1.0 PURPOSE

The purpose of this access procedure is to clearly identify the approvals, training and PPE requirements that will allow personnel to enter switch rooms and substation switch-houses designated for “Authorized Personnel Only” and safely perform certain tasks.

2.0 APPLICATION

This procedure, applies at any Vale locations indicated with approval on the cover page. In order to optimize available resources and thus lower operating costs, Vale non-electrical and qualified contractor personnel may be permitted into switch rooms and substation switch-houses to perform tasks such as switching, tagging, resetting, and other tasks. The procedure is not intended to create “free access” inside any known areas of increased electrical hazard. 2.1 EXCEPTIONS

This procedure does not apply to “precipitator switch rooms”.

4.0 COMMITMENTS

The Switch Room and Substation Access Procedure is committed to comply with the following:

5.0 DEFINITIONS

Substation1: A substation is a fenced-in area owned by the Vale Power Department or jointly by the Power Department and the plant’s electrical department; that is used to transform 44-69 kilovolt (kV) power to the lower voltage levels for distribution by the plants and mines. The fenced-in area is locked using a RACO key and adequate signage exists to remind personnel to contact the systems operator upon entry. Switch Room (can be referred to as junction box room underground): A room or fenced-in area owned by the plant’s electrical department used to transform power levels and distribute power to other areas and feed specific pieces of equipment. Adequate signage exists to warn of danger – the highest possible voltage inside, and that entry is for authorized personnel only. In an underground environment, the entry to a switch room may consist of a chain or other barrier having appropriate signage on it. Open Bus Switch Room2: If the switch room contains any exposed voltages in excess of 50 volts (V) (CSA-Z462-15, 4.3.4.4), it is considered to be an open bus switch room, and subject to stricter requirements due to the increased electrical shock hazard. Once the “open bus” in a switch room has been removed or guarded, it is no longer considered “open bus”.

Qualified: One who has demonstrated skills and knowledge related to electrical equipment and installations and has received safety training to identify and avoid hazards involved. Precipitator Switch Room: If the switch room allows precipitator operators to isolate dampers and 600-V power in order to perform static high voltage switching and grounding of the precipitators’ internal structure and external TR (transformer-rectifier) set, then it is exempted from this policy. The precipitator operators will continue to own and manage these switch rooms.

6.0 IMPLEMENTING THE PROCEDURE

Implementing the steps in this procedure will ensure that personnel are informed and instructed in all known hazards associated with specific tasks so risks can be managed. Hazard information must be disclosed before issuing a switch room device permit to help personnel manage the risks to as low as reasonably achievable. The plant or mine’s designated Electrical Authority plays a facilitating role in this access procedure and also assures its safe implementation. The designated Electrical Authority also has complete and final approval over the permits and may revoke these permits.

7.0 ROLES & RESPONSIBILITIES

The following roles and responsibilities are in effect to support and enforce the directives listed in this procedure. 7.1 ELECTRICAL AUTHORITY3

Facilitates and follows the approval process for all permit requests.

Tours personnel inside switch rooms and switch houses and observes their level of competence for the privileges assigned to them (or selects designate for this task).

Issues the “Switch Room Device Permit” (all copies).

Issues key(s) to the permitted individual.

Contacts permit holders at the end of their term to either extend the permit duration or reclaim the key(s).

Has complete and final approval over all permits and may revoke these permits.

Permitted individuals are responsible to:

Keep their required training current for the duration of the permit.

Follow plant specific rules.

Follow all restrictions and instructions as outlined on the permit and during the switch room orientation.

Keep a copy of the permit available at the worksite or implement a site specific method to easily verify who has been given privileges.

When escorting non-permitted individuals in the switch room, they must ensure that these individuals are working safely in accordance with the switch room rules and permit restrictions and instructions.

Ensure that the Vale, SHE Department’s Arc Flash PPE Protection Standard Procedure Instruction (in its most current version) is being followed by all individuals within the arc flash boundary. Those who cannot comply must not enter the boundary while the equipment is being interacted with.

Return key at the permit’s expiry date.

8.0 APPROVALS, TRAINING & GRANTING OF PERMITS

The following sections outline the procedures for approvals and granting of Switch Room Device Permits: 8.1 VALE NON-ELECTRICAL

1. Vale Supervisor of employee seeking access privileges will make a request to the plant’s Electrical Authority (Authority).

2. Employee’s Supervisor to provide proof to the Authority that the employee’s required, pertinent training (i.e. ZES, WHMIS, Fit Testing, etc.) is current.

3. Employee must attend the mandatory ESA course (Equal Code: ESA_SWITCH). The course needs only be taken once.

4. Employee must complete the Switch Room Device Permit (permit) with the Authority (see Appendices 3 and 4).

5. The Authority or his designate will tour the permitted individual into the switch room(s) and observe the level of competency for the tasks assigned.

6. The permit is to be issued along with a key(s) for the switch room(s).

Important Note on Open Bus Switch Rooms:

Permits for Open Bus switch rooms MUST NOT be issued.

Access to Open Bus switch rooms IS NOT permitted without an escort from qualified electrical personnel such as Vale electricians, Vale electrical staff, or a permitted electrician.

8.2 CONTRACTORS

1. Contractor’s plant contact to inform the plant Electrical Authority (authority) of the contractor seeking to obtain access privileges.

2. Contractor must provide proof of Certificate of Qualification (for electricians only) or Powerline Technician Certification (for linemen) or other necessary proof (i.e., specialized service groups) and proof of NORCAT Training.

Important Note on Open Bus Switch Rooms:

Permits for Open Bus switch rooms and substation switch-houses will only be granted to licensed or qualified electricians/technicians/linemen (electrical personnel) after completing switch room specific orientation.

Access to Open Bus switch rooms and substation switch-houses is not permitted for non-electrical contractors without an escort from qualified electrical personnel such as Vale electricians, Vale electrical staff or a permitted electrician/technician/linemen/engineer.

9.0 SWITCH ROOM AND SUBSTATION SWITCH HOUSE TAGGING BOOKS

The existing switch room and substation switch house tagging books are not required for equipment specific breakers, switches, and starters, regardless of voltage. The tagging books will be maintained for all distribution breakers and distribution switches rated 600V or greater.
