Highlights of the 25th Hong Kong College of Family Physicians Conferment Ceremony and the 23rd Dr. Sun Yat Sen Oration, 3rd June 2012 – Part 1
ThisyearmarkstheSilverJubilee,the25thAnniversaryoftheHongKongCollegeofFamilyPhysicians(HKCFP)FellowshipConfermentCeremony.Thisextremelyjoyfuloccasionhasbeencoupledwiththe23rdDr.SunYatSenOration.TheceremonywassuccessfullyheldlastSunday,on3rdJune2012attheHongKongAcademyofMedicineBuilding.
TheHKCFPhas invitedmanygovernmentandclinical leaders, includinggovernmentofficials,universitycolleagues,oursistercolleges’ representatives fromoverseasaswellas localspecialtycollegesandmanyother localandoverseasdignitariestojoin intocelebratethesuccessofourConjointExaminationFellows,ExitExaminationandDiploma inFamilyMedicine (DFM)candidates.Wearemostgrateful thatwehavereceivedtremendoussupportfromtheaforementionedcolleaguesandpeerstomakethisyear’sceremonyarealsuccess.
After the introductionof theOfficialPlatformparty,Dr.RubyLee,ourCollegePresident,deliveredanopeningspeech. Itwas followedbythewelcomingspeechofDr.JennieKendrick,Censor-in-Chiefof theRoyalAustralianCollegeofGeneralPractitioners(RACGP).AftertheexchangesofgiftsbetweentheHKCFPPresidentandtheRACGPCensor-in-Chief,ProfessorDorisYoungwasawardedanHonoraryFellowshipoftheHKCFPforhertremendousachievements inpromotingthedevelopmentofFamilyMedicineinHongKongandheroutstandingcontributionstotheCollegeinthepastyears.
Thereweretwenty-sixdoctorsawardedHKCFPFellowship,twenty-twodoctorsawardedRACGPFellowship,forty-twodoctorsawardedExitExaminationCertificate,andsevendoctorsawardedDFM.Awardsforthebestcandidate inFellowshipExamination2011,theHKCFPBestResearch2011,andHKCFPResearchFellowship2012werealldeliveredonstage.TheceremonywasconcludedbyProfessorLeeSumPinggivingusaveryenlightening23rdDr.SunYatSenOrationentitled“WitherHealthcare”.

Before elaborating my story of developing the Logo of Family Physicians Links, I would like to give my sincere gratitude to FP Links for providing me a precious opportunity to participate in this logo design competition.
When I first saw this competition announcement on FP Links Newsletter, I was really interested as it alerted me to the fact that there is no specific logo for the newsletter FP Links apart from the College logo. It really motivated me to apply
for the competition as it is a challenge for me to give birth to a logo, a unique one for FP Links, which marks a milestone for the College newsletter.
In the beginning, I had several raw ideas linked with logo design, for example a “cross” sign which gives people a health care related thinking, a “doctor” sign which links with physicians etc. I would like the new logo to express a concept that the link is providing all family doctors a media for communication and platform for information sharing. Finally, I chose “Stethoscope” as the logo’s major symbolic component since all doctors and the majority of the public are familiar with this significant instrument. With the thinking that it would be more meaningful if the sign is composed of “FP Links”, I designed the stethoscope by using 3 important letters – “F”, “P” and “L”. Referring to the above picture, the left part of the logo is a stethoscope, its right earpiece is a letter ‘P’ for Physicians, the left earpiece is a letter ‘F’ for Family and the lower part of stethoscope is a letter ‘L’ for Links. The right part of logo is the full name of FP Links with the black font meaning staidness and sensitivity. There are 2 people (family doctors) linked together in red and jointly you can see a “happy face”.

Eachyear,the19thofMayisaspecialdayforallofus,as it istheWorldFamilyDoctorDayproposedby theWONCAsince2010.On the specialdaythisyear,OurCollegehadaspecialpubliceventin collaboration with the Primary Care Office,DepartmentofHealth,andtheEducationBureauof the Hong Kong Government. This was theAward Presentation Ceremony of the ThemedCompetitionentitled‘FamilyDoctorasyourhealthpartner’, followedby theFamilyDoctorForumorganizedbythePublicEducationCommitteeofOurCollege. Itwassuccessfullyheld inCitywalk,TsuenWanintheafternoonof19thMay2012.

Family Physicians Engaging in Whole Person CareRICE 與我的成長歷程Dr. Dana LoMedical Officer, University Health Service, The Polytechnic University
剛拜讀過蔡惠宏醫生近日刊登於明報周刊題為“望聞問切”及“診症功力”的文章,內容提及RICE (RICE是指Reason, Idea, Concern and Expectation of Consultation)在診症過程的重要性。適逢剛剛完成六年家庭醫學專科培訓,我回望過去的歲月,原來我運用RICE的模式也隨著年月一起成長了。
猶 記得 第一次 聽 到 R I C E 概 念,始 於第一 年當上 駐院醫生(Res ident)的時候。那時候我在公立醫院接受家庭醫學專科的首年醫院培訓(Hospi ta l -based training),一星期才有一個下午回到所屬的家庭醫學部門上課(structured seminar),學習包括RICE等理論。由於首兩年必須被外借到例如內外婦兒骨等其他專科工作吸取經驗,有感其他專科醫生大多也不是這樣診症的,所以上完課之後自然一早把甚麼RICE理論拋諸腦後了。
第三年進入社區培訓(Community-based training),我被調回普通科門診(GOPC)工作。GOPC簡直是新紮醫生的“少林寺”,病人多但時間少,而病症的種類又未必一定簡單,稍一不留神,隨時跌入陷阱而不自知,但同時間又要確保準時完場以便開夜診時段。當年的我,診症技巧尚未成熟,只好以“日趕夜趕”的速度,及恐防趕得太急,會跌進陷阱的心情來診症。當時首要任務,是要能夠在GOPC安全地生存,那有心情思考RICE呢?
我在第四年已經適應了門診生活,開始準備專科的中期試(Conjoint Fellowship Examination - Clinical Examination)。我是一位很乖的考生,參加了各式各樣
的精讀班,也自製和互相交換模擬試題。很多人說問中RICE有分,所以大家也認為RICE多麼重要!我就是這樣抱著問少一條也隨時會不合格的心情,開始強迫自己平日診症時只要“得閒”就問人RICE,來熟練考試模式。當然,這樣問出來的機械化效果,現在回想起來,也感到甚為“搞笑”!
我真正能把RICE融入每天恆常診症中,是要待我到第五、第六年的專科培訓(Higher training),那時候我巳離開了醫管局轉到大學醫療保健處工作。大學的診症環境理想,我終於擁有自己的診症室,病人可以輕易選擇指定醫生覆診,我也可以因應病人需要,預約一些例如輔導或小手術等特別時段來安排病人覆診。原來,RICE不是靠背出來的,有時候未必會順次序出現,也未必要把這四條問題逐一以“我問你答”的形式詢問,因為不少時候只要細心聆聽和意會,病人往往已經明顯地自己表達出來了。因此,RICE並非在“得閒”的時候才可以問的奢侈問題;相反,如果能夠一早就洞悉到病人的RICE,從而適當地回應病人各方面的訴求,那診症自然會變得有效率。
我沒有奇怪,為何要在自己畢業那麼多年後,才開始明白RICE的意義,因為每一位醫生也必須經過多年來的經驗和培訓才能夠成長。尤其我們家庭醫學,先苦後甜;未必有手起刀落的快速成功感,但卻擁有一群與我們一起成長同行的病人。我深信:“不經一番寒切骨,那得梅花撲鼻香?”

I rememberthatourteammemberspresentedaresearchprojectonmanaginglowbackpainandwewereverymuchimpressedbytheconstructiveandenlightening feedback fromothergroupmembers.
ThenextsessionwasgivenbyProf.SamuelWongand he introduced to us the basic principles offormulating research questions and setting uphypothesis.Healsospokeonhowtouseandsetupresearchquestionnairesandpitfalls toavoidduringdata collectionandentry.Mostofuswerebasicorhigher traineesof theCollegeandweall foundit ratherdifficult to formulateaworkable researchquestion inourdailypractice. Prof.Wongclearlyexplainedwhatsortsof topicswereacceptable inFM research.Healso stressed the importanceoffeasibilityandlogisticswhenconsideringtheresearchtopics and encouraged us to communicate withexperts/cliniciansatourworkingenvironmentandtohaveextensive readingsof the literature in thefieldof interest. Mostofusaredoingorplanning
todoquestionnairestudy,butwhatmakesagoodquestionnaire?Fromhis lecture,weunderstoodtheimportanceofreliabilityandvalidityofquestionnaires t ud y and s te p s to fo l low in q ue s t ionna i redevelopment.
Statistics isalwaysachallengingtasktousresearchbeginners. The lastsessionof theworkshop ison introductionof literaturereviewandresearchprotocol/proposalwritinggivenbyProf.MartinWong.Hedemonstratedtousclearlyhowtostartanefficientliteraturereviewandwheretofindtherelevantinformationinthelargedatabaseworld.Hethenexplainedtoustheessentialcomponents of a complete research protocol/proposal, i.e.presentation,background,objectives,methods,ethicalconsiderations,timetable,resources,references and appendices. This can ef ficientlyfacilitatefutureplanningandimplementationoftheresearchproject.Gettingapaperpublishedisbynomeanseasy,howeverwithhisexperiencesharing,welearnedthetipsofhowtoprepareamanuscript instructuredformatsandclearlayoutssoundly.
Ifoundthisworkshopusefulnomatterforbeginnersorexperienced researchers since the topicsherecover almost all we need to know in performingclinic research.Therefore, Ihaveno reservation torecommend thisworkshop tomycolleagueswhohaveinterestinconductingclinicalresearch.

FDADrugSafetyCommunication:5-alphareductaseinhibitors(5-ARIs)mayincreasetheriskofamoreseriousformofprostatecancer.
On6thSeptember2011,theU.S.FoodandDrugAdministration(FDA)informedhealthcareprofessionalsthattheWarnings and Precautionssectionofthelabelsforthe5-alphareductaseinhibitor(5-ARI)classofdrugshasbeenrevisedtoincludenewsafetyinformationabouttheincreasedriskofdevelopinghigh-gradeprostatecancer.Thisriskappearstobelow,buthealthcareprofessionalsshouldbeawareofthissafetyinformation,andweightheknownbenefitsagainstthepotentialriskswhendecidingtostartorcontinuetreatmentwith5-ARIsinmen.
ThenewsafetyinformationisbasedonFDA’sreviewoftwolarge,randomizedcontrolledtrials—theProstateCancerPreventionTrial (PCPT)andtheReductionbyDutasterideofProstateCancerEvents(REDUCE)trial—whichevaluateddailyuseoffinasteride5mgversusplacebofor7yearsanddailyuseofdutasteride0.5mgversusplacebofor4years,respectively,forthereductionintheriskofprostatecancerinmenagedatleast50years.Thetrialsdemonstratedanoverallreductioninprostatecancerdiagnoseswithfinasteride5mganddutasteridetreatment.Thisoverallreductionwasduetoadecreasedincidenceoflowerriskformsofprostatecancer.However,bothtrialsshowedanincreasedincidenceofhigh-gradeprostatecancerwithfinasterideanddutasteridetreatment.
Drugsinthe5-ARIsclassarefinasteride(marketedasProscar(finasteride5mg)andPropecia(finasteride1mg)anddutasteride(marketedasAvodart).Dutasterideisalsoavailableincombinationwithtamsulosin,underthebrand-nameJalyn.
Proscar,Avodart,andJalynareapprovedtoimprovesymptomsofanenlargedprostategland(benignprostatichyperplasiaorBPH).ProscarandAvodartarealsoapprovedtoreducetheriskofurinaryretentionorsurgeryrelatedtoanenlargedprostate.Propeciaisapprovedtotreatmalepatternhairloss.
Theadditionalinformationforhealthcareprofessionalsisasfollows:
• Beawarethat5-ARIsmayincreasetheriskofhigh-gradeprostatecancer.• Priortoinitiatingtherapywith5-ARIs,performappropriateevaluationtoruleoutotherurologicalconditions,including
prostatecancer,thatmightmimicbenignprostatichyperplasia(BPH).• Beawarethattreatmentwith5-ARIsleadstoanapproximate50%reductionofprostate-specificantigen(PSA)by6months;
however,individualpatientsreceiving5-ARIsmayexperiencevaryingdecreasesinPSAvalues.Therefore,anyconfirmedincreaseinPSAwhileona5-ARImaysignalthepresenceofprostatecancerandshouldbeevaluated,evenifthatPSAisinthenormalrangeofmennottakinga5-ARI.
• Knowthat5-ARIsarenotapprovedforthepreventionofprostatecancer.
FinasterideLabelChanges
On11thApril2012,theU.S.FoodandDrugAdministration(FDA)announcedchangestotheprofessionallabelsforPropecia(finasteride1mg)andProscar(finasteride5mg)toexpandthelistofsexualadverseeventsreportedtoFDA,assomeoftheseeventshavebeenreportedtocontinueafterthedrugisnolongerbeingused.Erectiledysfunctionafterstoppinguseofthesedrugswasaddedasaknowneventin2011.Thenewlabelchangesinclude:
• ArevisiontothePropecialabeltoincludelibidodisorders,ejaculationdisorders,andorgasmdisordersthatcontinuedafterdiscontinuationofthedrug.
• ArevisiontotheProscarlabeltoincludedecreasedlibidothatcontinuedafterdiscontinuationofthedrug.• ArevisiontoboththePropeciaandProscarlabelstoincludeadescriptionofreportsofmaleinfertilityand/orpoorsemen
qualitythatnormalizedorimprovedafterdrugdiscontinuation.
Despitethefactthatclearcausallinksbetweenfinasteride(PropeciaandProscar)andsexualadverseeventshaveNOTbeenestablished,thecasessuggestabroaderrangeofadverseeffectsthanpreviouslyreportedinpatientstakingthesedrugs.Informationabouttheseadverseeventsmaybeimportanttoindividualpatients.Therefore,prescribersandpatientsneedtobeawareofthem,aspartofadiscussionofriskandbenefitsoffinasteridewhendeterminingthebesttreatmentoptions.
Proscar(finasteride5mg)andPropecia(finasteride1mg)bothcontaintheactiveingredientfinasterideandbelongtoaclassofprescriptionmedicinescalled5alpha-reductaseinhibitors.
Proscar(finasteride5mg)wasapprovedbyFDAin1992andisindicatedforthetreatmentofbothersomesymptomsinmenwithbenignprostatichyperplasia(BPH).ItisalsoapprovedtoreducetheriskofurinaryretentionortheneedforsurgeryrelatedtoBPH.
Propecia(finasteride1mg)wasapprovedbyFDAin1997foruseinmalesonlyandisindicatedforthetreatmentofmalepatternhairloss.
FDAbelievesthatfinasterideremainsasafeandeffectivedrugforitsapprovedindications.Healthcareprofessionalsandpatientsshouldconsiderthisnewlabelinformationwhendecidingthebesttreatmentoption.
