Welcome to the 2019 Annual Conference of the Indiana Primary Health Care Association (IPHCA). As your state association, IPHCA has a long and well-established tradition of convening Indiana’s community health centers for a great two-day opportunity to learn from our speakers, and from each other. We are thrilled that you have chosen to use your valuable time to at-tend this program.

IPHCA’s conference over the past four years have focused on increasing knowledge about social determinants of health. These programs were designed to support efforts by health centers to achieve improved out-comes by understanding and addressing the lived realities of our pa-tients. This year, our focus is on one area, patient addictions and effec-tive health center responses to addictive behaviors. The IPHCA staff have invested hundreds of hours in planning this program, with signifi-cant input from health center leaders. It is our sincere hope that you will enjoy the five plenary talks, as well as an array of wonderful break-out sessions, included in the program this year.

As you experience our 2019 conference, please give us your feedback, both through the evaluation forms that will be provided for structured communication about the program, and through any informal feedback you can provide to IPHCA staff. Your views matter to us. We will use program evaluations to inform future conferences.

It has been my pleasure to serve Indiana’s health centers over a period of ten years. As I prepare for retirement in August of this year, I want to wish you all well in the very important work that you do every day. It is an honor for all of us at IPHCA to support you in any and every way possible.

Empathic Inquiry: Patient Centered Approaches to

Social Determinants of Health Screening

This session will detail key principles for patient centered social needs screening gained from insights of the

Oregon PCA’s empathic enquiry collaborative. Attendees will learn action oriented principles for developing a patient

centered approach to social need conversations, social need screening workflows, and the role empathic inquiry plays in

trauma informed care.

Caring for Patients with Pain is a Team Sport The desire to help patients treat chronic or acute pain successfully exceeds the skill and resources of a single individual.

This session examines the roles and contributions of care team members in pain management including extended care

members. Attendees will learn strategies to increase provider competency in chronic pain management, they will under-

stand the six building blocks for team based opioid management.

Breathe: Healthy Steps to Living Tobacco Free: New Materials for Patent Education This session will introduce new guided educational resources for use with pregnant women and families with young children in an effort to reduce the disease and economic burden tobacco use disproportionately places on racial/ethnic minorities and low socio-economic status (SES) populations. These new materials focus on health literacy, using high-impact graphics, and cover timely topics such as e-cigarettes (vaping) and the effects of marijuana use during pregnancy and around children.

Harm Reduction and Needle Exchange: Aspire’s Story Attendees will learn about the challenges of running a needle exchange program, including legal requirements, local political concerns, funding restrictions, and benchmark requirements. Justice Steven David was appointed to the Indiana Supreme Court

in 2010 and now serves as the senior member of the Court. Prior

to being named the 106th Justice of the Indiana Supreme Court,

Justice David built a multi-faceted career of private practice,

corporate counsel experience, and stellar service as a military law-

yer. Justice David’s 35 years of public service includes 16 years as

a trial court judge in Boone County and 28 years of military service.

Rachel serves as the President and CEO of Central City Con-

cern, an organization that provides comprehensive solutions

to ending homelessness through health, housing, and

employment interventions. An internist with a specialty in

addictions medicine, Rachel has 15 years’ experience caring

for and designing programs for individuals with substance

use disorders, persistent pain, and mental health disorders.

Dr. Carlos serves as Chief of Internal Medicine for Eskenazi

Health. As a practicing Pulmonologist he has embraced the

teaching mission of the Indiana University School of

Medicine. He is a three time recipient of the “Golden Apple”

award for the top teaching professor for the school.

Mallori DeSalle is a licensed mental health counselor, a certi-

fied in specialized treatment areas and prevention and a

member of the Motivational Interviewing Network of Trainers.

At the School of Public Health at Indiana University,

Bloomington, she manages the Screening, Brief Intervention

and Referral to Treatment program within Prevention In-

sights.

Creating a Culture of Quality

Quality improvement can be like whack-a-mole. The measure we focus on this year improves until we focus on a

different measure next year. A graph of our progress on a measure looks like an ocean wave. How do we foster a

culture that improves quality across the board and at all levels of our organization? Seek first the thriving community

Evidence Based Screening, Brief Interventions and Treatment for Adolescent Substance Use Disorders

In this breakout session we will review state of the art brief screening tools for adolescent substance use. We will also

learn about evidence based models for the treatment of adolescent substance use disorders, as well as approaches that

one can use in a primary care setting that can enhance an adolescent’s motivation to change their own behavior.

Treatment & Recovery w/ Chronic Illness: The Role of Community Health Workers and Chronic Care Professionals Attendees will learn how chronic conditions adversely affect treatment and recovery. This includes the cost to the organ-ization and how CHWs– CCPs can alleviate that cost burden. The methods CCPs use to educate their patients will be discussed along with the process of obtaining a free Chronic Care Certification will be shared as well. Use of Telemedicine for Behavioral Health Treatment within Primary Care

In this session we will discuss the integration of telemedicine in to the primary care setting for treating behavioral health disorders. How to effectively utilize telemedicine without disrupting workflow, billing/reimbursement, and patient care.

Primary Care Conditions of Patients with SUD This session will cover the most common primary care conditions and experiences of patients with substance use disorders and how best to serve them in the community health center setting. Medicaid Promoting Interoperability: A Challenge to STOP Addictions This presentation is to explore the 2019 Medicaid PI objectives and look at them through the lens of the STOP theme. We will examine the objectives and talk through them in by meshing in social determinants of health opportunities and how PCMH criteria can play a part in improving your scores as well as address the addiction crisis.
