Welcome to the 2019 Annual Conference of the Indiana Primary Health Care Association (IPHCA). As your state association, IPHCA has a long and well-established tradition of convening Indiana’s community health centers for a great two-day opportunity to learn from our speakers, and from each other. We are thrilled that you have chosen to use your valuable time to at-tend this program.
IPHCA’s conference over the past four years have focused on increasing knowledge about social determinants of health. These programs were designed to support efforts by health centers to achieve improved out-comes by understanding and addressing the lived realities of our pa-tients. This year, our focus is on one area, patient addictions and effec-tive health center responses to addictive behaviors. The IPHCA staff have invested hundreds of hours in planning this program, with signifi-cant input from health center leaders. It is our sincere hope that you will enjoy the five plenary talks, as well as an array of wonderful break-out sessions, included in the program this year.
As you experience our 2019 conference, please give us your feedback, both through the evaluation forms that will be provided for structured communication about the program, and through any informal feedback you can provide to IPHCA staff. Your views matter to us. We will use program evaluations to inform future conferences.
It has been my pleasure to serve Indiana’s health centers over a period of ten years. As I prepare for retirement in August of this year, I want to wish you all well in the very important work that you do every day. It is an honor for all of us at IPHCA to support you in any and every way possible.

Creating a Culture of Quality
Quality improvement can be like whack-a-mole. The measure we focus on this year improves until we focus on a
different measure next year. A graph of our progress on a measure looks like an ocean wave. How do we foster a
culture that improves quality across the board and at all levels of our organization? Seek first the thriving community
and all else will follow. Stories from Maple City Health Care Center’s 30-year experiment.
James Gingerich, MD, Guardian of the Vision; Maple City Community Health Center
Evidence Based Screening, Brief Interventions and Treatment for Adolescent Substance Use Disorders
In this breakout session we will review state of the art brief screening tools for adolescent substance use. We will also
learn about evidence based models for the treatment of adolescent substance use disorders, as well as approaches that
one can use in a primary care setting that can enhance an adolescent’s motivation to change their own behavior.
Leslie Hulvershorn M.D., Associate Professor of Psychiatry, Adolescent Behavioral Health Research Program; IU School of Medicine
Treatment & Recovery w/ Chronic Illness: The Role of Community Health Workers and Chronic Care Professionals Attendees will learn how chronic conditions adversely affect treatment and recovery. This includes the cost to the organ-ization and how CHWs– CCPs can alleviate that cost burden. The methods CCPs use to educate their patients will be discussed along with the process of obtaining a free Chronic Care Certification will be shared as well. Paul Conrad, Director of Training and Workforce Development; ASPIN
Use of Telemedicine for Behavioral Health Treatment within Primary Care
In this session we will discuss the integration of telemedicine in to the primary care setting for treating behavioral health disorders. How to effectively utilize telemedicine without disrupting workflow, billing/reimbursement, and patient care.
Dr. Donna Vaughan, DBH, LMHC, Jessica Bosecker, LMHC; Windrose Health Network
Primary Care Conditions of Patients with SUD This session will cover the most common primary care conditions and experiences of patients with substance use disorders and how best to serve them in the community health center setting. This includes discussing what primary care conditions are common, treatment options, and resources to assist clinicians and public health workers.

Jennifer C Anglin, MS, CHES; Purdue Healthcare Advisors
The Role of Philanthropy in Addressing the Addictions Crisis While health centers have been successful using resources from HRSA to treat addiction, there are opportunities to en-gage with private philanthropy. This session will identify strategies to partner with foundations, share promising practic-es for foundation engagement and provide details on trends in giving around addiction services and the opioid
epidemic. Jason Patnosh, Associate Vice President, NACHC Partnership and Resource Development Marcia Haaff, Chief Executive Officer; Lutheran Foundation

Heidi Hart, Med, LCPC; CEO Terry Reilly Health Services
Don’t Fix This This breakout sessions will discuss patient examples where “fixing” gets in the way of patient progress. Practice, case review and group discussion will require an engaged participant, so come ready to work!
Mallori DeSalle, MA, LMHC, NCC, CMHC, CPS Prevention Insights, IU-Bloomington, School of Public Health
Building Community Collaborative Partnerships for Better Patient Care This is the map of our journey as an FQHC through community organizing to improve the medical outcomes of our most vulnerable patients. This panel presentation will endeavor to pass on tools to others in order that they may then develop community systems tailored to their own set of unique local challenges
