Ihave always been a patient advocate; when I began my career as a nursing assistant, at Madera Community Hospital more than 37 years ago, then

as a Registered Nurse and as a Family Nurse Practitioner. Now, as Chief Executive Officer I have the opportunity to

advocate for all our patients and lead the entire organization on a quest for excellence in healthcare quality, safety and service.

Leading an institution, vital to the physical and economic health of Madera County, is an awesome responsibility.

According to The American Hospital Association, General Acute Care Hospitals have been closing at a rate of about 30 per year. As Chief Executive Officer, it is my charge to ensure our hospital thrives. Thriving requires goal alignment from our nearly 900 employees and more than 250 credentialed Medical Staff. It also requires support from our community.

Today, as I complete my first year as CEO of Madera Community Hospital, I know each member of our team is committed to providing the best patient care. We understand our patients have a choice on where to receive healthcare services and we value our community physicians and healthcare providers who choose Madera Community Hospital.

Although we are unwavering in our compliance of Federal and State regulatory standards and Hospital Facilities Accreditation Program standards, we are not the hospital or health system you heard about 30 years ago, 5 years ago or even a year ago. Our employee written and adopted Standards of Behavior have created a culture of courtesy and respect for one another and guides our united purpose of delivering patient centered care. Everyone on our team is an advocate for patients.

Treasured Trustees

Madera Community Hospital is a private, not-for-profit, General Acute Care facility, governed by a

self-appointed volunteer Board of Trustees. While the governance structure isn’t unique in Fresno or Madera, it is unique in the State. Most hospitals are part of larger health or university system. Regardless of the governance structure, hospitals are highly regulated and treasured community health and economic resources.

Charged with ensuring all residents have access to quality medical care, Hospital Trustees have four main responsibilities: Hire the Chief Executive Officer, approve Medical Staff privileges, pass policy and approve the annual budget.

The approval of Medical Staff privileges is based on California Law prohibiting hospitals from employing

physicians. Many consumers are unaware of the regulation. The rationale for the prohibition on employment of physicians is based on the potential for conflict between a physician’s loyalty to the patient and the financial interests of the hospital.

The success of a hospital is highly dependent on the relationship between the Governing Board, the Chief Administrator and the independent, self-governing Medical Staff members. Madera Community Hospital Board of Trustees wisely developed their governance structure to include three officers of the Medical Executive Committee, elected by their peers: Chief of Staff, Vice Chair and Immediate Past Chief.

The community is grateful for the commitment of our Hospital Trustees and their proven commitment to healthcare excellence. ■

Clinic, Urgent Care, Employer Based Clinic, or our own RapidCare Clinic, the Emergency Department is the front door into every hospital.

Madera Community Hospital Emergency Department averages 100 daily patient encounters. In early 2017, Valley Emergency Physician Healthcare, Inc. (VEP) was invited to provide the medical provider care in our Emergency Department. In the last nine months, under the Medical Directorship of Scott French, MD and the clinical directorship of Brady Danby, RN, consumers have noted an uptick in efficiency as well as compassion.

EDAC, Madera Community Hospital’s Emergency Department Advisory Council is being developed to improve care based on community perception and need. “The delivery of Emergency Medicine is complicated to most consumers,” said Dr. French. “The intervals of care include multiple interruptions and interactions in an environment of high density decision making.” EDAC members will work collaboratively to co-design programs and policies to effectively impact health care challenges and enhance the patients’ experience in Madera Community Hospital’s Emergency Department.

The Emergency Department Advisory Council will consist of 7-10 Community Advisors, along with our Emergency Department Clinical and Operational Leadership. The Council Members agree to serve a two year term and meet in-person, monthly for 90 minutes. EDAC Members will establish their own charter and work plan. Physician Profile - Afab Naz, MD – Pediatrician

told me I was going to be a doctor. So, I began my pre-med courses.

MCH: Sounds like your parents were a great influence.Dr. Naz: Yes. My mom was a school teacher and my dad was a clerk in education. I had 7 siblings and we lived in a 1 bedroom house. My mom is the one who always told us, ‘you must study’ if you want to succeed. MCH: Where did you study?Dr. Naz: I graduated Medical School from the University of Karachi in 1972. I always knew I wanted to be a pediatrician and the US Healthcare Expansion in the ‘60’s and ‘70’s provided the opportunity.

After three years in the Pakistan Army, I came to the United States for a degree in pediatrics. At the time, only 5% of the doctors from Pakistan passed the Educational Council for Foreign Medical Graduates Exam, required to practice in the United States.

I earned Board Certification in May of 1980, by The American Board of Pediatrics and in October of 1980, I was awarded a Fellow by the American Academy of Pediatrics.

MCH: What brought you to Madera? After a year in Kansas City, practicing at a psychiatric facility,

I took a job at Cook County Hospital in Chicago. My plan was to practice for two years and pass Board Certification. I needed to pass the Boards before I could return to Pakistan. After passing my Boards I told my wife, just two more years in the US and we will be financial secure to return home, but this County seduced me.

In 1981, a former classmate, Mohammad Ashraf, MD, called me in Chicago and told me he was practicing in California and the hospital needed a pediatrician. I told him I couldn’t make the visit to Madera, because my wife was close to delivering our second child. Dr. Ashraf told me, ‘your wife is going to deliver regardless of where you are.’ I couldn’t argue with his logic so I made the visit to Madera.

California is one of five states in the United States where doctors cannot be employed by hospitals. The rationale for the prohibition on employment of physicians is based on the potential for

conflict between a physician’s loyalty to the patient and the financial interests of the hospital corporation. Doctors are independent practitioners licensed by the California Medical Board and governed by their peers.

MCH: First off, Happy Birthday. Dr. Naz: Thank you. My children surprised me with a

party at my son’s house. There were about 150 family and friends who attended

MCH: How many children do you have? Dr. Naz: I have 3 children, a daughter and two sons. My

daughter, Lizma, is the first born. She has a master’s degree in linguistics. Lizma runs my clinical research division. My oldest son, Ednann, is a doctor and administrator for Community Medical Centers. My youngest, Tabish is also a medical doctor. He is starting his Internal Medicine residency in Michigan. MCH: You are conducting clinical research in Madera?Dr. Naz: Yes. In fact, my daughter and I just returned from a clinical research meeting in Spain.

I am a principle investigator and have documented the effects of more than 100 pharmaceuticals and vaccines.

MCH: When did you know you wanted to be a doctor?Dr. Naz: Around the time I was in middle school; my dad

MCH: What made you stay in Madera?Dr. Naz: I had a good opportunity to build a practice but primarily, it was the lifestyle. The trees are green the weather is great and my commute in Chicago was more than an hour each way. The first 13 years in Madera, I lived one mile from my office.

The geographic similarity to Pakistan was a draw, but my wife didn’t like it. She liked the urban features and benefits of Chicago.

MCH: How about now?Dr. Naz: We moved to Madera in June. When the weather began to change, my wife went shopping for a coat for our daughter. She found a coat at a local boutique. She asked the store owner if she could return the coat if her husband wasn’t pleased with the purchase. The store owner said to her, ‘Mrs. Naz, take the jacket. If your husband doesn’t like it, bring the jacket back. If he does, mail us a check.’ This experience brought a new sense of appreciation for rural living. Nothing like this would have happened in Chicago.

MCH: The story sounds like Madera welcomed you and your family.Dr. Naz: Yes but I also earned the community’s respect and support. I have had the privilege of caring for so many of our community’s children. For the first several years, I answered calls and met parents and children any time or night and on the weekends.

MCH: Did any of your siblings or your wife’s family move to Madera?Dr. Naz: More than 100 of my relatives live in the Madera /Fresno area. In the last 40 years, all my brothers and sisters as well as all nine of my wife’s siblings moved here. My mother is buried in Madera and my father is buried in Fresno.

MCH: What would you like the community to know about our hospital?We have a great hospital. Most of the specialties are available. If anyone of our residents has a medical emergency, guess where you are going to go? Madera Community Hospital. If we are good for emergencies, we are good for routine care.

MCH: In addition to owning a successful pediatric medical and clinical research practice you also give your time toward advancing the health and wellbeing of the community.Dr. Naz: Madera is my place. All of my investments are in Madera. Whatever life I have left, it is important for me to do good. ■ Place Carpet One

n October of 2018, the American Cancer Society presented data on HPV Vaccination rates; noting Madera County leads the

state with 43% of the targeted youth population having received the vaccination. San Francisco followed with 39%. Other Counties in our region fell to 31% in Tulare, 27% in Fresno and 19% in Kings County. The California Dialogue on Cancer HPV Vaccination Roundtable is striving to attain an 80% vaccination rate in the targeted youth population.

The American Cancer Society awarded a small grant to the Hospital for the development and implementation of strategies to increase HPV Vaccination rates. CalViva Health is partnering with Madera Community Hospital Family Health Services to develop strategies to attain 80% vaccination compliance. “We are excited about the partnership with CalViva Health and our mutual commitment to creating opportunities for health promotion,” said Peter Garcia, RN, Director, Family Health Services “Parent education and health practitioner diligence are our primary tools for ensuring all youth between 11-12 are protected from nine different cancers.”

The HPV Vaccination is a covered health expense and can be administered during the well visit when also receiving the Tdap Vaccination, required prior to entering middle school.

The human papillomavirus, HPV, is a serious problem

causing several cancers. The HPV vaccination protects

against this virus and the cancers it can cause. The

virus is so common it will infect most people at some

point in their lives. Most infected people do not know

it. The best way to help prevent HPV infection is to get

vaccinated. The HPV Vaccination is expected to prevent

90% of HPV related cancers when given before children

are exposed to the virus.

2018 Employer of the Year

In a letter from Nick Gomez, President of San Joaquin Valley College, Madera Community Hospital was lauded for supporting 12 students through externships; ensuring students received quality hands-on practical skills in the workplace. “The skills our students learned through externships are immensely valuable and our college cannot accomplish this without the help and partnership of employers like Madera Community Hospital,” said Gomez.
