1. Minimum Mandatory Qualifications

Implemented STARLIMS v9 in both the clinical and environmental laboratories working closely with both PHL and STARLIMS personnel. This included integration with both Common Customer application and SDWIS applications. (EPA Safe Drinking water reporting application). During this effort my roles and responsibilities included business analyst, application developer and ST ARUMS configuration engineer. With minimal external resources our team successfully deployed STARLIMS V9, provided end user training and support. We also developed numerous crystal reports to satisfy business requirements.

Beyond my expert level knowledge in SDMS, LabX, etc. I also have a strong background in laboratory instrumentation and the software accompanying them. This domain of knowledge is essential to ensuring instruments are properly integrated with the STARLiMS LIMS via SDMS. It also guarantees the system can adapt to future changes in instrumentation (e.g., replacing Nicolet FTIRs with Shimadzu). The level of expert knowledge CQ Consulting provides is also required in order to provide a superior level of support to the end users in the field. This yet another major differentiator between CQ Consulting and other LIMS service providers. Methodology and Team Building Project Management I know how to successfully manage projects. I have 15 years of experience successfully managing software implementations and possess the PMI (Project Management Institute) PMP (Project Management Professional)

certification. I am an expert at developing and maintaining project plans, tracking and reporting project KPls (e.g., schedule variance, cost variance, work variance, etc.), managing issues, quantitative risk management, communication management, eliminating team impediments, etc.

SCRUM I have extensive experience with the Agile SCRUM framework, whether it is gathering requirements (user stories) and defining acceptance criteria, running a planning poker session to derive estimates, managing the daily stand­ups, monitoring team velocity, working with the product owner to groom the product backlog, or conducting a sprint retrospective. I have led countless sprint planning meetings, planning poker sessions, daily stand-ups, sprint demos and sprint retrospectives. CQ Consulting also highly encourages scheduled backlog grooming sessions. These are led by the Product Owner with assistance from the SCRUM master. Change happens. Whether the requirements change or business priorities change, it is essential for the project leadership team to ensure the backlog remains accurate so the team can focus on work that provides the greatest benefit

Testing When developing applications, I perform basic smoke testing once the work item is complete. This is highly informal testing in which the I run the application and checks basic functionality. After smoke testing, I will perform logical testing. During logical testing, the I test the application against the acceptance criteria specified in the associated user stories. Although the result is typically documented, this is still not considered formal testing. Testers generally perform a separate verification of the logical tests once the developer has completed their testing. As a final note, CQ Consulting recommends our developers to perform independent peer reviews on all designs and developed code if at all possible.

Once developer testing is complete, formal testing can be conducted. This is the process whereby the tester creates formal test cases to assist with the verification process. In highly regulated environments, test cases are typically required for every requirement (user story). However, in non-regulated environments, CQ Consulting encourages clients to utilize a risk-based approach for formal test cases. We also typically recommend independent execution of formal test cases; in other words, the individual that creates the test cannot be the one that executes it. The second type of testing performed by our customers (and typically the most valuable) is exploratory testing. While this is considered informal testing and does not satisfy verification requirements, it typically reveals usability defects as it provides an independent, non-technical review of the software.

Training A key aspect to any implementation is training. I have developed and executed comprehensive, instructor-led, role­based training programs that provide hands-on experience within a laboratory environment to accelerate adoption of STARLiMS LIMS solutions. I have also developed quick reference guides (QRGs), provided specialized one-on-one training, and developed and executed plans for continuing education (e.g., releasing quick reference videos along with release notes when a sprint product is formally released). Beyond technical capabilities, I also possess the critical communication skills required to provide effective training to laboratory users. Beyond end user training I have developed and delivered many training sessions for ST ARUMS developers whether it be formal classroom training or informal paired programing exercises.

4. Performance Standards

State law requires that all contracts for services include Performance Standards for measuring the overall quality of services provided that a Contractor shall meet in order to avoid assessment of damages. The State may be open to negotiations of Performance Standards prior to contract award, prior to the commencement of services, or at times throughout the contract duration.
