“The da Vinci Surgical System is an intricate non-programmable robotic tool that surgeons use—not a robot that performs surgery,” says obstetrician-gynecologist Marc Winters, M.D. “For operations such as minimally invasive hysterectomies, the da
Vinci represents a significant advancement in treatment over conventional open surgery and other minimally invasive techniques such as laparoscopy.” Because the system increases precision, the benefits of robotic-assisted surgery include less blood loss, a shorter hospital stay, less pain, a reduced risk of infection and faster return to daily activities.
SURGICAL PRECISION The da Vinci Surgical System acts as an extension of the surgeon’s hands. Composed of several components, the device includes
a large bedside cart with interactive robotic arms, a high-definition, three-dimensional viewing system, a computer console with specialized hand controls, and miniature surgical instruments. Procedures using the system are performed through small incisions instead of one large opening. The surgeon operates while seated at a computer console a few feet from the patient. The console contains hand controls that the surgeon manipulates to direct the machine’s robotic arms, wrists and micro instruments. The wrists have a range of motion and agility that far exceeds human ability. This accounts in part for the system’s incredible surgical precision. Furthermore, the robot’s tweezer-like “fingers” can hold miniature surgical instruments steadily for long periods of times without growing tired.
A 7-foot robot is now assisting surgeons at Saddleback Memorial. Its mission: to help make minimally invasive procedures even less invasive.

Throughout the procedure, the da Vinci’s advanced video system transmits highly detailed, 3-D digital images of the interior surgical area, which greatly enhances the surgeon’s ability to view the complex structures, tissues and organs inside the body.
SMALLER INCISIONS“Robotic-assisted technology has elevated gynecologic surgery to a whole new level,” notes Daniel Sternfeld, M.D., obstetrician-gynecologist. “With the da Vinci Surgical System, we can use minimally invasive techniques without compromising the complexity of the procedure being performed.” Saddleback Memorial surgeons use robotic-assisted surgery to treat a wide range of gynecological conditions, including ovarian and cervical cancer, uterine fibroids, endometriosis, abnormal vaginal bleeding and loss of uterine support (prolapse).
Smaller incisions, greater accuracy and shorter recovery times are among the benefits of operating with the da Vinci Surgical System.
During a conventional open hysterectomy, a 6- to 12-inch incision is made through the abdomen. Recovery can be painful and take up to six weeks. In contrast, surgery with the da Vinci is performed through two to four small incisions. “With the da Vinci’s magnified three-dimensional imaging and zoom-in capabilities, we can operate deep within the pelvic region through smaller incisions with great accuracy,” says Dr. Sternfeld.
Another surgical procedure performed with robotic-assisted technology is the prostatectomy, or removal of the prostate gland. This operation requires only five tiny incisions with patients returning home the
next day. Traditionally, the operation’s effect on the nerves that govern bladder control and sexual function have been a concern. The results of a da Vinci prostatectomy significantly surpass other surgical methods and are particularly
notable in two important areas: the post-surgical restoration of urinary control and the preservation of sexual function. In clinical comparisons of the da Vinci prostatectomy to laparoscopic removal of the prostate (another minimally invasive approach), 97 percent of men regained urinary function after robotic-assisted surgery versus 83 percent for the traditional minimally invasive approach. Those studies also showed that sexual function was restored in 86 percent of men after the da Vinci procedure, versus 76 percent of men who had laparoscopy.

da Vinci Surgical System. “There’s a great deal of enthusiasm about this new technology,” says Dr. Winters. The da Vinci Surgical System can be used in procedures that treat a range of conditions, including obesity, coronary artery disease, kidney disorders,

A Lot Is Resting on Your Heart Simple tests save lives
For nearly two months in the summer of 2009, unusual test results and ambiguous ultrasounds left doctors uncertain about the status of her pregnancy. “No one was sure whether I’d miscarried or not,” she says.
Tracy’s obstetrician, Brian Koperek, M.D., had rarely seen a case like this. “An abnormal appearing placenta was seen at the first sign of life on an eight-week ultrasound.”
A RARE CONDITIONDr. Koperek immediately referred his patient to Saddleback Women’s Hospital perinatologists—physicians who specialize in high-risk pregnancies. Providing one of the few high-risk pregnancy programs in the area, the hospital offers compassionate, technologically advanced care for high-risk pregnant moms and newborns.
The news from the perinatologists was both good and bad. The baby was growing and appeared healthy. But Tracy had a condition called placenta accreta, which occurs when
the placenta grows too deeply into the wall of the uterus. “The placenta supplies the baby with oxygen and nutrients,” Dr. Koperek explains. “Normally, it detaches easily from the uterine wall after the baby is delivered. But in
women with placenta accreta, removing the placenta can lead to severe and sometimes life-threatening blood loss.”
Babies of women with placenta accreta are also likely to be born too soon. To prevent premature labor and delivery, Tracy was admitted to Saddleback Women’s Hospital 24 weeks into her pregnancy. She remained there on bed rest until her daughter Kate’s birth in January 2010. “At first, being confined to bed for so long was unimaginable—especially over the holidays. But my family and the extraordinary staff at Saddleback made it a good experience.”
Also amazing was Tracy’s experience with the hospital’s new Blood Donor Center, which provides patients with the safest, highest-quality blood and blood products.
A CARING RESPONSEBecause the mother of two toddlers was at high risk of hemorrhage, doctors recommended that friends and family donate blood for her. The response was overwhelming. Forty-three people—some of whom she’d never met—donated blood. As it turned out, she would need much of it. “In spite of everything, people were amazed at how well I did,” she says.
Saddleback Memorial offers advanced care for high-risk pregnant moms and newborns. Kate was born weighing 3 pounds, 12 ounces and spent six weeks in the hospital’s neonatal intensive care unit (NICU). Equipped to care for the sickest, most fragile infants, the level III NICU is staffed by a full complement of board-certified neonatologists, pediatric subspecialists, baby-friendly experienced nurses, respiratory therapists, social workers and pediatricians. Under the staff’s extraordinary, round-the-clock care, Kate soon began to thrive. Today, she’s a normal weight for her age, healthy, and according to her parents, a modern miracle. “We will be forever indebted to the hospital for all the wonderful care we were given,” Tracy says. I can’t express enough how grateful we are.”

High-Risk Pregnancy Defying the odds
When conducted each year by your physician, these simple tests can identify cardiovascular risk factors at their earliest stages, when they’re more easily treated and managed.
“The onset and progression of cardiovascular disease typically occurs over a number of years,” says Michael Gault, M.D., medical director at the MemorialCare Heart and Vascular Institute at Saddleback Memorial Medical Center. “But many people don’t see a doctor until they experience chest pain or, worse, have a heart attack. Good heart health isn’t about treating cardiovascular disease after you begin to have symptoms—it’s about taking steps now to prevent problems later on.”
PREVENTION IS KEY About 80 million Americans currently have at least one form of cardiovascular disease, with coronary heart disease, stroke and high blood pressure among the most prevalent conditions. ”I can’t stress enough the importance of prevention,” says Dr. Gault. Healthy habits that help include being physically active, watching your weight, not smoking, managing stress, maintaining normal blood pressure and cholesterol levels, controlling blood-sugar levels if you have diabetes and adhering to a heart-smart diet.
Each year, thousands of people in Orange County turn to the Saddleback Memorial Heart and Vascular Institute for expert evaluation, diagnosis and treatment of cardiovascular disease.
