HehasbeenworkingwithbothNHSEnglandandPublicHealthEnglandtobringaboutatransformationofcarewiththeaimofincreasingvalueforbothpopulationsandindividualsandpublishedabookcalledHowToGetBetterValueHealthcarein2007.ThemeansofdoingthisthroughSystemsisnowcalledPopulationHealthcare.Theaimofpopulationhealthcareistomaximisevalueandequitybyfocusingnotoninstitutions,specialtiesortechnologies,butonpopulationsdefinedbyacommonsymptom,conditionorcharacteristic,suchasbreathlessness,arthritis,ormultiplemorbidity.ThisworkrequiresculturalchangenotstructuralchangeandthedevelopmentofPopulationHealthcareandPopulationMedicine,astyleofclinicalpracticewhichfocusesonallthepeopleinneedinthepopulationandnotjustthosewhoreferthemselvesorhavebeenreferredishiscurrentfocusbothinEnglandandinanumberofothercountries.

DrMelanieHiornsisaconsultantradiologistandDivisionalDirectorofMedicine,DiagnosticandTherapeuticServicesatGreatOrmondStreetHospitalforChildren(GOSH)inLondon.Herclinicalworkisfocussedonbodyradiologyandshehasaspecialinterestinthegenitourinarysystemandgastroradiology.Shehasbeeninaseniormanagementroleforeightyearsandhasresponsibilityforover700staffanda£50Mbudget.

Inthisregard,radiologycanbeseenasagreatsuccesswithagrowthindemand,whichfaroutstripstheGDPgrowthoftheUK,morecloselyparallelingthegrowthofsoutheastAsianeconomies.Thishoweverputspressureonradiologydepartmentsandonindividualradiologiststoincreasethenumberofexaminationsundertakenandtodecreasethetimeframeinwhichtheyarebothperformedandreported.

Nationallycommissionedevidence-basedguidelinesarewidelyemployedinUKmedicinetopromotesafeandcost-effectiveworking.However,itisveryrareforNICEguidelinestosuggestlessimagingaspartofbestpractice.NICEguidelinestendtobedisease-basedandhaveacumulativeimpactonradiologyasadisciplinethatintersectswithdiverseclinicalpathways.Therehasalsobeenanincreaseinexaminationcomplexityandrequesterexpectation.Theriseinexamnumbersisoftenaccompaniedbyadropinthepositivity-rateforagiventest,whichhasimplicationsforthosereportingthem.

Radiologytraininghaschangedwiththeadventofnewtechnologiesandtechniques.ThecurriculaforClinicalRadiologyandInterventionalRadiologyarereviewedandupdatedonanannualcycletomaintaincurrencywithtechnologicalchangesaswellastocomplywiththerequirementsoftheGMCandotherregulatorybodies.

TheShapeofTrainingreviewisunlikelytoimpactonthecurrentstructureoftraininginradiology.CorecompetencesshouldbemaintainedthroughouttrainingwithtraineesatCCT/CSThavingcompetencesingeneralradiology,oneortwospecialinterestareasorsubspecialtycompetenceinInterventionalRadiology.
