n  In reading the comments on approximately 100 reports, a person would be hard pressed to say they came across anything that would be consider a quality catch."

n  There are both good and bad reasons why quality concerns (catches) are not recorded. A couple of the bad reasons would be; pride and fear of reprisal. "

n  Generally speaking, the pipeline industry is a prideful lot with many big egos. "

n  People state “I’ve been doing this for over 40 years and don’t need no one telling me how to do it right”. The irony in that statement is that the double negative is an admission that someone truly does need to tell that individual how to do it right. "

n  Statements like that come from both inspectors and contractor personnel. By documenting quality (and safety) deficiencies it’s an acknowledgement that perhaps everything isn’t as great as some may think. "

bar” for inspection."n  Every other construction industry (bridges, piping, tanks, etc) has

a program in place."n  Regulators were looking for something to fill this void – better
