Ohio Gov. John Kasich took a bold step into the future last year for transforming health care delivery and reducing costs in Ohio through expanding Medicaid coverage and by urging cooperation and support of initiatives that can produce the long-term reforms called for in the Affordable Care Act.

We are pleased to deliver this presentation introducing an innovative local resource surely absent in Ohio. This is a business model flowing from the very words written in the health reform legislation; and answers the call of our Governor for the creation within his Office of Health Transformation the efforts of private and public institutions to work together in improving the health and welfare of Ohio citizens.

The idea of a Telemedicine Centers of Ohio that can work in tandem with Ohio Medicaid MCO subscribers and providers is fully responsive to Ohio’s State Planning Grant and the requirements in the Ohio Medicaid Integration Program awarded contracts starting in 2015.

Before we began this journey, we met with some of Ohio’s Medicaid contractors, and share their aspirations when assessing the impact of the ACA provisions and opportunities beginning January 2014. Notably, one Medicaid contractor leader responding to the Governor’s initiatives said: “we’re looking at improving clinical outcomes, and doing it in a more efficient and more effective way, so we can also produce some sort of reasonable savings”.

Getting and using more technology to accomplish those herculean tasks, without massive upfront capital investments, is made attractive by creating partnering arrangements that offer mutual benefits and can share mutual comparative market advantages. The private entrepreneurial sector offers access to technical innovations and capital that can support rapid adoption, remove capital and manpower risks, and offer expertise that often is not available or affordable in every community in Ohio.

When you look at the advantages for the Medicaid beneficiary to not have to travel sick to see a provider away from the home base, or wait maybe for an distant area provider to visit to see everybody scheduled; effectiveness and efficiency have cost consequences and begs for innovation

The challenge for Ohio’s Medicaid contract payors lay in seizing opportunities to alter its business model anchored in the status quo, and also to be bold enough to embrace disruptive innovations that introduce lasting continuous change and substantial cost savings. Just as the urgent care facilities in drug stores have shown us, and even some Internet webcam providers confirm; insured and non insured alike want local neighborhood health care options, and will go where they believe its available, convenient and affordable.

We now know the State of Ohio can substantially reduce its annual costs of care by using technology that affordably expands access to health care and eliminates the burdens for both patients and providers travel expenses. In fact, substantial savings results were all of Ohio’s selected Medicaid Integration awardee contractors and their providers encourage use of telemedicine visits that support follow up of its insured’s suffering with chronic conditions, support post hospital discharge, improve care of residents in nursing homes to reduce costly avoidable readmissions, and address the timely care for those living in home and community settings, many requiring frequent and high touch care and monitoring.

Using creative bold innovations in remote monitoring fused with initiatives supporting Ohio’s need to support dual eligibles with chronic medical conditions who desire to reside in affordable housing rather than by default in scarce and expensive institutional settings is a highly desirable pursuit. Not only does this use of technology drive coordination and care management, it keeps the patient provider relationship primary and intact, and it also helps drive down the health payer's cost for unnecessary hospitalizations and avoidable ER visits.

As seriously presented herein, Ohio’s Medicaid Providers now have some innovative options that offers comparative competitive advantages in the marketplace, and uniquely opens the door for successive innovations built around telemedicine technology and the shift in how the health care sector of the GDP re-engineers itself in fully implementing Patient-Centered Medical Homes (PCMHs) and use of Accountable Care Organizations (ACOs) as tools enabling the ACA mandates in the next few years..

Telemedicine Centers of Ohio is a specially staffed and equipped facility of designed medical suites to provide telemedicine origination and remote site directed and supervised health care services for medical providers and their patients. Telemedicine infrastructure includes EHR capture at point of care using state of the art integrated medical assessment devices to support HITECH Act compliance.

Initial sites are in Columbus, and other cities will be added as we grow. The Telemedicine Center fulfills four goals set by the Governor:

· Transforming the organization and delivery of healthcare services, including payment reform, health information technology and primary care workforce

· Ensuring collaboration among government agencies, healthcare providers and health plans, and consumers

· Implementing provisions of President Obama’s Patient Protection and Affordable Care Act

· Increasing quality and reducing costs for the state’s employee healthcare system and Medicaid programs

Ohio’s Telemedicine Centers Designed For Most Providers

Private mental health, nutrition, smoke cessation, group therapy sessions, caregiver training, post discharge education.

Pediatric exams, follow-up and wellness consultations, timely assessment and care giver education, ASD, tele- neurological and epilepsy, diabetic, and sleep apnea.

Rehab follow up, post hospital discharge transitional care, telestroke, and physical exams.

All medical services are directed by the patient’s chosen Provider located anywhere in Ohio, and fully conducted in compliance with CMS Part A/B payment requirements, and under Ohio laws. Certain Telemedicine Center suites will be leased and specifically equipped by Providers for their scheduled patients.

Telemedicine Centers support HIPPA compliance with its clients offering the highest level of security. We provide complete end-to-end data security – from behind the firewall, across networks, out to the device, and between apps.

The Center’s high-availability NOC leverages AES-encrypted transport of content to and from mobile devices – giving IT the tools they need to protect against data loss and theft risks while enforcing policy compliance. The Telemedicine Center is built using next-gen containerization, which includes secure data sharing between any of our secured app as well as app-level encryption independent of the device used.

Telemedicine Centers of Ohio Support Medical Care Access to SNFs, ASL, Pre-Schools, Home and Community Settings

Medical devices and technology have made it possible for the health care Providers to make rounds in SNFs, homes, schools, community centers, day care and adult day health centers, thereby supporting Ohio’s fragile populations of dual eligibles, and special needs normally not timely served—and the most costly to manage.
