ObservationsControl measures Timescale
Hazards and ConsequencesImmediate, and longer-term actions
1. Electrocution: Can be caused due to non-standard electric connections and improper insulation from electric cables coming from the mail lines.
2. Excavation work exposes equipment operators to whole body vibration from earthmoving equipment and ergonomic problems from poor seat and cabin design.
3. Objects and tools used by workers at height not restrained and could cause injury to workers and visitors working on ground.
4. Fire: as mentioned earlier, non standard electric connections with no insulation could cause sparks, also storage of fuel(kerosene) next to the electric board could lead to a major fire in case of a electric malfunction or electric spark.
5. First Aid boxes are not available at the site in case of any accident or incident no first aid would be available, this resulting in the person affected not getting any first aid treatment and could result temporary or permanent damage to the person affected and even causing death. Considering that the nearest nursing home facility available is about 20 kms away.
6. Trash from materials packaging, employee lunch/meal breaks, etc. Has been properly maintained, Recycle bins and garbage bins have been provided at the site.
Cables to be changed and new cables to be installed with proper ground connection, also cables to be double insulated with proper insulation at main joints. Supervisor to be notified and given proper training and timely inspection be done.
Workers be allowed sufficient rest in-between jobs, short shifts to implemented. Sufficient monitoring and new equipment be provided that will eliminate ergonomic issues. Survey to be conducted. Health Plan be put in place for workers encountering ergonomic problems.
Stack materials to prevent sliding, falling, or collapse. Use protective measures such as toe boards and debris nets. Restrain work equipments with the help of strings or ropes or cables depending on the tools used and the nature of work. Provide sufficient training, inspection and monitoring for workers working at height.
Cables to be insulated at joints, rewiring to be done, store of kerosene to be moved to an isolated area, fuel should be kept away from sources of ignition and in certified fuel storage containers, Warning Signs to be placed on the containers.
First aid boxes be made available at designated work sites and should be accessible in event of an accident or incident. On site doctor should be made available considering that there are no near by hospitals.
Good Practice.
3-5 days.
5-7 days.
2 days.
Fuel to be moved immediately and with high priority.
3 days.
To be Continued.
