Expectations for Behavior Data EntryAll behavior incidents resulting in an office referral must be entered into Infinite Campus.

Why Reliable Data MattersFor students to learn and teachers to teach, schools must be safe. Accurate and timely data is critical for enabling school and district leaders to effectively focus resources and interventions aimed at improving the safety and security of all students, staff and school facilities (National Center for Education Statistics, 2011).

Behavior Event Cross ReferenceBehavior Event Codes are district specific. As Behavior data is entered, the user must not only select a local Fayette County Behavior Event name (skipping, fighting, etc.) but must also select a KDE defined Board or Law Violation. A Cross Reference table is available to help in the selection. The Cross Reference table is found starting on Page 30.

Behavior LettersPer KDE Regulation, we are required to send a letter home each time a student is suspended. We must keep a copy of this letter on file for a minimum of five (5) full school years. Schools may create their own letter, however letters are also available in Infinite Campus. For instructions on how to print Behavior and Suspension Letters in Infinite campus, see Page 27.

Student Code of ConductThe Student Code of Conduct should be used by administrators when deciding consequences or Resolutions for the student.

The document can be found on the FCPS Website:

1. Go to http://www.fcps.net2. Under the “Students” channel, click “Code of Conduct”

Technical Center Behavior DataFor students attending a Technical Center (Eastside, Southside, Locust Trace), all Incidents with the exception of “Out of School Suspension” should be entered at the school in which it actually occurred.

If a Technical Center student is suspended from the Technical Center, the Incident must be entered at both schools because of the tie to attendance. Technical Centers should use the “VSSP3” Resolution code. If a Technical Center student is suspended from their A1 School, the Incident must only be entered at the A1 School.

“Double Dash” Report InformationAll “Double Dash” reports (named for the dashes to the left of the report name) in Infinite Campus are custom reports. If you are prompted for a login, always use your Windows account. The username is your full email address, and the password is your Windows account password.

Printing “Double Dash” ReportsTo print a Double Dash report, the data must first be exported and then printed. Commonly used export formats are PDF and Excel. To export, click the Save button, and choose the appropriate export type.

Identifying your Behavior AdminsBefore entering Behavior records, it is important to identify staff members who decide the consequence for each behavioral action (time out, SAFE, suspension, etc.). Typically this includes your principal and associate principals. Rights to identify a staff member as Behavior Admin are given only to Building Coaches. The school list containing Building Coach names can be downloaded here: Index > FCPS IC Support Site > Staff Information .

Building coaches can follow the steps below:

1. Click the Search tab; select the Staff category

2. Search for the staff member3. Go to: Index > Census > People4. Click the District Assignment tab5. At the bottom of the screen,

check the box labeled Behavior Admin

6. Click Save

Enabling Letters to Print in Spanish

Behavior Data Entry Staff typically will not have rights to maintain the “Spanish Letter” checkbox. Your Registrar can follow the steps below to identify the appropriate students.

1. Search for the student2. Click the appropriate

TermsIncident – An Incident is a group of Events linked by time and location. Incident reporting is similar to a police report – a record of occurrences at a specific time and place.

Event – An occurrence within an Incident. Each Incident must contain one Event.

Participant – A student involved in the Event.

Role – The function (Offender, Participant, Victim or Witness) the student played in the behavior Event.

Law and Board Violations - An item in either of two lists that contains the KDE violation. The violations are separated into Board violations and Law violations. Every Event code begins with a B for "Board" or an L for "Law". This signifies to the user the appropriate Board violation list or Law violation list that contains the match to the Behavior Event.

Resolution – The consequence assigned to the participant(s) of an Event.

Out of School Suspension – Disciplinary action that involves denying the student access to the school campus for any part of the school day. This means a student can be suspended for less than a full school day.

INSR – In School Removal – from KDE – “ any district resolution type in which student is removed from his/her normal educational environment during instructional time…”. For Fayette County Public Schools, all of the following Resolutions fall into this category

If multiple students are involved in one Event, be careful to add each student under the same Event.

This button is not visible until the student is added to the Event. Multiple Resolutions can be assigned to a student.

5 Complete Within the Incident, click the Status dropdown menu and select Complete

After all data entry has been completed for the Incident, change the Status to “Complete”. This is a manual process. Infinite Campus does not do this automatically.

Behavior ManagementAll Behavior data entry is done from the Behavior Management screen: Index > Behavior > Behavior Management .

1. New – This button allows the user to enter a brand new Incident and Event.2. Status Filter – This drop down allows the user to limit what he/she views in the Behavior

Management Tool. Any Incident under Submitted/In Progress does not have all data elements entered. Any Incident under Complete has all data elements entered.

3. Alignment Filter – This drop down should always say Discipline/Award.

Behavior IncidentThe Behavior Incident screen is accessed by clicking “New” or by clicking a previously entered Incident.

1. Status – The Status drop down is a manual field. The Status should be left at “In Progress” while the data elements are being added. After all Events, participants and Resolutions have been entered, the Status should be changed to Complete. Again, this is a manual step, not an automatic one.

2. Submitted By – This is the name of the staff member submitting the Incident. If the Incident happened in a classroom, the Submitted By name would be the teacher.

3. Alignment – Always Discipline4. Title – Leave this field blank. The system will populate the field with the Event Name when the

record is saved.5. Date of Incident – This field defaults to the current date. Change as needed to reflect the date

of the Incident. The date of the incident MUST be entered here.6. Time of Incident – This field defaults to the current time. Change to reflect the time of the

Incident. The time of the incident MUST be entered here.7. Damages – Enter a monetary amount here, if applicable8. Context – See a detailed description on Page 12.9. Context Description – Text field allowing more specifics on the Context.10. Behavior Event Location – A drop down list providing choices so that the site of the behavior

Event can be included in the Safe School Report.11. Details - Explain the reason for the discipline referral. These notes are usually taken verbatim

from the referral form. Student names should not be entered in this field, but the description should be as exact as possible. (Replaces the Event Details field except data entered here applies to the entire Incident rather than a specific Event within the Incident)

The Behavior Incident that occurs in a school setting OR while a student is under the authority of the school/district AND between the times the bell rings in the morning and the bell rings in the afternoon.

Incident occurs during instruction in the classroom

Incident occurs during lunch in the cafeteria

Incident occurs in the gym during a Pep Rally.

Incident occurs on a field trip

SN: School Sponsored Event, not during school hours

The Behavior Incident that occurs in a school setting OR while a student is under the authority of the school/district AND before the bell rings in the morning or after the bell rings in the afternoon OR on non-instructional days

Incident occurs during a home football game

Incident occurs at a school fall festival

Incident occurs on the school bus

Incident occurs at an away football game in which student was transported by the school or under authority of school personnel

NS: Non School Sponsored Event, during school hours

The Behavior Incident occurs off school campus AND between the times the bell rings in the morning and the bell rings in the afternoon

Incident occurs off campus during the time the student is supposed to be in attendance at school

NN: Non School Sponsored Event, not during school hours

Behavior EventThe Event is an occurrence within the Behavior Incident. The Behavior Event screen is access by clicking the New button in Behavior Management and clicking Add Event/Participant, or by opening an existing Incident and clicking Add Event/Participant.

1. Event Type – The short title for what the student(s) did wrong. An Event is an individual occurrence within an Incident. Every Event Code starts with a letter that corresponds to the FCPS Student Code of Conduct. “B” indicates a Board violation. “L” indicates a Law violation.

Behavior Events requiring entry of Harassment Type and Victim Participant Details: B140 Bullying, L430 Harassing Communication, L440 Harassment – Law, L567 Misdemeanor Stalking, B837 Threatening-Student, B840 Threatening-Staff, B890 Verbal Abuse.

2. Checkboxes Call Police – Select only if metro police were called. If a report is filed by an officer, the

Case Number should also be entered in the field near the bottom of the Event Details section.

Violence Indicator School Resource Officer – Indicates FCPS

law enforcement officers are involved. Gang Related Court Designated Worker – Indicates

event required involvement of a CDW3. Law and Board Violation – An item in either of

two lists that contains the KDE reported list of violations. Each Behavior Event must have a Law or Board violation selected.

4. Case Number – The case number as assigned by a law enforcement officer. All Law Violations must have a Case Number entered into Infinite Campus.

5. Motivation/Function – If known, select the motivator for the student to engage in the behavior.

6. Participant Details – This section ties a student(s) to the Event. See details on Page 14, Participant Details.

Participant Details 1. Participant Name – Student(s) involved in the selected Event. Multiple students can be

involved in an Event and multiple Events can be tied to an Incident. Once you select the student, additional Participant Details display.

2. Display on Portal – Do not check this checkbox. The district settings for Parent Portal do not allow viewing of Behavior records; however, this box should never be checked.

3. Role (previously called Event Role) – The function (Offender, Participant, Victim or Witness) the student played in the behavior Event.

4. Demerit – No district or state policy on this field. Schools may use the field at their discretion.5. Relationship to School: Leave this set to “1: Current Student”6. Injury7. Injury Description8. Details (Replaces Role Details) – This field provides an area for the administrator to add more

details for the behavior Event specific to each student involved. Do not enter any student names in this field.

9. If the event involves Bullying/Harassment, Drug/Alcohol or Weapon, additional entry is required.a. Bullying/Harassment – Enter the Harassment Type (required) and Description (optional)b. Drug/Alcohol – Enter the Drug Type (required) and Description (optional)c. Weapon – Enter the Weapon Type (required) and Description (optional)

Behavior ResolutionA Resolution is the consequence for the student’s action. A student may have more than one Resolution assigned for a specific Event. The Resolution screen is accessed by first creating the Incident and Event, adding Participants, then clicking “Add Resolution”.

1. Resolution Type – This field indicates the consequence (Conference, Out of School Suspension, etc.). If the Resolution is Restraint or Seclusion, you must also follow steps on Page 18, Restraint and Seclusion Resolutions.

2. Resolution Assign Date – When was the consequence assigned?

3. Resolution Start Date/Time – When will the consequence begin?

4. Resolution End Date/Time – When will the consequence end?

5. Duration in School Days – This field is used for Out of School Suspension Resolutions only. Estimate the number of school days in whole and half numbers (1.5, 2.0, etc.)

6. Behavior Admin Staff Name – The administrator who decided the consequence.

7. Apply To – Place a check beside the appropriate student, being careful if there is more than one Event attached to the Incident.

Important Info for Resolutions Error reports are available to catch date errors. See Page 23, Error Reports.

Resolution Dates must not be later than a student's withdrawal date. If a student withdraws while a Resolution is still in force, the behavior record must be adjusted to show an end date no later than the withdrawal date.

A withdrawal date and the last day of Suspension can be the same date. The date of withdrawal is a day of membership and is true in all public school districts in Kentucky.

If a student enrolls in a different school during a suspension, make the suspension End Date the same as the Withdrawal Date.

Resolutions for Suspension, SAFE and In School Suspension MUST include an End Date and End Time.

If a suspension spans a non-instructional day (including snow days), multiple Resolutions should not be used.

Behavior Resolution TypesThere are three different types of Resolutions:

1. Resolution Assignment for One Date – This means the student’s Resolution does not span more than one date. In the screen shot below, the data entry indicates the student was in ISS / S.A.F.E. on 10/6 only, from 8:25 – 3:15.

2. Continuous Resolution Assignment for Multiple Dates – The student began their Resolution assignment on one date and it continued on to one or more dates. There are no breaks in instructional time for the Resolution. In the screen shot below, the data indicates the student was in ISS / S.A.F.E. for three school days, from 10/6 – 10/8.

3. Resolution Assignment for Multiple Partial Days – The student was assigned the Resolution for multiple days, but only for a portion of each day. Each segment of time must be entered as a separate Resolution. The Resolution will display according to how many segments of time were assigned. See the screen shots below for an example of a student assigned ISS / S.A.F.E. for 1 hour on three separate dates.

Entering Behavior RecordsThe expectation for data entry is: All Behavior Incidents resulting in an office referral must be entered into Infinite Campus by the end of the school day following the Incident. The process below follows the Behavior Overview on Page 9.

1. Create the Incident (see Page 11 for details)a. Go to: Index > Behavior > Behavior Managementb. Click Newc. Enter the following: Submitted By, Title, Date of Incident, Time of Incident, Context (see

Page 12) and Context Description (if needed).d. Select the Behavior Event Location and enter a Location Description if needed.e. Enter Details as needed to describe what happened. Be careful not to type student

names in this field.2. Create the Event(s) (see Page 13 for details)

a. Click Add Event / Participantb. Enter the following: Event Type, checkboxes as appropriate, Law or Board Violation (see

Cross Reference Table starting on Page 30) and Case Number if the Event is a Law Violation or if police are involved.

3. Add Participant(s) (see Page 14, Participant Details for details)a. In the Participant Details section of the screen, enter the student’s name in the Student

Name field and click Searchb. Select the appropriate studentc. Enter the following: Role, Injury, Injury Descriptiond. Enter Details to describe how the student was involved in the Evente. Add additional students as needed by typing their name in the Student Name field and

completing steps a – d.f. If the event involves Bullying/Harassment, Drug/Alcohol or Weapon, enter the Type

(required) and Description (optional).g. Click Save

4. Add Resolution (see Page 15 for details)a. Click Add Resolutionb. Enter the following: Resolution Type, Resolution Assign Date, Resolution Start Date /

Time, Resolution End Date / Time, Duration in School Days (Suspension and ISS/SAFE only), Behavior Admin Staff Name.

c. Under Apply To, check the appropriate student names, being very careful if more than one Event is listed.

d. Click Savee. Add additional Resolutions as needed. Follow steps a – d until all Resolutions are entered.

5. If Resolution is Restraint or Seclusion, a Behavior Response is required (see Page 18 for details)6. Complete - If all data entry is complete, change the Status to Complete, click Save

Restraint and Seclusion ResolutionsIf a student is restrained and/or secluded, BOTH of the following are required (704 KAR 7:160):

A resolution of Restraint and/or Seclusion The entry of a Behavior Response in Infinite Campus

1. Add the appropriate resolution of Restraint or Seclusion by following the steps for Entering Behavior Records, Page 17, #4 – Add Resolution.

2. Click: Add Behavior Response (If Add Behavior Response is grayed out, change the record from COMPLETE to IN PROGRESS to activate the button)

3. Select the following:a. Response Type - Create a new behavior response for each Response Type. Restraint and

Seclusion must be entered as separate response types. If the technique changes during the SAME response, describe in the Response Details textbox.

b. Response Start Timec. Response End Timed. Pre-Response Interventions - Select the behavioral intervention(s) used immediately

prior to implementation of restraint or seclusion. User can choose multiple interventions.e. Pre-Response Actions – Enter the following information into the textbox:

i. Document an account of the actions by involved students and staff before the response

ii. Describe events leading up to the response, including possible factors contributing to the dangerous behavior.

iii. Document the effectiveness of any Pre-Response Interventionsiv. Describe how the student’s behavior posed an imminent danger of physical harm

to self or othersf. Response Details – Enter the following information into the textbox:

i. Document an account of the student’s behavior during the responseii. Describe how school personnel responded to the dangerous behavior

iii. Document interactions between the student and the school personnel during the response

iv. Document the effectiveness of this response type

Restraint and Seclusion Resolutionsg. Post-Response Details – Enter the following information into the textbox:

i. Document an account of the student and staff following the responseii. Document the effectiveness this response had in deescalating the situation

iii. Describe the planned positive behavioral interventions which shall be used to reduce the future need for restraint or seclusion of the student

iv. Document a referral to Program 504 or IDEA, if student not previously identifiedv. Describe basis for declining to refer the student to Program 504 or IDEA, if

applicablevi. Enter the Date and Time the Parent Notified and Method of Notificationvii. If a debriefing session was requested by the parent/guardian, enter the session

date, time and attendees. viii. Document any follow-up debriefing sessions

h. Student was injured during the response – check only if applicablei. Injury Severity

i. Apply To – Select student in which to apply Behavior Responsej. Guardian Contacted – Indicate that the guardian was contacted, this will open additional

fields to complete. This field is not required, however it is mandatory that a parent/guardian is contacted with each Restraint or Seclusion resolution.

i. Date – Enter the date the guardian was contacted ii. Time – Enter the time the guardian was contacted

iii. Contact Name – Enter the name of the guardian contactediv. Details – Enter any details discussed during the contact, such as the method of

contact.k. Participant Details – Search and add all staff involved in the behavior response. NOTE:

Do not add other students as participants to a response, all participants currently print on the offender’s behavior detail report and this is a violation of FERPA. The issue will be resolved in a future release of Infinite Campus.

i. Participant was injured during the response – Check if applicableii. Describe any injuries to the participant

iii. Injury Severity4. When all data has been entered on the Behavior Response, scroll to the top and check the

Complete checkbox.5. Check the record again to make sure Restraint or Seclusion have been added as Resolutions.6. Click Save7. Notify Principal or designee that the Restraint or Seclusion Incident has been completed in

Infinite Campus

Opening Existing Behavior RecordsThere are two ways to open an existing Behavior Record. Which option you choose is a personal preference. If you are unsure when the Incident occurred, please use option 2, and search by student.

1. Opening an Existing Behavior Record via Behavior Management:a. Go to: Index > Behavior > Behavior Managementb. If the Incident Status has been changed to Complete, in the Status Filter dropdown select

Completec. The Incidents are sorted by date. Click the appropriate Incident to view.

2. Opening an Existing Behavior Record via the Student Behavior Taba. Go to the Search tab, search for any of the students involved in the Incidentb. Go to: Index > Student Information > Generalc. Click the Behavior tabd. Click the appropriate Incidente. In the Incident Detail section, click the Incident ID link. This opens the Incident in the

Behavior Management tool.

Editing Behavior Records1. Search for the Incident to edit by using the steps for Opening Existing Behavior Records, Page 20.2. Make corrections to the Incident, Event(s), Participants and/or Resolutions3. Click Save in the upper left hand corner

Deleting Behavior RecordsPlease use caution when deleting! Deleting not only removes the part deleted, but also all underlying data. D E L E T E W I T H E X T R E M E C A U T I O N !

1. Deleting an Incident – This will delete not only the Incident, but all Events, Participants and Resolutions tied to it.

a. Search for the Incident to delete by using the steps for Opening Existing Behavior Records, Page 20.

b. Click the “Delete” button above the Incident

2. Deleting an Event – This will delete not only the Event but all Participants and Resolutions tied to it.a. Search for the Incident to delete by using the steps for Opening Existing Behavior Records,

Page 20. b. Click the Event to deletec. At the bottom of the Event and

Participant Details screen click Delete d. Click Save on the Incident

3. Deleting a Participant – This will delete not only the Participant but all Resolutions tied to the student.

a. Search for the Incident to delete by using the steps for Opening Existing Behavior Records, Page 20.

b. Click the appropriate Event to delete a Participant

c. In the Participant Details section, uncheck the checkbox beside the student’s name

d. Click Savee. Click Save on the Incident

4. Deleting a Resolutiona. Search for the Incident containing the Resolution to delete by using the steps for Opening

Existing Behavior Records, Page 20.b. Under the appropriate Event and Participant, click the Resolution to deletec. Click Deleted. Click Save on the Incident

Correcting Duplicate EventsWhen more than one student is involved in an Incident/Event, it is very important not to duplicate the Event. Use the steps below if it is necessary to combine multiple Events into one due to incorrect data entry. In the screen shot to the right, you will see three Events for “Fight – Student/Student”. When this is submitted to KDE, it will appear as three separate fighting Events, when it actually was one Event with two Participants and one Victim.

1. Search for the Incident to delete by using the steps for Opening Existing Behavior Records, Page 20.

2. Click the first event listed3. Check the boxes to add the other students attached to the Incident. In the screen shot to the

right, I will place checks beside Robin and Brandon’s name. Because Robin was a Victim, I must be careful to change her Role to Victim.

4. Click Save. This adds the additional two students to the first Event.

5. Make note of the Resolutions for the students moved to the first Event.6. Delete the duplicate Events using the steps on Page 21, Deleting Behavior Records.7. Click Add Resolution and add the Resolutions you noted to the students as needed. Use the

steps on Page 17, Add Resolution.8. Click Save on the Incident

Error ReportsThese reports must be run monthly and errors corrected.

Behavior Error ReportThis report will show you any Behavior Incident/Event with the following errors:

Bullying/Harassment Data Requirements - Behavior Events qualifying as Bullying/Harassment that do not list a Victim or do not have a Harassment Type indicated.

Date Issues – Incident Dates that do not fall within the school calendar dates Resolution End Date Missing Law/Board Violation Check – Law/Board Violation does not match Event Type. See Cross

Reference Table to make corrections. Call Police Check – Event coded as a Law Violation but the “Call Police” checkbox was not

checked. Overlapping Resolutions – Dates for ISS / SAFE and Out of School Suspension cannot overlap

1. Go to: Index > Behavior > Reports > Error Reports > Error Report2. Select the following:

SAFE Schools Error ReportThis report should be run in conjunction with the Behavior Error Report on Page 23. KDE’s Safe Schools Report will show you any Behavior Incident/Event with the following errors:

State Resolutions without a Start/End Date and/or Time State Resolution missing a Law or Board Violation Events without a participant selected Behavior Incident date not within enrollment Suspension/attendance dates and/or times inconsistent Student demographic information missing Behavior Resolution date not within enrollment Overlapping state Resolutions Behavior Response checks for Restraint and Seclusion

1. Go to: Index > KY State Reporting > Safe Schools2. Under Select Calendars select your school3. Click: Run Extract4. The first screen displayed is a list of errors. Fix all errors listed as soon as possible.

Suspension Attendance LinkageThis report will show where a student’s suspension in Behavior does not match in Attendance or vice versa. To correct these errors you need to work with the staff member who enters Attendance data to determine if the error is in Attendance or in Behavior.

1. Go to: Index > KY State Reporting > Edit Reports > Suspension Attendance Linkage2. Click: Generate Report3. All students listed must be corrected (with the exception of Early Start grade levels 96, 97, 98 and
