance is a step in their path to success. The following guidelines will ensure attendance success:

1. All absences including excused absences must be made up to complete the required hours for the course of study.2. Excused absences include those absences that are approved in advance or are taken for mitigating circumstances

beyond the control of the student such as illness, death in the family, etc. These absences must be accompanied by a doctor’s note to keep from applying to the $50.00 attendance fee. Part Time & Full time Day students are given 2 un-excused absences and Part Time PM Students are given 4 un-excused absences. (Only one un-excused can be use for a Saturday practical)

3. Students must call or message an instructor if they are planning to miss class if prior arrangements have not been made.

4. Students must let an instructor know that they will not be in attendance to use an un-excused. Students who do not call or message an instructor prior to class starting will not be allowed to use an un-excused absence.

5. Students who miss class for illness, death in the family etc. will not be allowed to use an excused absence even with the required documentation if they do not notify the school in advance of missing class.

6. Students must attend theory and practical classes according to the assigned schedule.7. Students must arrive for all assigned classes on time. Students must arrive by 8:45 am on Saturdays.8. Students are responsible for clocking in and out on the computer.9. Students must attend a minimum 80% of their required attendance hours. If this requirement is not met, the

student may be placed on school probation.10. Students must complete their 300 hours within a certain time frame designated from the schedule decided upon

when registering for the class. If they are not completed within that time there is an extra-instructional charge of$7.50 per hour over 300 hours. Extra-instructional charges must be paid in full before taking the schools final theory and practical test.

11. Students who miss class will have a $50.00 attendance fee added to the their school account for each missed day; Saturday’s attendance fee is $ 75.00. All fees must be paid within seven days. If payment is not made within two weeks, the student may not return to school until the fine and any additional fees are paid in full.

12. A student unexcused for two consecutive class days may be asked to meet with the school director and may be placed on school suspension.

13. A student that does not pay their scheduled payment on time will be accessed a $5.00 per day late fee and if not paid within 14 days of the scheduled due date, the student will be excused from school until payment is current.

14. Currently enrolled Students absent for 3 consecutive school days without prior notification or approval, will be officially withdrawn and are required to re-enroll and pay the applicable re-entry fee of $150.00 as well as any prior fee’s. Re-entry is not guaranteed and is based on availability and evaluation of past performance.

15. Students who miss school, theory class, or are tardy, are required to make up all tests, services, and required projects that are missed. (3 tardies/leaving early equal a $25.00 fine) being one minute late is considered a tardy. Arriving to school late by 30 minutes or more is considered a no-show and will be fined accordingly.

16. Students who leave school 30 minutes or more before their scheduled leave time will be considered a no show and fines will be assessed accordingly.

17. Any student with Attendance and No-Show fines will not be permitted to continue their studies until all fines are paid in full.

18. Any student not in attendance for Saturday practical work will be fined $75.00 unless prior arrangements (before beginning class) are made. Students may use one of their unexcused absences for a Saturday practical day.

19. Full time students are scheduled for 8 hours per day Tuesday-Friday and 6 hours on Saturdays. Students must be in attendance during their contracted hours.

20. Students may not leave the premises during clock-in hours. Students are given one 15 minute break for a 5 hour shift. There is a 30 minute lunch break for 8 hour students ONLY. If a student leaves school grounds they must clock out. If you take longer than the 30 minutes allowed for lunch you will be given a tardy. If a students lunch is more than an hour they will be given a no-show.

21. Students in attendance 6 or more hours receive two five minute breaks and students scheduled less than 6 hours are given one five minute break. Students who leave the building for their break must clock out for a break! Students taking excessive breaks or consistently going over their break time will be asked to meet with an instructor and possible disciplinary action will be taken.

22. Students clocking out 29 minutes before their scheduled leave time will be given a tardy.

Attendance Schedules: The part time schedule(s) is as follows: Daytime students are scheduled to attend Tuesday - Saturday from 9:00 a.m. to 3:00 p.m. Evening students are scheduled to attend Tuesday - Thursday from 5:30 p.m. to 9:00 p.m. & Saturday 9:00 a.m. to 3:00 p.m. The full time schedule is as follows: Tuesday - Thursday from 9:00 a.m. to 5:00 p.m, Friday & Saturday 9:00 a.m. to 3:00 p.m.

Leave of Absence Policy Students may request one leave of absence during their school period and must be done by submitting their request in writing and include the reason for the request, a requested start date for the leave, an expected date of return as well as the students signature. The students is required to apply in advance in advance of the leave unless unforeseen circumstances prevent the student from doing so. A students leave may be for a maximum of 30 days. There must be a reasonable expectation that the student will return from the LOA.The student will not be assessed any additional institutional charges as a result of the LOA. THE LOA together with any additional approved leaves must not exceed a total of 180 days in any 12 month period. A student granted an LOA that meets these criteria is not considered to have withdrawn, and no refund calculation is required at the time of the LOA approval. Upon approval of the LOA a LOA form will be completed. The LOA form must be signed by both the student and school administration. If a leave must be for more than the 30 days, this must be approved by the Owner or Director with documentation from the student informing the institution of the special situation. Any leave of absence will extend contract the same number of days as a leave of absence. Changes to the contract period on the enrollment agreement must be initialed by all parties or an addendum must be signed and dated by all parties. Students will not be assessed a re-entry fee if he/she returns from the approved leave as scheduled. A student not returning from an approved leave of absence as scheduled will be formally withdrawn from the institution. In this case the students withdrawal date for the purpose of calculating a refund is always the students last day of attendance.

Grading Procedures GRADING PROCEDURES: Students are assigned theory study and a minimum number of practical experiences. Theory is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion ONLY when rated as satisfactory or better. Practical skills are evaluated according to text procedures, performance standards established by the state licensing agency, and set forth in the Practical Skills Evaluation Criteria. Students must maintain a theory grade average of 75% and pass a FINAL written and practical exam with 75% prior to graduation. Students must make up failed or missed tests and incomplete assignments. Numerical grades are considered according to the following scale:

Excellent – Few flaws or flawless performance in practical work. Very Good - Work is above average and acceptable for practical floor performance. Satisfactory - Work is acceptable for practical floor performance. Unsatisfactory - Needs further instruction. Not able to work on practical floor.

Make-up Work Policy Each student is required to achieve at least 75% on all theory chapter exam reviews. If the exam review is not completed on time and a 75% is not achieved student will not be allowed to graduate until exam review is completed with a 75% or higher. If a student does not pass the final theory exam with 75% or above, that student will be required to re-take the theory exam within 3 school days (excluding Saturdays) at no charge. Each and any attempt beyond the first re-take, there will be a $25.00 fee. If the students does not pass the final practical exam with 75% or higher, that student will be required to attend all three classes the following month. Any attempt beyond the first re-take, there will be a $150 fee.

Satisfactory Academic Progress Policy / SAPP

• Satisfactory Academic Progress Policy is consistently applied to all students enrolled at ColorMy Nails School of Nail Technology.

• SAPP is printed in the catalog to ensure that all students receive a copy prior to the first day ofclass.

• Each student will be evaluated on both academic and attendance progress at 150 scheduledclock hours. Each Student Progress Report has an attendance percentage based on 150scheduled hours divided by Students’ actual hours. Students must maintain an 80%attendance rate and a 75% or better academic grade in order to be considered maintainingsatisfactory attendance progress.

• The maximum time allowed for a student to complete the course at satisfactory academicprogress is stated below:

SAPP evaluation periods are based on actual contacted hours at the instituation. Evaluations will determine if the student has met the minimum requirements for satisfactory academic progress.

• Students meeting the minimum requirements for academic and attendance at the evaluationpoint are considered to be making satisfactory academic progress and such progress will benoted on the hard copy. Students will receive a hard copy of their Satisfactory AcademicProgress Determination at the time of the evaluation.

• Students who fail to meet minimum requirements for attendance or academic progress maybe placed on warning for non-compliance and will meet with an instructor, school director orowner to determine a plan of action to get student back in compliance by contract end date.

• Students may re-establish satisfactory academic progress by meeting minimum attendanceand academic requirements by the end of the warning period. Warning period length will bedetermined on an individual basis, as determined by school instructor, director or owner.

• Any student who withdraws prior to completion of the course and wishes to re-enroll willreturn in the same satisfactory academic progress status at the time of withdrawal.

• Instructor will review SAPP results with student at the time of completion (approx. half waymark, see chart above) Students can request to review their SAPP any time throughout theirschooling by asking an instructor, the owner or school director.

Re-Entry Students Policy: Former students of Color My Nails School of Nail Technology who wish to re-enter must request approval from the school administration. The request will be reviewed and a decision made within 30 days of the request. Students who re-enter within 30 days of their last clock-in date will be charged the same contract amount as the original contract. ($7.00 multiplied by the number of clock hours needed to complete the state requirement of 300 clock hours equals total tuition due, tuition = $2100.00) Students who re-enter more than 30 days after their last clock-in date must start the course from the beginning. Tuition rates current at the time of re-entry will apply and tuition balance from previous enrollment(s) must be paid in full prior to re-entry. Students are only allowed to re-enter the school a total of two times. Acceptance of re-entry students is based on availability and a $150.00 re-entry fee will apply to all students.

• Color My Nails School of Nail Technology may accept student transfer hours from a previousaccredited school. A student’s transfer hours will be counted as both attempted and earnedhours for the purpose of determining when the allowable maximum time frame as beenexhausted.

Transfer Students Policy: Color My Nails School of Nail Technology may accept applicable verified hours from a student’s previous schooling, from an accredited school. All transfer hours must be verified and any costs associated with verification are the responsibility of the student. Hours applied toward Color My Nails School of Nail Technology training are at the discretion of the administration. Upon transfer hour acceptance the student will be required to pay the Enrollment Fee of $200.00 and Tuition of $7.50 per hour to meet the state requirement. The student is also responsible from any kit fee, if a kit is needed.The school may charge a $25.00 transcript fee for requests for transcripts to transfer to another school.

Evaluations and Advising All students are counseled and evaluated regarding their academic and practical progress on the following schedule:

• Fulltime Students: In the 4th week after contract start date.• AM Students: In the 5th week after contract start date.• PM Students: In the 9th week after contract start date.

Students may also have additional evaluations at any time at the Instructors discretion. Copies of counseling/satisfactory Academic progress reports are provided to students during their course of training. Consideration is given but not limited to, the following areas:

• Student's total hours in school (through previous month)• Student's attendance percentage (monthly and cumulative)• Student's theory/written grade average, monthly and cumulative (75% or higher is satisfactory)• Student's practical skills performance (quality satisfactory)• Student's goals, strengths, and/or areas needing improvement• Performance required for improvement in weak areas, if required• Follow up of previous counseling and performance expectations, if necessary• Other points of interest, if applicable, such as advising forms, leave of absence, probation and/or any additional needs

the Instructor and/or student may address

Student strengths and areas requiring improvement are identified and plans for needed improvement are discussed. If a staff member discovers during the counseling session that the student has need of counseling for something other than academic progress or matters relating to enrollment, a sincere attempt will be made to refer the student to the appropriate agency. A list of such agencies and organizations is maintained in the campus administrative office and school lunch room.

Students are counseled regarding progress on a regular basis according to the published Satisfactory Academic Progress Policy. Counseling regarding personal matters, licensing regulations, financial assistance (if applicable), and reciprocity, employment and continuing education opportunities is available to students as needed. The administration office has a list of agencies for referral if a student needs other assistance.

Standards of Conduct Rules & Regulations

• Students are expected to maintain high standards of personal hygiene.• Students are expected to maintain a high standard of conduct, showing professionalism and respect to

people and property.• Profanity and vulgarity on the school premises will not be tolerated and are considered grounds for

disciplinary action.• Students involved in any form of violence or the threatening of violence will be terminated immediately.• Any student found guilty of willful destruction or theft of school property may be terminated immediately.• Students may not under any circumstances, bring their children to school and collect hours for that day.• No personal calls except in case of emergency are to be received through the business phone. A student will

not be called to the phone unless it is an emergency.• Lockers are provided to give students personal belongings a secured area. It is the student’s responsibility

to keep the lockers locked. Students may have to share lockers. Color My Nails School of Nail Technology isnot responsible for lost or stolen items.

• The school maintains the right to inspect student lockers for sanitation purposes or kits at any time.• Students are supplied with a kit that includes very fine equipment. The student is responsible for the up

keep of this equipment. All items on the kit list are required to be in the building at all times the student isclocked in and receiving hours towards training. It is recommended that students mark all of theirbelongings with their name or initials and keep equipment not being used in locker or bring home at the endof your day.

• No Smoking is allowed in the building or within 50 feet of the building as per Utah law.• Students must park in designated areas. Failure to comply with parking is grounds for dismissal.• The heat/cooler thermostats are off limits to students unless directed by an instructor.• All debts owed to the school must be paid in full prior to graduation.

In order to maintain professional standard on the practical floor the following standards will be observed: • Any student who is disrespectful or abusive to a client or staff member may be subject to disciplinary action.• Students are required to accept/complete all services assigned to them. Even if required number of

services is completed.• Students must not visit with another student who is busy with a client.• An instructor must check all work before buffing and polishing.• All appointments are to be made by the receptionist, instructor or the student who answers the phone.

Students are not allowed to change appointments from one student to another student’s books.• Students are not allowed behind the front desk or in the office unless they have been assigned to work

there.• Students are not allowed to congregate in the reception area or in the office.• Students are not allowed to perform any service on a client without a receipt from the front desk.• Students who are not working on a client are responsible for applying their efforts toward the study of nail

technology, practical or theory.Sanitation

• Students must follow all sanitation codes as published by the State of Utah Department of Commerce,health department, and school rules and policies.

• Students must wash their hands before every client and upon leaving the restroom.• Each student must clean his or her work area after each client.• Each student must soak their implements in hydracide 10 minutes before using and between clients.• Kits must be kept clean, marked and ready for inspection at all times.• Any product that is spilled must be cleaned up immediately.• Students are responsible for cleaning up after themselves in the break room.• A sanitation calendar will be posted each month, in the practical and theory rooms. This calendar shows

which student is responsible for sanitation duty for morning, evening and Saturday classes.

Time Clock Rules and Procedures • Student’s hours are recorded on a time clock each day. It is the responsibility of students to use

the time clock correctly. Failure to do so may result in loss of hours.• Students with an 8 hour schedule will be given a 30 min. lunch break.• Students who do not clock in and out for lunch will be counseled, probated, and then suspended

from school if the problem continues to occur.• Under no circumstances should a student ever leave the premises during their scheduled hours

without the permission of an instructor. Students who leave the premises without instructorpermission and do not clock out may be dismissed from school.

• Students are required to clock their own time. Clocking time for another student may result insuspension or dismissal from school.

• Students must clock in by 8:45 AM on Saturdays. Appointments will be given to students who areclocked in and ready to serve a 9:00 AM appointment.

Clock In: Students will not receive hours prior to their clock in time. Example: If a student forgets to clock in when they arrive at 8:42 am, and they remember at 11:47 am (and clock in), no hours will be granted before the clock in time.

Tardiness and leaving early: All tardies and students who choose to leave 29 minutes or less early must pay a $25.00 fine for every 3 tardies. Tardiness/leaving early may be excused with a physicians note or letter of explanation prior to or within 1 day of the absence or tardy. (Leaving early is the same as a tardy)

Students who clock in 30 or more minutes late or out 30 or more minutes early will be considered a no show.

Clock In, Clock Out: If a student forgets to clock in or out and is also tardy, hours will be missed and must be made up. Example: If a student was tardy and also forgot to clock in, the hours missed before clocking in will not be added to their time.

Attendance /“No-shows”: All students must notify the school prior to any absence that is not pre-approved! Failure to notify the school of a an absence will result in a no-show and fine. All student absences that are unexcused will be given a $50.00 attendance/no-show fine (Saturdays $75). In order for an absence to be excused you must notify the school of the day and reason for the absence prior to being absent and must supply ring a Doctor’s note stating your illness, a funeral program or written documentation pertaining to the reason for the absence. ie- police record, court document, etc. when you return. Not notifying the school prior to an absence will be considered a no-show! “No-showing” an appointment that is on a student’s book is not tolerated. As stated in the school contract all No Shows will be charged a $50.00 fine, Tuesday through Thursdays, $75 on Saturdays. These rules will be strictly enforced. A receipt for the fine will be documented and given to the student as well as placed in the students file. All fines must be paid within seven days.

Note: Because we are a clock hour program, it is vital that students follow the clock rules and procedures. Failure to do so may result in incorrect hours.

Log of Practical Services Each student receives a Practical Log sheet in their workbook. This sheet is signed by their instructor and is for students to keep track on their own, all the services they perform on clients/students. When this sheet has been completed, it is turned in and kept in their student file for reference.

Service Requirements Students are required to perform and complete the daily service requirements and any weekday projects assigned to them once they have passed their pre-practical tests.

Daily service requirements include: 3.5 hour schedule: 1-2 services per day 5 hour schedule: 2-3 services per day 8 hour schedule: 4-5 services per day

Weekly service requirements include: 3.5 hour schedule: 6-10 services per week 5 hour schedule: 10-15 services per week 8 hour schedule: 20-25 services per week

These numbers allow students to complete their schooling within 8-18 weeks.

If services cannot be performed on clients, they are to be performed on students. Students are also required to complete a certain number of services on Mannequin hands.

Personal Services Students may work on one another daily to receive credit for any service. Paying clients will never be turned away. Therefore clients will take precedence over a free/personal service done on each other. Any time that colored acrylic is used on yourself or another student there is a $5.00 fee. Any school supply you borrow must be checked out/checked in by the front receptionist. If supplies are not returned by the end of your shift, you will be required to purchase/replace the items. No service may be done on yourself to receive credit.

Internal Complaint Procedures Students wishing to file an official complaint with the school may receive an internal complaint form from the office staff or from their instructor. Once the form is filled out in full, it will be placed in their file and the owner of the school will be notified and a meeting will be scheduled. Disciplinary Policies Students must understand that any infraction of the Standards of Conduct, Rules and Regulations, General Policies, or the enrollment agreement, could result in any of the following disciplinary actions:

Counseling: A student may be counseled for any infraction of the Standards of Conduct, Rules and Regulations or non-compliance with education requirements. If the student does not correct the problem, they may be placed on probation.

Probation: A student may be placed on probation for a specified time for any infraction of the Standards of Conduct, Rules and Regulations or non-compliance with education requirements. If the student does not correct the problem, they may be placed on a second probation or suspended three days depending on the severity of the infraction. If the problem is not corrected during the second probationary period, they may be suspended for three days or dismissed permanently at the discretion of the school administration.

Suspension: Enrollment may be immediately suspended for three days for any infraction of the Standards of Conduct, Rules and Regulations, non-compliance with education requirements, or general Policies and Procedures at the discretion of the school administration. Three tardies or attendance/no-show fines may result in suspension from school studies. During school suspension students are not to be on school premises. Students may also be suspended for not paying fines, fees and tuition payments on the due date. Students suspended for money owed will not be allowed to return to school until account is paid in full. Students with an outstanding balance after 30 days will be dropped and sent to collections.

Dismissal/Termination: Enrollment may be terminated at the discretion of the school administration for any reason deemed necessary to maintain the positive educational environment and general objectives of the school, or for any of the following reasons:

• Immoral or improper conduct• Noncompliance with educational requirements, Rules of Conduct, General Policies, and/or

Enrollment Contract.• Noncompliance with the school’s Satisfactory Progress Policy, noncompliance with state laws and

regulations.• There will be a $50.00/$75.00 fine for No-Showing any scheduled appointment. This fine must

be paid within 7 days.• There is a $50.00/$75.00 Attendance fee for any unexcused absence. This fine must be paid

within 7 days.• Noncompliance with state laws and regulations.• Any action which causes or could cause bodily harm to a client, a student, or employee of the

school.• Willful destruction of school property and theft.• Engaging in the manufacture, distribution, dispensation, possession, or use drugs and/or alcohol.• Not meeting all terms of probation or suspension.• 6 or more tardies may result in dismissal or termination from the school.

Color My Nails School of Nail Technology refund policy applies to all terminations for any reason, by either party including student decision, course or program cancellation or school closure.

Any monies due the applicant or student shall be refunded within 30 days of official cancellation or withdrawal.

If student (or in the case of student under legal age, his/her parent or guardian) cancels the enrollment and demands the money back in writing within three business days of signing the enrollment agreement, all monies collected by the school will be refunded even if the student has begun classes.

If a student cancels the enrollment after three (3) business days after signing the contract but prior to starting classes a refund of all monies paid to the school less an application fee, if applicable, and registration fee will be made. If a student is not accepted by the school a full refund of all monies paid to the school less an application fee (if applicable) will be made.

Students must cancel in writing. The "formal cancellation date" will be determined by the postmark on written notification, the date said notification is delivered to the school in person, the last day of attendance before an approved leave of absence, or after three days of consecutive no shows (no call no show) when a student is scheduled for class.

For students who enroll and begin class but withdraw prior to course completion or are expelled from the school (after three business days of signing the contract) the following schedule of tuition earned by the school will apply.

Any monies due a student or applicant who unofficially withdraws from the institution shall be refunded within 30 days of a determination by the institution that the student has withdrawn without notifying the institution. To determine unofficial withdrawals, the school monitors each student’s attendance by taking roll daily and posting weekly hours reports.

In the case of disabling illness or injury, death in the student's immediate family or other documented mitigating circumstances, a reasonable and fair refund settlement will be made. If the school is closed or the course is canceled subsequent to a student's enrollment, and before instruction in the course has begun, the school will either provide a full refund of all monies paid or completion of the course. Students who withdraw or terminate prior to course completion are charged a withdrawal fee of $150.00. This refund policy applies to tuition charged in the enrollment agreement. The kit charge and other miscellaneous expenses the student may have incurred at the institution (EG: extra kit materials, books, products, unreturned school property, etc.) will be calculated separately at the time of withdrawal. The student kit is not returnable and non-refundable.

If a course or program is canceled subsequent ot a student's enrollment, and before instruction in the course/program has begun, the school shall at its option:

Provide a full refund of all monies paid; orProvide completion of the course and/or program

If the school cancels a course and/or program and ceases to offer instruction after students have enrolled and instruction has begun, the school at is option:

Provided a pro rata refund for all students transferring to another school based on the hours accepted by receiving school; or Provide completion of the course and/or program; or Participate in a Teach- Out Agreement; or Provide a full refund of all monies paid

If the school closes permanently, and ceases to offer instruction after students have enrolled, and instruction has begun, the school must make arrangements for students. The school shall at its option:

Provide a pro rata refund; or Participate in a Teach- Out Agreement

If a student withdraws and owes a balance to the institution or attempts to pay the balance due with a non-collectible check, the student will be responsible for collection fees, costs, and reasonable attorney fees.

Color My Nails School of Nail Technology will make all reasonable efforts to collect any monies due to the school. If payment is not made within agreed upon terms and all reasonable efforts of collection on the schools part have been exhausted, student accounts maintaining a balance will be sent to collections.

Correspondence regarding cancellation and settlement for the school, banks, collection agencies, lawyers, or any other third parties representing the institution, acknowledge the existence of Color My Nails School of Nail Technology's withdrawal policy.

Graduation Requirements Students must meet the following requirements in the applicable course of study to qualify for graduation and be issued a Certificate of Completion:

• Successful completion of all phases of study, required tests, practical projects, and clinicassignments for the course.

• Completion of the course of study as required by the Division of Occupational andProfessional licensing.

• Pass the final written and practical exam with a 75%.• Complete all required exit paperwork and attend an exit interview.• All debts owed to the institution are paid in full.

Upon completion of the course of study and all graduation requirements, aColor My Nails School of Nail Technology certificate of completion will be awarded.

Option #1 – Pay first day of class: enrollment fee of $200.00 and kit fee of $700.00 or $2100.00. The tuition $2100.00 is to be paid in payments of weekly or bi weekly and needs to be paid in full two weeks before they are scheduled to graduate.

Option #2 – Students who pay in full on or before start day will receive a $200.00 discount.
