(d) Tandem End Office Multiplexing
A per minute of use charge applies for the use of multiplexing equipment on the endoffice side of the access tandem and to each FGA/BSA-A terminating minute of usebetween the dial tone office and the end office.
(e) Dedicated Tandem Trunk Port
A monthly rate applies, per port for each in service dedicated trunk on the serving wirecenter side of the access tandem.
Dedicated Tandem Trunk Port is billed as originating and terminating based on a PercentOriginating Usage (POU) factor of 50%.
Originating Calculation = PIU x Originating Rate x Quantity x POU
Terminating Calculation = PIU x Terminating Rate x Quantity x (100 – POU)

(F) Local Switching
(1) Dedicated End Office Port
A monthly rate applies, per line or per trunk, for each in service dedicated line or trunkterminating in the end office port.
Dedicated End Office Port is billed as originating and terminating based on a PercentOriginating Usage (POU) factor of 50%.
Originating Calculation = PIU x Originating Rate x Quantity x POU
Terminating Calculation = PIU x Terminating Rate x Quantity x (100 – POU)
(2) Shared End Office Trunk Port
A per minute of use charge applies to the shared end office trunk ports for termination ofcommon transport trunks for tandem-routed traffic.
(G) DNAL Termination
A monthly rate applies only to the DNAL BSA for each termination requested by thecustomer. One DNAL Termination charge applies per each point of termination.
(H) DNAL Mileage
The DNAL Mileage charge applies only to the DNAL BSA and is calculated according tomileage band. There are two rates that apply per band, i.e., a fixed monthly rate per mileageband and a monthly rate per band.

39.1 General Description
(A) This section of the tariff provides the regulations, rates and terms and conditions that apply totelecommunications services provided by the Telephone Company in the MetropolitanStatistical Areas (MSAs) in which the Telephone Company has received Phase II pricingflexibility pursuant to Subpart H of Part 69 of the Commission’s Rules. MSAs are divided intothe categories below:
(1) Full Service MSAs
Full Service Relief MSAs are those MSAs which qualified for Phase II pricing flexibility forall elements of service, i.e., local channels (channel terminations) between LEC endoffices and customer (end user) premises; entrance facilities; dedicated interofficefacilities; local channels (channel terminations) between an interexchange carrier's pointof presence and a serving wire center. The Full Service Relief MSAs are set forth inSection 39.2(A), following.
(2) Limited Service MSAs
Limited Service Relief MSAs are those MSAs that qualified for Phase II pricing flexibilityfor all elements of service except local channels (channel terminations) between a LECend office and a customer (end user) premise. The Limited Service Relief MSAs are setforth in Section 39.2(B), following.
(B) The services provided in MSAs pursuant to this section of the tariff are set forth in Section 39.3following. These services are comparable to the SWA Dedicated Transport Services inSections 6.9.2(A)(1), 6.9.2(A)(3), 6.9.2(A)(4), 6.9.2(B)(1), 6.9.2(B)(3), 6.9.2(B)(4), 6.9.2(F), and6.9.2(G). The general regulations, service descriptions and rate regulations for the SWADedicated Transport Services in Section 6 are also applicable to the services specified in thissection, except as provided below.
(C) Unless otherwise provided for in this section, regulations set forth in Sections 1,2,5, and 13 are(AT) also applicable.
