A 60-year-old male presented with right-sided flank pain. Oninitial evaluation with NCCT (KUB), he was found to haverenal calculi on right side with a rare congenital anomalyknown as tandem kidney [Figs. 1 and 2]. Tandem kidney orL-shaped kidney is one of the varieties of crossed renal ectopia

nephrolithotomy (PCNL) with holmium laser antegradeendopyelotomy.

The first reported case of crossed ectopia was described byPamarolus in 1654. Exact incidence of this anomaly isunknown, though the prevalence seen in autopsy series is1:2000.1 The anomaly occurs more commonly in males with aratio of 2:1, and left-to-right ectopia is seen three times morefrequently than right-to-left ectopia. They are mostly asymp-tomatic, but whenever urological problems are associated,they require appropriate surgical intervention.
