The Mentorship Program consists of 7 modules. Each module requires that 5 completed clinical cases with Dr. Petrungaro’s mentorship.

Description of Program

Benefits of Participating inthe Mentorship Program

• Learning objectives of the specific case• Outline of specific treatment plan• Goals the participant doctor is trying to achieveAfter each case is completed:• Evaluation form after procedure is completed regarding their learning experience• Test administered covering the didactic and hands-on portions of the specific cases treated• Continuing education credit 2.5 hours per case completed as outlined above

Prior to the inception of each case, the following will be assessed:

• Implant placement, basic or advanced• Bone grafting/ membrane usage• Minimally invasive protocols• Optimizing implant esthetics• Optimizing immediacy and streamlining the surgical/ restorative process• Patient management pre-operative/ intra-operative/ post-operatively• Case presentation, implant surgical/ restorative procedures• Implant prosthetic procedures• Surgical operatory setup/ instrumentation/ supplies• Perform more complex bone replacement procedures• Have a resource center to help manage complex cases efficiently and by state-of-the-art procedures• Learn at your own pace, with one-on-one instruction while treating your own patients

Dr. Petrungaro will work closely with you to incorporate the above procedures into your practice. Cases can be brought to his office and participating doctors can work hand in hand on your own patients as Dr. Petrungaro instructs and demonstrates how the procedures are done and teaches each step of the way.

Please note all surgical fee’s are assessed by Petrungaro Periodontics and Aesthetic Implan-tology for services rendered to patients under the mentorship program. All surgical supplies and products will be provided by Petrungaro Surgical Solutions for use in the Mentorship Program. All surgical services will be completed at 30 N. Michigan Avenue, Suite 1220 Chicago, IL. The participant doctors will be completing the prosthetic aspect, and assessing their own fees for the prosthetic portion.
